Closure of radial forearm free flap donor-site defect with proportional local full-thickness skin graft: case series study of a new design

  • Objectives: The aim of this study was to describe a simple, convenient, and reliable new technique using local full-thickness skin graft (FTSG) for skin coverage of a donor-site defect of the radial forearm free flap (RFFF). Patients and Methods: Between April 2016 and April 2021, five patients with oral squamous cell carcinoma underwent mass resection combined with RFFF reconstruction. After RFFF harvesting, donor-site defects were restored by proportional local FTSG. Results: The donor-site defects ranged in size from 24 to 41.25 cm2, with a mean of 33.05 cm2. Good or acceptable esthetic outcomes were obtained in all five patients. There was no dehiscence, skin necrosis, wound infection, or severe scarring at the graft site through the end of the postoperative follow-up period, and no patient had any specific functional complaint. Conclusion: The proportional local FTSG showed promising results for skin coverage of the donor-site defect of the RFFF. This technique could decrease the need for skin grafts from other sites.

A new experimental protocol to quantitatively assess the motor control capability of low-back pain patinents during dynamic trunk movement

  • An experimental protocol was developed and tested in this study in order to quantify the motor control capability of the trunk movement for both healthy subjects and low-back pain (LBP) patients. Information processing capacity (bits/second) (Fitts, 1954) and dynamic motor perfor- mance such as flexion/extension velocity and acceleration were measured as motor control parameters under the controlled range of motion (ROM). In this study, the original experimental propocol (Kim et al., 1993, 1994) was re-designed to reduce the length of the test via a series of statistical analyses for clinical application. The accuracy of the shortened protocol was statistically examined and indicated no difference conpared to the original protocol in terms of evaluating information processing capacity. This protocol was also tested among ten healthy subjects and ten LBP patients for validation purpose. The results showed that the information processing capacity was not significantly diffenent between two groups due to the large variation although there was an apparent mean difference. Average movement time showed a significant increase in LBP patients com- pared to healthy subjects. In conclusion, it was found that the new short experimental protocol could quantify the motor control capability of neuromuscular system of the trunk and also showed the applicability to patient population.

Comparison of conventional and new endoscopic band ligation devices for colonic diverticular bleeding

  • Background/Aims: Endoscopic band ligation (EBL) is used to treat colonic diverticular bleeding (CDB). An endoscopic variceal ligation device for esophageal varices is used as a conventional EBL device (C-EBL). A new EBL device (N-EBL) was developed by Sumitomo Bakelite Co. in August 2018. We aimed to evaluate the clinical outcomes of N-EBL compared with those of C-EBL. Methods: Seventy-nine patients who underwent EBL for CDB at St. Luke's International Hospital, Japan, between 2017 and 2020 were included in this retrospective study. Patients were divided into the C-EBL and N-EBL groups. Their clinical outcomes, including achieving initial hemostasis, early rebleeding, procedure time, and EBL-associated adverse events, were evaluated. Results: Of the 79 patients, 36 (45.6%) were in the C-EBL group and 43 (54.4%) were in the N-EBL group. The rate of achieving initial hemostasis was 100% in the C-EBL group and 93.0% in the N-EBL group. No significant difference was noted in the early rebleeding rate between the groups (p=0.24). The N-EBL group achieved a shorter median EBL procedure time than the C-EBL group (14.2 minutes vs. 18.2 minutes, p=0.02). No adverse events were observed in either group. Conclusions: The N-EBL device is safe and useful and may reduce EBL procedure time.

