The Effect of Blood Nutrient Index on Depression (성인 여성의 혈액 영양소 지표가 우울지표에 미치는 영향)

  • The purpose of this study is that the blood nutrient indicator in adult women, is the depressive indicator. This study recruited 77 pre-menopausal women, and excluding missing values, 53 (mean age 44.2±6.5 years) blood indexes were statistically confirmed and then analyzed the effect of indexes on the Beck Depression Inventory. The Beck's Depression Inventory(BDI)'s average of the study subjects was 7.6±7.2, and most of the depression symptoms were within the normal range. Only two women were very serious and other two women were serious state. As a result of this study, 21 questions of BDI was classified into 5 sub-factors. Of these, 2 factors were considered physical depression, 1 factor was mood depression, 1 factor was cognitive depression, and 1 factor undecided. As a result of the correlation analysis, triglyceride, LDL(low density lipoprotein) cholesterol and potassium showed statistically significant positive relationship with depressive factors. On the other hand, HDL(high density lipoprotein) cholesterol, sodium and chloride ions showed negative relationship with depressive factors. When multiple regression analysis was performed. Considering the depression factors was dependent variable, and triglyceride, cholesterol, blood sugar, and ketone bodies was considered as independent variables. HDL(high density lipoprotein) cholesterol had a statistically significant negative effect on the depression factor 3. Considering the depression factors was dependent variable, and minerals were considered as independent variables. Potassium had a positive effect, and chloride ions had a negative effect on the depressive factor 3. This study was limited to 77 pre-menopausal women. In the future, post-menopausal women, men, and clinical neurological disorders group will be possible.

A Survey About Awareness and Necessity of Community Based Dysphagia Therapy of Community Dwelling Older Adults (지역사회 거주 노인들의 연하장애 인식과 중재 필요성)

  • Objective : This study aimed to investigate the awareness and experience of community-based dysphagia therapy and related education in community-dwelling older adults. Methods : A total of 89 older adults were recruited from a public health center in Gyeonggi-do. Awareness, experience, and related education regarding community-based dysphagia therapy were analyzed using descriptive statistics. Results : We analyzed 89 questionnaires. Awareness, treatment experience, and education regarding dysphagia were low; however, the importance and intention to participate were high. Respondents wanted education about proper chewing and safe swallowing, oral health, oral motor exercise, and participation in community-based dysphagia programs in public health centers. The reason for the lack of experience in dysphagia education and therapy is insufficient information and opportunities. The respondents had a good understanding of dysphagia symptoms. Conclusion : Dysphagia therapy maintains swallowing and eating functions as a life-long Activity of Daily Living, and is a very important area in community rehabilitation. Based on the results of this survey, the necessity and importance of community-based dysphagia were identified. It is time to provide correct information and develop a systematic education program for community-based dysphagia therapy. Occupational therapists need to play an active role in improving quality of life by early detection and providing proper intervention.

Factor Structure of the Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile in Infants and Toddlers With Mental Disorders and the Difference Among Mental Disorders (정신장애 영유아에 대한 K-CBCL 1.5-5 (Korean-Child Behavior Checklist 1.5-5) 조절곤란 프로파일의 요인구조와 정신장애 간 차이검증)

  • Object : This study aimed to identify the factor structure of Korean-Child Behavior Checklist 1.5-5 Dysregulation Profile (K-CBCL 1.5-5 DP) in infants and toddlers with mental disorders and verify differences in K-CBCL 1.5-5 DP among the diagnosis groups. Methods : The participants were 265 mothers of infants and toddlers with mental disorders who completed K-CBCL 1.5-5 DP. The data was analyzed using AMOS 25.0 and SPSS 25.0. Results : First, the bifactor model was the most suitable for the factor structure of the K-CBCL 1.5-5 DP. Second, there were significant differences among the diagnosis groups, such as communication disorders, pervasive developmental disorders, emotional disorders, and developmental delays. It was confirmed that the pervasive developmental disorder and emotional disorder groups showed significantly higher dysregulation compared with the communication disorder group. Conclusion : This study confirmed that infants and toddlers had dysregulation problems. Using the bifactor model, the multidimensional nature of the K-CBCL 1.5-5 DP was assessed. It was also meaningful that dysregulation could contribute to onset and deepening of symptoms of pervasive developmental disorders and emotional disorders in infancy.

