Submasseteric Abscess (깨물근하 농양)

  • Purpose: The masserteric space is an important tissue compartment of the face, but a disease in it is difficult to diagnose and treat. The submasseteric abscess is located between the masseter muscle and mandibular ramus with different appearances such as sepsis, infection, or tumor. Especially the common misdiagnosis of submasseteric abscess is acute or chronic parotitis. The purpose of this report is to pay special attention to the possible diagnosis of submasseteric abscess for the symptoms of unilateral cheek swelling and tenderness that accompany marked trismus. Methods: A 11-year-old boy came to our hospital because of facial swelling, tenderness, and trismus in a history of left cheek swelling and toothache. We diagnosed his case as submasseteric abscess by CT scan and surgical intervention was performed. Under general anesthesia, the abscess was opened by the intraoral incision and considerably massive pus was drained. Results: Swelling, tenderness, and trismus became to subside during postoperative 10 days and general condition and vital signs became stable. After 6 months, CT scan showed that both masseteric muscles were symmetric and there was no periosteal reaction of the mandible. Conclusion: In conclusion, submasseteric abscess is a rare infection with the symptoms of cheek tenderness and marked trismus. A detailed medical history and clinical examination of a patient as well as computed tomography(CT) are important tools in the accurate diagnosis and efficient treatment of the submasseteric abscess. Adequate drainage, removal of cause, and antibiotic infusion are the management of choice.

A Case of Childhood Biliary Stricture after Blunt Trauma Managed by Endoscopic Therapy (소아에서 둔상 후에 발생한 담도협착의 내시경적 치료 경험 1례)

  • Isolated injury to the extrahepatic biliary tract following blunt abdominal trauma is rare, and there is little information especially in children regarding the endoscopic diagnosis and management of occult injury to the biliary tract. We experienced a 5-year-old boy who presented with jaundice 16 days after blunt abdominal trauma and was diagnosed as isolated distal common bile duct stricture by ultrasonography of abdomen. We could get more detailed information about the injury by endoscopic retrograde cholangiopancreatography. We could also manage this isolated common bile duct stricture successfully with endoscopic nasobiliary drainage and plastic stent insertion instead of surgical correction. There appeared to be no recurrence of stricture as evidenced by biochemical test and ultrasonography during 2 years of follow up.

Differences in Sexual Knowledge, Attitudes and Perspectives of Sexual Value of Female Students as related to Sexual Characteristics (성관련 특성에 따른 여학생의 성지식, 성태도 및 성가치관의 차이)

  • Purpose: The purpose of this study was to identify differences in sexual knowledge, attitudes and perspectives of sexual values for female students as these factors related to sexual characteristics. Methods: The participants were 835 girls in junior or high school from Seoul and Kyunggi province in Korea. Data were collected using self-report questionnaires and analyzed using descriptive statistics, t-test, F-test and Scheffe with the SPSS 14.0 WIN program. Results: The major results were as follows: First, for sexual knowledge, there were significant differences for girls from homes with conservative atmosphere and for those who had experiences related to porno. Second, related to sexual attitudes, students who had experience related to porno, were significantly more positive, and had a more open attitudes than students with no experience. Third, for perspectives of sexual value, students who perceived a conservative home atmosphere, who had boy friend, and who had experiences related to porno, had a more positive sexuality, and attitudes towards sexual equality. Conclusion: The findings indicate that it is necessary to develop special sex education programs for both school and parents so that teachers and parents can guide these students appropriately.

A study on the utilization of a high school health clinic in Seoul (서울시내 일개 고등학교 학생들의 양호실 이용에 관한 조사)

  • This study was aimed at finding the status of utilization of school health clinic at a boy's high school in Seoul, from March 1988 to February 1990. Data were collected from the records of school health clinic. Major findings are as follows: 1. The average frequency of visits per person was 1.62, 1.92, 1.53 per a year respectively. 2. Students in grade two had the highest frequency of visits of the school health cliniclin.2 during 3years. 3. Injury was the most frequent type of complaint treated at the school health clinic and G-I trouble was the next. Cough with tonsilitis and e.t.c schowed no rate difference between years. 4. The rate of utilization was higher in April, June and September than other months, and lower in Feburary, December. In general, the type of complaints does not seem to be affected by season. 5. In weekday's utilization, the rate of Monday and Tuesday was highest. 6. The average hospital-refer rate for 3 years was $1.02\%$, and that of Injury was highest.

The Sixth Grade Children's Understandings of the Greenhouse Effect (초등 6학년 학생들의 온실효과에 대한 이해)

  • The purpose of this study was to investigate elementary school students' conceptions about the knowledge source and the mechanism about the greenhouse effect, and the relationship between the greenhouse effect and the global warming. Participants were 157 sixth graders (boy : n=79, girl : n=78) from six elementary schools located in the same city. The open-ended questionnaire was used to examine the students' spontaneous ideas depicted by their drawings and/or writings. The results of this study are as follows: First, although there is no the content on the greenhouse effect in the elementary school curriculum document, the children in this study indicated that the main source of their knowledge about the greenhouse effect came from 'school class' (31.8%); Second, although the children did not take a class about the greenhouse effect itself, 14.0% of children had a relatively high level of the mental model on the phenomenon; Third, more than 90% of the children did not have the correct understanding about the relation between the greenhouse effect and the global warming.

