임상간호사의 이직경험 (A Study On The Experiences of Turnover among Hospital Nurses)

  • The purpose of this research was to understand the structure of the experiences of turnover among hospital nurses. The research question was 'What is the structure of the experiences of turnover among hospital nurses?' The sample consisted of 16 hospital nurses who experienced one or more turnover. The unstructured interview were carried out from July 15. through August 31., 1997. Intervewed by audio-recording and analyzed by Van Kaam's method. There were 361 descriptive expressions and priority classifications. The result summerized as 107 common elements, 38 syntheses of hypothetical definitions and 6 identifications of the structural definition. The structural definitions and hypothetical definitiones were as follows ; 1. There were different views on the turnover intention. Hard task, dissatisfaction of initial expectancy, uncomfortable relationship with doctors, inappriate reward, uncomfortable relationship with workers of other parts, uncomfortable relationship with patients, unreasonable promotion, uncomfortable relationship with co-worker, helplessness, poor environment of working conditions. 2. Motivating factor of turnover can be varied : self development, problem of social support, boring task, problem of relation-ship with the senior, unfair promotion, night duty. 3. Social supportive factors were co-worker support, senior support, self control, family support, time support. 4. There were different views on the job satisfaction. Task, relationship with patient, recognition, professionalism, promotion, working environment, relationship with co-worker, reward.

신생아중환자실 간호사의 간호업무량 분석 (Nursing Time Use in a Newborn Intensive Care Unit (NICU))

  • This study examined nursing care in a Newborn Intensive Care Unit (NICU) by reviewing nursing activities for the newborns. Through direct observation, time used for nursing care according to the nursing activity, shift, day of the week, and position of the nurses was measured. This study was done on November 15, 21, 24, 1999 at a university medical center hospital and included eight nurses and 179 newborns as the study subjects. The data were collected from the medical records, and by using a nursing activity record for the NICU, and a nursing activity time record for the NICU. The first step in the data collection process was to develop a list of nursing activities which was done through a literature review, examination of medical affairs and duty records. Content validity was measured by a panel of three professors who were experienced clinicians. In the second step two pre-training sessions were held with three sophomore student nurses who then measured the time for each nursing activity using a stopwatch. The data were analyzed using frequencies for nursing activities, averages, percentages and ANOVA for differences between shift and between days of the week, and percentages and t-test for differences according to position of the nurse. The results are as follows: 1) The total number of activities was 156, direct or indirect nursing activities. Direct nursing activity classified according to physical, educational, emotional/social/economic/spiritual needs. There were 109 direct nursing activities in 16 fields. 2) The order of nursing activities, according to time required, was record keeping, nutritional care, measurement/observation, medication, hygiene care, examination and specimen collection, and checking supplies, and according to frequency, measurement/ examination, record keeping, nutrition care, hygiene care, elimination care and medication. 3) According to shift, direct care during the night shift at 313.4 minutes was the longest time and indirect nursing care during the night shift at 252.2 minutes was the highest time. 4) For days of the week, Monday had the highest time for direct care 275.8 minutes (34.6%) and Wednessday had the highest time for indirect nursing care 269.6 minutes (36.1%). 5) For nursing time according to position of nurse, general nurses had the highest for direct care (330.7 minutes), nurse managers for indirect nursing activities (239.0 minutes) and general nurses for individual private time (63.9 minutes). The results of this study show that the major nursing time consuming activities included record keeping, nutrition care and measurement/examination. For newborns, time needs to be allowed for care to be sensitive, sophisticated and specialized rather than concentrated on indirect nursing tasks such as record keeping. Therefore, it is imperative to develop computerized systems that support a systematic approach to record keeping which is more efficient. Moreover, nursing needs according to shift, day or position of nurse can be utilized in assessing nursing resources through a computerized process.