A New Scale(NS) Score System to Predict Outcome of Intracranial Aneurysm Using TCD (TCD를 이용한 두개강내 동맥류의 예후 예측 가능한 New Scale(NS) Score System)

  • Objective : By conducing a review of clinical outcomes for patients with aneurysm treated using current microneurosurgical techniques and intensive care unit management, we speculated that grading systems based only on clinical condition or CT finding after admission failed to provide a significant stratification of outcome between individual grades of patients, because these systems did not include the factor for postoperative vasospasm. We hypothesized that postoperative blood flow velocity could have a significant impact on outcome prediction for patients surgically treated for intracranial aneurysms. Methods : We conducted a analysis on patient- and lesion-specific factors that might have been associated with outcome in a series of 55 aneurysm operations performed with measurements of blood-flow velocity with transcranial Doppler ultrasonography(TCD). In the new scale(NS) score system, 1 point is assigned additionally for the case with Hunt and Hess(H-H)/World Federation of Neurological Surgeons(WFNS) Grade IV or V, Fisher Scale(FS) score 3 or 4, aneurysm size greater than 10mm, patient age older than 60 years, blood-flow velocity higher than 120cm/sec, and posterior circulation lesion. By adding the total points, a 6-point scale score(score 0-6) is obtained. Results : Age of patient, size of aneurysm, clinical condition(H-H grade and WFNS), FS score, and blood flow velocity(TCD 1day after operation) were independently and strongly associated with long-term outcome. When NS scores were applied to 55 patients with at least 6 months follow-up, the correlation of individual scores with outcome was strongly validated the retrospective findings. Conclusion : It was speculated that TCD could be used to assess postoperative vasospasm and to monitor noninvasively the patients with aneurysmal SAH. This NS score system is easy to apply, divide patients into groups with different outcome, and is comprehensive, allowing for more accurate prediction of surgical outcome.

A clinical study of Aroma Acupuncture on chronic headache patients (만성두통환자에 대한 아로마침의 임상적 연구)

  • Objective : Because the cause and etiology of chronic headache is not yet fully explained, the treatment of this symptom is not simple. This study compares the effects of aroma acupuncture and normal acupuncture applied on chronic headache patients, in order to establish a primary data for further studies of new treatments and developments of new practical acupunctures. Methods : 38 clinical experiment participants were gathered and through a questionnaire patients who experienced headache for more than 4 hours a day and more than 15 days per month were qualified as Chronic Headache patients. The qualified patients were classified in to two groups, aroma acupuncture group(Aroma AT group, n=23) and normal acupuncture group(AT group, n=15). Treatment was applied 2 times a weeks for 8 weeks. The acupoints, GV20, HN23, ST8, HN46, TE17, GB20, LI20, LI11, LI14, ST36, and LR3 were stimulated for 20 minutes in both the groups. The effects of both groups were analyzed using VAS scores and BPI (Brief Pain Inventory). Results : The VAS pain scores improved significantly in both groups. The decrease rate was larger in the Aroma AT group(p<0.05). The BPI scores for the both groups also decreased. The improvement in the Aroma AT group was larger(p<0.05). Conclusion : Aroma acupuncture applied on chronic headache patients showed effects of relieving pain, improving general activities, changing the mood, and improving sleeping behaviors. These effects were analyzed using VAS scores and BPI. The Aroma AT group showed significant changes of VAS and BPI compared to the AT group, suggesting that this new practical acupuncture is very effective when treating chronic headaches. Further researches are needed to evaluate the distinct functional mechanism of aroma acupuncture, but this study is meaningful in the sense that it will act as a fundamental study to build on.

Body Impedance Measurements for edematous patients (부종환자에서 임피던스를 이용한 수분변화의 예측)

  • The assessment method of human body composion by bioelectrical impedance is very simple, safe, rapid and noninvasive. Based on prediction formulas for total body water from bioelectrical impedance, the observed weight loss should be associated with an increase in impedance. However in edematous patients for dialysis, the calculated total body water loss as calculated from impedance were overestimated and significantly higher than the weight loss after dialysis. So determination of impedance were made in 50 edematous patients before, during and after dialysis. Mean weight loss, which was assumed to be only loss of water was 1719$\pm$ 866 gr and mean impedance change was 71.0 $\pm$ 23.0 Ohm under 50kHz. Body weight loss was highly correlated [r>0.81 with the increase in body impedance under variable frequencies[1, 10, 20, 30, 40, 50 kHz . But there were no differences between frequences. In conclusion, clinical application of bioelectrical impedance method is useful for individual edematous patients with new correlation equation[Y=230+26.8X, X;Impedance change, Y;Calculated total body water loss .

