AN EXPERIMENTAL STUDY OF THE EFFECT OF THE METHYLCHLOROTETRACYCLINE ON GROWTH OF CORONAL SUTURE IN RATS (Demethylchlorotetracycline이 백서의 관상봉합 발육에 미치는 영향에 관한 실험적 연구)

  • To study some informations on the morphogenesis and developmental process of the coronal suture in rats, the author performed daily oral administrations of demethylchlorotetracycline, a kind of tetracycline group, in the amount of 30mg/kg of body weight to the female rats from the 7th day of pregnancy to the time of delivery. Microscopic evaluation was undertaken on the fetal rats in the experimental group. The subject of this experiment were defined to the fetal rats of each group at 1st, 3rd, 7th and 14th day after birth. All these fetal rats were sacrificed and the heads were removed. All the tissue sections were fixed with $10\%$ formalin, Bouin' and Carnoy' solution and then stained by Hematoxylin-Van Gieson stain, or Feulgen and Rossenbeck, Periodic acid Schiff, and prepared for alcian blue reaction. The results were as follows; 1. The directions of osteogenic fibers were arranged irregulary during first 3 days, but after the 7th day they tended to change radial directions like control group. 2. The density of deep stained cells by Feulgen-Rossenbeck reaction were shown leu in the experimental group than that in the control group in first 3 days, but there was shown no significant difference between both groups after the 7th day. 3. PAS reaction in early stage was generally negative in the experimental group unlike as in the control group, but diffuse reaction was observed in the loose middle zone like as in the control group after 14th day. 4. Alcian blue reaction was negative in cambial zone, and slightly positive in uniting zone compared with control group in early stage. After 14th day, however, there was observed a tendency of moderately positive reaction.

Diagnostic Usefulness and Limitation of Fine Needle Aspiration Cytology of Lymph Node - Analysis of 176 Cases Confirmed by Biopsy - (림프절 세침흡인 세포검사의 진단적 유용성과 한계 - 생검으로 확진한 176 예의 분석 -)

  • The accuracy of fine needle aspiration cytology(FNAC) of the lymph node was investigated through a review of 176 FNAC cases and the corresponding biopsies. We chose 157 FNAC cases after the exclusion of 19 inadequate ones. Sensitivity of malignancy was 94.0%, specificity 100%, false negativity 6.0%, and false positivity 0.0%. The overall diagnostic accuracy was 96.8%. Sensitivity of metastatic carcinoma was 98.0% and that of malignant lymphoma was 87.9%. False negative cases included one metastatic carcinoma and four malignant lymphomas. The aspirates of metastatic carcinoma with false negativity exhibited a diffuse smear of keratin debris without viable cells, which led to the difficulty in differentiation from benign epithelial cyst. The cases of malignant lymphoma with false negative diagnosis were two Hodgkin diseases, one Lennert's lymphoma, and one peripheral T cell lymphoma in the histologic sections. On the analysis of 39 cases of tuberculosis, 17 cases(43.6%) were diagnosed as tuberculosis, 4(10.3%) as granulomatous lymphadenitis, 3(7.7%) as necrotizing lymphadenitis, and 15(38.5%) as reactive hyperplasia or pyogenic inflammation. Sensitivity of tuberculosis was 53.9%. In conclusion, lymph node FNAC is an excellent non-invasive diagnostic tool for the diagnosis of metastatic carcinoma. The diagnostic accuracy of malignant lymphoma could be improved with flow cytometry or polymerase chain reaction for antigen receptor genes. For the FNAC diagnosis of tuberculosis, AFB stain, culture, and PCR would be helpful as adjuvant techniques.

  • This report deals with a 9-year-old neutered male Yorkshire terrier that had been suffered from vomiting, bloody diarrhea, anorexia. On exploratory laparotomy, pale-pink $10{\times}6cm$ duodenal mass was found and submitted to the Department of Veterinary Pathology, College of Veterinary Medicine, Seoul National University. Histologically, the duodenal mass consisted of compact sheet of poorly demarcated, highly infiltrative neoplasm. The neoplastic cells were round to polygonal and contained scant to moderate amounts of granular basophilic cytoplasm and eccentrically located irregularly round nuclei with stippled chromatins. The neoplastic cells were positive to lamda light chain immunohistochemically but were uniformly negative for toluidine blue stain and giemsa stains. Based on these results, this case was diagnosed as primary extramedullary plasma cell tumor of duodenum.

