AN EXPERIMENTAL STUDY ON THE EFFECT OF VARIOUS HYDROXIDES UPON THE PULP IN DOGS' TEETH (수종수산화물(數種水酸化物)이 치수조직(齒髓組織)에 미치는 영향(影響)에 관(關)한 실험적연구(實驗的硏究))

  • After a vital pulpotomy in dogs' teeth, the responses of the remaining pulp tissue under hydroxides (calcium hydroxide, magnesium hydroxide, aluminium hydroxide and zinc hydroxide) were studied histologically. The class V cavities were prepared on the teeth and the pulp was amputated. Each hydroxide was placed over the amputated tissue and the cavity was sealed with zinc oxide eugenol cement. Animals were sacrificed after 3 days, 1, 2, and 3 weeks following the operation. The teeth were decalcified, sectioned and stained with hematoxylin and eosin. Microscopic examination reveals as follows; 1. Calcium hydroxide: Inflammatory change was seen in the superficial portion of the remaining pulp tissue at the 3rd day and 1st week. The incompleted calicified material began to be deposited from the canal wall at the 2nd week, and the advanced calcified material was seen at the 3rd week. 2. Magnesium hydroxide: Severe inflammatory change was seen in the superficial portion of the remaining pulp from the 3rd day and the 1st week samples. Inflammatory change was decreased at the 2nd week and the slight calcified material was deposited from the root canal at the 3rd week. 3. Aluminium hydroxide: Severe inflammatory changes were seen in the remaining pulp tissue, the blood vessel was dilated, and the odontoblasts were destroyed at the 3rd day and 1st week. The fibrous degeneration spread to the apex at the 2nd week. There was no evidence of newly formed odontoblasts or deposition of calcified material underneath aluminium hydroxide. 4. Zinc hydroxide: The micrscopic picture was destructive. A thick necrotic layer was found under the amputated surface at the 3rd day and 1st week. Granulation tissue formation as well as chronic inflammatory changes extended to the apical area in the pulp tissue. Also there were no sign of odontoblastic formation or calcified material at the 2nd and 3rd week.

  • Kikuchi-Fujimoto disease is a rare disease first described in 1972 by Kikuchi and Fujimoto et al. The disease is described as a benign and unusual self-limiting histiocytic necrotizing lymphadenitis of unknown origin, which is characterized histologically by necrotic foci surrounded by histiocytic aggregates. is usually manifested with lymphadenopathy and high fever. This disease mostly affects young Asian women between 20 and 30 years of age and has rarely been reported in children. Main symptoms are indolent or light tender, enlarged lymph nodes in the neck area. The correct diagnosis requires the histologic examination of the lymph node. Kikuchi-Fujimoto disease is easily confused histologically and clinically with lymphoma and systemic lupus erythematosis histologically and clinically. Although it is an uncommon cause of fever of unknown origin, early recognition of KFD is very important and will minimize potentially harmful and unnecessary evaluations and treatments. We reported a case, a 23-year old man who had Kikuchi-Fujimoto disease with a literature review.

A Case of Combined Germ Cell Tumor in Testis (고환의 원발성 결합성 생식세포 종양 -A case report with literature review-)

  • Primary germ cell tumor of the testis is, rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ ceil tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and $9{\times}6{\times}5$ cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.

Peroxidase Activity during Leaf Infection of Mulberry (Morus alba L.) with Brown Leaf Spot Fungus Myrothecium roridum

