Carpal Tunnel Syndrome among Packing Workers in A Rayon Manufacturing Factory (모 사업장 포장부서 근로자들에서 발생한 수근터널증후군에 대한 조사연구)

  • The carpal tunnel syndrome is one of the most common peripheral nerve entrapment syndromes. The typical symptoms are pain, numbness and paresthesia in the median nerve territory of the hand. Recently, it is widely recognized that occupational factor is regarded as the important cause of the carpal tunnel syndrome. Clinical study is performed in the 42 female workers who is repetitively working at packing department in a rayon manufacturing factory from November 1991 till March 1992. The study included a questionnaire, physical examinations, and the neurophysiological test. The summary of the results obtained was as follows : 1. Among 42 packing workers, 9 workers(21.4%) were diagnosed as carpal tunnel syndrome by electromyography. The affected side was bilateral in 4 workers(9.5%), right in 4 workers(9.5%), and left in one worker(2.4%). 2. Among 42 subjects, 28 workers(66.7%) complained the clinical symptoms related to carpal tunnel syndrome, 11 workers(26.2%) showed positive Phalen sign, and 7 workers(16.7%) showed positive Tinel sign. 3. Researchers regard electromyographic finding as the gold standard for diagnosis of carpal tunnel syndrome. The sensitivity and specificity of the clinical symptoms to diagnose the carpal tunnel syndrome were 0.89, 0.39 respectively. If the carpal tunnel syndrome is diagnosed by the combination of the positive findings of the symptoms and the physical examinations, either Tinel or Phalen sign, the sensitivity and specificity were 0.67 and 0.76 respectively. Considering above results, though this small number of workers is not adequate for epidemiologic conclusions, carpal tunnel syndrome seems to be an important occupational disorder among packing workers in a rayon manufacturing factory.

A Clinical and Electrophysiologic Study of Carpal Tunnel Syndrome (수근관 증후군의 임상 및 전기신경생리 소견)

  • Carpal Tunnel Syndrome(CTS) is a common entrapment neuropathy of the median nerve at the wrist. An Electrophysiologic study has been widely used for the diagnosis of carpal tunnel syndrome. The subjects of this study were 48 cases (88 hands) with clinically suspected carpal tunnel syndrome who underwent electrodiagnostic examination from Jan 1, 2001 to Sep 30, 2001, The results were as follows: 1. Among 48 persons with a clinically suspected carpal tunnel syndrome, 40 patients were female 83.33$\%$ and the patients who are above 60 years old were 37.50$\%$. 2. Electrodiagnostic results were 22 cases (45.84$\%$) with bilateral carpal tunnel syndrome and 10 cases (20.83$\%$) with normal. 3. Physical findings consisted of tingling sensation in 48.86$\%$ of the involved hands, positive Phalen's Sign in 20.46$\%$ of them, thenar atrophy in 15.91$\%$ of them, and weakness in 14.77$\%$ of them. 4. Electrophysiologic studies showed a decreased sensory conduction velocity in 20 cases (22.73$\%$) of total hands, a prolonged latency in 3 cases (3.41$\%$) of them, abnormal sensory and motor fiber in 33 cases (37.50$\%$) of them, and normal in 27 cases (30.68$\%$) of them. Considering above results, we had better make a diagnosis precisely the patients with clinically suspected carpal tunnel syndrome through subjective symtoms, physical examinations, and electrophysiologic studies.

Clinical Practice Patterns for Carpal Tunnel Syndrome in Korean Medicine: An Online Survey (손목터널증후군의 한의 임상 진료 현황 조사를 위한 온라인 설문조사)

  • Objectives We conducted an online survey to understand the current status of Korean Medicine clinical practice related to carpal tunnel syndrome (CTS) to develop the CTS Korean Medicine Clinical Practice Guidelines (CPG). Methods A thoroughly reviewed questionnaire was distributed by e-mail to 21,719 Korean Medicine doctors between March 26 and April 30, 2021. All raw data were arranged and analyzed systematically. Results The response rate among the doctors was 4%. Of the respondents, 82.1% responded that it is necessary to develop the CTS Korean Medicine CPG. We also obtained data related to the specific diagnosis, treatment, and prognosis management methods used at various treatment sites for patients with CTS. Conclusions This online survey will be helpful in developing the CTS CPG, which will contribute to the standardization of guidelines and improvements in the reliability of Korean Medicine. Our findings will provide valuable data and resources for future clinical studies on CTS.

