소아심장외과 중환자실 장관영양 프로토콜 개발 및 효과평가 (Development and Implementation of a Feeding Protocol for Infants in a Pediatric Cardiac Intensive Care Unit)

  • Purpose: It is crucial to provide adequate enteral nutrition for postoperative recovery, wound healing and normal growth in infants in pediatric cardiac ICUs. This study was done to develop a feeding protocol using the vaso-active inotropic (VAI) score and to evaluate the impact of nutritional outcomes following the new feeding protocol for infants who underwent cardiac surgery. Methods: This study consisted of three phases. First, a feeding protocol was developed based on a literature review. Second, ten experts rated the content validity. Third, a comparison study was conducted to evaluate the impact of the new feeding protocol. Data were analyzed using SPSS Version 20. Results: Twenty-nine infants were enrolled in the pre-protocol group, and 22 infants in the post-protocol group. Patients in the 2 groups were similar. Time to reach feeding goal was significantly decreased from 56.0 (27-210) hours to 28.5 (10-496) hours in the post-protocol group (Z=-4.22, p<.001). Level of enteral feeding knowledge among nurses increased significantly after implementation of the protocol. Conclusion: The feeding protocol using VAI score facilitates the achievement feeding goal to decrease feeding interruptions and help nurses in their practice. Larger studies are necessary to examine clinical outcomes following the implementation of this feeding protocol.

성인간호학 학습목표 개발을 위한 간호실무 조사연구 (A Survey of the Scope of Nursing Competency for Developing Learning Objectives In Adult Health Nursing)

  • Nurses in today's challenging health care settings need to be skilled critical thinkers and clinical experts. The nurse must be able to use a broad knowledge base to mobilize resources, coordinate actions and evaluate outcomes in complex new situations. So the national licensing examination for registered nurses is change to improve the quality of professional competency of nurses in Korea. Prior to this, learning objectives should be developed and improved periodically. The purpose of this study is to describe the nursing competency to provide base line data for developing learning objectives in adult health nursing. This study was conducted by means of a questionnaire which was developed by the researcher after reviewing the literature. The questionnaire was based on learning objectives which were developed by a nation-wide nursing faculty majoring in adult health nursing. The subjects were 45 nurses in a middle level hospital. The collected data were treated using SPSS Win 7.5 Statistical Package so as to obtain such descriptive statistics as mean score, frequency, and to test reliability test, nonpar-Friedman test. To summarize the major findings in this study, it showed the scope of nursing competency and can guide the direction of study and methodological criteria to develop learning objectives. Recommendations for further research are: firstly, it is necessary to state learning objectives with learners' behavioral terminology; secondly, to overcome locality in scope of this study, there is a need to analyze with nation-wide sampling by an in-depth statistical analysis; thirdly, because the subjects of this study are mostly three-year graduate nurses, there is a need to compare this study with other studies of different subjects.

활동기준원가계산(ABC)을 이용한 간호활동 분석 및 간호서비스 원가분석;일 산부인과 간호단위를 중심으로 (Analysis of Nursing Activities and Cost of Nursing Service Based on the ABC System)

  • The purpose of this study is to examine a possibility of applying the ABC system to analyze the cost of nursing service on one obstetrics and gynecology (OB/GYN) unit in a major medical center. The activities of nurses on an OB/GYN unit were analyzed for 4 days for 96 hours. 2 week days and 2 days on weekend. Total cases included in the study were activities of 12 charge nurses and 12 staff nurses. Activities were categorized into direct and indirect activities in order to calculate the cost of activities. Nursing activities were defined multidimesionally in order to utilize the ABC system. The 60 nursing activities were classified into the direct nursing activities and the indirect nursing activities based on a literature review, interview, and survey results. The direct activities were further categorized into the single nursing activities and the multiple nursing activities. The indirect activities were classified into the indirect nursing service activities and the general management activities. The major findings of this study were as follows : 1. There were differences in activities according to the positions and duties of the nurses. The charge nurses mostly performed the indirect nursing service activities and the general management activities. Almost all of their indirect nursing activities spent on each patient were similar. The staff nurses performed the single nursing activities and the multiple nursing activities. 2. The activities of staff nurses included medication, patient assessment, rounding and organizing the unit. patient education, nursing treatment, admission and discharging of patients. There was no differences in types and amount of time spent between the nurses on day-time duty and evening-time duty, but the patient assessment activity increased during the night-time. 3. The cost of post partum nursing services for women who had a normal vaginal delivery using the ABC system included the cost incurred by direct nursing activities provided by the staff nurses (85.9%), indirect nursing activities incurred by the staff nurses(19.0%) and the nursing activities by the charge nurses and the head nurse(14.1%). The ABC system is a relatively new method of cost analysis. The results of this study can provide the nursing and hospital managers with useful information on cost control. It is suggested that more studies should be done using the ABC system and extend the scope of studies to include value analysis to aid the Activity-Based Management(ABM) and/or the reengineering of hospital process.

