Purpose: This study aims to investigate the clinical characteristics of children diagnosed with the novel influenza A (H1N1) in the winter of 2009 at a single medical institution. Methods: Out of 545 confirmed cases of influenza A (H1N1) in children, using the real time RT-PCR method at Kosin University Gospel Hospital from September to December of 2009, 149 patients and their medical records were reviewed in terms of symptoms, laboratory findings, complications and transmission within a family. Results: Median age of subjects was 7 years (range: 2 months-18 years). New cases increased rapidly from September to reach a peak in November, then declined rapidly. Most frequently observed symptoms were fever (96.7%), cough (73.2%), rhinorrhea (36.9%) and sore throat (31.5%). Average body temperatures on the 1st, 2nd and 3rd hospital day were $38.75{\pm}0.65^{\circ}C$, $38.08{\pm}0.87^{\circ}C$ and $37.51{\pm}0.76^{\circ}C$, respectively. Complete blood counts and biochemical tests performed on the first admission day showed within the reference values in most cases. Of the 82 patients with simple chest radiography, 18 (22%) had pneumonic lesions; multi-focal bronchopneumonia in eleven, single or multi-segmental lobar pneumonia in five, and diffuse interstitial pneumonia in two patients. All of the 149 patients improved from their symptoms and discharged within 9 days of admission without any late complication. Conclusion: Children with 2009 pandemic influenza A (H1N1) at our single institution displayed nonspecific symptoms and laboratory findings, resembling those of common viral respiratory illnesses, and did not appear to develop more severe disease.
본 연구는 수도권 소재 대학에 재학 중인 일부 보건계열 대학생들을 대상으로 신종인플루엔자에 대한 지식정도와 자기 효능감, 감염의 심각성, 행동의도를 조사해 보고, 성별에 따른 차이를 조사한 결과 다음과 같은 결론을 얻었다. 1. 조사대상자의 인구사회학적 특성으로는 여학생이 57.0%이었으며, 전공은 치위생과 학생이 56.8%로 가장 많았으며, 신종인플루엔자에 대한 교육경험은 78.1%가 받아본 적이 없다고 응답하였다. 2. 신종인플루엔자에 대한 지식은 '신종인플루엔자는 사람과 사람 사이에서 전염될 수 있다(94.6%)', '젊고 건강한 사람도 아이나 노인과 같이 신종인플루엔자에 감염될 수 있다(93.4%)', '열이 37.8도가 넘으면 신종인플루엔자 감염이 의심된다(93.2%),' '신종인플루엔자는 주로 감염된 사람의 기침이나 재채기를 통해 감염된다(91.3%)' 등의 항목에서 높은 정답률을 보였고, '신종인플루엔자는 조류인플루엔자와 동일한 것이다(16.3%)', '돼지고기를 잘못 먹으면 신종인플루엔자에 감염될 수 있다(20.5%)', '보통 독감백신주사를 맞으면 신종인플루엔자를 예방할 수 있다(25.6%)' 등의 항목은 낮은 정답률을 보였다. 신종인플루엔자에 대한 지식을 점수화하면 평균은 7.8점인 것으로 나타났다. 3. 성별에 따른 신종인플루엔자에 대한 자기효능감은 남학생이 여학생 보다 높게 나타나 유의미한 차이가 있었다.(P < 0.05). 4. 성별에 따른 신종인플루엔자에 대한 감염의 심각성은 여학생이 남학생보다 높게 나타나 유의미한 차이가 있었다(P < 0.01). 5. 성별에 따른 신종인플루엔자에 대한 행동의도는 '아는 사람이 신종인플루엔자에 걸리면 만나는 것을 고려해 볼 것이다'가 남학생 3.79점, 여학생 3.98점으로 여학생이 높게 나타나 유의미한 차이가 나타났고(P < 0.05), '신종인플루엔자가 발생한 지역은 여행이 망설여진다'는 남학생 3.93점, 여학생 4.07점으로 여학생의 행동의도가 남학생보다 높게 나타났다(P > 0.05).

Background: The live bird market (LBM) plays an important role in the dynamic evolution of the avian influenza H5N1 virus. Objectives: The main objective of this study was to monitor the genetic diversity of the H5N1 viruses in LBMs in Indonesia. Methods: Therefore, the disease surveillance was conducted in the area of Banten, West Java, Central Java, East Java, and Jakarta Province, Indonesia from 2014 to 2019. Subsequently, the genetic characterization of the H5N1 viruses was performed by sequencing all 8 segments of the viral genome. Results: As a result, the H5N1 viruses were detected in most of LBMs in both bird' cloacal and environmental samples, in which about 35% of all samples were positive for influenza A and, subsequently, about 52% of these samples were positive for H5 subtyping. Based on the genetic analyses of 14 viruses isolated from LBMs, genetic diversities of the H5N1 viruses were identified including clades 2.1.3 and 2.3.2 as typical predominant groups as well as reassortant viruses between these 2 clades. Conclusions: As a consequence, zoonotic transmission to humans in the market could be occurred from the exposure of infected birds and/or contaminated environments. Moreover, new virus variants could emerge from the LBM environment. Therefore, improving pandemic preparedness raised great concerns related to the zoonotic aspect of new influenza variants because of its high adaptivity and efficiency for human infection.