Differences in Clinical Characteristics and Surgical Outcomes of Patients with Ischemic and Hemorrhagic Pituitary Adenomas

  • Objective : Ischemia and hemorrhage of pituitary adenomas (PA) caused important clinical syndrome. However, the differences on clinical characteristics and surgical outcomes between these two kinds apoplexy were less reported. Methods : A retrospective analysis was made of patients with pituitary apoplexy between January 2013 and June 2018. Baseline and clinical characteristics before surgery were reviewed. All patients underwent transsphenoidal surgery and were followed up at least 1 year. Results : Total 67 cases (5.8%) among 1147 pituitary tumor patients were enrolled, which consisted of 28 (~2.4%) ischemic PA and 39 (~3.4%) hemorrhagic PA. There were more male patients in the ischemic group compared with hemorrhagic group (78.6% vs 53.8%, p=0.043). However, the mean age, tumor size and functional tumor ratio were significant higher in the hemorrhagic group. Headache was more common in ischemic PA (82.1%) than that of hemorrhagic PA (51.3%, p=0.011). Magnetic resonance imaging findings found that mucosal thickening and enhancement of the sphenoid sinus was observed in 15 ischemic PA patients (n=27, 55.6%), but none in patients with hemorrhagic PA (n=38, p<0.0001). It was worth noting that the rate of pre-surgical hypopituitarism in ischemic PA patients were seemed higher than that in hemorrhagic PA patients, but not significant. The two groups got a total tumor resection rate at 94.1% and 92.9%, independently. No significant difference on the operative time, blood loss in operation and complications in perioperative period was observed in two groups. After operation, cranial nerve symptoms recovered to normal at 81.8% of ischemic PA patients and 82.6% of hemorrhagic PA patients. Importantly, the incidence of postoperative hypopituitarism partially decreased in both groups, among which the rate of hypothyroidism in ischemic PA patients significantly decreased from 46.4% to 18.5% (p=0.044). Conclusion : Patients with ischemic PA presented different clinical characteristics to the hemorrhagic ones. Transsphenoidal surgery should be considered for the patients with neuro-ophthalmic deficits and might benefit for pituitary function recovery of the apoplectic adenoma patients, especially pituitary thyroid axis in ischemic PA patients.

A Case of Chronic Meniere's Disease Improved by Korean Medicine Treatment Including Pharmacoacupuncture of Cervical and Temporomandibular Area - A Case Report (경추부 및 하악부 약침 치료를 포함한 한의복합치료로 호전된 만성 메니에르병 환자 1례 - 증례 보고)

  • Meniere's disease is characterized by episodic vertigo, fluctuating hearing loss, tinnitus and ear fullness. The main pathological finding is endolymphatic hydrops, but the etiology of disease and effective treatment methods are still disputed. Recently, Cervical spine disorders(CSD) and Temporomandibular disorders(TMD) have been attracting attention as one of the causes of Meniere's disease. A 65-year-old female Meniere patient with musculoskeletal problems in the cervical and mandible area was treated by Korean medical therapies including pharmacoacupuncture treatment. After 5 weeks of treatment, there was no meaningful change in hearing level evaluated with pure tone audiometry, but the subjective symptoms of Meniere improved significantly. Numerical rating scale (NRS) decreased from 10 to 0 for hearing loss, 10 to 3 for tinnitus and 8 to 3 for ear fullness. Also NRS of cervicalgia was reduced from 5 to 0 after treatment. The result suggests that the Korean medical therapy including pharmacoapuncture targeting CSD and TMD could be safe and effective method for patients with Meniere's disease.

Development and Assessment of an ICT-Based Non-Face-to-Face Lifestyle Program to Improve the Mental Health of Older Adults: A Pilot Study (고령자의 정신건강을 위한 ICT 기반 비대면 라이프스타일 프로그램: 파일럿 연구)

  • Objective : This study aimed to develop an information and communication technology (ICT)-based, non-face-to-face lifestyle program for older adults and assess its applicability. Methods : The program was developed on the basis of the ADDIE model which comprises 5 stages: analysis, design, development, implementation, and evaluation. In this study, a step-by-step identification was performed in 8 stages. Results : The results of the program analysis showed a significant decrease in depressive symptoms and loneliness scores, and an increased quality of life scores. Conclusion : The findings suggest that the ICT-based non-face-to-face lifestyle program developed in this study can motivate older adults to better understand their lifestyles for successful aging, while helping senior citizen centers resume stagnant projects.

MRI Findings of COVID-19 Associated Acute Necrotizing Encephalopathy in Two Pediatric Patients: Case Report and Literature Review (소아에서 발생한 COVID-19 연관 급성 괴사성 뇌병증의 MRI 소견: 2건의 증례 보고 및 문헌 고찰)

  • Acute necrotizing encephalopathy (ANE) is a rare immune-mediated complication of a viral infection commonly involving the bilateral thalamus and has been reported mainly in children. Here, we describe the MRI findings of coronavirus disease 2019 (COVID-19)-associated ANE in two pediatric patients, including a 7-year-old girl with fever and mental change, and a 6-year-old girl with fever and generalized seizures. Brain MRI revealed symmetrical T2 fluid attenuated inversion recovery high-signal intensity lesions in the bilateral thalamus with central hemorrhage. In one patient, the thalamic lesions showed a tri-laminar pattern on the apparent diffusion coefficient map. This report emphasizes the importance of creating awareness regarding these findings in patients with COVID-19, particularly in children with severe neurological symptoms. Furthermore, it provides a literature review of several documented cases of COVID-19 presenting with bilateral thalamic hemorrhagic necrosis, suggesting a diagnosis of ANE.

