목적 : 침윤성 방광암 환자를 대상으로 TURBT (Transurethral Resection of Bladder Tumor)과 유도 항암화학요법(neoadjuvant MCV chemotherapy) 후 방사선 항암화학 병용요법(cisplatin chemotherapy and radiotherapy)을 받은 환자에서 치료의 적응도, 국소 반응율, 방광 보존율과 생존율을 알아보고자 하였다. 대상 및 방법 : 1990년 10월부터 1998년 6월까지 TURBT 시행 후 병리 조직검사 상 침윤성 방광 이행상피암으로 진단 받은 병기 T2-T4, Grade II 환자 21명을 대상으로 하였다. 치료과정은 TURBT 후 4주 간격으로 2회의 유도 MCV(methotrexate, cisplatin, vinblastine) 복합 항암화학요법을 시행하고 그 후 cisplatin과 방사선 동시 병용요법을 시행하였다. 방사선치료는 $6\~15$ MV X-ray를 사용하여 하루 1.8 Gy씩 $39.6\~45\;Gy$를 전 골반에 $4\~5$주간에 걸쳐 조사하였다. 치료 종료 후 방광내시경을 통해 관해율을 판정하여 완전 관해인 경우 $1\~2$주 후 보강 방사선조사를 cisplatin과 병용하여 19.8 Gy를 시행하였다. 부분 관해인 경우 근치적 방광 절제술을 예정하였고 수술이 불가능한 경우에는 항암제 병용 강화 방사선치료를 받도록 하였다. 환자의 추적관찰기간은 $34\~67$개월로 중앙값이 49.5개월이었다. 결과 : 총 26명의 환자가 유도 항암화학요법을 위해 등록되었으며 유도 항암화학요법과 방사선 항암화학 병용요법을 완료한 환자는 21명$(81\%)$이었다. 유도 항암화학요법과 방사선 항암화학 병용요법 후 방광내시경을 통해 완전 관해 판정을 받은 환자는 21명 중 7명$(33\%)$이었고 부분 관해 환자는 14명$(67\%)$이었다. 완전 관해 환자 중 강화 항암제 병용 방사선 19.8 Gy를 받은 환자는 7명 중 6명$(85\%)$이었고, 이들 중 5명$(14\%)$은 재발 없이 방광을 보존하였다. 부분관해 환자중 1명이 근치적 방광적출술을 받았으며 3명이 TURBT와 2회의 MCV 항암요법을, 10명이 추가의 항암화학 방사선 병용요법을 받았다. 근치적 방광적출술을 받은 1명은 수술 소견상 방광 내 종양이 존재했으나 수술 후 재발이 없었다. 전체 21명의 환자중 12명$(58\%)$이 방광을 보존하면서 생존하고 있고 8명이 방광암으로 사망하였으며 1명이 다른 질병으로 사망하였다. 전체 환자의 중앙 생존기간은 27개월이며, 5년 생존율은 $55\%$ 였다. 유도 항암화학요법과 항암제 방사선치료 후 완전 관해 환자의 경우 5년 생존율은 $80\%$, 부분 관해 환자 $14\%$로 의미 있는 차이를 보였다(p=0.001). 유도 항암화학요법과 항암제 병용 방사선치료를 받은 환자에서 grade 3 이상의 혈액학적 부작용의 빈도는 각각 백혈구 감소증 6명$(29\%)$, 혈소판 감소증 1명$(4\%)$이었으며 이로 인한 화학요법의 지연은 1명$(4\%)$에서 관찰되었다. 결론 : 침윤성 방광암 환자에서 유도 항암화학요법 및 항암제 병용 방사선치료를 적용하여 방광 보존을 및 5년 생존율에 있어 만족할 만한 결과를 얻었다. 완전한 TURBT와 유도 MCV 항암요법과 cisplatin 방사선 병용요법에서 완전관해 여부가 방광보존가능성과 생존율에 영향을 미치는 요소로 생각되고 만약 완전관해를 이루지 못하면 근치적 방광적출술을 시행하는 것이 바람직할 것이다.

