Effect of Ankle Position on Hallux Flexion Force and Muscles Activity of Abductor Hallucis (발목자세가 엄지발가락 굽힘 힘과 엄지벌림근의 근활성도에 미치는 효과)

  • Purpose : There is no validated method for measuring the toe flexor strength that can isolate the intrinsic muscles of the foot from the extrinsic muscles. This study compared the hallux flexion force (HFF) and muscle activity in the foot and ankle according to ankle position [plantarflexion (PF), neutral, and dorsiflexion (DF)]. Method : The study enrolled 17 subjects. In the sitting position, the HFF and activities of the abductor hallucis (AbdH), tibialis anterior (TA), and gastrocnemius (GCM) muscles were measured using a digital dynamometer and a wireless electromyography system, respectively. Subjects were instructed to flex the great toe maximally in three different ankle positions. Three 5-second trials were performed to measure the HFF and muscle activities in each condition. Repeated-measures ANOVA was used to compare the variables and paired t-tests with the Bonferroni correction were used for post-hoc pair-wise comparisons. The significance level was set at .016. Result : The HFF in DF was significantly greater than in any other ankle position (p<.01). The TA activity was greatest in ankle DF and that of the GCM was greatest in PF (both p<.01). However, there was no significant difference in AbdH activity according to ankle position. Conclusion : These results suggest that selective strength measurement of the foot intrinsic muscles in HFF should be performed with the ankle in the neutral position.

A Study on the Optimal Position of Lightning Arrester on Joint Operation of Neutral Wire and Overhead Grounding Wire through Lightning Surge Analysis in Combined Distribution System (혼합배전계통에서 뇌과전압 해석을 통한 중성선과 가공지선 혼용 운전시 적정 피뢰기 위치에 관한 연구)

  • This paper studies the validity about a joint operation of neutral wire and overhead grounding wire in combined distribution systems. The overhead grounding wire and neutral wire are currently installed separately and grounded by common. However there is no any ineffectiveness or electrical problem in case of the proposed system, such system can be operated at real distribution system. Therefore this paper describes the suitability of a joint operation through lightning surge analysis on combined distribution systems and analyzes the optimal position of lightning arrester on joint operation of neutral wire and overhead grounding wire. Lightning surge analysis is carried out by EMTP/ATPDraw to obtain the overvoltage of overhead line and underground cable in various conditions such as location and current types of lightning stroke. Over voltage gained by the analysis show that the insulation strength of the joint operation case is not stable compare with the current operation case.

  • It is very common to use the powered hand tools to enhance the productivity in various types of industry. But the use of the powered hand tools could cause health problems such as cumulative trauma disorders and vibration white fingers. In this study. the effects of hand-arm vibration and anatomical hand position on localized muscle fatigue were analyzed. Eight healthy male subjects volunteered for the study. Vibration frequencies of 0, 40, 80, 100, 150, and 200Hz and hand position of flexion and ulnar deviation were selected for the independent variables of the experiment. Median frequency shifting was used as a dependent variable. The results indicated that at the vibration frequency of 40Hz and accelation of 2g, the muscle fatigue was the greatest. For the hand position. there was significant difference between neutral and flexion. and neutral and ulnar deviation, but no difference between flexion and ulnar deviation. These results could be applied in designing powered hand tools to minimize the health problems.

Effects of Modified Low-Dye Taping on Stroke Patients with an Excessive Pronated Foot (과도하게 엎침된 발을 가진 뇌졸중 환자에게 적용된 수정 발바닥활 지지 테이핑의 효과)

  • PURPOSE: Stroke patients may develop an over-pronated foot, resulting in a lower medial longitudinal arch. This can lead to a structural change of the foot due to deformation of the musculoskeletal system. The purpose of this study was to examine the effects of modified low-dye taping on stroke patients with an excessively pronated foot. The effect of the taping on the foot after light daily activity was also examined. METHODS: The subjects consisted of 21 stroke patients with an excessively pronated foot, as measured by the navicular drop test. First, their navicular heights were measured at a relaxed standing position (measure 1) and while standing in the subtalar neutral position (measure 2). Modified low-dye taping was applied to each subject's affected foot and the navicular height was then measured for the standing posture (measure 3). Finally, each subject walked around for 10 minutes and the navicular height was measured again (measure 4). RESULTS: Statistical analyses showed that the navicular height value at the relaxed standing position (measure 1) was significantly lower than for the other 3 measurements. That is, the modified low-dye taping was effective in maintaining a subtalar neutral position, even after a 10-minute walk, for stroke patients with an excessively pronated foot. CONCLUSION: The results suggest that modified low-dye taping applied to stroke patients with an excessively pronated foot could be an effective way to place the subtalar joint in a neutral position, and that its effect can be sustained for light daily activities.