  • Periarticular cysts in the shoulder joint are relatively rare. The most common are paralabral cysts, which can cause suprascapular nerve entrapment syndrome. Acromioclavicular juxtaarticular cysts have been described in association with full-thickness rotator cuff tears and a degenerated acromioclavicular joint. Intramuscular cysts of the rotator cuff are a relatively rare and unknown type of periarticular cyst. We report a case of an intramuscular cyst of the rotator cuff that was associated with a tear of the rotator cuff.

A Case Report of Spinal Pharmacopuncture and Herb Medicine for Dry Mouth (구강건조증 환자에 대한 척추 약침 및 한약 치료 1례)

  • Xerostomia is a symptom of subjective complaints due to decreased salivary secretion or various other causes, which is common in about 30% of elderly people. Parasympathetic stimulants or artificial saliva are used as remedies, but this has side effects, and there are limitations in completely resolving symptoms. In this study, an 88-year-old woman with subjective dry mouth syndrome for the previous two to three years was treated with spinal pharmacopuncture and herbal medicine (Yookmigihwang-tang). The patient received pharmacopuncture at the thoracic 3rd, 6th, and 10th level twice a day and took herbal medicine three times daily for seven days. Both the degree of oral dryness and behavior were improved after treatment of Korean medicine, especially in the case of pharmacopuncture for inner core muscles.

Morphological types and morphometrical measurements of the suprascapular notch in both dry bones and human cadavers: anatomical study to improve the outcomes of the diagnostic and interventional procedures in the shoulder region

  • Understanding the anatomy of suprascapular area helps the clinicians and surgeons in management of any disability at the shoulder region. This work aimed to clear the different morphological and morphometrical types of suprascapular notch (SSN). Unknown 120 dry human scapulae of both sides and 60 formalin-embalmed cadaveric upper limbs (40 males and 20 females) were used in the present study. Three main morphological forms of SSN were reported: J, U, and V-shaped. J-shaped notch showed the highest incidence followed by U-shaped then V-shaped one. Morphometrically, type (III) notch was the most prevalent in both dry bones and cadavers, while the incidence of type (II) was the lowest form. Also, the measurements of superior transverse diameter, middle transverse diameter and vertical dimension of the different types of the notch showed no side or sex significant difference. The suprascapular foramen with ossified superior transverse scapular ligament (STSL) was seen in 5.8% of dry bones and 10% of cadaveric specimens. Fan and band-shaped ossified transverse scapular ligaments were reported. Absence of SSN was seen in 10.8% of dry bones, 7.5% of male and 10% of female specimens with left side predominance. V-shaped, absence, and ossified STSL were considered as predisposing factors of suprascapular nerve entrapment syndrome. Knowledge of the morphology and morphometric parameters of SSN is of great clinical significance for anatomists, radiologists, physiotherapists, orthopedics and neurosurgeons to perform good diagnosis and best planning for surgical or arthroscopic interventions within the shoulder region.

Effect of Ischemic Compression on Sternomastoidcleido Muscle on Chronic pain in Upper Trapezius Muscle (목빗근에 대한 허혈성 압박법이 위등세모근의 만성 통증에 미치는 영향)

  • Recently, incorrect and unhealthy postures have become increasingly prevalent due to reasons such as smartphone use. Consequently, imbalanced muscle contraction occurs. In particular, if the oblique muscle is contracted, the accessory nerve will be compressed, causing ischemic pain in the upper trapezius muscle. To investigate the effect on pain in the upper trapezius muscle, this study applied the ischemic compression method to the accessory nerve capture point at the 1/3-point of the oblique cervical muscle. In this study, the ischemic compression method was applied to eight women twice a week for four weeks, and pain was evaluated before, immediately after, and three weeks after application. The visual analogue scale, McGill Pain Questionnaire, and pressure dynamometer were used to evaluate subjects' pain. As a result of this study, it was confirmed that the pain was significantly reduced after treatment, and that the pain reduction was still effective three weeks after completion of the intervention, indicating that the intervention on the accessory nerve compression point on the oblique cervical muscle was both effective and long-lasting for pain reduction in the upper trapezius muscle. Future studies should increase the number of subjects to check not only pain reduction in the upper trapezius muscle, but also improvement of neck function.
