본 연구에서는 차아염소산나트륨 용액 및 고압증기멸균이 근관 치료용 Ni-Ti 파일의 주기적 피로 파절에 미치는 영향을 알아보고자 하였다. 파일 첨단 크기가 #30이고 .06 taper를 가지는 Hero $642^{(R)}$와 $K3^{(R)}$파일 70개를 10개씩 7개의 군으로 나누었으며, 6% 차아염소산나트륨 용액에 15분 (N15군), 30분 (N30군), 고압증기멸균은 $121\;^{\circ}C$의 온도, 0.12 MPa의 압력에서 20분간 시행하는 것을 일 회로 하여 5회 (H5군), 10회 (H10), 고압증기멸균 5회 후 차아염소산나트륨 용액 15분(H5N15), 고압증기멸균 10회 후 차아염소산나트륨 용액 30분 (H10N30) 처리 후 주기적 피로 파절 실험을 수행하여 파일이 파절될 때까지의 시간을 초 단위로 측정하였다. 대조군은 전처리를 하지 않고 주기적 피로 파절을 시행하였다. 95%의 신뢰도로 one-way analysis of varience를 이용하여 통계 처리하였으며, 다중 비교는 Tukey방법으로 분석하였다. 파일의 표면을 주사전자현미경으로 관찰하였다. 주기적 피로 파절에 소요된 시간은 Hero $642^{(R)}$는 각 군별로 유의할 만한 차이가 없었으며, $K3^{(R)}$는 N15, N30, H5N15군에서 대조군에 비해 짧은 시간이 소요된 것으로 나타났다 (p < 0.05). 주사전자 현미경에서 관찰했을 때 Hero $642^{(R)}$와 $K3^{(R)}$모두 N15, N30군에서 파일의 표면과 날이 부식이 되어 대조군에서 관찰된 일정 방향으로 배열된 정형화된 예리한 빗살무의가 부분적으로 파괴된 형태가 관찰되었으며, N15군에 비해 N30군에서 파일의 형태가 파괴된 양상이 뚜렷했으며, Hero $642^{(R)}$보다 $K3^{(R)}$가 심하게 부식된 양상이었다. H5군과 H10군에서는 $K3^{(R)}$ 파일은 날 부분이 무디어진 양상을 보였고, Hero $642^{(R)}$는 형태 파괴가 뚜렷하지 않았으나 H10군에서 표면의 빗살무늬가 약간 무디어진 양상을 보였다. H5N15군과 H10N30군에서는 그다지 뚜렷한 형태변화는 없었다. 본 연구 결과 $K3^{(R)}$파일을 사용 시에는 가능한 한 차아염소산나트륨에 적은 시간 노출시키는 것이 파일의 부식으로 인한 형태의 손상을 방지할 수 있고, 주기적 피로 파절에 걸리는 시간도 감소시킬 수 있을 것이다.
근관 치료시 많이 사용되고 있는 $K-flex^{(R)}$ S-S 파일 (Maillefer, USA)과 $Profile^{(R)}$ Ni-Ti 파일 (Maillefer, USA), $K-3^{(R)}$ Ni-Ti파일 (SybronEndo, USA)의 부식 정도를 보기 위하여 총 360개의 20번 file을 1) 멸균소독 방법 (고압가열멸균소독, E-O gas 소독) ,2) 근관세척 액 ($5,25\%$ 차아염소산나트륨, 생리식염수) ,3) 멸균소독 횟수 (1, 5, 10회 ) 에 따라 36개 실험군으로 나누었다. 각각의 파일을 각 군별로 처리한 뒤, 세 명의 검사자가 광학 현미경 하에서 25배의 배율로 검사하여 0; 부식 없음, 1; 경도의 부식, 2; 중등도의 부식, 3; 심한 부식으로 점수화 하였다. 심한 부식을 보이는 파일의 표면은 SEM으로 관찰하였고 Kruskal-Wallis test (p < 0.05)를 이용하여 통계 분석 처리하여 차아염소산나트륨 접촉 여부와 멸균소독 방법의 차이에 의한 부식 정도를 평가하였다. 그 결과 고압가열멸균소독을 10회 시행한 파일은 통계적으로 유의한 차이를 보여 부식 정도가 심하였고, 1회나 5회 고압가열멸균소독을 시행한 파일과 E-O 가스로 소독한 파일들은 부식의 정도가 경미하여 통계적으로 유의하지 않았다. 차아염소산나트륨 접촉 여부와 제조사 혹은 파일 재료에 따른 유의성 있는 차이는 없었다