Changes in Cervicocephalic Joint Position Sense in Sustained Forward Head Posture (지속적인 전방머리자세 시 머리-목 관절 위치 감각의 변화)

  • Purpose : The aim of this study was to verify differences in cervicocephalic joint position sense error after different sustained sitting postures in healthy young adults. Methods : Twenty-five healthy adults(12 men, 13 women) participated in this study. Repositioning errors of neck movement were observed in participants during joint repositioning tasks. During 2 test days with a 1-week interval, the participants performed forward head posture and upright sitting posture in random order. Both head-to-neutral(HTN) and head-to-target(HTT) tasks were performed on each day. On the first day, the participants sat slouched or upright for 10 minutes. Then, they sat upright and moved their heads at a self-selected speed with their eyes-closed to pre-determined neutral and target positions as accurately as possible. The participants noticed that when they reached a pre-determined position, the errors between pre-determined neutral and target positions and current position was recorded. The tasks consisted of flexion, extension and lateral bending. On the second day, the same test was performed after another sitting posture for 10 minutes. Repositioning error values were collected by using a smart phone-based inclinometer. The mean value for three trials was used for data analysis. A paired t-test was used for statistical analysis. Results : Significant differences in joint repositioning errors were found between the repositioning error after different sitting postures on the sagittal plane for both the HTN and HTT tasks (P<.05). No significant differences in errors on the coronal plane were found (P>.05). Conclusion : Cervicocephalic joint position sense can be affected by sitting postures, especially on the sagittal plane.

Effects of Wrist Position on Electromyographic Activities of Flexor Digitorum Superficials and Extensor Digitorum during Drilling and Pinch Grip (드릴작업과 집기작업 시 손목 자세가 손가락굽힘근과 손가락폄근의 근 활성도에 미치는 영향)

  • The purpose of this study was to investigate influence of wrist position on the electromyographic(EMG) activities of the flexor digitorum superficialis (FDS) and extensor digitorum (ED) during drilling and pinch grip. Eighteen healthy subjects had been recruited and each subject performed two tasks (drilling and pinch grip) at three different wrist positions ($30^{\circ}$ flexion, neutral, $30^{\circ}$ extension). The EMG amplitude of each muscle was normalized to the amplitude in the maximal voluntary contraction (MVC). Repeated one-way ANOVA was used to compare the differences of EMG across wrist positions. The FDS EMG activity in both drilling and pinch grip was lowest at $30^{\circ}$ wrist extension and the highest at $30^{\circ}$ wrist flexion. The ED EMG activity was lowest when the wrist was neutral in both tasks. From the results of this study, we can conclude that the desirable wrist positions for drilling and pinch grip tasks are slightly extended or neutral position. Therefore, flexed wrist position should be avoided to reduce the excessive work load on the finger muscles during the tasks.

Comparison of Three Supraglottic Airway Devices in Neutral and Cervical Neck Collar Position (융복합적 기술이 적용된 성문위기구들의 삽관용이성 비교)

  • Purpose of this study was to compare the easiness of insertion of three Supraglottic airway devices(SADs) in a neutral and cervical neck collar position in a manikin setting. Three different SADs-Laryngeal Mask Classic (LMA), I-gel and Streamlined Liner of the Pharynx Airway(SLIPA) were applied. Thirty nine emergency medical technicians (EMTs) who are working in Korea 119 rescue services were taught briefly about the use of the LMA, I-gel and SLIPA in a manikin setting. The time to effective ventilation was shorter in I-gel ($10.5{\pm}2.2$, $11.4{\pm}5.7$) and SLIPA ($12.3 {\pm}5.1$, $12.0{\pm}6.8$) than LMA ($17.2{\pm}3.2$, $18.5{\pm}5.5$) in a neutral and cervical neck collar position, respectively(p<0.05). There was no difference between head positions in each of the airways. In conclusion, in the speed of insertion, I-gel and SLIPA were superior to LMA in the neutral and cervical collar setting of a manikin.

