Transcutaneous electrical nerve stimulation for pain during propofol injection: a randomized clinical trial

  • Background: Propofol is a short-acting intravenous sedative widely used for procedural sedation and general anesthesia. However, pain during propofol injection is a distressing adverse effect. This study was designed to investigate whether transcutaneous electrical nerve stimulation (TENS) could reduce pain during propofol injection compared to sham TENS. Methods: In a randomized controlled trial, 80 patients were allocated to two groups: the active TENS group received electrical stimulation via two electrodes on the venous cannulation site, whereas the sham TENS group received no stimulus. After 20 min following TENS, propofol 0.5 mg/kg pain was injected intravenously and pain was evaluated using a four-point score (0 = none, 1 = mild, 2 = moderate, 3 = severe). Adverse effects associated with TENS were also recorded. Results: The overall incidence of pain during propofol injection was 47.5% in the TENS group and 87.5% in the sham group (P < 0.001). The incidence of moderate pain was significantly lower in the TENS group (7.5%) than in the sham TENS group (42.5%) (P < 0.001). There were no complications associated with TENS. Conclusion: Pre-treatment with TENS significantly reduced the incidence and intensity of pain during propofol injection.

키네시오 테이핑과 경피신경전기자극이 만성 뇌졸중 환자의 균형 및 보행기능과 삶의 질에 미치는 영향 (The effect of kinesio taping and transcutaneous electrical nerve stimulation on balance and gait function and quality of life in stroke patients)

  • 본 연구는 만성 뇌졸중 환자에게 키네시오테이핑과 경피신경전기자극을 결합 적용하여 단일방법과 비교하기 위함이다. 본 연구는 뇌졸중환자 29명을 대상으로 테이핑군(10명), 경피신경전기자극군(10명), 테이핑과 경피신경전기자극 결합한 결합군(9명)으로 나누었다. 중재기간은 4주간 주 5회, 30분씩 실시하였다. 평가에는 Timed Up & Go(TUG)와 Berg balance Scale(BBS)를 이용하여 균형평가를 실시하였고 보행 평가는 Functional Gait Assessment(FGA)와 10-metre walk test를 이용하였다. 삶의 질 평가는 EuroQol-5 dimension (EQ-5D)를 이용하였다. 연구결과 세 군 모두에서 TUG, BBS, FGA, 10 meter walk test, EQ-5D에 유의한 개선이 나타났다(p<.05). 세 군간 중재효과 비교에서 결합군이 나머지 두 군보다 FGA에 유의한 증가를 보였다. 본 연구를 통해 경피신경전기자극과 테이핑 결합이 단일적용 보다 FGA에 더욱 효과적인 것을 알 수 있었다. 하지만 FGA를 제외하고 다른 변수간에 차이가 없었으므로 앞으로의 연구에서는 더 많은 대상자 수와 중재기간이 필요할 것으로 사료된다.

  • The conduction velocity of the nerve is of importance to the diagnosis of various negromuscular disorders. A major technical problem encountered in nerve stimulation technique is the control of stimulus artifact and the convenience of nerve stimulator. The remained artifacts must be removed by processing the contaminated signal. This paper discusses about the artifact cancellation algorithms in constant voltage type nerve stimulator(CVS) and constant current type nerve stimulator(CCS).

경피적 전기 신경 자극의 적용 부위에 따른 자율신경계 활성도 변화 비교 및 평가 (Comparison and Evaluation of Autonomic Nervous System (ANS) Activity According to the Stimulus Position of Transcutaneous Electrical Nerve Stimulation (TENS))

  • Purpose: The aim of this study is to find the location of acupoints that are effective for increasing parasympathetic nervous system(PNS) activity among acupoints used to relieve symptoms such as dizziness and nausea. Materials and Methods: Twenty healthy adult men participated in this experiment. Transcutaneous electrical nerve stimulation(TENS) was applied to P6(Neiguan), ST36(Zusanli) and BL20(Pishu) for 30 minutes. For 5 minutes before and after stimulation, Electrocardiogram(ECG), Electrodermal activity(EDA), Respiration, and Skin temperature(SKT) were measured using biosignalsplux Professional(biosignalsplux; Portugal). Paired t-test was performed for the results before and after stimulation, and a one-way ANOVA test between stimulations was performed for the rate of change before and after stimulation. Results: When each acupoint was stimulated with TENS, it was found that the PNS was generally activated. In addition, when considering the change in biosignals after stimulation, the participants stimulated with P6 showed the most consistent results. Conclusion: As a result of stimulating each acupoint, it was found that stimulating P6 can effectively increase PNS activity. These results indicate that methods of stimulating P6 may be most effective in alleviating symptoms of motion sickness.

