Primary Malignant Lymphoma of Lung -A Case Report- (원발성 폐 림프종 치험 1례)

  • Primary malignant lymphomas of the lung are rare and known often to be localized, solitary pulmonary lesions, in chest radiograph. Because they are highly treatable contrast to the other primary lung cancer, the distinction is important. A 35-year old man who was admitted for a solitary pulmonary nodule in the right middle lobe. Percutaneous needle aspiration disclosed diffuse, small cell lymphoma. Bone marrow biopsy showed no evidence of neoplastic lymphoid cell infiltration. There were a walnut sized mass involving right middle lobe with a small satellite nodule at 2cm distal to the right upper lobe bronchial orifice. The histopathology of the bilobectomized specimen showed diffuse, small cell, malignant, non-Hodgkin`s lymphoma. Immunologic subtype was defined as B-cell type.

  • The diffuse nodular lung lesion has a diagnostic problem and should be made differential diagnosis. A chest X-ray of 62-year-old male patient with dyspnea showed small-sized scattered multinodular lesion on entire lung field. Bronchoalveolar lavage and transbronchial needle aspiration showed the non-specific findings, so open lung biopsy was done and revealed bronchioloalveolar cell carcinoma.

Clinical Investigation of Incidentally Found Thyroid Carcinoma in Mass Screening (집단 검진에서 발견된 갑상선 우연암종에 대한 임상적 고찰)

  • Background and Objectives: The introduction of highly sensitive imaging techniques has made it possible to detect many non-palpable nodules, or“incidentaloma”in the thyroid. Discovery of these lesions raises concerns about their malignancy, but the optimal strategy for managing these lesions has not been clearly established. This study was designed to evaluate the usefulness of ultrasonographic exam with new diagnostic criteria and presume the value of mass screening for thyroid cancer. Materials and Methods : Mass screening for thyroid cancer were carried out in conjunction with mass screening for breast cancer. The subjects were 630 women aged 30 years or over. Thyroid glands were examined with 10 MHz transducer ultrasonography by one radiologist. Needle aspiration biopsy were performed when suspicious of malignancy under the new diagnostic criteria. Results and Conclusion : The new ultrasonographic criteria to diagnose thyroid cancer provided useful information and ensured more accurate evaluation. 7 cases of thyroid cancer were discovered and successfully managed. It should be further demonstrated that there is sufficient value of mass screening for thyroid cancer to perform it independently despite early cancer detection.

  • Aim: Breast cancer is one of the most common cancers of women in India with high fatality rate. Over a 1 year study period 105 consecutive biopsy or fine needle aspiration cytology confirmed breast cancer patients were interviewed by direct questionnaire method regarding risk factors attending Surgery and Radiotherapy OPD of Medical College Kolkata, West Bengal while taking other 105 patients attending Surgery Department for some other disease as controls. The data were compiled in MS Excel 2007 and analyzed by Epi info 3.5.1 software. Among the cases, rural residence, illiteracy and low socio-economic status was significantly higher than controls. Late onset of menarche, late onset of menopause, ever OCP usage, breast feeding for 1-2 years and age of 1st childbirth between 20-30 years were found to be significant protective factors. People should be made aware regarding the modifiable risk factors to prevent breast cancer.

  • A 5-year old female shih-tzu referred to the veterinary Teaching Hospital, College of Veterinary Medicine, Konkuk University. The patient was presented with a mass of ventral to pubic region and intraperineal region. CBC, chemistry, x-ray, fine needle aspiration, biopsy and MRI was performed. The dog was definitively diagnosed as lipoma based on cytologic and histopathologic examination. In order to remove the mass, an approach with two direction was made. First, intraperineal region was removed about 40%, and then ventral to pubic was removed the remainder. And inserted penrose drain for obliterate dead space to prevent the accumulation of serous discharge. The dog is clinically normal and being monitored for development of any abnormal signs for the time being. In this case, diagnostic MRI was very useful procedure. it revealed accurate location of a lipoma and useful manner for decision of surgical approach to remove.

  • We managed surgically a case of local recurrence in esophageal cancer Twenty month after transthoracic subtotal esophagectomy and csophago-gastrostomy, he su(fared from dysphagia. Chest CT and percutaneous needle aspiration biopsy showed . Local recurrence involving residual esophagus, thyroid gland, posterior membraneous portion of trachea. We did cervical esophagectomy, laryngectomy thyroidectomy, partial resection of trachea and reconstruction with free jejunal antograft successfully.

  • Metastatic squamous carcinoma of the head and neck may rarely present as a cyst in the cervical region. The true incidence of metastatic cystic neck mass is unknown. It is difficult to differentiate metastatic cystic neck mass from congenital cystic neck mass with physical examination and fine needle aspiration biopsy. So the differential diagnosis is dependent on the age of the patient, and therefore in the patient over the 40 years of age, the possibility of a metastatic neck mass should be considered. We report a metastatic cystic neck mass which had been misdiagnosed as a branchial cleft cyst.

Cystic Changes in Lymph Nodes with Metastatic Squmous Cell Carcinoma (낭종성 측경부전이를 동반한 두경부 편평상피암 2례)

  • For many years it has been reported that seemingly benign neck cysts may contain carcinoma. Cystic metastases have often mistaken for either branchial cleft cysts or benign mass. Authors experienced two cases which presents cystic cervical metastatic cancer One was a tonsillar carcinoma and the other was a tongue carcinoma. Patients with a cystic squamous carcinoma in the neck likely have a primary in upper aero-digestive system and It is known that the tonsil is most common site. Radiologic examination and fine needle aspiration biopsy of the cyst proved to be non-diagnostic. The development of cervical lymph node metastases before clinical signs of carcinoma of the tonsil is also well recognized. So, in old patients, thorough head If neck examination, panendoscopy and ipsilateral tonsillectomy is mandatory to identify a primary carcinoma prior to cyst excision.

Biphasic Pulmonary Blastoma detected by Pneumothorax (기흉에 의해 발견된 이상성 폐 모세포종)

  • Pulmonary blastoma is a rare primary lung neoplasm composed of a mixture of immature mesenchyme and epithelium that morphologically mimics embryonal pulmonary structure. Microscopically, tumors could be divided into two classes; those composed solely of malignant glands of epithelial components (well-differentiated fetal adenocarcinoma) and those with biphasic appearance. A 51-year-old man was admitted with left sided pneumothorax. After closed thoracostomy, chest X-ray showed a circular mass in the left lower zone. The tumor was diagnosed as adenocarcinoma as a result of percutaneous needle aspiration biopsy, and left lower lobectomy was performed. The patient was diagnosed as having biphasic pulmonary blastoma on postoperative histological assessment. This case is the first reported patient with biphasic pulmonary blastoma detected by pneumothorax.
