  • Chronic pelvic pain in women is a very annoying condition that is responsible for substantial suffering and medical expense. But dealing with this pain can be tough, because there are numerous possible causes for the pelvic pain such as urologic, gynecologic, gastrointestinal, neurologic, or musculoskeletal problems. Of these, musculoskeletal problem may be a primary cause of chronic pelvic pain in patients with a preceding trauma to the low back, pelvis, or lower extremities. Here, we report the case of a 54-year-old female patient with severe chronic pelvic pain after a transcutaneous electrical nerve stimulation (TENS) accident that was successfully managed with image-guided trigger point injections on several pelvic stabilizing muscles.

  • In this paper, the safety distances and maximum permissible power (MPP) of resonant wireless power transfer systems are defined and derived with regard to human exposure to electromagnetic field (EMF). The definition is based on the calculated induced current density and electric field in the standard human model located between the transmitting and receiving coil. In order to avoid the adverse health effects such as stimulation of nerve tissues, the induced current and electric field must not exceed the basic restriction values specified in EMF safety guidelines. The different combinations of diameters of the coils and the distance between the two coils are investigated and their effects are analyzed. Two versions of EMF safety guidelines (ICNIRP 1998 and ICNIRP 2010) are used as bases for safety distance calculation and the difference between the two guidelines are discussed.

  • Neuropathic pain is caused by functional abnonnalities of structural lesions in the peripheral or central nervous system, and occurs without peripheral nociceptor stimulation. Trigeminal neuropathy always pose differential location difficulties as multiple diseases are capablc of producing them: they can be the result of traumatism, tumors, or diseases of the connective tissue, infectious or demyelinating diseases, or may be of idiopathic origin. There are a number of mechanisms described as causing neuropathy. They can be described as ectopic nerve activity, neuroma, ephatic trasmission, change of sodium channel expression, sympathetic activity, central sensitization, and alteration in central inhibition systems. More than I mechanism may be active to create individual clinical presentations. In order to provide better pain control, the mechanism-based approach in treating neuropathic pain should be familiar to physicians.

A Case of Generalized Myasthenia Gravis Combined with Psoriasis (전신성 건선이 병합된 중증 근무력증 1예)

  • There have been several reports about coexistence of myasthenia and other autoimmune disease. Psoriasis is a papulosquamous disease defined by erythematous plaques with a silvery scale and a T-cell-mediated autoimmune disease. We report a case of a 49-year-old man with generalized myasthenia gravis (MG) superimposed by psoriasis. MG was diagnosed by clinical symptoms, increased acetylcholine receptor antibody titer and repetitive nerve stimulation test. Psoriasis was diagnosed by clinical manifestations and specific skin biopsy findings. MG and psoriasis are both autoimmune diseases. The coexistence of MG and psoriasis suggest a close connection of pathogenesis.

  • The nerve impulse is induced by the stimulation of neuron or axon, and this stimulated voltage decays along the propagation distance and time if it is subtreshold potential. This behavior can be estimated using the electrical equivalent circuit because it is very similar to propagation phenomenon of electrical circuit to which Ohm's law is applied. Therefore, I calculated various biometric parameters of body, and then analyzed the propagation behavior of stimulated potential voltage using the distributed parameters of electrical circuit in this paper.

  • Evidence based physical therapy interventions for this patient was applied in 15 clinical sessions. Considering the patient's two main impairments, two physical therapy interventions were delivered with WOMAC index outcome measurement. From this case report, I observed that a combined physical therapy interventions consisting of manual therapy, therapeutic exercise, and TENS Unit in patients with knee osteoarthritis may result in decreased pain, stiffness and increased physical function.

Effect of Sling Exercise on Muscle Activity and Pain in Patients with Rotator Cuff Repair

  • Purpose: This study was conducted to determine the effects of sling exercise on muscle activity and pain in patients with rotator cuff repair. Methods: This research evaluated 20 rotator cuff repair patients, divided randomly between 10 controls and 10 in an experimental group who performed a sling exercise. Both the experimental and control groups underwent transcutaneous electrical nerve stimulation and performed continuous passive motion. Muscle activity was measured with a surface electromyograph and pain was measured with the visual analogue scale. The paired t-test was used to compare groups before and after the experiment. The independent t-test was conducted to assess differences in the degree of change between the two groups before and after the experiment. Results: Within-group comparison of both the experimental and control group showed significant differences in muscle activity and pain. Comparison of the groups revealed significant differences in muscle activity between groups group, but not in pain. Conclusion: These results indicate that sling exercise is effective for improving muscle activity of rotator cuff repair patients.

  • Authors have experienced the treatment for the uppermost frequent cases, herpes zoster, frozen shoulder and low back pain among those who visited our pain clinic. 1. Herpes zoster The Patients who received the treatment within 4 weeks of the onset of the disease, not only healed herpes without leaving post herpetic neuralgia but also crust formation was fast. 2. Frozen shoulder Over 90% of the patients who received suprascapular nerve block accompanied with trigger points electric stimulation was very efficient procedure, futhermore, authors felt keenly the necessity each patients co-operation and individual endurance for this treatment. 3. Low back pain An epidural steroid administration to the patients who did not have any organic disturbances was effective treatment. If this non-effective, search other cause of the disease or operation was advisable one.
