한방치료로 호전된 소음인 임신부의 삼차신경통 치험 1례 (A Case Report of Trigeminal Neuralgia of Pregnant Soeumin Treated by Traditional Korean Medicine)

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment on a patient with trigeminal neuralgia during pregnancy. Methods: We treated a facial pain patient diagnosed as trigeminal neuralgia during pregnancy with acupuncture, cupping and herbal medicines. Herbal medicines were prescribed according to the patient's Sasang constitution which was Soeumin. The severity of symptom was evaluated by Numeric Rating System (NRS). Results: After treatment, the patient's facial pain caused by trigeminal neuralgia was improved. NRS score was decreased from 6 to 0. Conclusions: This case report shows that traditional Korean medical treatment and Sasang constitution medicine is effective for the patient with trigeminal neuralgia during pregnancy.

특발성(特發性) 설인신경통(舌咽神經痛) 환자 치험 1례 (A Case Report of Patient with Idiopathic Glossopharyngeal Neuralgia)

  • Objective : This study reports the effect of Korean medicine on one case diagnosed as idiopathic glossopharyngeal neuralgia. Methods : We experienced one case with idiopathic glossopharyngeal neuralgia treated with acupuncture, herbal medicine and gargling. Associated symtoms had been estimated with change of VAS(Visual Analogue Scale), severity of dysphagia, and diet type. Result : After the treatment, symptoms were decreased and almost disappered. In this case, VAS and severity of dysphagia decreased from 10 to 1 and from severe to subsided, and diet type changed from unable to DD4(Dysphagic Diet 4) during 21 days. Conclusion : This study suggests that Korean medicine is effective on idiopathic glossopharyngeal neuralgia.

특발성 삼차신경통의 체형교정에 따른 호전 사례보고 (A Improving Case Report of Idiopathic Trigeminal Neuralgia by Body-form Correction)

  • Objectives : To understand spatial cause hypothesis of idiopathic trigeminal neuralgia, the body truncus area was divided into intracranial, thoracic and pelvic cavity and was illustrated, the subjective pain degrees of idiopathic trigeminal neuralgia were compared with the illustraion of three cavity. Methods : The frontal view of pictures of idiopathic trigeminal neuralgia subject truncus area were illustrated into three circles which was treated with conservative methods like the pelvic correction, cervical correction, FCST, posture training without drugs and acupuncture. The spatial analysis of three circle vertical centerlines were compared with the pain degrees. Results : The vertical centerlines of three circles were agreed with the body gravity centerline depending on the treatment progresses. namely, as the parts of truncus were matched to the body gravity centerline, the degrees of pain were decreased. Conclusions : The vascular pressure on the trigeminal nerve which was causing the idiopathic trigeminal neuralgia might be induced by the spatial misalignment of truncus area because the spatial misalignment of it can press to move vascular into trigeminal nerve partially. Further study will be progressed.

설인신경통 환자에서 Phenytoin 정주의 효과 -증례 보고- (The Effect of IV Infusion of Phenytoin for Glossopharyngeal Neuralgia -A case report-)

  • Glossopharyngeal neuralgia is a rare syndrome that involves episodic bursts of pain in the sensory distributuion of the ninth cranial nerve. The nature of the pain is characterized by excruciating shock-like pain in the region of the tonsillar fossa or pharynx and can radiate to the ear or the angle of the jaw. Like trigeminal neuralgia, glossopharyngeal neuralgia typically responds to anticonvulsant agents such as carbamazepine. However, dose of carbamazepine needs to be increased gradually to avoid side effects. If the patient can not tolerate until effective carbamazepine level is reached, phenytoin can be administered intravenously at the same time that oral carbamazepine therapy is begun. We present fifty-three year old female patient suffering from glossopharyngeal neuralgia who did not respond to initial carbamazepine therapy, but responded to concomitant intravenous infusion of phenytoin.

대상포진후신경통의 한방 치료 국내 연구 현황 : 증례 연구 분석 (Trends of Korean Medicine Treatment for Postherpetic Neuralgia : Analysis of Case Studies)

  • Objectives : The purpose of this study is to analyze the trends of Korean medicine treatment on postherpetic neuralgia. Methods : Domestic databases(OASIS, KISS, RISS, KISTI, NDSL, KMbase) were used to search case studies related to Korean medical treatment of postherpetic neuralgia. Total 9 studies were selected. Results : A total 15 prescriptions were used 22 times in 14 cases. Among them, Sipjeondaebo-tang(十全大補湯) was the most frequently prescribed. Total 91 herbs were used and they were classified as tonifying and replenishing medicine(補益藥), exterior-releasing medicine(解表藥) and blood-activating and stasis-dispelling medicine(活血祛瘀藥) in the order of mostly used. In 8 studies, in addition to herbal medicine treatment, other Korean medicine treatments such as acupuncture, pharmacopuncture, moxibusiton, cupping therapy and external agents were performed at the same time. Most frequently used acupoints were 足竅陰(GB44) and 商陽(LI1). Conclusions : This is the first study to analyze the trends of Korean medicine treatment for postherpetic neuralgia. Although there are some limitations, this study is thought to be helpful in the domestic treatment of postherpetic neuralgia in the future.