Solitary Cervical Neurenteric Cyst in an Adolescent Patient

  • Spinal neurenteric cysts are uncommon congenital lesions, furthermore solitary neurenteric cysts of the upper cervical spine are very rare. A 15-year-old boy having an intraspinal neurenteric cyst located at cervical spine presented with symptoms of neck pain and both shoulders pain for 2 months. Cervical spine magnetic resonance (MR) imaging demonstrated an intradural extramedullary cystic mass at the C1-3 level without enhancement after gadolinium injection. There was no associated malformation on the MR imaging, computed tomography, and radiography. Hemilaminectomy at the C1-3 levels was performed and the lesion was completely removed through a posterior approach. Histological examination showed the cystic wall lined with ciliated pseudostratified columnar epithelium containing mucinous contents. Neurenteric cyst should be considered in the diagnosis of spinal solitary cystic mass.

Intracranial Pial Arteriovenous Fistulas

  • Intracranial pial arteriovenous fistula (AVF) is a rare cerebrovascular lesion that has only recently been recognized as a distinct pathological entity. A 41-year-old woman (Patient 1) presented with the sudden development of an altered mental state. Brain CT showed an acute subdural hematoma. A red sylvian vein was found intraoperatively. A pial AVF was revealed on postoperative angiography, and surgical disconnection of the AVF was performed. A 10-year-old boy (Patient 2) presented with a 10-day history of paraparesis and urinary incontinence. Brain, spinal MRI and angiography revealed an intracranial pial AVF and a spinal perimedullary AVF. Endovascular embolization was performed for both lesions. The AVFs were completely obliterated in both patients. On follow-up, patient 1 reported having no difficulty in performing activities of daily living. Patient 2 is currently able to walk without assistance and voids into a diaper. Intracranial pial AVF is a rare disease entity that can be treated with surgical disconnection or endovascular embolization. It is important for the appropriate treatment strategy to be selected on the basis of patient-specific and lesion-specific factors in order to achieve good outcomes.

A Long-Term Survival Case of a Primary Malignant Intracerebral Nerve Sheath Tumor

  • We report a long-term survival case of a primary malignant intracerebral nerve sheath tumor (MINST) occurring in the right frontal lobe of a 13-year old boy. After the gross total resection (GTR), we have performed radiation therapy but it recurred 50 months after the surgery, so the second GTR was performed. Later, second tumor recurrence was found 4 months after the second surgery. Subsequently the third GTR, radiotherapy, and chemotherapy were carried out. At present, the patient has been remaining alive for 77 months without evidence of tumor recurrence. According to the previous reports, the primary MINST is very rare : there are only 8 cases reported. It is also a fast-growing, invasive tumor with poor outcome. This is the first case that had no recurrence for 50 months after the surgery among the reported cases that had been followed up for more than 5 years. It is supposed that a period of recurrence free survival after GTR and low mitotic activity are associated with the patient's prognosis. A GTR followed by adjuvant radiation therapy and chemotherapy will be recommended to patients of MINST.

Primary psoas abscess confused with hip pathology (고관절 이상으로 오인된 일차성 요근 농양 1례)

  • A healthy, 14-year-old boy presented with right hip pain and consequent fever after falling out of bed while sleeping. The patient could not walk and complained of severe pain with active and passive motion, which consisted mainly in extension and internal rotation of the right hip. Laboratory analysis of the peripheral blood identified leukocytosis and increased levels of acute phase reactants. Magnetic resonance imaging of the hip, which was performed with the expectation of right hip pathology, revealed cellulitis and abscess in the right psoas muscle and associated inflammatory changes in the adjacent presacral fat plane but showed no abnormal lesions in the adjacent pelvic bone and spine. Staphylococcus hominis was cultured from the blood. With empirical antibiotic therapy, the patient recovered fully. We report a case of primary psoas abscess confused with hip pathology in an immunocompetent child without underlying disease.

Coronary artery fistula associated with single coronary artery (단일관상동맥 동정맥루 1례)

  • A case of a single coronary artery complicated with a coronary artery fistula (CAF) to the right ventricle is extremely rare, and its management strategy and prognosis are not clear. A 5-year-old boy was hospitalized for evaluation of a continuous heart murmur. Transthoracic echocardiography suggested a CAF to the right ventricle, with an enlarged left coronary artery. Cardiac catheterization confirmed the CAF terminating at the right ventricle and the absence of a right coronary artery. The fistula was ligated at the right ventricular side under cardiopulmonary bypass. At follow-up 18 months later, the child was clinically asymptomatic, and coronary angiogram showed no recurrence of the fistula.