Purpose: To investigate whether preoperative chemoradiotherapy (PCRT) followed by local excision (LE) is feasible approach in clinical T2N0 rectal cancer patients. Materials and Methods: Patients who received PCRT and LE because of clinical T2 rectal cancer within 7 cm from anal verge between January 2006 and June 2014 were retrospectively analyzed. LE was performed in case of a good clinical response after PCRT. Patients' characteristics, treatment record, tumor recurrence, and treatment-related complications were reviewed at a median follow-up of 49 months. Results: All patients received transanal excision or transanal minimally invasive surgery. Of 34 patients, 19 patients (55.9%) presented pathologic complete response (pCR). The 3-year local recurrence-free survival and disease free-survival were 100.0% and 97.1%, respectively. There was no recurrence among the patients with pCR. Except for 1 case of grade 4 enterovesical fistula, all other late complications were mild and self-limiting. Conclusion: PCRT followed by an LE might be feasible as an alternative to total mesorectal excision in good responders with clinical T2N0 distal rectal cancer.
Intracranial germ cell tumors (ICGCT) occur in 2-11% of children with brain tumors between 0-19 years of age. For treatment of germinoma, relatively low radiation doses with or without chemotherapy show excellent 10 year survival rate of 80-100%. Past studies showed that neoadjuvant chemotherapy combined with focal radiotherapy resulted in unacceptably high rates of periventricular tumor recurrence. The use of generous radiation volume which covers the whole ventricular space with later boost treatment to primary site is considered as standard treatment of intracranial germinomas. For non-germinomatous germ cell tumors (NGGCT), 10-year overall survival rate is still much inferior than that of intracranial germinoma despite intensive chemotherapy and high-dose radiotherapy. Craniospinal radiotherapy combined with cisplatin-based chemotherapy provides the best treatment outcome for NGGCT; 60-70% of overall survival rate. There is a debate on the surgical role whether surgery can contribute to improved treatment outcome of NGGCT when added to combined chemoradiotherapy. Because higher dose of radiotherapy is required for treatment of NGGCT than for germinoma, it is tested whether whole ventricular irradiation can replace craniospinal irradiation in intermediate risk group of NGGCT to minimize radiation-related late toxicity in the recent studies. To minimize the treatment-related neural deficit and late sequelae while maintaining long-term survival rate of ICGCT patients, optimized administration of chemotherapy and radiotherapy should be selected. Use of technically upgraded radiotherapy modalities such as intensity-modulated radiotherapy or proton beam therapy is expected to bring an improved neurocognitive outcome with longitudinal assessment of the patients.

Gastroesophageal cancer is one of the most common types of cancer worldwide. Despite significant developments in management, 5-year survival in the developing world is less than 20 percent. Due to restricted research about the impact of preoperative chemotherapy (POC) on tumor resection, pathological response and postoperative complications in Iran, we designed and implemented the present retrospective cross- sectional study on 156 patients with gastroesophageal cancer (GEc) between 2013 and 2015 at Shariati Hospital of Tehran. Two groups were included, the first group had previously received preoperative chemotherapy and the second group had only undergone surgery. All patients were followed for at least one year after the operation in terms of tumor recurrence, relapse free survival and one-year survival. The two groups were eventually compared regarding tumor resection, pathological response, postoperative complications, recurrence rate and survival. The mean age was $66.5{\pm}7.3years$ and 78 percent were male. The tumor resectability, pathological response and postoperative complications in the group which received POC were 93.5%, 21.8% and 12.8%, respectively, and in the surgery alone group figures for tumor resection and postoperative complications were 76% and 29.5%, respectively. Also based on our study the 5-year survival in the POC group was better (79.5% vs. 66.5%). Using standard neoadjuvant regimens (preoperative chemotherapy/chemoradiotherapy) beforesurgery could increase tumor resectability, pathological response, and improve the general status of the patients. Therefore using POC may be recommended over surgery alone.
