Three Cases of Chronic Adult Atopic Dermatitis (만성 성인형 아토피 피부염의 한방 치험 3례)

  • Introduction: Atopic dermatitis is chronic eczematous dermatosis which is related strongly to a family history of atopic disease and hereditary disposition. Its threshold of itching is low so that severe itching occurs and causes secondary eczema. Adult atopic dermatitis has mostly severe symptoms and progresses to chronic relapsing. Objective: The purpose of this study was to estimate the efficacy of oriental medical treatment and management on chronic adult atopic dermatitis. Subject: We studied 3 patients who visited Dongguk University Gangnam Oriental Hospital Dept. of Dermatol. with chronic adult atopic dermatitis from August 2001 to April 2002 and were observed for over 4 months. Methods: We treated them with herb-medication, acupuncture, negative therapy, aroma therapy, full spectrum visible ray therapy(by carbon arc) and external application(Jaun-go). We used the SCORAD index to assess the severity of atopic dermatitis. Results: There were 3 patients with chronic adult atopic dermatitis we could check for over 4 months by clinical scoring using the SCORAD system. 1. The grades of the severity of atopic dermatitis were lower only by the oriental medical treatments and management. 2. Intensities of relapse cases were lower then before the oriental medical treatments. 3. The secondary treatment term for relapse cases was shorter then the first treatment term after oriental remedies were used. Conclusion: Considering the above results, more study is needed on the chronic relapse cases with adult atopic dermatitis, the factors of increasing seventy of adult atopic dermatitis, the prognostic factors of adult atopic dermatitis, and the clinical interchange of chronic adult atopic dermatitis between oriental medicine and western medicine.

Meralgia Paresthetica Treated with Acupuncture Plus Myofascial Release Technique: Case Report (대퇴신경지각이상증 환자에 대한 침치료 및 근막이완요법 병용 치험 1례)

  • Objectives : The purpose of this case is to report the improvement after treatment about a patient with meralgia paresthetica. Methods : We treated the patient with acupucture therapy, cupping therapy, electroacupuncture therapy, percutaneous radiofrequency thermoablation and myofascial release technique by Turbo SASO from $26^{th}$ June 2015 to $3^{rd}$ July 2015 by evaluating femoral function with VAS score. Results : After 5 times of treatment, this patient achieved excellent outcome following the technique, showing that clinical symptom as able to walked and pain was disappeared, VAS changed from 10 to 3 and the result of patrick test came out negative. Conclusions : The various symptoms appear in the Meralgia paresthetica such as numbness, paresthesia, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve(LFCN). Oriental medical treatment for meralgia paresthetica resulted in satisfactory results by diminishing the symptoms progressively during the five days of treatment. Differential diagnosis was based on careful physical examination. More research of meralgia paresthetica is needed.

  • This study examined 19 papers published from 1977 to 2000 based on the objective frame evaluation. This purpose of this study was to examine the trend of Hwabyung research and to serve as a guide for the future 'Hwabyung' study. The results of studies were as follow. In the design of research, clinical studies are definitely dominant. As for the fields of research, the concept of Hwabyung was studied more frequently than the others. As for the authorship. psychiatrists prevailed but nursing professionals are on the increase. Research subject in clinical and survey study, patient was definitely dominant and the place of study was almost always a hospital (75%), data collection was higher on interview (58.3%). Hwabyung was considered unique culture bounded syndrome related to Korean culture. Frequent etiologic factor of Hwabyung were a husband's extra-marital affair, conflicts between houses wives and mothers-in-laws, and financial loss and suffering. From the incidence of Hwabyung, a greater number of patients with Hwabyung were middle aged women in the low economic and educational classes, and these were connected with the culture and the family system. The symptoms of Hwabyung included psychological and physical symptom, neurological disorder and disease behavior. Defense mechanisms and coping strategies for Hwabyung were somatization, suppression, orality, withdrawal, isolation, regression, help-seeking, complaining, and wreaking anger. Treatment of Hwabyung were medication, effort by herself, communication with families, consultation with psychologist, acupuncture, negative therapy, moxa, and Qi-kong. Psychiatric therapy, behavior therapy, nursing intervention on multi-interdisciplinary approach and psychiatric nursing approach were recommended for the nursing care of a Hwabyung patient.

The Result of Radiation Therapy of Vocal Cord Cancer for 15 Years (성문암 방사선치료 15년 성적)

  • To assess the result of radiation therapy for fifteen years experience, a total of 81 cases of pathologically proven vocal cord cancer had been analysed according to patient survival retrospectively. All the patients had been treated with radiation therapy using Co-60 teletherapy unit in curative aim. The results are as follows ; 1. According to AJCC staging, (ive year survival rate was $75.0\%$ in stage I, $73.1\%$ in stage II, $36.0\%$ in stage III, and $20.0\%$ in stage IV. 2. According to T-staging, five year survival rate was $75.0\%$ in T1, $73.1\%$ in T2, $24.3\%$ in T3, and $25.0\%$ in T4. 5, According to nodal status, five year survival rate was $59.4\%$ in negative node group and $14.2\%$ in positive group. 4. According to the histologic grade, the better in differentiation, the more in number of cases and the better in prognosis. 5. In summary, five year actuarial survival rate was $55.5\%$ and ten year survival rate was $49.8\%$ and ten year survivors totalled 12 cases.