활동기준원가계산(ABC)을 이용한 간호활동 분석 및 간호서비스 원가분석;일 산부인과 간호단위를 중심으로 (Analysis of Nursing Activities and Cost of Nursing Service Based on the ABC System)

  • The purpose of this study is to examine a possibility of applying the ABC system to analyze the cost of nursing service on one obstetrics and gynecology (OB/GYN) unit in a major medical center. The activities of nurses on an OB/GYN unit were analyzed for 4 days for 96 hours. 2 week days and 2 days on weekend. Total cases included in the study were activities of 12 charge nurses and 12 staff nurses. Activities were categorized into direct and indirect activities in order to calculate the cost of activities. Nursing activities were defined multidimesionally in order to utilize the ABC system. The 60 nursing activities were classified into the direct nursing activities and the indirect nursing activities based on a literature review, interview, and survey results. The direct activities were further categorized into the single nursing activities and the multiple nursing activities. The indirect activities were classified into the indirect nursing service activities and the general management activities. The major findings of this study were as follows : 1. There were differences in activities according to the positions and duties of the nurses. The charge nurses mostly performed the indirect nursing service activities and the general management activities. Almost all of their indirect nursing activities spent on each patient were similar. The staff nurses performed the single nursing activities and the multiple nursing activities. 2. The activities of staff nurses included medication, patient assessment, rounding and organizing the unit. patient education, nursing treatment, admission and discharging of patients. There was no differences in types and amount of time spent between the nurses on day-time duty and evening-time duty, but the patient assessment activity increased during the night-time. 3. The cost of post partum nursing services for women who had a normal vaginal delivery using the ABC system included the cost incurred by direct nursing activities provided by the staff nurses (85.9%), indirect nursing activities incurred by the staff nurses(19.0%) and the nursing activities by the charge nurses and the head nurse(14.1%). The ABC system is a relatively new method of cost analysis. The results of this study can provide the nursing and hospital managers with useful information on cost control. It is suggested that more studies should be done using the ABC system and extend the scope of studies to include value analysis to aid the Activity-Based Management(ABM) and/or the reengineering of hospital process.

LED의 간헐조명과 RGB 비율에 따른 상추의 품종별 생육 특성 (Growth Characteristics of Lettuce under Different Frequency of Pulse Lighting and RGB Ratio of LEDs)

  • 본 연구는 상추((Lactuca sativar L.)의 3가지 품종에 대해서 RGB LEDs의 각각의 다른 비율과, 듀티비 50% 조건의 RB LEDs를 이용한 여러 가지 주파수를 가지는 펄스광 조사가 상추의 생장과 형태형성에 미치는 영향을 살펴보기 위하여 수행되었다. 파종 후 육묘 기간을 거쳐 유사한 외형을 갖는 묘를 선발하여 재배 룸의 온도와 습도, $23{\pm}1^{\circ}C/50-60%$(주간)과 $18{\pm}1^{\circ}C/70-85%$(야간) 조건에서 담액수경재배로 4주간 재배하였다. 광합성유효광량자속밀도(PPFD)는 재배베드 위에서 $110{\pm}3{\mu}mol{\cdot}m^{-2}{\cdot}s^{-1}$, RGB 비율은 6:3:1, 5:2.5:2.5, 3:3:4, 2:2:6, 1:1:8 이었다. 50% 듀티비를 갖는 펄스광은 RB LEDs로 구성되었고 설정된 주파수는 50, 100, 500, 1,000, 5,000, 10,000, 25,000Hz(20, 10, 0.1, 0.04ms) 이었다. RGB 비율 6:3:1에서 적축면 상추의 생체중은 다른 RGB 처리구와 비교하여 유의적으로 높은 값을 보였으나, 대조구인 형광등 처리구와는 유의적 차이가 발생되지 않았다. RGB 비율 1:1:8의 조건에서, 롤로로사는 생체중, 그랜드 래피드는 엽수와 생체중이 다른 RGB 처리구와 비교하여 유의적으로 낮았다. 청색광의 비율이 증가할수록, 3개 품종 모두에서 엽장이 감소하면서 엽형이 둥근 형태로 발달하였다. RB LED로 구성된 LED 광조건 하에서 50% 듀티비 조건과 처리된 여러 주파수의 증가 또는 감소에 따른 상추의 생육 및 형태형성에 미치는 경향성을 발견하기 힘들었다.