Clinical Studies on Ribostamycin sulfate (외과영역에서의 감염증에 대한 Vistamycin 의 사용경험)

  • Ribostamycin sulfate is a new aminoglycoside antibiotic which is effective against organisms resistant to other conventional antibiotics, Particularly E. coli, and Staphylococcus group. Our clinical studies were performed on 20 patients who were admitted to Thoracic Surgical Dept of National Medical Center from May '77 to September '77, and the following results were obtained; 1. Among 20 patients, 13 were excellently improved and 4 responded moderataly, but 3 patients were not improved. So, 85% of the patients were controlled with Ribostamycin sulfate administration. 2. The rate of effectiveness of Ribostamycin sulfate was 85.7% against E. coli, 92.3% against Staphylococcus, and 75% against klebsiella, revealing 83.3% in average. 3. No side effects were observed. 4. From our clinical studies, Ribostamycin sulfate is expected to possess in high therapeutic value in surgical infectins caused by both gram positive and negative microorganisms.

The Life of Patients with a Heart Transplant (심장 이식 수혜자의 삶)

  • Purpose: The main purpose of this study was to develop a substantive theory on the life of patients with heart transplantation in the context of Korean society and culture. The question for the study was 'What is the life of patients like with a heart transplant?'. Method: A grounded theory method guided the data collection and analysis. Participants for this study were 12 adults who regularly visited a Cardiovascular ambulatorium in a medical center. The data was collected through an in-depth interview and analyses were performed simultaneously. Result: 'Developing a new life to live on borrowed time' was the core category in this study. It revealed two types of life, one is living in peace and another is thinking positive. Conclusion: This study provides a framework for the development of individualized nursing interventions to care for patients with Heart Transplantation. The findings may provide pointers for health professionals about ways to improve support for heart transplant recipients.

An Acoustic Analysis of Diadochokinesis in Patients with Parkinson's Disease (파킨슨병 환자 대상 조음교대운동의 음향적 분석)

  • The acoustic analysis of diadochokinesis(DDK) has been used to evaluate dysarthria. However, there has not been an automatic method to evaluate dysarthria. The aim of this study was to introduce a new automated program to measure DDK tasks and to apply this to clinical patients with idiopathic Parkinson's disease(IPD). Fourty-seven patients with IPD and a healthy control group of twenty participants were selected with every DDK task recorded three times. Twenty-five acoustic parameters in the program were developed. The relevant parameters were times of DDK, pitch related parameters, intensity parameters which were analyzed by 2-way ANOVA. Significant differences between the groups were found in the times of DDK, pitch related parameters, and intensity parameters. The findings indicated that the pitch of control group was more stable than that of the IPD. Even though the patients with IPD had a higher intensity value, this phenomenon was caused by the weakness of the IPD group who could not control their speech with a breath.

Treatment effects of Sympathicotomy and Clinical study by Oriental medicine On 10-20 years old Hyperhidrosis patients (10대 다한증 환자의 수술 치료후 만족도 조사와 한의학적(韓醫學的) 임상고찰(臨床考察))

  • Back ground: The purpose of this study was to examine the patient's satisfaction of Sympathicotomy and clinical observation by Oriental medicine. Methods: We studied 93 hyperhidrosis patients after Sympathicotomy. They were treated on East-West Hyperhidrosis Clinic, Kyung Hee University Medical Center, from October 1999 to February 2002. Resurt and Conclusion: The most patients were satisfied with treatment effects of Sympathicotomy and there were few side effects. The only complain was compensatory sweating. Sympathicotomy can be recommended as a useful treatment for hyperhidrosis. Clinical study by Oriental medicine on hyperhidrosis patients was showed that hyperhidrosis could be observed at Taeeumin, Soyangin and Soeumin. We can find the new treatment of compensatory sweating, by Sasang Constitutional Medicine and Oriental diagnosis.