Inter-rater·absolute Reliability and Concurrent Validity of Tinetti-gait Scale (Korean version) in Stroke Patients (뇌졸중 환자들의 한국판 Tinetti-보행 척도의 측정자간·절대적 신뢰도와 동시 타당도)

  • PURPOSE: The purpose of this study was to investigate the inter-rater absolute reliability and the concurrent validity of the Tinetti-gait scale that was translated into Korean for chronic stroke patients. METHODS: Fifty-two patients with post-stroke participated in this study. Inter-rater reliability was analyzed by Intraclass Correlation Coefficient ($ICC_{3,1}$) and Kappa coefficient, and absolute reliability was analyzed by the Standard Error of Measurement (SEM), and the Minimal Detectable Change (MDC). Concurrent validity was analyzed by correlating between the Tinetti-gait scale and physical functions. The physical functions were measured by using the Dynamic Gait Index(DGI), 10m walking test(10WT), One Leg Standing Test of affected/non-affected side(OLST), Sit to Stand test(STS), (Fugl Meyer assessment of Lower Extremity(FM-LE). RESULTS: The inter-rater reliability of the Tinetti-gait scale was high; $ICC_{(3,1)}=.91$ (95% CI=.85~.95) (very reliable), the range of Kappa coefficient were .73~.92 (substantial~good). The inter-rater agreement of the each item in Tinetti-gait scale ranged from .74 to .92 (95 % CI=.59~.95) (reliable~very reliable). The SEM and MDC were .56 and 1.55, respectively. In the results of concurrent validity, there were moderate positive correlation between Tinetti-gait scale and DGI (r=.78), 10WT (r=.74), OLST (r=.65~.73), FM-LE (r=67). And there was moderate negative correlation between Tinetti-gait scale and STS (r=-79) (p<.01). CONCLUSION: The Tinetti-gait scale(Korean version) was a reliable and valid tool to measure gait ability in patients with chronic stroke. Thus, it could be a useful tool for examining a gait ability of post-stroke patients. Further study should be conducted to investigate the usability to predict fall risk of post-stroke patients of the Tinetti-gait scale.

Factors Associated with Attendance in a Nutrition Education Program for Hyperlipidemic Patients

  • This study was conducted to examine factors with attendance in hyperlipidemia nutrition eduction program among 101 hyperlipidemic outpatients (38 males 63 females) at Samsung Medical Center We employed the Health Belief Model (HBM) as the theoretical framework The individual nutrition education and counseling program was scheduled with 4- half hour session sin 2 to 4 weeks intervals. Upon initiation of the program a trained dietitian surveyed HBM constructs and psychosocial factors. The following were included perceived susceptibility to cardiovascular disease(CVD) perceived severity to CVD percieved benefits to diet modification perceived barriers to persistence in maintaining therapy and self efficicacy and social support from family Sociodemographic data health factors stress level nutrition knowledge, and 24-hour dietary recall behavior were also surveyed All these data was analyzed according to the number of nutrition sessions attended The subjects were 55.9$\pm$9.4 year old and 24.6$\pm$kg/㎡ Sociodemographic factors were not associated with the number of nutrition sessions attended HBM constructs and psychosocial factors were significantly associated with the number of nutrition sessions attended. According to Spearman correlation coefficients. From stepwis regression analyses using HBM constructs as independent variables perceive barriers to persistence in maintaining diet therapy (negative) proved to be the strongest predictors for the number of nutrition sessions attended (partial R$^2$= 72.3%) followed by perceived severity to CVD and self efficacy (model R$^2$=76.6% The findings indicate that HBM constructs and psychosocial factors were closely associated with patient attendance It suggests that information and guidance to minimize patients perceived barriers to diet therapy might help to improve patients to scheduled appointments in nutrition education programs.

Thermal-Induced Osteonecrosis of Adjacent Vertebra after Intradiscal Electrothermal Therapy

  • A 42-year-old man was admitted to our hospital with complaints of low back pain and intermittent right thigh pain. Twelve weeks before admission, the patient received intradiscal electrothermal therapy (IDET) at a local hospital. The patient still reported low back pain after the procedure that was managed with narcotic analgesics. Follow-up magnetic resonance imaging (MRI) was performed, and his referring physician thought the likely diagnosis was spondylodiscitis at the L4-5 spinal segment with a small epidural abscess. At admission to our department, the patient reported aggravated low back pain. Blood test results, including the erythrocyte sedimentation rate and C-reactive protein levels, were slightly elevated. Biopsy samples of the L4, L5 vertebral bodies and disk were obtained. The material underwent aerobic, anaerobic, fungal, mycobacterial cultures and histologic examination. Results of all cultures were negative. Histologically, necrosis of the bone was evident from the number of empty osteocyte lacunae. In addition, there was no evidence of infection based on biopsy results. No antibiotic treatment was administered on discharge. Repeat computed tomography and MRI performed 12 months after IDET showed a bony defect in the L4 and L5 vertebral bodies, and a decrease in the size of the L4-5 intervertebral disc lesion. We report a case of lumbar vertebral osteonecrosis induced by IDET and discuss etiology and radiologic features.