변화의 과정에 있는 방문간호사의 직무분석: FGI와 DACUM을 적용하여 (Job Analysis of Visiting Nurses in the Process of Change Using FGI and DACUM)

  • Purpose: This study conducted a job analysis of visiting nurses in the process of change. Methods: Participants were the visiting nurses working for the Seoul Metropolitan city. On the basis of the Public Health Intervention Wheel model, two times of the focus group interview (FGI) with seven visiting nurses and one time of the Developing a Curriculum (DACUM) with 34 visiting nurses were performed. A questionnaire survey of 380 visiting nurses was conducted to examine the frequency, importance and difficulty levels of the tasks created by using the FGI and DACUM. Results: Visiting nurses' job was derived as the theme of present versus transitional roles. The present role was categorized as 'providing individual- and group-focused services' and 'conducting organization management', while the transitional role was categorized as 'providing district-focused services' and 'responding to new health issues'. The job generated 13 duties, 28 tasks, and 73task elements. The tasks showed the levels of frequency (3.65 scores), importance (4.27 scores), and difficulty (3.81 scores). All the tasks were determined as important, exceeding the average 4.00 scores. The group- and district-focused services of the tasks were recognized as more difficult but less frequent tasks. Conclusion: The visiting nurses exert both present and transitional roles. The transitional roles identified in the present study should be recognized as an extended role of visiting nurses in accordance with the current changing healthcare needs in South Korea. Finally, the educational curriculum for visiting nurses that reflects the transitional roles from the present study is needed.

Vancomycin Therapeutic Drug Monitoring 운영 실태 조사와 업무 개선 방안 (An Improvement Plan with Assessment of Therapeutic Drug Monitoring Service for Vancomycin)

  • The objective of this study was to analyze and to improve therapeutic drug monitoring(TDM) service of vancomycin in a local hospital. Patients with TDM service between September 2005 and December 2008 were included and the data were collected for vancomycin use and components of TDM. During that period, 421 cases of TDM service of vancomycin in 236 patients were retrospectively reviewed. The first dosages of vancomycin were appropriate in 135(57.2%) patients and administration of vancomycin was discontinued in 126(53.4%) patients due to therapeutic failure or adverse drug reaction. MRSA was identified in 191(80.9%) patients and 135(70.7%) samples for the identification were sputum. According to the TDM reports, 232(55.1%) serum samples were obtained at the steady-state conditions and 55.5% of the samples that were drawn before the steady-state was due to the physician's inappropriate knowledge about the steady-state. Based on the time of vancomycin administration, 35.8% of the samples were not obtained at the recommended sampling time. For the patients in general wards, the most common reason for the incorrect samples was routine serum sampling by the laboratory medicine phlebotomists between 6 and 8 a.m. except sunday. In contrast, samples drawn by nurses or physicians at inappropriate time were the most common reason for the incorrect samples with patients in the intensive care units. Physicians accepted 68.5% of the recommendations for vancomycin dosage and administration. In conclusion, TDM service of vancomycin needs to be improved in inappropriate sampling time and vancomycin dosage. For solving these problems, current team made of TDM pharmacists and physicians of laboratory medicine can be expanded to include a physician of infectious diseases, nurses and laboratory medicine phlebotomists as new members. Through the TDM service of vancomycin by the new team, we can settle the problems and make the guideline for the scientific controversies associated with therapeutic monitoring of vancomycin.