사업장 근로자의 구강건강신념과 구강보건교육 인식에 관한 조사연구 (A Study on the Oral Health Belief and Oral Health Education Awareness of Workers in the Workplace)

  • Objectives : A survey was conducted to investigate oral health belief and education awareness of oral health of workers in the workplace and to obtain the basic data necessary to promote their oral health and design an oral health education program. Methods : This study conducted a self-administered survey. Results : The following conclusion was made. For oral health beliefs based on the type of work that the workers did, workers in production and technical services had more benefits than those in other fields(security, guard, etc.), and workers on night duty had more oral health hygiene problems than that of regular day-time workers. The longer the period of service was, the more sensitive and serious the workers were and the less beneficial the work was; workers working for eight hours or less had higher levels of importance than those working for ten hours. They were well aware of the need for oral health education but were less aware of the need for an oral health room. Conclusions : It is necessary to emphasize oral health beliefs when developing an oral health education program that promotes oral health for workers in the workplace. If an oral health education program attempts to reflect the concerns of workers in the workplace and provides preferred contents and methods on oral health education, the program is expected to promote the active and positive participation of the workers.

임상간호사의 억압된 분노, 용서 및 삶의 질 간의 관계 (The Relationship among Anger-in, Forgiveness, and Quality of Life in Clinical Nurses)

  • Purpose: The study was to identify relationships among the Anger-in, Forgiveness, and Quality of Life in Clinical Nurses. Methods: Data were collected by questionnaires from 205 clinical nurses who worked at the 4 hospitals located in Busan. Data were collected from September 26 to October 10, 2013. Anger-in, Forgiveness, and Quality of Life were measured using a structured questionnaire. The instruments included Anger-in Scale, Forgiveness Scale, Quality of Life Scale. The collected data were analyzed using frequency, percentage, independent t-test, One-way ANOVA, $Scheff{\acute{e}}$ test and Pearson's correlation coefficients by SPSS/WIN 18.0 for windows. Results: There were significant correlations among anger-in and forgiveness(r=-.34, p<.001), anger-in and quality of life(r=-.33, p<.001), forgiveness and quality of life(r=.27, p<.001). There was a statistically significant difference in Anger-in of participants according to nurse's satisfaction of job. Forgiveness showed significant different according to nurse's perceived health status, belief of religion, turnover intension, and job satisfaction. Quality of life was significantly different according to nurse's age, perceived health status, work period, work pattern, night duty, turnover intention, motivation of choice in nursing and job satisfaction. Conclusion: These results suggested that reducing anger-in level and enhancing forgiveness level would increase quality of life among clinical nurses.

LED에 의한 항로표지 시설의 감성식별 조명에 대한 연구 (A study on the emotional identification lighting of AtoN facility by LED)

  • 해안 또는 항만에 설치되는 항로표지 시설에 3색 LED를 적용하여 식별을 용이하게 하는 시각적인 감성식별 조명에 대한 연구로 항만 양측의 홍등 및 녹등에 의하여 입구를 확실하게 표시하고, GPS에 의한 시각동기와 타이머를 통하여 항만 유도등을 좌우동시 동기 및 일렬순차 동기시키는 콘트롤러를 구현하였다. LED에 의한 등대 구조물을 다양한 색상으로 표현하고 또한 높이에 따른 층별 색상으로 조화시키는 감성적 조명으로 야간에 항만의 수많은 조명과 구별되어 항해사들의 표지식별에 대한 혼란을 예방하고, 야간항만의 경관을 미화시키는 기대효과가 있다. 선박의 항만진입을 돕는 유도등의 동기 및 순차점멸 그리고 항만안전 메시지를 전달하는 CW 발광에 대한 기능을 부가하였고, 동명기를 고휘도 LED로 대체하여 전력소비를 크게 감소시킨다.