  • Small cell neuroendocrine carcinoma of the uterine cervix is a distinct subtype of cervical cancer that appears analogous to oat cell carcinoma and carcinoid tumors of the lung. It has been assumed to be derived from the neural crest via argyrophilic cells in the normal endocervix. We have recently encountered a case of small cell neuroendocrine carcinoma of the uterine cervix coexisting with adenocarcinoma which was argyrophil negative. A 66-year-old multiparous woman was admitted because of vaginal bleeding for 2 months. Cervicovaginal smear revealed several scattered clusters and sheets of monotonous small cells with some peripheral palisading in the background of hemorrhage and necrosis. Radical hysterectomy specimen revealed an ulcerofungating tumor on endocervical canal which was composed of two components. Major component of the tumor was made up of monomorphic population of small oval-shaped tumor cells arranged in sheets and partly in acinar structures or trabecular fashion. Other component was adenocarcinoma, endocervical well-differentiated type. Argyrophilia was present on the Grimelius stain and immunohistochemical studies revealed diffuse positivity to neuron-specific enolase and carcinoembryonic antigen. Electron microscopic examination showed clusters of small round to oval cells, which had a few well-formed desmosomes and several membrane-bound, dense-core neurosectetory granules.

  • We described the findings of fine needle aspiration cytology of the lung from a patient with Wegener's granulomatosis. Early diagnosis and prompt treatment of the patients with Wegener's granulomatosis is essential for a better prognosis. However, the variety of clinical presentations and nonspecific radiologic infiltrates of Wegener's granulomatosis frequently make the diagnosis difficult. Although an open lung biopsy is required for a firm diagnosis, fine needle aspiration cytology & biopsy preparation can also provide an adequate tissue sample, when the findings of fine needle aspiration are considered with clinical manifestations and ANCA value in the serum. The cytologic smears showed scattered necrotic tissue fragments entrapping many neutrophils and occasional epithelioid cells. Multinucleated giant cells were infrequently observed. Ziehl-Neelsen stain for acid fast bacilli was negative. All the cytologic features recapitulated the histopathologic findings of purulent and necrotizing granulomatous inflammation seen in Wegener's granulomatosis.

  • Comparision of the classical taxonomy and the numerical taxonomy was carried out on 108 strains isolated from Chung-mu bay and it was confirmed that correlation coefficient measured between strains represented similarities. Gram stain is the most important test in the classical taxonomy, while Gram positive and Gram negative strains were distributed evenly over the whole cluster and coccus and rod were separated in the numerical taxonomy. Both the numerical taxonomy and the classical taxonomy showed high similarity in the classification between the genera. The strains collected in bottom layer marked lower taxonomic similarity between strains than those in upper and middle strains. The distance between the inside (Group I) and the outside (Group II) strains of the bay was larger than that between the strains collected in high and low tide.

Tuberculous Peripleural Abscess: Collective Review (결핵성 늑막주위 농양)

  • Fifty -six cases of tuberculous peripleural abscess were experienced in the department of thoracic and cardiovascular surgery, college of medicine, Hallym university from January 1980 to June 1990. Tuberculous peripleural abscess seems to originate from the space between the parietal pleura and endothoracic fascia. But rib caries, originated by hematogenous spread of mycobacteria to the rib, shows the rib destruction first, thereafter periosteal erosion and regional tissue involvement follows. In our 56 cases, results were as follows: 1. Their age ranged from 6 to 82 years, and female dominant [M: F=21: 35]. 2. The locations of abscess were 31 right, 23 left, and 2 sternal portions. 3. On X \ulcornerray findings, 37 cases showed active or old lesion of the tuberculosis in the lung field, 7 cases periosteal destruction of the ribs, and 29 cases pleural thickening. 4. Operative findings showed cold abscess with multiple fistulous tracts leading to intercostal space in most of the cases, and their origin were presumed to be from the space between the endothoracic fascia and parietal pleura. 5. The pus showed negative AFB stain in most of the cases except 3 cases. 6. Partial costectomy and radical curettage with drainage were performed in all cases. 7. 7 cases recurred after the first operations, but no recurrence after second operations.