임상연구환자를 돌보는 병동간호사와 임상연구간호사의 역할기대와 협력 인식 (Role Expectations and Cooperation of Staff Nurses and Clinical Research Nurses who take care of Clinical Research Patientser)

  • 입원을 경험하는 임상연구 환자가 증가하고, 이에 따라 이들을 돌보는 간호사들의 업무가 증가하고 있으나, 모호한 역할 경계로 혼란이 발생하고 있다. 본 연구는 임상연구업무 중 병동간호사의 참여가 가능한 직접실무 제공업무, 교육 및 상담업무, 옹호 및 조정업무에 대한 역할기대와 병동에서의 협력인식 및 협력 장애요인을 파악하기 위한 조사연구이다. 대상자는 병동 간호사 66명, 임상연구간호사 65명이었다. 자료수집은 SAS 9.3통계 프로그램을 이용하여 기술 통계, two-sample t-test, 𝑥2 test 로 분석하였다. 연구결과, 임상연구 간호업무의 직접실무 제공영역, 교육 및 상담영역, 옹호 및 조정영역을 포함한 역할기대 점수는 병동간호사는 4.56±0.41점, 임상연구간호사는 3.83±0.60점으로 서로에 대한 역할기대 점수가 높게 나타났다. 통계적으로 병동간호사의 역할기대가 유의하게 (p<.001) 임상연구간호사보다 더 높게 나타났다. 두 군 간의 협력 인식은 5개 항목에서 유의한 차이가 나타났다. 협력장애요인으로는 업무과중과 임상연구에 대한 인식부재가 높은 순위로 나타났다. 높은 역할기대는 역할 갈등의 요소가 된다. 그러므로, 새로운 임상연구 간호업무에서의 병동간호사와 임상연구간호사의 역할 정립을 위하여 절차 마련과 효과적인 실무 관행의 정착, 공식적 시스템 운영이 필요할 것으로 사료된다. 두 군 간의 협력을 증진하기 위해서는 서로를 동반자로 생각하고 상대에 대한 전문성을 인정할 수 있는 상호 신뢰하고 협력하는 문화가 형성되어야 할 것으로 사료된다.

만성질환 관리를 위한 PDA폰 기반 이동형 방문건강관리 시스템 개발 (A Study on a PDA-Based Ubiquitous Healthcare System for the Management of Chronic Diseases)

  • Purpose: The rapid change to an aging society generates an increase in the incidence of chronic diseases. Many chronic patients have been facing their illnesses without enough preparation. In order to solve these problems, we designed and tested a public healthcare service based on ubiquitous technology. Method: Telemedicine has emerged as new medical care system of chronic disease. However, public potential of its technology is difficult to know under current traditional health care system. In this work, we developed a Personal Digital Access (PDA) phone based healthcare system by trained visiting nurses for elderly patients. A field test was performed by SeongBuk Public Health Center in Seoul, Republic of Korea (ROK). Result: Surveys were generated to assess the effects of this system compared to conventional public health system. Findings of trials demonstrate that healthcare coordination enhanced by PDA phone technology is satisfactory to the patients and nurses compared to previous one because prompt responses arouse their recognition of health. Conclusion: Ubiquitous healthcare system applied to public health service proved to be efficient and time-saving to monitor and control the chronic illnesses of large population.

