당뇨병성 다발신경병증 환자에서 체감각유발전위검사 시 근위부 자극법의 적용 (Application of Proximal Stimulation for Somatosensory Evoked Potentials in Patients with Diabetic Polyneuropathy)

  • Background: Somatosensory evoked potential (SSEP) is valuable for the evaluation of the central pathway. However, peripheral neuropathy sometimes renders the test useless by preventing the conduction from reaching the CNS. We postulated that the peripheral conduction problems could be overcome by proximal stimulation in SSEP and wanted to verify this in the study. Methods: Twenty patients with diabetic sensorimotor polyneuropathy were included. SSEP was elicited by stimulating the median and posterior tibial nerves. We compared the effect of distal and proximal stimulations in each SSEP in the aspect of presence/absence and various latencies of resultant waves. Results: Among the 40 cases, proximal stimulation caused reappearance of subsided waves in 10 cases (25%). In the median nerve SSEP, proximal stimulation made EN1 and CN2 visible which were not evident when distally stimulated. In the posterior tibial nerve SSEP, there was also improvement of forming waves when proximally stimulated. Conclusions: In the diabetic polyneuropathy, proximal stimulation of SSEP is more effective than the conventional distal stimulation in evaluating central pathway.

Comparison of Temporal Bone Parameters before Cochlear Implantation in Patients with and without Facial Nerve Stimulation

  • Background and Objectives: Facial nerve stimulation (FNS) is a complication of cochlear implantation (CI). This study compared the thickness and density of the bone separating the upper basal turn of the cochlea (UBTC) and the labyrinthine segment of the facial nerve (LSFN) on preoperative computed tomography (CT) in patients with and without FNS after CI. Subjects and Methods: Adult patients who underwent CI from January 2011 to February 2017 with preoperative CT at a tertiary referral hospital were considered for this retrospective case-control study. Patients were divided into two groups: with FNS (n=4) and without FNS (n=53). The density and thickness of the bone between the LSFN and UBTC were measured on preoperative CT. Charts were reviewed for other parameters. Results: A statistically significant difference was seen in the thickness (p=0.007) but not in the density (p=0.125) of the bone between the UBTC and LSFN. Four patients had FNS at the mid-range electrode arrays, and one of them additionally had FNS at the basal arrays. Conclusions: Decreased thickness of the bone between the UBTC and LSFN can explain postoperative FNS, confirming the histologic and radiologic findings in previous studies, which indicated that the thickness of the temporal bone between the LSFN and UBTC is less in patients who experience FNS. While the density in this region was also less, it was not statistically significant.

  • Functional Electrical Stimulation is the clinical application of a small electric current to the intact nerves of the body, in order to trigger a muscle contraction. This contraction is then incorporated into a functional activity, for example walling. A clear distinction needs to be made between therapeutic stimulation and functional stimulation. The former being of an exercise orientation, where one would relax while the stimulation works on its own. Functional electrical stimulation on the other hand, incorporates this elicited muscle movement into an everyday activity, like standing, walking, reaching out etc. But recently, New method used to FES which updated electrode insult in muscle and small nerve branch. A lot of new research build up many countries such as England, USA, Japan, Spain, Canada. So I had been write this paper for introduce new FES method, and I hope to more enhanced motivation of therapist for the functional electric stimulation.

환경강화와 말초신경 전기자극이 뇌손상 백서의 기능회복에 미치는 영향 (The effect of Environmental Enrichment and Peripheral Nerve Electrical Stimulation on Functional Recovery after Brain in rats)

  • Purpose: To investigate and analyze effects of environmental enrichment(EE) and nerve stimulation that follows in application times with change of functional recovery(1, 3, 7 and 14 days). Methods: Focal ischemic brain injury was produced in 100 Sprauge-Dawley rats through middle cerebral artery occlusion(MCAO). Neurobehavioral assessment were selected, such as tilting plane testing, horizontal wire testing, vestibulomotor function testing and complex neuromotor function test, then they were randomly divided into five groups; Group I : Sham group, Group II: MCAO group, Group III: MCAO and ES group, Group IV: MCAO and EE group, Group V: MCAO and EE and ES group. Results: In neurobehavioral assessment, group V were significantly difference from other groups on between-subject effects. Conclusion: Our findings suggest that in focal ishemic brain injury, combined environmental enrichment and peripheral nerve electrical stimulation is more improved that the improvement of exercise function recovery than non treatment group.

The Effect of Frequency of Transcutaneous Electrical Nerve Stimulation (TENS) on Maximum Multi-finger Force Production

  • Objective: The purpose of this study was to investigate the effect of transcutaneous electrical nerve stimulation (TENS) treatment on maximum voluntary force (MVF) production. Methods: Ten healthy, young subjects (5 males and 5 females) participated in the study. MVF was recorded after a fifteen minute session of TENS stimulation under two conditions: low frequency (4 Hz) at maximum tolerable level and high frequency (110 Hz) at maximum tolerable level. TENS was provided simultaneously via self-adhesive electrodes placed on the finger pads of the index, middle, ring and little fingers. MVF was also recorded in a baseline condition with no TENS treatment. Data were collected in three different sessions on three consecutive days at the sametime of the day. Results: Results from the study show that on an average, MVF increasesby 25% for the index, middle and little fingers for TENS treatment with 4 Hz frequency as compared to the baseline condition. However, the 110 Hz condition did not result in a significantly different MVF than the baseline condition during individual finger pressing tasks. In addition, while producing MVF with all the four finger stogether, MVF was 30% higher for the 4 Hz conditionin comparison to the baseline condition, and 15% higher for the 110 Hz condition in comparison to the baseline condition respectively. Conclusion: The results suggest that stimulation ofafferent fibers onthe glabrous skinwith TENS could have a net facilitatory effect on the maximum motoroutput.