Mitochondrial myopathies caused by prolonged use of telbivudine

  • Background: Telbivudine is a nucleoside analogue used for the treatment of chronic hepatitis B, but it often develops mitochondrial toxicity leading to symptomatic myopathy. In this study, three patients with telbivudine induced myopathy were enrolled in order to investigate the nature and pathogenesis of mitochondrial toxicity caused by long-term use of telbivudine. Methods: Clinical features, laboratory findings, muscle pathology, and quantitation of mitochondrial DNA were studied in three patients. Results: Patients presented with progressive muscle weakness with high serum creatine kinase levels. Light microscopic findings of muscle pathology showed ragged red fibers that reacted strongly with succinate dehydrogenase stain, but negative for cytochrome c oxidase activities. Electron microscopy revealed abnormal mitochondrial accumulation with rod shaped inclusions. The quantitative peroxidase chain reaction showed a depletion of mitochondrial DNA in skeletal muscle of the patients. Conclusions: Nucleoside analogues including telbivudine are potent inhibitors of viral DNA polymerases. However, they are not specific for viral DNA and can disturb mitochondrial replication at the same time. All nucleotide analogues should be used with close clinical observation in order to avoid development of mitochondrial myopathy.

Treatment of the Mycobacterium chelonae Infection after Fat Injection

  • For recent years, use of autologous fat injection has increased significantly in facial contouring surgery. Along with such increase in use, complications like atypical mycoplasma infection have been also on the increasing trend. The authors report two cases of Mycobacterium chelonae infection that occurred after autologous fat injection. Patients were treated as infection that resistant to common antibiotics and results were negative to routine culture and Gram staining. Acid-fast bacillus stain, polymerase chain reaction (PCR) test and mycobacterial cultures were conducted for diagnosis under suspicion of atypical mycoplasma infection. Then, combination antibiotics therapy, surgical treatment, and steroid injection were performed for treatment. Both patients were diagnosed with Mycobacterium chelonae in PCR test. They were positive to mycobacterial cultures. Combination antibiotics therapy was repeated to improvement of symptom. However, they could not be free from side effects such as deformation in facial contour, scar and pigmentation even after full recovery. When chronic wound infections after autologous fat injection, we must suspect atypical or mycobacterial infection and conduct examinations for a early diagnosis and proper antibiotic therapy that is effective to the nontuberculous mycobacteria.

A Case of Necrotizing Meningoencephalitis in a Shih-Tzu Dog (시츄견의 괴사성 뇌막뇌염 증례 보고)

  • Necrotizing meningoencephalitis (NME) is a unique idiopathic nonsuppurative inflammatory disease of central nervous system in small-sized breed dogs. A 9-year-old intact male Shih-Tzu dog with anorexia, vomiting, salivation and intermittent seizures was submitted to the Jeju National University for diagnosis. Grossly, there were no obvious lesions in the brain, except dilatation of most blood vessels in meninges. Histopathologically, brain revealed severe multifocal nonsuppurative inflammation in perivascular area of meninges and cerebral cortex. Some areas of cerebral parenchyma were replaced with lots of macrophages contained periodic acid-Schiff positive materials. Many new-formed blood vessels were observed around the necrotic regions using Gomori reticulum stain. Immunohistochemistry and reverse transcription-polymerase chain reaction were negative for toxoplasmosis and canine distemper virus. Based on the gross, histopathologic features and antigen detection methods, this case was diagnosed as NME. Here we reported the NME in relatively uncommon breed, Shih-Tzu dog, than other small breed dogs.