Correlation Between Salt Content, Microbial Diversity, and Biogenic Amine Concentration in Commercial Ganjang (시판 한식간장 염도와 간장 미생물 다양성 및 바이오제닉 아민 농도와의 상관관계)

  • Biogenic amines in food, produced through amino acid decarboxylation, can cause allergies, digestive issues, and neurological symptoms. Recently, various studies have been conducted to reduce the levels of biogenic amines in food. This study analyzes the impact of salt concentration in Ganjang on microbial diversity and biogenic amine production. Results show a statistically significant correlation between salt concentration in Ganjang and species richness and species abundance indices. Although the alpha-diversity index did not significantly correlate with biogenic amine levels, higher salt concentration resulted in a statistically significant decrease in histamine and tyramine levels. An analysis of the correlation between microbial distribution and biogenic amines based on the salt concentration in Ganjang revealed that the distribution of Lactobacillus sp. and Bacteroides sp. increased as the salt concentration increased while the levels of biogenic amines decreased. On the other hand, the distribution of Tetragenococcus sp., Chromohalobacter sp., and Halomonas sp. decreased with increasing salt concentration, accompanied by an increase in biogenic amine levels. The results of this study suggest that within the salt concentration ranges in Ganjang, an increase in salt concentration is associated with an increase in the distribution of Lactobacillus sp., Bacteroides sp., Streptococcus gallolyticus, and Pseudomonas sp. This increase in microbial distribution is presumed to be related to a reduction in biogenic amine production or an enhancement in biogenic amine degradation.

Prevalence of Dysphagia and Educational Needs in Community-Dwelling Older Adults: Focusing on Chuncheon, Gangwon-do State (지역사회 거주 노인의 연하장애 유병률과 교육 요구도 조사: 강원특별자치도 춘천 지역을 중심으로)

  • Objective : This study investigated the prevalence of dysphagia in older adults in Gangwon Special Self-Governing Province and its impact on Quality of Life (QoL) and educational needs. Methods : A cross-sectional survey was conducted from August 1 to 15, 2023, with 207 cognitively normal older adults from three senior welfare centers. Data included general characteristics, Korean-version of the Eating Assessment Tool (K-EAT-10), Swallowing-QoL, and educational needs related to swallowing disorders. Dysphagia was defined by a K-EAT-10 score of 3 or higher. Participants were divided into normal and risk groups for analysis using chi-square and Independent t-test. Results : The prevalence of dysphagia risk (K-EAT-10 score of 3 or higher) was 41.5% (86 individuals). Among the 92 individuals reporting dysphagia symptoms, 64 (69.6%) were at risk. QoL was significantly lower in the risk group, especially among those living alone or with denture discomfort. The most preferred educational need was training in eating solid foods (Mean = 4.27), with higher needs in the dysphagia risk group. Conclusion : This study underscores the prevalence of dysphagia, its impact on QoL, and the need for educational programs. Emphasizing prevention, public awareness, and early detection, along with the role of dysphagia rehabilitation experts and community collaboration, is essential for supporting healthy aging.

Treatment of Dysphagia in a Cardiac Transplantation Patient: A Case Study (심장 이식 수술 환자의 연하장애 재활치료: 단일 사례 연구)

  • Objective : This case study aimed to explore dysphagia symptoms in post-cardiac transplantation patients and evaluate the effectiveness of dysphagia rehabilitation therapy in an occupational therapy clinic. Methods : A patient with post-cardiac transplant dysphagia underwent videofluoroscopic swallowing studies to assess the Functional Dysphagia Scale (FDS), Penetration-Aspiration Scale (PAS), and Dysphagia Outcome and Severity Scale (DOSS). Assessments were performed three times at two-week intervals. The treatment, administered from July 19 to August 21, 2023, included eight sessions of 30 minutes each. Treatments included corrective techniques (tongue-hold maneuver, chin tuck against resistance, mendelsohn maneuver, effortful swallowing, and respiratory muscle exercise) and compensatory strategies (chin-tuck position, multiple swallowing, modification of volume, and viscosity), alongside caregiver education. Results : The FDS scores decreased from 50 to 30, PAS scores improved from 5 to 3, and DOSS scores improved from Stage 2 to Stage 4. Conclusion : Dysphagia rehabilitation therapy improved swallowing function in a post-cardiac transplantation patient. However, further studies are required to confirm these findings.
