Managing large infected midline abdominal defects are clinically challenging and technically demanding. The alloplastic materials, regional flaps, and component separation are usually infeasible because of the size, location, depth, and state of the defects. In these cases, the free flap is the only option with a large well-vascularized tissue that is free to inset regardless of the location. Herein, we report a case of 44-year-old man with a large infected midline abdominal wall defect who was completely treated with a latissimus dorsi myocutaeous free flap followed by negative pressure wound therapy.

The radiographic diagnosis of osteomyelitis can be suspected early with deep soft tissue swelling, but the actual bone changes of osteomyelitis may be delayed as long as 10 to 14 days after onset or may be totally aborted by antibiotic therapy. Recognition of osteomyelitis by bone imaging is far more rapid than by conventional radiographic examination and can be used on admission to establish the diagnosis. Ten patients suspected of having early, acute osteomyelitis were studied by TC-99m Pyrophosphate bone imaging. Radiographs taken at the same time were all negative. Of these 9 patients showed positive bone images. The bone imaging provides a safe, accurate, noninvasive technique for the early diagnosis of osteomyelitis.

Purpose: The purpose of this study was to investigate sexual life in men after stroke Methods: The subjects were 60 post-stroke patients who visited a convalescent center for disabled people in Seoul, Korea. The subjects were discharged from the hospital and received physical therapy. Six items of sexual relationship were used to measure sexual life after stroke. Results: The most common sexual changes identified after stroke included decline in satisfaction, libido, coital frequency, satisfaction, and ability to perform sexual activities. Only 20.0% of the subjects reported that they discussed sexual life with health professionals. Conclusion: The change in sexual life was perceived as negative effects on their attitude toward sexuality. Some attitudes or improper beliefs that could negatively influence sexual readjustment after stroke nay be corrected through adequate counseling. Nurses should present not only accurate information regarding sexual concerns but also they can provide health education and sexual counseling in their practice.

저자는 한약 복용, 침을 이용한 저주파 전기자극 치료, 이침요법, 부항요법, 행동수정요법, 식이 및 운동지도 등을 중심으로 한 기존의 비만 치료 방법으로 체중 및 체지방 감소를 시도했으나 실패한 경험이 있는 환자를 대상으로 새로운 치료 기술인 약주요법을 위주로 한 프로그램을 적용하여 체중, 체지방률, 복부비만도 등의 지표에서 좋은 경과를 보였으며 기존의 치료에 비해 치료 만족도가 높았던 증례를 경험하였기에 보고하는 바이다.

Progressive pigmented purpuric dermatosis(Schamberg's disease, purpura simplex) is an uncommon eruption characterized by petechiae and patches of brownish pigmentation, particularly on the lower extremities. Lesions remain for months or years and present only a cosmetic problem. there is no hematologic disease, venous insufficiency, or associated internal disease. The most characteristic feature is orange brown, pinhead-sized "cayenne pepper" spots. It is hard to find similar disease in Oriental Medicine, however it could be though related with 瘀血. We observed and treated a 25 old female with progressive pigmented purpuric dermatosis on her lower extremities, without pain and itching sign. About 1 year after our treatment, herb-medication. acupuncture treatment, negative therapy and applied aroma oil in order to remove the 瘀血(a kind of congestion) & inner heat and promote the circulation of her blood, the area of pigmented purpuric dermatosis was decreased remarkably and the colour was lighter. She is been treated continuously now and satisfied with the efficacy of treatment.

Systemic lupus erythematosus is a disease of unknown etiology that affects many organ system and is characterized by the presence of multiple autoantibodies that participated in immunology mediated tissue injury. A 36 years-old female patient was admitted to ward due to high fever and erythematous rash on face and hole body. She exhibited itching sense, joint pain, nausea, fatigue, sensitivity to light. The homatologic finding revealed anemia, decrease of lymphocyte, low platelet count, but LE cell, Anti nuclear antibody(ANA) were negative. In the point of Differentiation of Syndrome(辨證), SLE can be thought to be a category of Seasonal febril disease(溫病). This subject diagnosed as Domination of intense evil heat(熱毒熾盛), and was administrated with Chungonpadocyem-gamibang(淸瘟敗毒飮加味方). The clinical and laboratory findings of our patient were improved by herb medication, acupuncture therapy and general supportive measure.

Tumor cell invasion and metastasis are a complex multistep process that involves the degradation of extracellular matrix proteins by matrix metalloproteinases (MMPs). Tissue inhibitor of metalloproteinase-1 (TIMP-1) acts as a negative regulator of matrix metalloproteinase and thus prevents tumor cell invasion and metastasis by preserving extracellular matrix integrity.(omitted)

Purpose: Deficits in lower-extremity function and trunk control ability have a negative impact on individuals with hemiplegia. This case report aimed to describe the effect of trunk stability exercises using proprioceptive neuromuscular facilitation (PNF) on trunk control ability, balance, and gait in a patient with hemiplegia. Methods: A 77-year-old man with hemiplegia and trunk and lower extremity impairment participated in this four-week training intervention. Results: The patient demonstrated improvements in trunk control ability, balance, and gait performance. Outcome measures (Fugl-Meyer Assessment Lower Extremity (FMA-LE), Trunk Control Test, Berg Balance Scale, Timed Up and Go test, 10 Meter Walk test) were measured before and after the training program. Conclusion: The results of this case suggest that a trunk stability exercise using a PNF program may improve trunk control ability, balance, and gait in a patient with hemiplegia.

Pleomorphic liposarcoma is extremely rare in the head and neck region. It can be diagnosed by identifying multivacuolated lipoblast with pleomorphism. Most studies have reported that pleomorphic liposarcoma shows an aggressive behavior with poor prognosis. The treatment of choice is wide resection to ensure negative margin. The authors report a case of pleomorphic liposarcoma in a 67-year-old female, who was treated with primary resection of tumor and postoperative radiation therapy. The patient is well without any recurrence or metastasis after 1 year of follow-up.
