말초신경초 종양의 특징을 지닌 개 신경종의 조직병리학적 및 면역조직화학적 진단 (Canine nervous-tissue tumors with features of peripheral nerve sheath tumor: histopathological and immunohistochemical findings)

  • Canine peripheral nerve sheath tumors (PNSTs) are spindle cell tumors that arise from Schwann cells, perineural cells, fibroblasts or all of them. Based on the morphology and biologic behavior, PNSTs are divided into benign PNST (BPNST) and malignant PNST (MPNST) forms. The aim of this study is to diagnose the two cases of neoplastic tissue samples with features of PNSTs by the histopathology and immunohistochemistry. The study was performed using two specimens from small animal clinic. The first case, A was a mass, 3~4 cm in diameter, extruded from vaginal mucosa of 10-year-old spayed female mixed-breed dog. And the second case, B was a subcutaneous mass, 1.5 cm in diameter, which is originated from right hind leg of 9-year-old castrated male mixed-breed dog. Two cases were stained with hematoxylin and eosin (H&E) for histopathological examination. And also immunohistochemistry (IHC) was performed by the avidin-biotin peroxidase complex (ABC) method with antibodies specific for the following proteins: S-100 protein, smooth muscle actin (SMA) and epidermal growth factor receptor (EGFR). In results, Antoni B schwannoma pattern characterized by pleomorphic, round and fusiform polygonal cells was seen in A. In B, Antoni A pattern, densely packed spindle cells arranged in interlacing bundles was seen in addition to Antoni B pattern. In IHC, cytoplasms of neoplastic cells were diffusely labeled for S-100 expression in A and B. For SMA, both A and B show negative expression. And for EGFR, A shows negative expression but B shows partially positive expression in areas of Antoni B schwannoma pattern. The histopathologic features of two cases coupled with the S-100 immunoreactivity led to a diagnosis of PNST. For SMA, both A and B show negative expression. The diagnosis of A will be a BPNST with the negative result and B will be a MPNST with the positive result for EGFR.

갑상선 유두상미세암종에서 갑상선외 침윤, 다원성 및 경부전이에 대한 Matrix Metalloproteinase 2 및 Tissue Inhibitor of Matrix Metalloproteinase 2 발현의 의의 : 부정적 결과 보고 (Expression of Matrix Metalloproteinase 2 and Tissue Inhibitor of Metalloproteinase 2 in Papillary Microcarcinoma of the Thyroid Gland are Not Related with Extrathyroid Invasion, Multifocality and Lymph Node Metastasis of the Tumor)

  • Objectives: To investigate the role of MMP-2 and TIMP-2 in the invasion and metastasis of thyroid papillary microcarcinomas. Materials and Methods: We performed immunohistochemical study on MMP-2 and its tissue inhibitor (TIMP-2) using tissue microarrays containing 2 cores of 40 microPTC and 8 non-neoplastic thyroid tissue. The expression intensity was semiquantitatively scored as -, ${\pm}$, +1, +2, and +3. Results: Both MMP-2 and TIMP-2 expression was observed in all tumors(100%) and in 1 of 8 non-neoplastic tissue(12.5%), and the positive staining was restricted to the epithelial cells. In 17 and 23 tumors with or without extrathyroid invasion, respectively, 8(47%) and 10(43%) cases showed moderate to strong(+23) positivity for MMP-2. TIMP-2 expression was moderate to strong in 13 cases(76%) and 16 cases(70%) in each group. In multifocal and solitary tumors, 3 of 6(50%) and 11 of 21(52%) cases showed moderate to strong MMP-2 expression, and 5/6(83%) and 15/21(71%) showed moderate to strong TIMP-2 expression. Conclusion: There is no relationship between MMP-2 or TIMP-2 expression and extrathyroid invasion or tumor multifocality in papillary microcarcinoma of the thyroid gland.

갑상선 결절의 Ki67과 p27 발현도에 대한 분석 (Significance of Ki67 and p27 Reactivities in Various Thyroid Disorders)

  • Objective: The expression of Ki67, a proliferation marker, and p27, a cyclin dependent kinases(CDKs) inhibitor, has been studied in various human neoplasms. This study was carried out to determine whether these markers are useful in distinguishing benign from malignant lesions of the thyroid or predicting biologic behavior of malignant lesions. Material and Methods: Using immunohistochemical techniques with monoclonal antibodies to Ki67 and p27, we analyzed the expression of Ki67 and p27 in various thyroid disorders(25 follicular adenomas, 47 follicular carcinomas, 16 papillary carcinomas, 20 adenomatous goiters and 40 normal thyroid tissues). The labeling indices(LIs) were determined by counting cells expressing these markers in 1000 cells per immunostained slide. Results: Neoplastic thyroid diseases showed higher expression of Ki67 and lower expression of p27 than non-neoplastic diseases(p<0.05). The expression of p27 was significantly different between follicular adenomas($LI=55.4{\pm}5.7$) and follicular carcinomas($LI=23.2{\pm}10.2$). There was, however, no significant correlation between the degree of Ki67 and p27labeling indices and types of carcinoma or clinical aggressiveness of diseases. Conclusion: The degree of Ki67 and p27 expression was useful in distinguishing between benign from malignant thyroid lesions, particulary between follicular adenoma and follicular carcinoma, but was not directly proportional to the tumor aggressiveness.