전기 자극으로 유발한 음경 발기력 측정 흰쥐 모델에 대한 HTE001의 발기력 상승효과 (Effect of HTE001, an Herbal Formulation, on Electric Stimulation-induced Penile Erection in Rats)

  • Objectives : This study was conducted to investigate the effect of HTE001, a multi-herbal mixture consisting of 10 herbs, Cornus Frutus, Schizandrae Fructus, Rubi Fructus, Cnidi Fructus, Acanthopanacis senticosi Radix, Cinnamomi Cortex, Eucommiae Cortex, Allii Bulbus, Rehmanniae Radix and Ginseng Radix, on electrostimulation-induced penile erection in rats. Methods : Intracavernous pressure (ICP) and mean arterial blood pressure (MAP) were simultaneously monitored through electric stimulation of the cavernous nerve after the oral administration of HTE001 (30, 100, 300 mg/kg) in normal rats. Statistical analysis was performed on maximal intracavernous pressure (ICP), maximal intracavernous pressure/mean arterial blood pressure (ICP/MAP) ratio, and the area under the curve (AUC) of ICP/MAP ratio. Results : Oral administration of HTE001 300 mg/kg caused the ICP to increase in a frequency-dependent manner. And HTE001 300 mg/kg treatment group showed the highest value in the ICP/MAP ratio and the AUC value of the ICP/MAP ratio compared to the control group at 2 Hz, 6 Hz and 10 Hz, respectively without an effect on the mean arterial blood pressure under the same stimulation of the cavernous nerve. Conclusions : These results show that HTE001 improve penile erection and prolong the decay period in normal rats without affecting mean arterial blood pressure, and suggest that HTE001 could be a good therapeutic candidate to treat erectile dysfunction.

슬관절 자극이 횡격신경 및 흡식중추신경에 미치는 영향 (Effect of Knee Joint Stimulation on the Activity of Phrenic Nerve and Inspiratory Nuron in the Cat)

  • 연구배경 : 생체의 운동은 움직임 자체를 수행하기 위한 근육, 관절 및 이를 관장하는 신경계와 운동중 적절한 산소를 공급하기 위한 심혈관계 및 호흡계의 조절이 동반되는 매우 복잡한 기전에 의하여 유지되고 있다. 이들의 상호관련성을 밝히기 위하여 많은 연구들이 진행중이나 특히 관절과 호흡중추의 연계성에 대하여는 아직도 연구가 미진한 편이다. 이에 본 연구는 관절의 정상적인 움직임이 호흡계에 어떠한 영향을 미치는지를 규명하고자 $\alpha$-chloralose로 마취한 고양이의 분당 호흡수, 횡격신경과 흡식신경섬유의 변화를 관찰하였다. 방법 : 26마리의 성숙한 숫고양이에서 슬관절 자극중 횡격신경 및 흡식중추신경의 활동성을 기록하기 위하여 쌍극백금전극과 탄소섬유전극을 이용하여 세포외에서 기록하였다. 슬관절을 자극하기 위하여 슬관절의 굴곡-신전운동, 슬관절 동맥을 통한 화학적인 자극 및 슬관절 신경의 전기적인 자극법 등을 사용하였다. 결과 : 슬관절의 정상 운동각도내에서 120 Hz.의 빠른 운동을 1분간 시킨 경우에는 분당호흡수와 분당호흡신경활동이 유의하게 증가하였으며 3분간 운동을 시킨 경우에는 호흡수, 일회호흡신경활동 및 분당호흡신경활동이 모두 유의하게 증가하였다. 그러나 60Hz.의 운동에 대하여는 전체적인 호흡활동에 뚜렸한 변화가 없었다. 과격한 운동중에 형성되는 젖산의 혈중농도와 같은 농도의 젖산을 슬관절내에 주입한 결과 주입후 30초 동안에 호흡수 이외의 일회호흡신경활동과 분당 호흡신경 활동이 유의한 증가를 보였다. 또한 potassium chloride에 대하여는 젖산의 경우와 유사한 반응을 보였으나 반응기간이 짧았다. 구심성 신경중 I, II군만을 선택적으로 흥분시킬 수 있는 강도인 1V로 슬관적 신경을 자극한 경우 자극기간에 한하여 호흡수를 제외한 일회호흡신경활동 및 분당호흡신경활동이 유의한 증가를 보였다. 또한 구심성 신경을 모두 흥분시키는 강도인 20V 자극에 대하여는 호흡계의 전체적인 항진효과가 있었으며 1V 자극에 비하여 매우 큰 반응을 나타내었다. 흡식중추의 신경세포에서 기록한 결과 횡격신경에서 나타난 반응과 유사한 결과를 얻었다. 결론 : 슬관절 구심성 신경중 제 I, II 군 신경의 흥분에 의하여 호흡중추는 직접적으로 활성화될 수 있으며 이러한 효과는 운동중의 슬관절로부터 중추로 유입되는 정보의 양에 비례하여 증가하는 것으로 생각된다. 따라서 이러한 사실은 슬관절의 정상적인 움직임에 의해서도 생체의 호흡기능이 항진될 수 있다는 것을 시사하고 있다.

  • Brachial plexus block (BPB) under ultrasound guidance has come to be widely used. However, nerve injury has been reported following ultrasound-guided BPB. We hypothesized that BPB under ultrasound guidance in conjunction with real-time electrical nerve stimulation would help us prevent nerve injury and do more successful procedure. Here, we report the successful induction and maintenance of ultrasound-guided BPB and the achievement of good peri- and postoperative pain control using a conductive catheter, the EpiStim$^{(R)}$.