Changes in Pain Following the Different Intensity of the Stretching and Types of Physical Stress

  • Background: Both the rapid concentric and eccentric contractions during exercise repeatedly impose excessive stress on muscle tissue. The hamstring muscles are very susceptible to injury due to the tensile stress. Various interventions are currently being undertaken to prevent strain injury before exercise. Stretching is the most common method and is known to have a positive effect on flexibility and muscle performance. However, relatively few studies have investigated the potential negative factors of stretching. Objects: The purpose of this study was to examine changes in pain following the different intensity of the stretching and types of physical stress. Methods: The subjects were divided into three groups based on the intensity of stretching: 100% (S100), 75% (S75), and 50% (S50) of the measured force at the point of discomfort in static stretching and 100% (P100), 75% (P75), and 50% (P50) of the maximum voluntary isometric contraction in Proprioceptive Neuromuscular Facilitation (PNF) stretching. The pain individual subjects perceived after stretching was measured via a Visual Analog Scale (VAS) and compared between the groups Results: Despite the decrease in the intensity of static stretching, no decrease in VAS value was observed. In PNF stretching, a significant decrease was observed at P50 compared to P100. S100 was significantly higher than P75 and P50. Conclusion: Previous studies have shown that PNF has a superior or the same effect on flexibility in comparison with static stretching. This effect was maintained even in moderate intensity. PNF stretching performed under moderate rather than high intensive static stretching, which causes pain and discomfort, might be recommended in clinical settings.

The effects of a group cognitive behavioral therapy program using video communication for pregnant women with depressed mood in Korea: a pilot study

  • Purpose: This study presents the development of a group video communication-based cognitive behavioral therapy (CBT) program for depressed pregnant women. It also provides the results of a preliminary test of its effects on their depression, automatic thoughts, and dysfunctional attitudes. Methods: In this quasi-experimental single-group pre- and posttest design study, 13 pregnant women participated in a 4-week, eight-session group CBT program, based on Beck's cognitive theory and using video communications from November 2020 to January 2021. Pregnant women between 14 and 32 weeks who were members of an online maternity and parenting community and residing in the cities of Changwon and Gimhae, Korea, were invited to voluntarily participate. Trained nurses led CBT sessions of 3-4 participants per group via video communication. Participants were assessed pre- and postintervention with self-report questionnaires for measurement of depression, automatic thought, and dysfunctional attitude after normality test according to the Shapiro-Wilk test of the variables. The data were analyzed using paired t-test and Pearson correlation coefficients. Results: Depression (t=7.90, p<.001), automatic thoughts (t=4.89, p<.001), and dysfunctional attitudes (t=2.42, p=.032) significantly decreased after the 4-week online program. There were statistically significant correlations among the three variables. Conclusion: This program was found to be effective in reducing depression, automatic thoughts, and dysfunctional attitudes. Above findings suggest that a group CBT program using video communication can be an effective therapeutic modality that helps pregnant women at risk for depression alleviate their negative emotions related to depression.

Effect of Sensory Feedback Type on Correct Sitting Posture Learning on Healthy Adults (감각 되먹임 종류가 건강한 성인 남성의 올바른 앉은 자세 학습에 미치는 영향)

  • PURPOSE: The growing number of people exposed to a static sitting posture has resulted in an increase in people with a poor posture out of the optimally aligned posture because of the low awareness of a correct sitting posture. Learning the correct sitting posture by applying sensory feedback is essential because a poor posture has negative consequences for the spine. Therefore, this study examined the effects of the sensory feedback types on learning correct sitting posture. METHODS: Thirty-six healthy adult males were assigned to a visual feedback group, a tactile feedback group, and a visuotactile feedback group to learn the correct sitting posture by applying sensory feedback. The spine angle, muscle activity, and muscle thickness were measured in the sitting position using retro-reflexive markers, electromyography, and ultrasound immediately after, five minutes, and 10 minutes after intervention. RESULTS: The intervention time was significantly shorter in the visuotactile feedback group than the visual feedback group (p < .05). Compared to the pre-intervention, the repositioning error angles of the thoracic and lumbar vertebrae of all groups were reduced significantly immediately after intervention and after five minutes. After 10 minutes, there was a significant difference in the thoracic and lumbar repositioning error angles of the tactile feedback group and the visuotactile feedback group (p < .05). No significant difference was noted at any time compared to the pre-intervention in all groups (p > .05). CONCLUSION: The use of tactile and visuotactile feedback in intervention to correct the sitting posture is proposed.