고양이의 땀샘 선암종 증례 (Apocrine sweat gland adenocarcinoma in a cat)

  • A 11-year-old female mixed cat with subcutaneous mass around the left 5th mammary glands was presented to local animal hospital. According to history taking, the mass recurred 2 times on the same site of abdomen. After surgical excision, subcutaneous mass was referred to Pathology Department of Veterinary Medicine in the Jeju National University. Grossly, round to oval, milky yellow or pale red nodules, measuring 0.1${\sim}$1 cm in diameter, were occupied in the subcutis. Microscopically, the most neoplastic sweat glands were proliferated in the dermis and subcutis. Most tubules were lined by round to oval shaped epithelium with eosinophilic cytoplasm, hyperchromatic nuclei with high mitotic figures and severe central necrosis. The neoplastic epithelium also had periodic acid-Schiff-positive diastase-resistant cytoplasmic granules, but was negative for Perl's iron stain. Based on the gross, histopathologic and special staining, this cat was diagnosed as apocrine sweat gland adenocarcinoma. In our best knowledge, this is the first report of apocrine sweat gland adenocarcinoma around abdominal mammary gland in a cat.

고양이에서 수신증이 동반된 신선종 (Renal Adenoma with Hydronephrosis in a Cat)

  • 식욕부진 및 구토를 동반한 6세령 중성 수컷 고양이(domestic short hair)가 진료를 위해 지역 동물병원에 내원하였다. 복부방사선 및 초음파검사상 우측 신장의 종대 및 심한 수신증 소견을 나타내었다. 외과적으로 적출한 신장에 대하여 병리학적 검사를 실시하였다. 신장 단면절개 시, 2개의 유백색 종괴 및 심한 신우 종대가 확인되었다. 종괴의 대부분은 단일형태의 분화도가 높은 세관 구조로 이루어져 있었으며, 입방 내지 원주형 세포들이 한 층으로 배열되어 있고 내강에 유두상으로 돌출되어 있었다. 면역조직화학기법을 통해 종양세포는 cytokeratin (CK) MNF116에 대하여 강한 양성반응을 보였으나, CK 7에는 음성으로 나타내었다. 따라서 임상증상, 병리학적 소견 및 면역조직화학기법을 통하여 본 증례는 수신증이 동반된 유두상 신선종으로 확진되었다.

요크셔테리어에서 발생한 난소 유두모양샘암종 (Ovarian Papillary Adenocarcinoma in a Yorkshire Terrier Dog)

  • 10년령의 중성화되지 않은 암컷 요크셔테리어가 심한 복부확장과 출혈성 삼출물에 대한 평가를 위해 내원하였다. 복부 방사선과 초음파 검사상에서 삼출물과 무에코의 낭성 구성물 들을 지닌 복강 내 종괴가 확인되었다. 복수의 세포검사에서 공포화된 세포질을 지닌 종양성 외피 세포들과 현저한 핵들이 관찰되었다. 자기공명영상에서는 잘 분획되고, 이질성의 큰 종괴가 복강 내를 채우고 있었다. 부검 상에서는 우측 난소로부터 유래한 큰 종괴가 관찰되었다. 조직병리학적 검사에서는 종양성 외피세포들로 이루어진 유두모양 구조들로 종괴가 구성되어있는 것을 확인 할 수 있었다. 환견은 임상 및 조직병리학적 소견들에 근거하여 유두모양의 악성 난소 샘암종으로 최종 진단되었다.

미니어쳐 핀셔에서 신경증상을 동반한 폐션암 (Pulmonary adenocarcinoma with brain metastasis in a Miniature Pincher)

  • A 13-year-old female Miniature Pincher was euthanized after suffering from respiratory insufficiency and seizure. At necropsy, firm tan masses approsimately 4 to 5 cm in diameter were noted at the right caudal love and left cranial lobe of the lung. On cross sections of the cerebellum and the verebrum, several compressive firm round masses, 0.5 to 1 cm in diameter, were noted. similar looking nodules were also present in the mediastinal lymph nodes. Histopathologically, the neoplastic masses consisted of cuboidal shape epithelial cells that form glandular structure. The neoplastic cells have hyperchromatic nuclei with prominent nucleoli and moderate amount of cytoplasm. The degree of mitosis was high (4-6/400X). Multiple areas of necrosis, hemorrhage, and tumor emboli were also noted. Metastasis to the mediastinal lymph nodes, pancreas, and brain was confirmed. Based on the gross and histopathologic examinations, a diagnosis of primary metastatic pulmonary adenocarcinoma was made. Clinicaly recognized neurological signs were therefore due to brain metastasis.