Background/Objectives: The aim of this national population-based retrospective study was to analyze the status and treatment outcome in patients with hypopharyngeal cancer. Materials & Methods: Participants were included in the KNHIS national sample cohort who received a KNHIS health check-up in 2008 and 2009, and we followed these individuals until 2017. Patients were defined as having hypopharynx cancer if they had admissions records for hypopharynx cancer in their national health insurance data from 2010 to 2017. Results: The study cohort included 3,922 patients. According to our nationwide data, 3,533(90.1%) were male with a median age of 65.03±11.04 years at the time of diagnosis. Among parametric models for hypopharyngeal cancer prognosis, old age (Hazard ratio [HR]:1.92; 95% confidence interval[CI]:1.76-2.09), female (HR:0.77; 95% CI:0.66-0.89), and low socioeconomic status (HR:1.216; 95% CI:1.114-1.327) were significantly associated with survival. Compared with concurrent chemoradiotherapy, patients who received no treatment (HR, 1.88; 95% CI, 1.31-2.70), neoadjuvant chemotherapy followed by surgery (HR, 1.21; 95% CI, 1.04-1.41), and chemotherapy alone (HR, 1.16; 95% CI, 1.03-1.27) showed poor prognosis in hypopharyngeal cancer. Conclusion: Our data indicated that age, sex, and income were significant predictors of lifetime survival in patients with hypopharyngeal cancer. Treatment modalities were also associated with prognosis. The data have implications for treatment investigations and prevention strategies.
목적 : 비소세포폐암으로 근치적 방사선치료를 시행받은 환자의 생존율 및 예후인자를 알아보기 위하여 본 연구를 시행하였다. 방법 : 1992년 1월부터 1993년 12월까지 수술이 불가능한 비소세포폐암으로 원자력병원 치료방사선과에서 40Gy 이상의 근치적 방사선치료를 시행받은 62명의 환자에 대해 후향적으로 치료 결과를 분석하였다. 병기별 분포는 병기 IIIA 14예, 병기 IIIB 48예였다. 방사선치료는 일일 선량 1.8-2.0Gy씩 주 5회로 원발병소에 총 40.0-70.2Gy의 방사선을 조사하였다. 환자중 37예에서 유도 학학요법을 시행하였다. 결과 : 방사선 치료 후 반응은 완전 관해가 3예, 부분관해가 34예였다 그외는 무반응이였다. 전체환자의 중앙 생존기간은 11개월이었으며 1년 생존율은 45.0%, 2년 생존율은 14.3%였고 5년 생존율은 6.0%였다. 병기에 따른 1년 생존율 및 중간 생존기간은 각각 병기 IlIA에서 28.6%, 6.5개월, 병기 lIIB에서 50.3%, 13개월이였다. 단변수 분석에서 생존율에 영향을 미치는 예후인자로는 T 병기, 병기, 방사선 치료 직후의 반응이 통계학적으로 의미가 있었고(P<0.05) 연령, 성별, 병리p적 소견, N 병기, 쇄골상 림프절 전이 유무, 화학 요법 시행유무 등은 통계학적으로 의미가 없었다(P>0.1). 그외 치료전 전신상태는 통계학적 의미는 없었지만 생존율에 영향을 주는 경향을 보여주었다(0.05
목적 : 항문암의 치료에 있어 고전적으로는 복회음부절제술이 주된 치료였으나 현재는 화학방사선병용요법이 주된 치료방법으로 정립되었다. 저자들은 서울대학교병원에서 항문암으로 치료 받은 환자의 임상적 특성을 조사하고, 치료방법에 따른 치료성적과 예후인자를 분석하고자 하였다. 대상 및 방법 : 1979년 8월부터 1990년 7월까지 서울대학교병원 치료방사선과에서 근치적 또는 수술 후 방사선 치료를 받은 42명의 환자를 대상으로 후향적으로 분석하였다. 표피양암종이 38명으로 방사선치료가 4명에서, 복회음부절제술 및 수술 후 방사선치료${\pm}$화학요법이 19명에서, 화학방사선요법이 15명에서 시행되었다. 화학방사선요법은 복합화학요법$(5-FU\;1,000\;mg/m^2\;D1\~5,\;cisplatin\;60\;mg/m^2\;D1)$을 3회 시행 후 원발병소 및 영역림프절에 50.4 Gy를 조사하였고, 양측 서혜림프절에도 동일양을 조사하였다. 잔존암이 있는 경우 복합화학요법을 3회 추가 실시하였다. 중앙추적기간은 85개월이었다. 결과 : 전체 항문암 환자의 5년 생존율은 $80.3\%$이었다. 치료방법에 따른 5년 생존율은 복회음부절제술 및 수술 후 방사선치료${\pm}$화학요법군, 화학방사선요법군에서 각각 $88.9\%,\;79.4\%$이었으며 두군간의 생존율의 차이의 통계적인 의미는 없었다(p=0.495). 화학방사선요법을 시행 받은 환자군에서의 항문보존율은 $86.7\%$였다. 예후인자 중 단변량분석에서는 연령(p=0.0164)과 수행능력(p=0.0007)이 유의성을 보였으며, 다변량분석에서는 연령(p=0.0426)과 수행능력(p=0.0068) 및 서혜림프절 전이여부(p=0.0093)가 통계학적으로 유의하였다. 결론 : 항문암의 치료에 있어서 화학방사선요법을 시행할 경우 기존에 알려진 바와 같이 복회음부절제술과 유사한 생존율을 보이며, 항문기능을 보존할 수 있는 치료 방법임을 확인할 수 있었다. 나아가 병행화학요법이 아닌 선행화학요법을 시행하여 수반되는 합병증을 줄일 수 있는 가능성을 확인하였다.
