Neuronal Vacuolation in a Pekingese (Pekingese에서의 Neuronal Vacuolation)

  • A 6-month-old female Pekingese was euthanized due to poor progrosis after 1 month history of neurologic signs that include depression, ataxia, urination and defecation difficulty. At necropsy, no significant gross abnormalities were noted Histologically, neuronal vacuolation was noted in the brain, primarily cerebellum and occasionally in the brain stem area. Neuronal necrosis and secondary axonal swelling were also observed. Differential diagnoses were able to rule out other diseases which can induce neuronal vacuolation such as lysosomal storage disease, prion infection, and postvaccinal change.

  • Over the last 30 years or so, it has been recognized that neurologic disorders could impair laryngeal function, and that neurogenic etiology could be discerned frequently. This has led collaborations between laryngologists and neurologists and focus on the management of neurogenic dysfunction of the larynx and pharynx, including central and peripheral disorder. The author introduced anatomical structure, nerve distribution and neurophysiology of the larynx for understanding its basic functions. The symptoms, diseases from laryngeal dysfunction and the development of diagnosis and management were also discussed.

Occipital Dysplasia in Dogs (개의 후두골 이형성)

  • Occipital dysplasias in five dogs showing various neurologic signs of unknown origin were diagnosed with clinical examination and skull radiographic assessment at this university teaching hospital. The severities of occipital dysplasia were Grade III in four dogs and Grade II in one dog. Major clinical signs were ataxia and convulsion. In skull radiographs, there were dorsal extents of the foramen magnum reached nuchal crest in all cases. Also, hydrocephalus or vertebral malformation was complicated in three cases.

  • Headache, like low back pain, is one of the most common of pain conditions. Many data suggest that nerve block can be one of effective treatments in managing headache except pure psychologic or surgical origin, because mechanism of headaches have neurologic, vascular or local tissue pathology. We experienced two types of headache; episodic tension-type headache, and benign exertional headache; successful treatment consist of nerve block and modulation of exercise, respectively.

  • In an attermpt to aviod the deleterious effects of cardiopulmonary by pass such as pulmonary complication neurologic complication and renal failure off-pump CABG has been rediscovered and developed. We experienced off-pump CABG in 2 cases with unstable angina complicated with COPD and report herein the cases with review of literature.

  • Malignant transformation of fibrous dysplasia is a rare condition and the incidence is estimated at 0.4% for fibrous dysplasia and 4% for Albright's syndrome. The authors did not find a reported case of malignant change at skull base around the orbit in the literature. We experienced a case of fibrous dysplasia, in which neurologic symptoms were aggravated due to malignant change around the orbit, and report its favorable outcome obtained with total surgical removal.

  • Intraoperative pulmonary thromboembolism is a high mortality situation. Early mortality in patients with pulmonary thromboembolism varies from 2% in normotensive patients to 30% in patients with cardiogenic shock. The use of extracorporeal cardiopulmonary resuscitation can improve survival and neurologic outcomes of cardiac arrest. We report a case of intraoperative massive pulmonary thromboembolism with circulatory collapse and cardiac arrest during anesthesia for pelvic bone fracture surgery, which were rescued by extracorporeal membrane oxygenation.

  • The effects of low-level laser irradiation on cells and tissues, known as photobiomodulation therapy (PBMT), are the basis of photomedicine. Several investigations have evaluated the therapeutic effects of PBMT for neuronal regeneration and differentiation in animal models and humans. Recently, intranasal PBMT (iN-PBMT) has shown potential as a treatment method for neurologic disorders. In this review, we have summarized the various modes of iN-PBMT delivery and their application in the treatment of brain disorders.

  • Pediatric intracranial arteriovenous malformations (AVMs) are challenging lesions managed by pediatric neurosurgeons. The high risk of hemorrhage and neurologic injury is compounded by the unique anatomy of each malformation that requires individualizing treatment options. This article reviews the current status of pediatric AVM epidemiology, pathophysiology and clinical care, with a specific focus on the rationale and methodology of surgical resection.