Objectives: To compare the flexural cyclic fatigue resistance and the length of the fractured segments (FLs) of recently introduced M-Pro rotary files with that of RaCe rotary files in curved canals and to evaluate the fracture surface by scanning electron microscopy (SEM). Materials and Methods: Thirty-six endodontic files with the same tip size and taper (size 25, 0.06 taper) were used. The samples were classified into 2 groups (n = 18): the M-Pro group (M-Pro IMD) and the RaCe group (FKG). A custom-made simulated canal model was fabricated to evaluate the total number of cycles to failure and the FL. SEM was used to examine the fracture surfaces of the fragmented segments. The data were statistically analyzed and comparisons between the 2 groups for normally distributed numerical variables were carried out using the independent Student's t-test. A p value less than 0.05 was considered to indicate statistical significance. Results: The M-Pro group showed significantly higher resistance to flexural cyclic fatigue than the RaCe group (p < 0.05), but there was no significant difference in the FLs between the 2 groups (p ≥ 0.05). Conclusions: Thermal treatment of nickel-titanium instruments can improve the flexural cyclic fatigue resistance of rotary endodontic files, and the M-Pro rotary system seems to be a promising rotary endodontic file.

Objectives: This study was to evaluate the antibacterial effect of different instrumentation and irrigation techniques using confocal laser scanning microscopy (CLSM) after root canal inoculation with Enterococcus faecalis (E. faecalis). Materials and Methods: Mesiobuccal and mesiolingual canals of extracted mandibular molars were apically enlarged up to a size 25 hand K-file, then autoclaved and inoculated with E. faecalis. The samples were randomly divided into 4 main groups according to the system of instrumentation and irrigation: an XP-endo Shaper (XPS) combined with conventional irrigation (XPS/C) or an XP-endo Finisher (XPF) (XPS/XPF), and iRaCe combined with conventional irrigation (iRaCe/C) or combined with an XPF (iRaCe/XPF). A middle-third sample was taken from each group, and then the bacterial reduction was evaluated using CLSM at a depth of $50{\mu}m$ inside the dentinal tubules. The ratio of red fluorescence (dead cells) to green-and-red fluorescence (live and dead cells) represented the percentage of bacterial reduction. The data were then statistically analyzed using the Kruskal-Wallis test for comparisons across the groups and the Dunn test was used for pairwise comparisons. Results: The instrumentation and irrigation techniques had a significant effect on bacterial reduction (p < 0.05). The iRaCe/XPF group showed the strongest effect, followed by the XPS/XPF and XPS/C group, while the iRaCe/C group had the weakest effect. Conclusions: Combining iRaCe with XPF improved its bacterial reduction effect, while combining XPS with XPF did not yield a significant improvement in its ability to reduce bacteria at a depth of $50{\mu}m$ in the dentinal tubules.

Objectives: This study evaluated 2 nickel-titanium rotary systems and a complementary protocol with an ultrasonic tip and a small-diameter instrument in flattened root canals. Materials and Methods: Thirty-two human maxillary second premolars with flattened canals (buccolingual diameter ≥4 times larger than the mesiodistal diameter) at 9 mm from the radiographic apex were selected. The root canals were prepared by ProDesign Logic (PDL) 30/0.01 and 30/0.05 or Hyflex EDM (HEDM) 10/0.05 and 25/0.08 (n = 16), followed by application of the Flatsonic ultrasonic tip in the cervical and middle thirds and a PDL 25/0.03 file in the apical third (FPDL). The teeth were scanned using micro-computed tomography before and after the procedures. The percentage of volume increase, debris, and uninstrumented surface area were analyzed using the Kruskal-Wallis, Dunn, Wilcoxon, analysis of variance/Tukey, and paired and unpaired t-tests (α = 0.05). Results: No significant difference was found in the volume increase and uninstrumented surface area between PDL and HEDM (p > 0.05). PDL had a higher percentage of debris than HEDM in the middle and apical thirds (p < 0.05). The FPDL protocol resulted in less debris and uninstrumented surface area for PDL and HEDM (p < 0.05). This protocol, with HEDM, reduced debris in the middle and apical thirds and uninstrumented surface area in the apical third (p < 0.05). Conclusions: High percentages of debris and uninstrumented surface area were observed after preparation of flattened root canals. The HEDM, Flatsonic tip, and 25/0.03 instrument protocol enhanced cleaning in flattened root canals.