Purpose: The purpose of this study was to explore the intentions and influencing factors of intentions to care for New Influenza A ($H_1N_1$) patient Methods: This study involved a descriptive design using self-administered questionnaire. Intentions to care for H1N1 patient was evaluated by prediction tool, based on the Theory of Planned Behavior (TPB). The data were analyzed by SPSS 17.0 using descriptive statistics, t-test, ANOVA with a Scheffe test, Pearson's correlation and multiple regression analysis. Results: The level of intention was high, attitude was negative, subjective norm was high, and perceived behavioral control was moderate. The general regression model with intention as a dependent variable was statistically significant (F=39.31, p<.001). 28.1% of variance in intention was explained by subjective norm (t=8.75, p<.001), and perceived behavioral control (t=4.28, p<.001). Among the predictors, subjective norm had the greatest effect on intention (${\beta}=.44$). The nurse with the higher subjective norm and more positive perceived behavioral control reported the higher intention. Conclusion: The findings of this study suggested that the various aspects of nurse's characteristics should be considered when establishing strategies to improve the nurse's intention for care of infectious disease.
New epidemics of infectious diseases often involve health care workers. In this short communication we present a case report of a health care professional who became the first case of influenza H1N1 virus to be notified in the United Arab Emirates. There are several issues related to workplace considerations and general public health, including preventive measures, the need for isolation of the patient, dealing with contacts, return to work, and communication with the workforce.
2006년 10월부터 2008 년 6월까지 총 인플루엔자 의사 환자 1,822건의 인후도찰물 및 비인후도찰물에서 277건의 인플루엔자바이러스를 분리했다. 절기별로는 2006~2007 절기의 1,154검체 중 52건(4.5%), 2007~2008절기의 668검체 중 210건(31.4%)에서 인플루엔자바이러스를 분리하였다. 인플루엔자바이러스 A/H1N1의 HA 유전자의 경우, 2008~2009 절기의 백신주인 A/Brisbane/59/2007과는 96.7%~97.7%, A/Solomon Islands/3/2006 96.5%~97.3%, A/New Caledonia/20/99와는 95.6%~96.6%의 유사성을 나타냈으며, NA 유전자의 경우, A/Brisbane/59/2007과는 97.8%~98.5%, A/Solomon Islands/3/2006과는 96.7%~97.6%, A/New Caledonia/20/99와는 96.8%~97.7%의 유사성을 보여 2008~2009절기의 백신주인 A/Brisbane/59/07과 가장 유사성이 컸다. 인플루엔자바이러스 A/H3N2의 분리주 중 1주를 제외한 모든 분리주가 HA 유전자에서 2008~2009 절기 백신주인 A/Brisbane/10/2007과는 98.4%~99.7%의 유사성을 보였고, A/Wisconsin/67/2005와는 96.5%~97.5%의 유사성을 보였으며, NA 유전자에서는 A/Brisbane/10/2007과는 98.9%~99.4%, A/Wisconsin/67/2005와는 98.0%~98.6%, A/California/7/2004와는 98.3%~98.9%의 유사성을 보였다. 인플루엔자바이러스 B의 HA 유전자의 경우는 2주를 제외하고는 2008~2009 절기의 백신주인 B/Florida/4/2006과는 96.5%~99.7%의 유사성을 보였으며, B/Malaysia/2506/2004와는 86.7%~87.7%의 유사성을 보여 B/Florida/4/2006과의 유사성이 크게 나타났다. NA 유전자의 경우는 reassortant분리주가 96.7%와 97.3%의 유사성을 나타내는 것을 제외하고는 B/Florida/4/2006에 98.9%~100%의 유사성을 나타냈으며, 분리주 유행시기의 백신주인 B/Malaysia/2506/2004와는 94.5%~96.7%의 유사성을 나타내어 2008~2009 절기의 백신주와 더 큰 유사성을 보였다. HA 유전자에서는 conserverd receptor binding site는 아미노산의 치환 없이 모든 분리주에서 잘 보존되어 있었으며, N-linked glycosylation site도 인플루엔자바이러스 A/H1 1주, A/H3 1주를 제외하고는 모두 같은 수의 N-linked glycosylation sites를 가졌으며, 인플루엔자바이러스 B의 경우는 2008~2009 절기의 백신주보다 1개가 많은 4개의 N-linked glycosylation sites를 가지고 있었다. Antigenic sites의 경우는 인플루엔자바이러스 A/H1의 Sb의 3개의 아미노산에서 백신주들과 다른 아미노산을 가지고 있으며, A/H3에서는 A, B, E 부위에서 는 아미노산의 변화가 나타났고, C, D 부위에서는 변화가 없었다. 인플루엔자바이러스 B의 4개의 분리주에서는 150 loop와 160 loop에서 B/Florida/4/2006과 비교하여 1개의 아미노산에서 치환이 나타났으며, 190 helix에서 모든 분리주가 B/Florida/4/2006과 비교하여 1개의 아미노산에서 치환이 나타났다.