FNS를 사용한 하반신마비자의 일어서기 (Functional Neuromuscular Stimulation for Paraplegic Standing)

  • Functional neuromuscular stimulation(FNS)를 사용하여 하반선마비사가 앉은 상태에서부터 일어나 균형을 유지할 수 있도록 하기 위하여 FNS의 자동제어 시스댐을 개발하였다. Output feedback gain matrix를 최적제어이론을 사용하여 계산하기 위하여 비선형 선체모델을 직립위치에 대하여 선형화하였다. 제어기는 측정된 output에 의하여 매순간 각 선체조인트에 필요한 torque할 계산 하며, 이 조인트 torque는 근육이 최소한의 에너지를 사용하게끔 각 단육에 배분된다. 컴퓨터 시뮬레이션결과, 이 제어 시스템을 사용하여 각 조인트가 약 45도 정도 구부버진 상태에서 부터 직립위치를 회복할 수 있으며 팔의 움직임으로 대표되는 외부로부터의 방해를 반았블 때에도 직립 위치를 자동적으로 유지할 수 있었다.

고양이에서 근육감각신경 활성화로 유발된 승압반사 (Arterial Pressor Response Elicited by Activation of Muscle Afferent Fibers in the Cat)

  • This study was performed to investigate the mechanism of changes in arterial blood pressure, as a typical example of somatosympathetic reflex, induced by activation of muscular afferent nerves. Cats were anesthetized with ${\alpha}-chloraloae$ (60 mg/kg, i.p.). Afferent fibers in muscle nerve were activated by various method muscle contraction, electrical stimulation of muscle nerves, intraarterial injection of some algesic substances and noxious mechanical stimulation etc-and the evoked changes in arterial blood pressure were monitored. The effects of intravenous or direct spinal administration of morphine on the changes in arterial blood pressure induced by activation of the muscle afferent fibers were observed and also the effects of spinal lesions made in the $L1{\sim}L3$ spinal cord on them were studied to identify the ascending spinal pathways of the somatosympathetic reflexes. Followings are the results obtained. 1) The stimulation of medial gastrocnemius nerve under non-paralyzed condition with C-strength, low frequency (lower than 20 Hz) stimuli elicited a depressor response and a pressor response was elicited with C-strength, high frequency stimuli, of which the maximal response was observed at 100 Hz stimulation. 2) When the animal was paralyzed, depressor response to stimulation of the medial gastrocnemius nerve was observed with C-strength, $0.5{\sim}5Hz$ stimuli although the amplitude of the depressor response was decreased. The maximal pressor response was observed during stimulation with C-strength, $20{\sim}100Hz$ stimuli. 3) Intraarterial injection of some algesic substances induced marked pressor responses while noxious mechanical stimulation of the medial gastrocnemius muscle was not enough to elicit any significant changes (larger than 10 mmHg) in arterial blood pressure. 4) Systemically administered morphine (2 mg/kg) lowered the arterial blood pressure immediately and persistently and it was reversed by administration of naloxone. Direct spinally administered morphine did not elicit any changes. 5) The pressor response elicited by the activation of muscle afferent nerves was strengthened by systemic morphine administration while the depressor response tended to decrease. 6) Morphine administered on the spinal cord directly, decreased pressor response but did not change depressor response. From the above results it is concluded that there are separate groups of afferent nerves in the medial gastrocnemius nerve, which elicit pressor and depressor responses and the spinal ascending pathways of them are also separated from each other.