삼차신경통 진단 및 치료의 중요 고려사항 (Clinical Considerations of Trigeminal Neuralgia)

  • 삼차신경통은 삼차신경의 하나 이상 분지에서 나타나는 안면 편측 통증 질환으로 짧은 찌르는 듯한 통증이다. 삼차 신경통의 가장 흔한 원인은 인접 삼차신경이나 삼차신경근 부위를 압박하는 혈관에 의하거나 특발성으로 나타나는 특발성 삼차신경통과 구조적 병소에 의해 나타나는 증후성 삼차신경통으로 분류된다. 증후성 삼차신경통은 수막종, 청신경종, 표피양암종 등이 주된 원인이며, 감각 결함, 운동 결함, 운동실조, 안구진탕 등과 같은 신경계 증상이 동반된다. 삼차 신경통의 치료는 크게 약물 치료와 외과적 치료가 있다. 이 중 가장 효과적으로 사용되는 약물은 Carbamazepine이며, 졸음, 현기증, 불안, 오심, 식욕부진 등의 다양한 부작용을 나타낼 수 있다. 특히 간독성, 골수 기능 억제 등의 위험한 부작용이 나타날 수 있으므로 주기적으로 혈액검사, 간 및 신장 기능 검사가 필요하다. 이에 삼차신경통의 임상 증례를 통해 증후성 삼차신경통과 특발성 삼차신경통의 예를 들어 진단과 치료를 시행함에 있어 여러 고려 사항들 중 특히 우리가 소홀히 하거나 간과하기 쉬운 중요 고려사항을 되짚어 보고자 한다.

후두신경통과 신경차단 (The Effects of Nerve Blocks in the Management of Occipital Neuralgia)

  • Background: Occipital neuralgia is characterized by pain, usually deep and aching, in the distribution the second and/or third cervical dorsal root. Two broad groups of patients include primary occipital neuralgia with no apparent etiology and secondary neuralgia with structural pathology. Patients with occipital neuralgia can develop autonomic changes and hyperesthesia. In patients who have not improved with conservative treatment, we have carried out various nerve blocks and evaluated the effectiveness. Methods: In a series of 20 occipital neuralgia patients with no apparent etiolgy, we have carried out great occipital nerve blocks with needle TEAS. In patients who have not improved more than 75% on VAS with great occipital block, we have carried out C2 ganglion blocks and in patients who have not improved more than 75% with C2 ganglion block, C3 root blocks, C2/C3 facet joint blocks have been carried out in due order. Results: In 3 patients out of 10 patients who have not improved with great occipital nerve block, C2 ganglion block led to pain relief. A good response of C3 root block was achived in 2 of 7 patients without response to C2 ganglion block and C2/C3 facet joint block led to improvement in 1 of 5 patients without response to C3 root block. Conclusions: Nerve blocks like great occipital nerve block, C2 ganglion block, C3 root block, or C2/C3 facet joint block were effective in the patients who have not improved with conservative treatment.

Atypical triggers in trigeminal neuralgia: the role of A-delta sensory afferents in food and weather triggers

  • Background: Trigeminal neuralgia is a debilitating craniofacial pain syndrome that is characterized by paroxysms of intense, short-lived electric shock-like pains in the trigeminal nerve distribution. Recently, the presence of triggers has become one of the key diagnostic criteria in the 3rd edition of the International Classification of Headache Disorders. Light touch is the most common trigger, however other non-mechanical triggers, such as cold weather and certain foods, have been thought to provoke trigeminal neuralgia anecdotally. We aimed to characterize the prevalence and characteristics of these atypical triggers. Methods: We conducted a retrospective, cross-sectional study of atypical triggers in trigeminal neuralgia patients seen in a tertiary pain clinic in Singapore. Patients were recruited via clinic records, and study data were identified from physician documentation. Results: A total of 60 patients met the inclusion criteria. Weather triggers were observed in 12 patients (20%), of which five patients (8%) reported strong winds, 4 patients (7%) reported cold temperatures, and 3 patients (5%) reported cold winds as triggers. Fifteen patients (25%) had a specific food trigger, of which 10 patients (17%) reported hard or tough food, 5 patients (8%) reported hot/cold food, 4 patients (7%) reported spicy food, and 2 patients (3%) reported sweet food as triggers. Conclusions: Although trigeminal neuralgia is most commonly triggered by mechanical stimuli, atypical triggers such as cold temperatures and certain foods are seen in a significant proportion of patients. These atypical triggers may share a common pathway of sensory afferent Aδ fiber activation.