Purpose: Advanced breast cancer traditionally has been perceived as a contraindication to immediate breast reconstruction, because of concerns regarding adjuvant treatment delays and the cosmetic effects of radiotherapy to breast reconstruction, so delayed reconstruction is usually preferred in advanced breast cancer patients undergoing mastectomy. However, with the improved outcome using multimodality therapy, consisting of perioperative chemotherapy and radiotherapy, immediate breast reconstruction is now being performed as surgical option for selected advanced breast cancer patients. Additionally, advanced breast cancer patients may be needed soft tissue coverage of an extensive skin and soft tussue defect after mastectomy. Current authors have experienced several types of immediate breast and chest wall reconstruction for advanced breast cancer. Methods: From December of 2007 to June of 2009, 14 women performed for immediate breast and chest wall reconstruction for advanced breast cancer. They had been treated with neoadjuvant chemotherapy or chemoradiotherapy followed by modified radical mastectomy or radical mastectomy. Four different techniques were used immediate breast and chest wall reconstruction, which are pedicled TRAM flap (4 cases), extended LD flap with STSG (3 cases), thoracoabdominal flap (4 cases) and thoracoepigastric flap (3 cases). Results: The mean age was 53 years and mean follow up period was 9 months. Patients' oncologic status ranged stage IIIa to stage IV. Two patients had major complications: partial flap necrosis of TRAM flap and one distal necrosis of thoracoabdominal flap. Three patients with stage IV disease died from metastases. Conclusion: The result of this study suggests that immediate breast and chest wall reconstruction can be considered as surgical option for advanced breast cancer. But we need long term follow up and large prospective studies for recurrence and survival.

Objective: To provide an evidence-based guide for the MRI interpretation of complete tumor response after neoadjuvant chemoradiation therapy (CRT) for rectal cancer using visual assessment on T2-weighted imaging (T2) and diffusion-weighted imaging (DWI). Materials and Methods: PubMed MEDLINE, EMBASE, and Cochrane Library were searched on November 28, 2019 to identify articles on the following issues: 1) sensitivity and specificity of T2 or DWI for diagnosing pathologic complete response (pCR) and the criteria for MRI diagnosis; 2) MRI alone vs. MRI combined with other test(s) in sensitivity and specificity for pCR; and 3) tests to select patients for the watch-and-wait management. Eligible articles were selected according to meticulous criteria and were synthesized. Results: Of 1615 article candidates, 55 eligible articles (for all three issues combined) were identified. Combined T2 and DWI performed better than T2 alone, with a meta-analytic summary sensitivity of 0.62 (95% confidence interval [CI], 0.43-0.77; I2 = 80.60) and summary specificity of 0.89 (95% CI, 0.80-0.94; I2 = 92.61) for diagnosing pCR. The criteria for the complete response on T2 in most studies had the commonality of remarkable tumor decrease to the absence of mass-like or nodular intermediate signal, although somewhat varied, as follows: (near) normalization of the wall; regular, thin, hypointense scar in the luminal side with (near) normal-appearance or homogeneous intermediate signal in the underlying wall; and hypointense thickening of the wall. The criteria on DWI were the absence of a hyperintense signal at high b-value (≥ 800 sec/mm2) in most studies. The specific algorithm to combine T2 and DWI was obscure in half of the studies. MRI combined with endoscopy was the most utilized means to select patients for the watch-and-wait management despite a lack of strong evidence to guide and support a multi-test approach. Conclusion: This systematic review and meta-analysis provide an evidence-based practical guide for MRI assessment of complete tumor response after CRT for rectal cancer.