Objectives: This study evaluated the effect of repeated uses and autoclaving in the instrumented area, fracture resistance, and time of instrumentation of thermally treated nickel-titanium reciprocating systems. Materials and Methods: Two hundred simulated canals were instrumented using Reciproc Blue and WaveOne Gold. Each file was used up to 10 times or until fracture. The instrumented area was measured in pre- and post-operative images, using ImageJ software. Kaplan-Meier survival analysis evaluated the number of uses of instruments before fracture. Instrumented area and time of instrumentation were analyzed by Mann-Whitney U test and Kruskal-Wallis. Correlations among the number of uses and instrumented area were measured. The level of statistical significance was set at p < 0.05. Results: Reciproc Blue presented a higher estimated number of uses in comparison with WaveOne Gold (p = 0.026), but autoclaving did not affect the resistance to fracture of instruments (p > 0.05). The instrumented area was different among the evaluated groups (p = 0.039), and the instrumented area along the uses of both tested instruments was reduced. With the time of instrumentation, there was also a significant difference among the evaluated groups; the groups without sterilization cycles were faster, in comparison to those submitted to autoclaving (p = 0.010). Conclusions: Reciproc Blue was more resistant than WaveOne Gold, suffering later fracture. Additionally, the sterilization cycles did not influence the estimated number of uses of thermally treated reciprocating instruments, but the instrumented area of root canals was reduced along with the repeated uses of both instruments.

Objectives: This study aimed to evaluate and compare the efficacy of the S1 reciprocating system and the D-Race retreatment rotary system for filling material removal and the apical extrusion of debris. Materials and Methods: Sixty-four freshly extracted maxillary canines were shaped with size 10 and size 15 K-files, instrumented using ProTaper Gold under irrigation with 2.5% sodium hypochlorite (NaOCl), obturated according to the principle of thermo-mechanical condensation with gutta-percha and zinc oxide eugenol sealer, and allowed to set for 3 weeks at 37℃. Subsequently, the teeth were divided into a control group (n = 4), the D-Race rotary instrument group (n = 30), and the S1 reciprocating instrument group (n = 30). After classical retreatment, the canals were subjected to a complementary approach with the XP-Endo Shaper. Desocclusol was used as a solvent, and irrigation with 2.5% NaOCl was performed. Each group was divided into subgroups according to the timing of radiographic readings. The images were imported into a software program to measure the remaining filling material, the apical extrusion, and the root canal space. The data were statistically analyzed using the Z-test and JASP graphics software. Results: No significant differences were found between the D-Race and S1 groups for primary retreatment; however, using a complementary cleaning method increased the removal of remnant filling (p < 0.05). Conclusions: Classical removal of canal filling material may not be sufficient for root canal disinfection, although a complementary finishing approach improved the results. Nevertheless, all systems left some debris and caused apical extrusion.

