  • The typical radiologic findings of a small radiolucent nidus with surrounding sclerosis and night pain responding to aspirin make the diagnosis of osteoid osteoma not too difficult. We report a case of multicentric osteoid osteoma with unusual clinical and imaging features which were strongly suggestive of subacute sclerosing osteomyelitis.

  • Detection of wooden foreign body represents a clinical challenge. Wood is typically radiolucent, so wooden foreign bodies are generally cannot be seen on survey radiography. Failure to locate and remove foreign bodies can lead the patient to the long-term secondary inflammatory reactions or infections. The dog described in the present report ingested a wooden foreign body(cotton swab stick) a year ago. The foreign body remained in the intestinal tracts which were attached to each other due to the prior abdominal operation. The wooden piece in the intestine functioned as a nidus and inorganic matters were gathered forming calculus on the outer layer of foreign body. In the radiograph, the foreign body appeared to be a chicken bone which of it's inner area was more radiolucent than the outer layer. Because the wooden foreign bodies that have remained for long time in the intestinal tract can be seen like a bone by mineralization, the diagnosis of the wooden foreign bodies should be done prudently.

  • Although Osteoid osteoma has occurred in every bone of the body, it is rarely found at the elbow and in younger children than 5 years old. In 2 years old child, diagnosis can be delayed because of nonspecific clinical, radiological features and making it difficult for the child to communicate with physician. We report a case of osteoid osteoma of the distal humeral metaphysis with intracapsular nidus and synovitis of elbow in 2 years old girl who was treated by a surgical excision.

Study on a Herb pair of Pericarpium Zanthoxyli(Zanthoxyli Fructus) in 『Donguibogam』 (『동의보감(東醫寶鑑)』에 활용된 천초(川椒)(촉초(蜀椒), 화초(花椒)) 약대(藥對)에 관한 연구(硏究))

  • Objectives : The purpose of this study is to find out herb-pair prototype composed of Pericarpium Zanthoxyli in "Donguibogam", to investigate origin of them. Methods : We researched the usages of Pericarpium Zanthoxyli compatibility as following procedure : 1. Choosing a related words such as Pericarpium Zanthoxyli(蜀椒, 川椒, 花椒) 2. Searching prescriptions: We searched prescriptions containing the related words in "Dongeuibogam" through "wook system", analyzed the selected prescription and found out herb-pair prototype composed of Pericarpium Zanthoxyli. Results : We found 12 fomulas in which Pericarpium Zanthoxyli herb pair was combinated and We made a Table 1. for the description of name of fomula, indication and herb configuration. Conclusions : Pericarpium Zanthoxyli was paired with below herbs; 檳榔(Arecae Semen), 細辛(Asari Radix), 苦楝根(Meliae Radix), 蔥白(Allii Fistulosi Bulbus), 茴香(Foeniculi Fructus), 巴豆(Crotonis Semen), 蓽撥(Piperis Longi Fructus), 露蜂房(Vespae Nidus), 肉桂(Cinnamomi Cortex), 艾葉(Artemisiae Argyi Folium), 細茶(Camellia sinensis), 雄黃(Realgar), 杏仁(Armeniacae Amarum Semen).

  • 유골골종(osteoid osteoma)은 특징적으로 직경 약 1cm미만의 원형 또는 타원형의 유골질제부(nidus)가 병소의 중심부에 있고, 그 주위는 반응성골질로 둘러싸여 있다. 그러므로 반응성골질 병소에서 제부의 확인은 유골골종의 진단에 매우 중요하다. 더우기 유골골종의 치료는 반드시 제부를 제거하여야 하기 때문에, 제부의 위치를 정확히 아는 것이 중요하다. 그러나, 유골골종의 제부는 단순 X-선 촬영상에서 잘 안나타나는 경우가 많아 진단에 어려움이 있다. 또한 골스캔도 유골골종의 진단에 많이 이용되고 있으나, 단순 신티그램에서 제부의 정확한 위치 파악은 어렵다. 이에 저자들은 바늘구멍조준기 골스캔을 시행하여 유골골종 제부에는 아주 진한 동위원소 집적이 보였고 주위의 반응성 골질 부위에는 제부보다 약한 동위원소 집적이 보여 특징적인 '이중 농도 집적' 소견을 얻었기에 바늘구멍조준기 골스캔이 유골골종의 진단에 매우 유용한 검사로 사료되어 보고하는 바이다.