목적 :. 자궁경부의 신경내분비암은 비교적 드문 질환으로 본원에서 광학현미경 및 면역조직화학적 진단을 받고, 치료받은 환자를 대상으로 임상적 특징, 예후 및 치료방침에 대해 알아보고자 한다 대상 및 방법: 1994년 5월부터 2001년 10월까지 본원에서 조직학적으로 자궁경부 신경내분비암으로 진단받은 환자 13예를 대상으로 하였다. 환자의 평균 연령은 56세(32$\~$78세)이며, 병기분포는 IB 5명, IIA 5명, IIB 3명이었다. 수술을 시행한 환자는 5명으로 이 중 3명은 수술 후 방사선치료를 추가하였다. 근치적 방사선치료를 시행한 환자는 9명이며, 이 중 1명은 방사선치료 후 근막외 자궁적축술을 시행하였고, 선행 혹은 동시항암화학요법을 시행한 환자는 9명이었다. 방사선치료는 병기에 따라서 전골반부 4,500$\~$5,400 cGy까지 외부조사 후, 강내조사를 주 2회, A point에 1회당 500 cGy로 3,000$\~$3,500 cGy를 조사하였다. 결과 : 추적관찰기간은 3개월에서 104개월로 평균 36개월이었다. 5년 생존율은 61.5$\%$이었으며, 병기별 5년 생존율은 병기 IB 60.0$\%$, IA 60.0$\%$, IB 66.7$\%$로 통계적으로 유의한 차이가 없었다(p=0.99). 전체 13명 중 5명(38.5$\%$)이 사망하였으며, 이 중 4명에서 골반부위 및 복부대동맥 주위 임파절 전이가 확인되었고, 이들 모두 원격장기 전이로 사망하였다. 사망자의 평균생존기간은 5개월이며, 임파절 전이 유무에 따라서 통계적으로도 유의한 차이(p=0.0001)를 관찰할 수 있었다. 나머지 8명은 무병생존중이다. 결론 : 자긍경부의 신경내분비암은 초기병기일지라도 임파절전이와 원격전이가 많고 병의 진행이 급속히 이루어지는 양상을 보이므로 국소치료인 수술이나 방사선치료와 함께 전신적인 복합항암화학요법을 병행하는 것이 좋을 것으로 생각되며 적절한 병합치료가 환자의 생존율을 향상시킬 것으로 생각된다.

③ 당 사이트는 회원으로부터 제기되는 의견이나 불만이 정당하다고 객관적으로 인정될 경우에는 적절한 절차를

제 11 조 (회원의 의무)
① 이용자는 회원가입 신청 또는 회원정보 변경 시 실명으로 모든 사항을 사실에 근거하여 작성하여야 하며,
허위 또는 타인의 정보를 등록할 경우 일체의 권리를 주장할 수 없습니다.
② 당 사이트가 관계법령 및 개인정보 보호정책에 의거하여 그 책임을 지는 경우를 제외하고 회원에게 부여된
ID의 비밀번호 관리소홀, 부정사용에 의하여 발생하는 모든 결과에 대한 책임은 회원에게 있습니다.

제 4 장 서비스의 이용

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는

- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
