COVID-19 환자에서 음압격리들것을 이용한 흉부 CT 검사에 대한 전문가 의견 설문: 영상품질과 감염위험 (Expert Opinion Questionnaire About Chest CT Scan Using A Negative Pressure Isolation Strecher in COVID-19 Patients: Image Quality and Infection Risk)

  • 목적 급성호흡기질환인 코로나바이러스감염증-19 (coronavirus disease 2019; 이하 COVID-19) 환자를 대상으로 한 음압격리들것(negative presurre isolation stretcher; 이하 NPIS)을 이용한 흉부 전산화단층촬영(이하 CT) 방식에 대해 전문의들의 경험과 의견을 확인하고자 하였다. 대상과 방법 격리입원 중인 COVID-19 환자에게 NPIS를 이용한 흉부 CT 검사를 시행했던 9개 의료기관 소속 27명의 전문의들을 대상으로 1) 조영증강 흉부 CT가 필요한 경우, 2) NPIS를 이용한 흉부 CT의 영상품질, 그리고 3) CT 조영제주사를 위한 NPIS 개방과 CT 검사실의 오염을 주제로 설문조사를 진행하였다 결과 조사대상인 9개의 의료기관들은 기관당 한 해 평균 116건의 COVID-19관련 흉부 CT 검사를 NPIS와 함께 진행했으며. 전체 검사 건수 중 평균 24건(21%)이 조영증강 흉부 CT였다. 설문에 참여한 호흡기내과 전문의 9명 중 5명(56%)은 환자의 혈중 D-dimer 이상이 확인되면 조영증강 CT가 필요하다고 의견을 밝혔다. 한편 영상의학과 전문의 9명 모두는 NPIS로 인한 흉부 CT 영상의 품질은 폐렴 또는 폐혈전 진단이 가능한 수준이라 답하였다. 또한 감염내과 전문의 9명 중 5명(56%)은 NPIS의 개방으로 인한 CT 검사실의 이차감염은 소독을 통해 예방할 수 있는 수준으로 생각하고 있었다. 결론 격리입원 중인 COVID-19 환자에서 NPIS와 함께 흉부 CT를 진행하더라도 CT의 품질은 진단이 가능한 수준이며 NPIS가 CT 검사실 내에서 잠시 개방되더라도 CT 검사실의 감염확산 위험은 높지 않은 것으로 전문의들은 인식하고 있었다.

COVID-19 중환자를 돌보는 간호사들의 경험 (Nurses' experiences of caring for severe COVID-19 patients)

  • Purpose : This study aimed to understand and describe the nurses' experiences caring for severe COVID-19 patients at isolation room with negative pressure in South Korea. Methods : Data were collected through individual in-depth interviews from February 16 to March 31, 2021 with 11 nurses who had been caring for severe COVID-19 patients at isolation room. Verbatim transcripts were analyzed according to Colaizzi's phenomenological analysis. Results : As a result, 5 theme clusters were extracted about nurses' experiences. The 5 theme clusters, "Adapting to unexpected adversity", "Accepting unresonable and heavy work", "Dulness and disconnection from the outside world", "Changing in perception of new infectious diseases", and "Pathetic isolated patients" emerged. Conclusion : The results of this study provided a deeper understanding of nurses struggling to care for COVID-19 severe patients. This study is expected to be useful in providing basic evidence for improving intensive care practices and for preparing policies in other infection disease situations.

  • Purpose: The MERS(Middle East Respiratory Syndrome) outbreaks in Korea highlighted dramatically the failings of traditional hospital environment for controlling or preventing infections among both patients and healthcare workers. MERS is transmitted by droplets that can be airborne over a limited area. The point should be emphasized that MERS in South Korea was predominantly a hospital-acquired (not a community-acquired) infection, because approximately 93% of MERS cases were resulted from exposure in hospital settings. This paper tries to suggest the design guidelines of negative pressured isolation ward for the sake of proper control of severe respiratory infectious diseases. Methods: Literature survey on the design guideline and regulations of airborne infection wards in Korea, Europe U.K. and CDC of U.S. have been carries out. 4 special infection wards in Hongkong, Germany, Japan and Korea have been surveyed in order to make the best use of the experiences related to facility design and operations. Results: Operating system influencing the facility design, space organizations of infectious ward including required space and zoning, and circulations of patients, staffs and materials are proposed. Implications: The results of this paper can be the basic data for the design of the airborne infection ward and relevant regulations. Afterwards in-depth study such as the development of space standards for the single bedroom, locker room and so on could be explored.

팬데믹 상황에서 음압병실 근무 간호사의 COVID-19 환자 간호 경험 (Nurses' Experience with Caring for COVID-19 Patients in a Negative Pressure Room Amid the Pandemic Situation)

  • Purpose: The purpose of this study was to explore nurses' experience with caring for COVID-19 patients in a negative pressure room amid the spread of the pandemic. Methods: This study was a qualitative research, and focus group interviews were used to collect data. Three focus groups comprising 19 nurses were interviewed from February 17 to 25, 2021. All interviews were recorded and transcribed verbatim with the consent of the participants. The verbatim transcripts were scrutinized using thematic analysis. Results: Two main themes emerged from the analysis: 'Struggling in an isolated space' and 'Limitations of nursing infrastructure and system'. The nurses caring for COVID-19 patients experienced anxiety and fear about the infection, physical exhaustion, emotional burnout, and a sense of duty as a nurse. They also acknowledged the lack of guidelines, increased task and burden, limitations of nursing care, and the demand for improving the limitations of the nursing system. Conclusion: The results of this study demonstrate that nurses caring for COVID-19 patients encounter physical and emotional problems within the limited healthcare system. The study suggests that comprehensive interventions are needed for nurses. Furthermore, detailed guidelines, strengthening of nursing personnel, and improvements to the nursing system are vital to effectively cope with the pandemic. The government and medical institutions should be aware of the needs of nurses and what they are going through, and make efforts to improve the quality of life of healthcare workers and create a safe healthcare environment.

소아 코로나바이러스감염증-19에서 발생한 열성 경련 (Febrile Seizure Associated With COVID-19 in a Child: Case Report and Literature Review)

  • 기저 질환 없이 평소 건강하던4세 남아에서 COVID-19와 연관된 열성 경련이 발생하였다. SARS-CoV-2 양성으로 확인되어 음압격리병동에 입원한 당일 발열과 함께 25분간 전신 간대성 발작이 있었고 산소흡입, 항경련제 투여 후 소실되었다. 말초혈액, 혈액화학, 전해질, 혈액기체분석, 급성기반응물질, 면역혈청, 특수화학 검사 결과들은 참고치 내에 있었으나, 소변검사에서 케톤이 검출되었다. 가슴X선 영상검사에서 활동성 폐병변이 관찰되지 않았고, 뇌파 검사에서 이상 소견 없었다. 뇌 자기공명영상 검사에서 뇌실질 내외의 병변은 없었다. 발작은 재발하지 않았고 입원 12병일째 퇴원하였으며 신경학적 후유증은 없었다.