경희공진단과 향부자팔물군자탕가감으로 호전된 대상포진 후 신경통 치험 1례 (A Case Report of Postherpetic Neuralgia Treated with Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam)

  • The most common definition of postherpetic neuralgia is the presence of pain more than a month after the onset of the eruption of zoster. This case report suggests the effect of Korean medicine on postherpetic neuralgia. An 85-year-old male patient with postherpetic neuralgia was treated with a combination of Korean medical remedies. Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam were applied for 23 days. The Numerical Rating Scale was used to measure pain intensity. The Barthel Index and Beck Depression Inventory were used for the functional assessment and depression evaluation, respectively. After treatment, the patient's pain intensity decreased. Additionally, the patient's independence increased during activity, and the severity of their depression decreased. Thus, this case suggests that a combination of Korean medicine treatments might be effective for postherpetic neuralgia. In particular, it shows that Kyung Hee Gongjin-dan and Hyangbujapalmul-tang-gagam can be therapeutic options in the treatment of postherpetic neuralgia.

韓.洋方 協診治療를 위한 三叉神經痛의 文獻的 硏究 (Study of literature on the Trigeminal Neuralgia for cooperative treatment of Oriental and Occidental medicine)

  • I examined and referred to literatures of every generations on the nicknames, causes, herb medications and acupucture treatments of ptosis(上胞下垂). And then the results were obtained as follows. We've compared and analyzed Occidental and Oriental medical causes, symptoms and treatments of Primary trigeminal neuralgia and wanted to get better effects by a cooperative analysis. So the examination and analysis of the recent treatment tendency and reference bibliography show the following results. 1. Trigeminal neuralgia is nerve systematic disease appearing in the distribution scope of trigeminal nerve. It's characterized by extreme pain accompanying with a repeated and simultaneous fit from several seconds to 1-2 minutes. 2. Though there are many hypothesis on the trigeminal neuralgia, but now many doctors agree that when trigeminal nerve is under the local out of sheath conditions resulting from receiving a chronic stimulus, and the nucleus of trigeminal nerve fire, owing to decrease of pain control function and abnormal occurrence of action potential, it would be appeared. 3. The Oriental medical name of trigeminal neuralgia is generally Dootong, Doopoong, Myuntong, Pyundootong, Pyundoopoong, and Myuntong is the nearest in Occidental medicine. 4. The Oriental medical cause of trigeminal neuralgia is usually divided into Wekam and Naesang. The first one is caused by Poonghan, Poongyul, Damhwa and wicked energy enter into the body, the mechanical energy is obstructed and can't move any more, so the pain appears by them. The other cause is the hurt by emotion. And it would be loss of the transportain of liver and obstructed, so result into Kanwulhwahwa, Kanpoongnaedong and the pain appears. 5. There are two methods of curing trigeminal neuralgia. As a medication, primary method is prescribing Carbamazepine and the second is using Phenytoin or Baclofen. And as a operation, Drug injection of trigeminal nerve, Amputation of branches of trigeminal nerve, Retrogasserian glycerol rhizotomy, Radiofrequency gangliolysis, Neurovascular decompression can be used. 6. There are several herb medicines for Trigeminal neuralgia. First, Chungung is good for Hwaejeetong, Keopoongjedam, Hwalhyuljeetong. Second, Jeongal, Jiryong, Okong is used for Sikpoonghekyung, Tongkyungjeetong. Third, Baekjee, Sesin, Cheonma, Manhyungja is efficacious in Sinonhepyo. Sanpoongjeetong. Fourth, for falling of liver's Wulhwa, Yongdamcho, Hyungge, Kukwha can be used. And also Saengjihwang, Hwangkm is good for going down the fever of Yangmyungwiyul and finally, Baekkangjam. Moryu can be effective for Jaumjamyang, Haekyungjitong. The other medicines can be used as assistant analgesics, and it also efficacious. 7. Generally the points of pain on the face and the points of Soyangkyung and Yangmyungkyung is used for Acupuntual therapy, because the two meridians passed on the face. Hakwan. Sabaek, Kwanryo, Keoryo, Hyubkeo, Taeyang, Jeechang, Younghyang, Eoyo, Chanjuk. Yangbaek. Sajukkong. Dooyoo, Kwangsangjum, Sengjang, Poongjee is used for taking near point and Joksamlee, Naejung, Habkok is used for taking distant point. 8. Dansam or Danggui injection which have a effect for Hwalhyulhwaeo, Sokyunghwalak and Vit B1, Vit B2, Vit B12, $2\%$ Hydrochloroprocaine, $1\%$ Lidocaine injection to pain point for local analgesics had so good effect. And external application and moxibustion are used for another treatment. 9. It proved that through mouse model, both Herb medication group and Drug medication group are efficacious for trigeminal neuralgia similarly and also the cooperative medication group shows more effective result than the only drug medication group.