  • The antrolith is a rare disease in the maxillary sinus. We experienced a 67-year-old female whose chief complaint was a intermittent dull pain of clinical, radiological and histological findings, we diagnosed it as antrolith in the left maxillary sinus and obtained results as follows: 1. Three radiopaque substances in the left maxillary sinus was revealed as 'Antroliths'. 2. These antroliths were movable in the left maxillary antrum and combined with maxillary sinusitis. 3. We supposed it internal origin of nidus in this case.

  • Survival rates of preterm infants have improved in the past few decades, and central venous catheters play an important role in the intensive medical treatment of these neonates. Unfortunately, these indwelling catheters increase the risk of intracardiac thrombosis, and they provide a nidus for microorganisms during the course of septicemia. Herein, we report a case of persistent bacteremia due to methicillin-resistant Staphylococcus aureus in an extremely low birth weight (ELBW) infant, along with vegetation observed on an echocardiogram, the findings which are compatible with a diagnosis of endocarditis. The endocarditis was successfully treated with antibiotic therapy, and the patient recovered without major complications. We suggest a surveillance echocardiogram for ELBW infants within a few days of birth, with regular follow-up studies when clinical signs of sepsis are observed.

  • We present reconstruction of a complicated scalp-dura defect using acellular human dermis and latissimus dorsi myocutaneous free flap. A 62-year-old female had previously undergone decompressive craniectomy for intracranial hemorrhage. The cranial bone flap was cryopreserved and restored to the original location subsequently, but necessitated removal for a methicillin-resistant Staphylococcal infection. However, the infectious nidus remained in a dermal substitute that was left over the cerebrum. Upon re-exploration, this material was removed, and frank pus was observed in the deep space just over the arachnoid layer. This was carefully irrigated, and the dural defect was closed with acellular dermal matrix in a watertight manner. The remaining scalp defect was covered using a free latissimus dorsi flap with anastomosis between the thoracodorsal and deep temporal arteries. The wound healed well without complications, and the scalp remained intact without any evidence of cerebrospinal fluid leak or continued infection.

  • A 43-year-old Korean woman with billary ascariasis accompanied by cholellthiasis is reported. Her chief complaints were abdominal pain, nausea, and vomiting. She had the past history of several attacks of abdominal pain in her childhood. ciliary stones were recovered from the left hepatic duct after cholecystectomy, which contained degenerated cuticle or body wall, and numerous eggs of Ascaris lumbriooides. It is strongly suggested that the ciliary stones were formed from the dead Ascmis worm(s). This is a rare case of biliary ascariasis during the recent 5 years in Korea.

Effects of Insect Crude Drugs on Blood Coagulation and Fibrinolysis System

  • The in vitro anticoagulant and fibrinolytic activities of crude extracts from insects were evaluated in order to find effective therapeutic drugs for the treatment of myocardial and cerebral thrombosis. We prepared three types of extracts (water, methanol and ethylacetate) from 28 insects for use as raw materials for the activity assays. The fibrinolytic activity was tested using the fibrin plate method and the activated partial thromboplastin time and thrombin time were measured for blood clotting activity. With regards to the fibrinolytic system, water extracts of six kinds of insects displayed a remarkable level of activity with a plasmin-like action. The water extracts of [Catharsius molossus, Eupolyphaga sinensis, Huechys sanguinea, Mantidis $o\ddot{o}theca$, Mimela splendens, and Polistes mandarinus (Vespae Nidus)] exhibited the activity. On the other hand, the methanol extracts did not display any fibrinolytic activity. In terms of the coagulation system, an aqueous extract of silkworm Tongchunghacho (Paecilomyces japonica), Oxya japonica japonica and Buthus martensi (Scorpion) increased the clotting time significantly longer (181 times) than the control. These results suggest that crude drugs from insects are useful sources for the development of new drugs for use in treatments involving blood coagulation and fibrinolysis.