Coarticulation and vowel reduction in the neutral tone of Beijing Mandarin

  • The neutral tone is one of the most important distinguishing features in Beijing Mandarin, but there are two completely different views on its linguistic function: a special tone(Xu, 1980) versus weak stress(Chao, 1968). In this paper, the acoustic manifestation of the neutral tone will be explored to show that it is closely related to weak stress. 122 disyllabic words in which the second syllable carries the neutral tone, including 22 stress pairs, were uttered by a native male speaker of Beijing dialect and analysed by Kay Digital Sonagraph 5500-1. The results of the acoustic analysis are presented as follows: 1) The first two formants of the medial and the syllabic vowel moves towards that of central vowel with a greater magnitude in the syllable with the neutral tone than in the syllable with any of the four normal tones. Also the vowel ending, and nasal coda /n/ and / / in the syllable with the neutral tone tends to be deleted. 2) In the syllables with the neutral tone, there are strong carryover coarticulations between the medial and syllabic vowel and the preceding unvoiced consonant. In general, the vowel is affected to move towards the position of the central vowel with more greater magnitude by coronal consonant than by labial or velar consonant. 3) In the syllable with the neutral tone, when and only when it precedes a syllable with tone-4, the high vowel following [f], [ts'], [s], [ts'], [s], [tc'] or [c] tends to be voiceless. 4) It can be seen from the acoustical results of 22 stress pairs that the duration of the syllable with the neutral tone is on the average reduced to 55% of that of the syllable with the four normal tones, and the duration of the final in the syllable with neutral tone is on the average reduced to 45% of that of the final in the syllable with the four normal tones(Lin & Yan 1980). 5) The FO contour of the neutral tone is highly dependent on the preceding normal tone(Lin & Yan 1993). For a number of languages it has been found that the vowel space is reduced as the level of stress placed upon the vowel is reduced(Nord 1986). Therefore we reach the conclusion that the syllable with neutral tone is related to weak stress(Lin & Yan 1990). The neutral tone is not a special tone because the preceding normal tone.

  • Shoulder MRArthrography was performed to get an accurate diagnosis about complex anatomical structure in shoulder joint. We carried out how the changes of anatomical rotations in shoulder joint could bring certain diagnosis effects on MRI images for various shoulder humerus positions; Neutral position, Internal rotation position and External rotation position. In addition, we prepared an aid tool in oder to maintain the right posture of a patient. This aid tool was made by adapting Modeling Design Program. By virtue of this aid, we obtained the following result. Shoulder MR Arthrography by the External rotation position for anatomical structure diagnosis was the most suitable in diagnostic evaluations of important anatomical structures in shoulder joint such as Biceps tendon, Supera-spiatus tendon, Sub-scapularis tendon, Labrum and Sub-acromial space.

Effect of Asymmetrical Tonic Neck Reflex on Weight Bearing of the Extremities (비대칭성 긴장성 경반사가 사지의 체중지지에 미치는 영향)

  • The purposes of this study were 1) to compare weight distribution and ratio of the extremities between normal and hemiplegic children in quadruped position and 2) to compare the effect of ATNR on weight bearing of the extremities between normal and hemiplegic children. The subjects fer the study were 48 children(24 normal, 24 hemiplegic) between the ages 3 to 6. They were teated weight distribution and ratio of the extremities in the neutral position of head and by passive right and left rotation of the head in the quadruped position. The data wert analized by paired t-test. The results were as follows: 1. In the neutral position of head, normal group was not difference on weight distribution and ratio of the extremities and hemiplegic group was more weight bearing on the sound upper extremity than the affected upper extremity(p<.01). 2. When the head rotated to the dominant side or sound side passively, there was not a significant difference between normal and hemiplegic group. 3. When the head rotated to the nondominant side or affected side passively, there was a significant difference between nondominant upper extremity nf normal and affected upper extremity of hemiplegic group(p<.05).