생약복합제제(生藥複合製劑)와 항암제(抗癌劑)의 병용투여(倂用投與)에 관한 연구(硏究) (제2보)(第2報) -cis-Platin 및 Mitomycin C와 가미대보탕(加味大補湯)의 병용투여(倂用投與)에 의한 부작용(副作用) 경감(輕減) 효과- (Studies on Combined Usage of Combined Preparation of Crude Drugs and Anti-neoplastic Drugs(II) -Alleviative effect of cis-Platin and Mitomycin C through Combined Usage of Gamidaebo-Tang-)

  • The water extract of Gamidaebo-Tang depressed the lethal toxicity of cis-platin$(45{\mu}M/kg,\;s.c.)$, and mitomycin C (6mg/kg, s.c.). Significant depression of renal toxicity(indicated by an increased blood urea nitrogen value and creatinine value) of cis-platin $(35{\mu}M/kg)\;s.c.)$, was observed in mice treated with Gamidaebo-Tang. In rats treated with mitomycin C and Gamidaebo-Tang, the increased LDH activity and reduced total protein and albumin contents by mitomycin C were significantly depressed. The number of WBC was significantly increased in rats treated with mitomycin C but in the rats treated with mitomycin C and Gamidaebo-Tang, the number of WBC was increased and the hematocrit value and the number of RBC and Hgb were significantly increased in the dose-dependent manner. Therefore, bone marrow toxicity of mitomycin C in the rats treated with Gamidaebo-Tang was depressed. In the end, alleviative effects of the side actions of cis-platin and mitomycin C were acknowledged by combined usage of Gamidaebo-Tang and these anti-neoplastic drugs.

The Value of I-Scan Image-Enhanced Endoscopy in the Diagnosis of Vocal Cord Leukoplakia

  • Background and Objectives : Detection of vascular abnormalities in vocal cord (VC) leukoplakia is important for the diagnosis of neoplastic change of the mucosa. The aim of this study was to investigate the value of i-scan in the differential diagnosis of VC leukoplakia based on visualization of abnormal vascular features. Material and Methods : Fifty-two patients with leukoplakia were enrolled in the study. Images of their larynx obtained using conventional white light endoscopy and an i-scan-enhanced endoscopy (Pentax DEFINA EPK-3000 Video Processors, with Pentax VNLJ10) were reviewed. The microvascular features of the lesions and vascular changes were analyzed and the results were compared with the histopathologic diagnosis. Results : Among the 52 leukoplakia patients, 7 (13.5%) patients had squamous hyperplasia, 10 (19.3%) mild dysplasia, 2 (3.8%) moderate dysplasia, 14 (26.9%) severe dysplasia, 4 (7.7%) carcinoma in situ, and 15 (28.8%) invasive squamous cell carcinoma on histopathologic examination. Using i-scan-enhanced endoscopy, abnormal vascular change with neoplastic neoangiogenesis was detected in most cases of malignant VC lesion [severe dysplasia : 9/14 (64.3%), carcinoma in situ: 2/4 (50.0%), and invasive squamous cell carcinoma : 11/15 (73.4%)]. Conclusion : i-scan-enhanced endoscopy is a useful optical technique for the diagnosis of VC leukoplakia. Our results suggest that i-scan may be a promising diagnostic tool in the early detection of laryngeal cancer.

흉막의 여러 가지 양성 및 악성 종양 혹은 종양 같은 질환들의 영상 소견: 임상 화보 (Imaging Features of Various Benign and Malignant Tumors and Tumorlike Conditions of the Pleura: A Pictorial Review)

  • 흉막의 종괴는 다양한 양성과 악성 종양이 있고 종양은 아니지만 종양 같은 질환들에 의해 발생한다. 일차 흉막 종양에는 고립섬유종양, 악성중피종, 그리고 원발성 흉막 비호지킨 림프종이 있다. 흉막에 발생하는 가장 흔한 종양은 전이성 질환이며, 림프종, 백혈병, 다발성 골수종 등의 혈액 종양을 가진 환자에서 드물게 나타난다. 흉수는 주로 흉막의 악성 질환과 연관이 있다. 드물지만 흉막 종양이 만성 농흉에서 생길 수 있으며, 그중 가장 흔한 것은 비호지킨 림프종이다(농흉과 연관된 림프종). 종양이 아닌 흉막 종괴로는 다양한 양성 질환에서 보일 수 있으며, 여기에는 결핵, 석면 흉막판, 그리고 흉막 유리체가 포함된다. 이 임상화보에서 저자들은 흉막의 다양한 양성과 악성 종양 및 종양성 질환들에 대한 특징적인 전산화단층촬영 소견에 대해 알아보고자 한다.