Purpose: There was a global increase in the prevalence of oseltamivir-resistant influenza viruses during the 2007-2008 influenza season. This study was conducted to investigate the occurrence and characteristics of oseltamivir-resistant influenza viruses during the 2007-2008 and 2008-2009 influenza seasons among patients who were treated with oseltamivir (group A) and those that did not receive oseltamivir (group B). Methods: A prospective study was conducted on 321 pediatric patients who were hospitalized because of influenza during the 2007-2008 and 2008-2009 influenza seasons. Drug resistance tests were conducted on influenza viruses isolated from 91 patients. Results: There was no significant difference between the clinical characteristics of groups A and B during both seasons. Influenza A/H1N1, isolated from both groups A and B during the 2007-2008 and 2008-2009 periods, was not resistant to zanamivir. However, phenotypic analysis of the virus revealed a high oseltamivir $IC_{50}$ range and that H275Y substitution of the neuraminidase (NA) gene and partial variation of the hemagglutinin (HA) gene did not affect its antigenicity to the HA vaccine even though group A had a shorter hospitalization duration and fewer lower respiratory tract complications than group B. In addition, there was no significant difference in the clinical manifestations between oseltamivir-susceptible and oseltamivir-resistant strains of influenza A/H1N1. Conclusion: Establishment of guidelines to efficiently treat influenza with oseltamivir, a commonly used drug for treating influenza in Korean pediatric patients, and a treatment strategy with a new therapeutic agent is required.
Avian-origin H3N2 canine influenza A viruses (CIVs) have become enzootic in China and Korea and have sporadically transmitted to North America, causing multiple epidemics. We isolated six CIVs in Korea from CIV-infected patients during 2014-2017 and conducted whole genome sequencing and phylogenetic analyses. Results revealed that CIVs have circulated and evolved in Korea since the early 2000s and then diversified into a new clade, probably contributing to multiple epidemics in China, the USA, and Canada. Our findings bridge an evolutionary gap for understanding the global transmission of CIVs, emphasizing the significance of continuous monitoring of CIVs.
Cause of high lethality and dissemination to human being, new development of rapid method for the detection of highly pathogenic Avian Influenza Virus (AIV) is still necessary. For the detection of AIV subtype H5N1, typical pathogenic AIV, new method to confirm sub-typing of this virus is also needed. For the purpose of ultra-rapid detection and sub-typing of hemagglutinin and neuraminidase of AIV, this study was planned. As the results we could demonstrate an ultra-rapid multiplex real-time PCR (URMRT PCR) for the detection of AIV In this study, the URMRT PCR were optimized with synthesized AIV H5- and AIV Nl-specific DNA templates and GenSpector TMC, which is a semiconductor process technology based real-time PCR system with high frequencies of temperature monitoring. Under eight minutes, the amplifications of two AIV subtype-specific PCR products were successfully and independently detected by 30 cycled ultra-rapid PCR, including melting point analysis, from $1{\times}10^3$ copies of mixed template DNA. The URMRT PCR for the detection of AIV H5N 1 developed in this study could be expected to apply not only detections of different AIVs, but also various pathogens. It was also discussed that this kind of the fastest PCR based detection method could be improved by advance of related technology in near future.
It's a threat for the public health that H1N1 (Influenza virus A) causes disease and transmits among humans. WHO (world health organization) declared that the infections caused by the new strain had reached pandemic proportions. The approved neuraminidase inhibitors (Zanamivir and Oseltamivir) and related investigative drug (BCX-1812) are potent, specific inhibitors of influenza A and B viruses. These drugs are highly effective to prevent influenza A and B infections. Early therapeutic use reduces illness duration and respiratory complications. Recently, we found one of the potent inhibitor of erystagallin A ($IC_{50}$ of 2.04 ${\mu}M$) for neuraminidase target, this inhibitor shows most similar structure to its natural substrate, sialic acid. Therefore, we chose 1l7f to get the receptor structure for docking study among many crystal structures. A docking study has been performed in Surflex-Dock module in SYBYL 8.1. In the present study, we attempt to compare the docking studies of pterocarpin and erystagallin A with neuraminidase receptor structure. In the previous report, the methoxy group of pterocarpin had H-bonding with Arg residues. The present docking results for erystagallin A showed the backbone of hydroxyl group shows significant H-bonding interactions with Arg152 and Arg292. The results showed that erystagallin A interacts more favorably with distinctive binding site rather than original active site. Therefore, we tried to reveal plausible binding mode and important amino acid for this inhibitor using docking and site id search calculations of Sybyl. The results obtained from this work may be utilized to design novel inhibitors for neuraminidase.