항로표지 시설의 LED 감성조명을 위한 RGB 원격 콘트롤러의 구현 (An Implementation of the RGB Remote Controller for LED Emotion Lighting of AtoN Facilities)

  • 해안 및 항만에 설치되는 항로표지 시설에 3색 LED를 적용하여 야간에 식별을 용이하게 하는 감성적 시각의 조명과 항만 진입등의 점멸제어 방식에 대하여 연구하였다. 항만 양측에 홍등, 백등의 항로표지를 주변의 조명과 차별화하여 식별을 쉽게 하였고, GPS에 의한 시각동기와 타이머를 통하여 항만 진입등을 좌우 및 일렬순차 점등으로 동기시키는 통합형 콘트롤러를 설계하였다. LED에 의한 AtoN(Aids to Navigation) 시설을 높이에 따른 층별 색상으로 조화시키고 감성조명으로 표현하여 항해사의 표지식별에 대한 혼란을 방지하며 야간항만의 경관을 미화시키는 효과를 얻었다. 또한 선박의 항만진입을 인도하는 메시지를 전달하는 모르스 부호 발광의 기능을 부가적으로 구현하였다. 등명기는 반영구적인 고휘도 LED로 대체되므로써 수명과 소비전력에 대한 효율성을 증대하였다.

15세기 궁궐 정전(正殿) 영역의 내부공간 이용방식에 관한 연구 -문헌을 중심으로- (A Study on the Usage of the Interior Space of Jeongjeon Zone at the Royal Palace in the 15th Century -Focused on the study of the literature-)

  • This study is to comprehend the interior space of the Royal Palace in the 15th Century, the early years of Joseon Dynasty. The subject of this study is limited to the center of the Royal Palace, that is Jeongjeon(正殿, the royal audience chamber) and Haengrang(行廊, which encloses Jeongjeon on four sides and has many rooms). It is very important to understand the usage of the interior space because the architectural space consists of the space unified by the organic function of the interior and the exterior space. But there are few studies on the interior space of Jeongjeon and Haengrang at the Royal Palace. Therefore, the purpose of this study is to understand the interior space of those buildings. The result of this study is following. Haengrang has several uses such as a night duty room, a storehouse, a government office or a banquet hall etc. So the interior spaces were finished with various methods that were suitable for the use of each room, and the material of the floor were the ground, Maru(the wooden floor) or Ondol(the Korean traditional heating system) There were held many kinds of ceremonies in Jeongjeon, and the government officials could not enter the inside of that building and took part in the ceremony on the front court of Jeongjeon, except the men performing the ceremony. But the high ranking officials could enter the inside when King gave a banquet and there, they prostrated themselves before King. They sat down with their legs crossed on the ground floor instead of sitting on a chair. When King held tea ceremony with Chinese envoys in Jeongjeon, they sat on Gyoui(交倚, a kind of armchair). Then, the government officials performing the ceremony in Jeongjeon prostrated himself around the King and the Chinese envoys and others stood around them.

해상교통관제사의 스트레스 수준 측정에 관한 연구 (A Study on Occupational Stress of the VTS Operators)

  • 해상교통관제사들의 과도하고 장기화된 스트레스가 그들의 삶의 질 뿐만 아니라 제공되는 서비스의 질에도 부정적인 영향을 미쳐 결과적으로 업무과중을 일으켜 선박사고 발생 위험률을 높일 수 있다는 데에 주목하고 스트레스 수준을 측정함과 아울러 스트레스에 영향을 미치는 직무스트레스 유발요인들을 추출해 보았다. 표준화된 측정도구(KOSS, PWI)로서 관제사들의 스트레스 수준을 측정해 보고, 배경적 변수(연령, 근무경력, 해상교통량 등)에 따라 스트레스에 차이가 있는가를 분석하였다. 그 결과, 관제사들은 일반인보다 높은 수준의 사회심리적 스트레스를 겪고 있었으며 연령 및 근무경력 별 스트레스 수준은 역U자 형태를 보였다. 또한 스트레스를 유발하는 직무환경적 요인들을 규명하였으며, 스트레스 관련 교육 및 근무환경 개선, 새로운 휴식시설 마련 등은 직무스트레스를 유의하게 낮게 함을 밝혔다. 그리고 근무환경, 교대근무 등의 직무환경적 스트레스 요인으로서 관제사의 직무스트레스를 설명할 수 있는 회귀모형을 도출하였다. 본 연구는 인적재해발생의 주요 원인인 직무스트레스에 관하여 고찰하고 해상교통관제 분야의 인적요인 연구에 필요한 기초자료로 삼고자 한다.