Objectives: This study investigated the nanoleakage of root canal obturations using calcium silicate-based sealer according to different drying methods. Materials and Methods: Fifty-two extracted mandibular premolars with a single root canal and straight root were selected for this study. After canal preparation with a nickel-titanium rotary file system, the specimens were randomly divided into 4 groups according to canal drying methods (1: complete drying, 2: blot drying/distilled water, 3: blot drying/NaOCl, 4: aspiration only). The root canals were obturated using a single-cone filling technique with a calcium silicate-based sealer. Nanoleakage was evaluated using a nanoflow device after 24 hours, 1 week, and 1 month. Data were collected twice per second at the nanoscale and measured in nanoliters per second. Data were statistically analyzed using the Kruskal-Wallis and Mann-Whitney U-tests (p < 0.05). Results: The mean flow rate measured after 24 hours showed the highest value among the time periods in all groups. However, the difference in the flow rate between 1 week and 1 month was not significant. The mean flow rate of the complete drying group was the highest at all time points. After 1 month, the mean flow rate in the blot drying group and the aspiration group was not significantly different. Conclusions: Within the limitations of this study, the canal drying method had a significant effect on leakage and sealing ability in root canal obturations using a calcium silicate-based sealer. Thus, a proper drying procedure is critical in endodontic treatment.
최근, 다양한 경사도의 니켈-티타늄 근관기구와 더불어 다양한 경사도의 gutta-percha cone이 소개되었다. 효율적인 근관 충전을 위해서는 짧은 시간에 적은 수의 기구 및 재료를 사용하여 균일한 gutta-percha로 근관을 충전할 필요가 있다. 본 연구에서는 0.06 경사도의 니켈-티타늄 전동화일로 형성된 만곡 근관에서 측방 가압법으로 근관충전시 비표준화 medium 크기 master gutta-percha cone의 충전효율을 ISO 표준화 규격의 master cone과 비교하여 평가하고자 하였다. 60개의 모형레진 만곡근관에서 0.06 경사도의 니켈-티타늄 전동화일 ($ProTaper^{TM},\;ProFile^{(R)}$, Dentsply-Maillefer, Ballaigues, Switzerland)을 사용하여 근단부 근관이 30번 크기가 되게 crown-down법으로 근관을 형성하였다. 근관은 master gutta-percha cone, AH 26 실러 (Dentsply-DeTrey, Konstanz, Germany), 그리고 type A accessory cone (Dentsply-Maillefer, Ballaigues, Switzerland)을 사용하여 측방가압법으로 충전하였으며, 이 때 사용된 master gutta-percha cone(Diadent, Chongju, Korea)과 Finger spreader(B type, Dentsply-Maillefer, Ballaigues, Switzerland)에 따라 임의로 15개씩 4개의 군으로 나누어 실험하였는데, St/SS 군에서는 ISO 표준화 규격의 30번 master cone과 스테인레스 스틸 finger spreader를, St/NT 군에서는 ISO 표준화 규격의 30번 master cone과 니켈-티타늄 finger spreader를, Non-St/SS 군에서는 비표준화 medium 규격 master cone과 스테인레스 스틸 spreader를, 그리고 Non-St/NT 군에서는 비표준화 medium 크기 master cone과 니켈-티타늄 finger spreader를 각각 사용하였다. 충전된 근관은 $37^{\circ}C$, 상대습도 $100\%$하에서 24시간 보관한 후, 치근단 1, 3및 5 mm수준에서 횡절단하여 입체현미경 하에서 관찰하고 컴퓨터에 저장한 다음, $Auto^{(R)}$CAD 2000 프로그램을 이용하여, 형성된 근관 및 gutta-percha 충전물의 외형을 추적하여 근관내 gutta-percha 면적비를 계산하였다. Gutta-percha 면적비의 결과치는 two-way ANOVA를, 그리고 accessory cone 수는 one-way ANOVA 및 Duncan's multiple range test를 이용하여 통계 분석하여 다음과 같은 결과를 얻었다. 스테인레스 스틸 finger spreader를 사용한 경우 및 니켈-티타늄 finger spreader를 사용한 경우 공히, 모든 치근단 수준에서 비표준화 medium 크기 master cone 사용군이 ISO 표준화 규격의 master cone 사용군에 비해 유의하게 높은 gutta-percha 면적비를 나타내었다 (p < 0.01). 비표준화 medium크기 master cone 사용군에서는 표준화 규격의 master cone 사용군에 비해 유의하게 적은 수의 accessory cone이 사용되었다 (p < 0.01).
본 연구는 치근첨부위의 급한 만곡을 가진 근관에서 엔진구동형 니켈-타이타늄기구를 이용해 근관 성형시 근관형태 변화에 대한 비교 연구를 함으로써 어떤 기구가 근관의 본래 형태를 잘 유지하는가를 구명하기 위하여 시행되었다. $\sharp$15 finger spreader를 이용해 만곡된 근관의 주형을 형성하고 epoxy resin으로 모형 근관을 갖는 레진 블록 96개를 제작하였다. 모형 근관은 만곡의 반경이 1.5mm, 3.0mm, 4.0mm 및 6.0mm를 가지며, 근관의 만곡도는 90도로 제작하였다. 엔진구동형 니켈-타이탸늄 파일로 ProFile, ProTaper, Hero 642 그리고 $K^3$를 사용하였으며 크라운다운법을 이용하여 제조회사의 지시에 따라 300rpm으로 근관을 성형하였다 시술 전$\cdot$후 사진을 디지털 카메라를 이용하여 촬영한 후, 이미지분석기를 이용하여 근관의 변화량을 측정하였다. 근관의 만곡을 따라서 치근첨으로부터 1mm간격으로 계측점을 총 9부분을 설정하여 원래의 근관형태가 유지되는 정도를 측정하였다 각 계측점 에서 근관의 중심축의 변위량, 근관의 폭의 변화량을 측정하였다. 계측점에서 각 기구들간의 유의성은 Kruskal-Wallis 분석법을 이용하였고 기구간에 검정은 Mann-Whitney U-test분석법을 이용한 결과 급격히 만곡된 근관 성형시에 반경 1.5mm의 만곡에서는 엔진구동형 니켈-타이타늄 파일을 사용시 주의를 요하며, 반경 3.0mm 이상의 만곡에서는 다른 엔진구동형 니켈-타이타늄 파일보다는 ProFile의 사용이 권고된다.

① 서비스 이용은 당 사이트의 업무상 또는 기술상 특별한 지장이 없는 한 연중무휴, 1일 24시간 운영을
원칙으로 합니다. 단, 당 사이트는 시스템 정기점검, 증설 및 교체를 위해 당 사이트가 정한 날이나 시간에
서비스를 일시 중단할 수 있으며, 예정되어 있는 작업으로 인한 서비스 일시중단은 당 사이트 홈페이지를
통해 사전에 공지합니다.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