한국간호교육 100년 발달사적 연구(1900-2000) (History of Korean Nursing Education (1900-2000))

  • Only one hundred years of history in nursing education in Korea is not enough to develop the subject fully as a science. However, the development of Korean nursing education is a great historical event, because Korean nursing education has been accepted by a male-oriented Korean society and has led to a new paradigm in the new millenium. These results are largely due to the Korean people's high enthusiasm for education, as well as Korean women's diligence. I think these 100 years of history can be divided into five periods: (1) the Sunlight period (1900-1911) (2) the New born period (1912-1945) (3) the Settle-down period (1946-1960) (4) the Marked Growth period (1961-1980) (5) the Jumping Period (1981-2000) These classifications are characterized by changes in the educational system, a changed nursing curriculum, educational goals, educational outcomes, and implications. The characteristics of historical development of Korean nursing education was evaluated in three dimensions: structures, contents, and outcome. The structure of Korean nursing education consists of a 3-year program and a 4-year program. Most nursing leaders in Korea hope that these two programs will converge into one system. Secondly, the contents of nursing education in Korea underwent very active changes, according to historical development. These changes in the nursing curriculum have been developed to provide a professional education and develop nursing education as a modern science. Lastly, as to the results of nursing education in Korea, the education was needed to turn out advanced specialists in nursing. In the new millenium, knowledge and information will be the driving forces behind social development. A nation's level of development and creativity in nursing education is the most potent determinant of the future of Korean nursing. The best way to prepare for future challenges will be to create the backbone of a nursing education system. Hence, well-educated nurses in graduate programs should be turned into advanced specialists in nursing. These groups will upgrade the image of Korean nurses, and will have strong influence to improve patient care and the health situation in Korea.

간호사의 심리적 계약이 발언행동에 미치는 영향 (The effect of psychological Contract of Nurse on Voice Behavior)

  • 본 연구는 간호사의 심리적 계약이 발언행동에 미치는 영향을 확인하여 조직의 인적자원관리와 효율적 병원 경영을 위한 기초자료를 제공하고자 하였다. 연구 참여에 동의한 간호사를 대상으로 실시한 서술적 조사연구이다. 연구결과 심리적 계약의 조직의무 이행 인식정도에서 거래적 계약과 관계적 계약 모두 낮게 나타났으며 발언행동은 지지적 발언, 건설적 발언, 저항적 발언, 부정적 발언 순으로 나타났다. 심리적 계약과 발언 행동의 하위 영역 관계를 보면 거래적 계약과 건설적 발언은 양의 상관관계가 있었으며(p<.05), 관계적 계약과 부정적 발언은 음의 상관관계가 있었다(p<.05). 따라서 의료기관은 새로운 정책, 조직 변화를 수행하고자 할 때 간호사의 역할 변화에 따른 내적 외적 보상이 이루어질 수 있는 시스템을 구축하는 것이 필요하다. 또한 간호사가 정책이나 조직변화에 적극 참여할 수 있도록 조직문화를 개선시키는 것이 필요하다.

상급종합병원 간호사의 그릿과 간호업무환경이 간호업무성과와 재직의도에 미치는 영향: 온정적 합리주의 리더십의 매개효과 (The Effects of Grit and Nursing Work Environment of Nurses in Tertiary Hospitals on Nursing Performance and Retention Intention: Mediating Effect of Compassionate Rationalism Leadership)

  • Purpose: To identify the effect of a new leadership type by confirming the effect of clinical nurses' grit and nursing work environment on nursing performance and retention intention with the mediating effect of compassionate rationalism leadership in the process. Methods: A proportional stratified sampling method was used with 45 tertiary general hospitals nationwide as the sampling unit. An online survey targeting nurses who have been working continuously for more than one year was conducted from October 14 to November 9, 2022. The final analysis included 1,256 questionnaires. The correlation between the participants' variables was analyzed using Pearson's correlation coefficients. The mediating effect was confirmed using a regression analysis, and bootstrapping. Results: The compassionate rationalism leadership had a mediating effect on the relationship between grit and nursing performance (F=203.21, p<.001), nursing work environment and nursing performance (F=109.48, p<.001), grit and retention intention (F=149.90, p<.001), and nursing work environment and retention intention (F=25.14, p<.001). Conclusion: The compassionate rationalism leadership of nursing managers had a positive effect on nursing performance and retention intention. Therefore, with the development and application of educational programs of compassionate rationalism leadership, it can be used to improve nursing performance and operate an efficient nursing organization.