Neurogram을 이용한 인공와우 자극기법 평가 연구 (Evaluation of Stimulus Strategy for Cochlear Implant Using Neurogram)

  • Electrical stimulation is delivered to auditory nerve (AN) through the electrodes in cochlear implant system. Neurogram is a spectrogram that includes information of neural response to electrical stimulation. We hypothesized that the similarity between a neurogram and an input-sound spectrogram could show how well a cochlear implant system works. In this study, we evaluated electrical stimulus configuration of CIS strategy using the computational model. The computational model includes stochastic property and anatomical features of cat auditory nerve fiber. To evaluate similarity between a neurogram and an input-sound spectrogram, we calculated Structural Similarity Index (SSIM). The results show that the dynamic range and the stimulation rate per channel influenced SSIM. Finally, we suggested the optimal configuration within the given stimulus CIS. We expect that the results and the evaluating procedure could be employed to improve the performance of a cochlear implant system.

Neuromodulation for Atrial Fibrillation Control

  • Trigger and functional substrate are related to the tone of autonomic nervous system, and the role of the autonomic nerve is more significant in paroxysmal atrial fibrillation (AF) compared to non-paroxysmal AF. We have several options for neuromodulation to help to manage patients with AF. Neuromodulation targets can be divided into efferent and afferent pathways. On the efferent side, block would be an intuitive approach. However, permanent block is hard to achieve due to completeness of the procedure and reinnervation issues. Temporary block such as botulinum toxin injection into ganglionated plexi would be a possible option for post-cardiac surgery AF. Low-level subthreshold stimulation could also prevent AF, but the invasiveness of the procedure is the barrier for the general use. On the afferent side, block is also an option. Various renal denervation approaches are currently under investigation. Auditory vagus nerve stimulation is one of the representative low-level afferent stimulation methods. This technique is noninvasive and easy to apply, so it has the potential to be widely utilized if its efficacy is confirmed.

흰쥐의 좌골 신경 자극을 통한 광전 자극의 가능성에 대한 연구 (Feasibility of Optoelectronic Neural Stimulation Shown in Sciatic Nerve of Rats)

  • 본 연구는 외부 전원 없이 광다이오드만을 이용하여 생성한 광전 자극을 통해 신경계를 효과적으로 자극하는 방법에 대한 것이다. 광을 통한 전류원 생성 및 전달은 생체 내에 집적된 광소자를 삽입하고 외부에서 광을 통해 신호와 전력을 전달을 한다. 이 기술은 특히 '눈' 이라는 광학적인 연결통로를 이용할 수 있는 인공망막과 같은 시스템에 매우 효과적이다. 그러나 광전 소자를 내부 전원 없이 구동시키는 경우, 광전류가 생체 저항에 직접적인 영향을 받게 되므로 자극에 충분한 전류를 생성할 수 없다. 무 전원 광다이오드를 통해 생성되는 광전류를 신경 자극에 적용하기 위해서는 생체 저항의 크기에 관계없이 활동 전위 생성에 충분한 전류 공급을 할 수 있는 안정된 전류원이 필요하다. 이를 위해서 본 연구에서는 병렬 저항을 도입하였다. 병렬 저항 추가 시 생체 저항을 포함한 전체 저항 값이 낮아지므로, 광원의 세기에 따라 최대의 광전류에 근접한 값을 얻을 수 있게 된다. 그러나 병렬 저항 값의 크기를 낮출수록 자극에 쓰이지 않는 전류량이 늘어나므로, 자극 전류량의 극대 값을 찾기 위해서는 병렬 저항 값의 최적화가 필요하다. 실험을 통해 측정된 실제 자극 전류량이 최대가 되는 병렬 저항 값의 범위는 500Ω∼700Ω 이고, 이때 전류량은 580uA∼860uA 이며 전류 효율은 47.5∼59.7%이었다. 자극의 크기와 빈1도를 변화시키면서 쥐의 좌골 신경을 자극하여 눈으로 확인 가능한 떨림 현상을 확인하였으며, 다채널 기록기를 이용해 활동 전위를 측정하였다. 이를 통해, 인공 망막에서의 광 자극 가능성을 확인할 수 있었다.