  • Peroxidase activity was measured in brown leaf spot pathogen (Myrothecium roridum) inoculated potted mulberry (Morus alba) during pre-symptomatic to various symptom development stages and compared with corresponding healthy leaf tissues. The enzyme showed a pH optimum of 7.0 and the activity was linearly increased up to 15 min of incubation. The peroxidase had a broad substrate specificity and the rates of oxidation were in the rank of pyrogallol> guaiacol> ascorbate at pH 7.0. Catechol at 10 mM inhibited 89% of guaiacol-peroxidase and 76% pyrogallol-peroxidase activities, indicated higher non-specific peroxidation in pyrogallol dependent assay system in mulberry than guaiacol. The optimum requirement for the guaiacol dependent assay was 0.2 ml (${\approx}40-60{\mu}g$ equivalent of protein) of crude enzyme source. Excepting the 8th leaf from the apex, the peroxidase activity did not vary appreciably in different leaf positions. In pre-symptomatic phases, an initial (1 to 5 min) rise of peroxidase activity was noticed in inoculated leaves, and then maintained a plateau up to 300 min. In contrary, non-infected tissue showed a slightly increased trend of enzyme level up to 420 min. In infected tissue, a sharp transient increase (3.1 fold) of peroxidase activity appeared between 300 - 420 min post infections. Afterwards, significantly different but steady maintenance of enzyme levels were observed in two treatments. On the other hand, during symptom development, a sharp increase in peroxidase activity was noticed up to 4th grade of lesion appearance (25.1 % to 50% of leaf area infection), and then declined slightly. However, in non-infected but same age healthy leaves, such huge fluctuations of enzyme level did not apparent. A high positive correlation $(R^2=0.92)$ between peroxidase activity and leaf spot development grades was also marked. The result implies that pre-symptomatic burst (between 1 - 5 and 300 - 420 min) and subsequent increased trend of guaiacol peroxidase activity may require for the symptomatic manifestation of Myrothecium leaf spot in mulberry.

Oral Malignant Melanoma in a Labrador Retriever (리트리버 개의 구강유래 악성 흑색종 발생 예)

  • A 12-year-old, intact male Labrador Retriever was admitted to the Veterinary Teaching Hospital of Konkuk University for the evaluation of bleeding mass in the oral cavity. The mass was first noted 2 years ago and has been growing up to a size of $5{\times}6$ cm. On complete blood count(CBC), leukocytosis and mild anemia were observed. Pulmonary metastasis was found in the thoracic radiographic examination. Blackish necrotic oral mass at the left mandibular area was diagnosed as a malignant melanoma based on the histopathological examination. Thus hemimadibulectomy and chemotherapy (carboplatin) were performed. The patient eventually died due to severe secondary infection 3 months after the last treatment.

Effect of New Herbicides(CGA 82725 & DOWCO 453) on Membrane Permeability in Bean Leaf Tissue (새로운 제초제(除草劑)가 강남콩잎의 세포막(細胞膜) 투과성(透過性)에 미치는 영향(影響))

  • Leakage of electrolytes from leaf discs of treated bean (Phaseolus velgaris L.) plant was the criterion used to investigate the effect of four herbicides on the permeability of leaf-cell membrane. CGA 82725 (2-propynyl 2-((3, 5-dichloro-2 pyridinyl) oxy) phenoxy prpanate) at $10^{-3}M$ increased significantly cell membrane permeability within 1 h after 12 h treatment. Significant increase in cell membrane permeability was also detected at $10^{-2}M$ of DOWCO 453 (Haloxy-methyl 2-(4 (3-chloro- 5-(trifuluoromethyl)-2-pyridinyl) phenoxy) propanate. The effect of dinoseb (2-(1-methylpropyl)-4, 6-dintrophenol) on cell premeability was detected at $10^{-4}M$ after 12h. The highest conductivity measurement was obtained from paraquat (1, 1'- dimethyl-4, 4'-bipyridinium ion). Increase in cell membrane permeability was not always associated with injury symptoms such as appearance of necrotic area in leaves.

Arthroscopic assisted Core Decompression of Humeral Head Osteonecrosis - Technical Note - (상완 골두 골괴사증에서의 관절경하 핵심 감압술 - 수술 술기 -)

  • Purpose: We introduce arthroscopic assisted core decompression for humeral head osteonecrosis. Operative technique: After diagnostic shoulder arthroscopy is performed using posterior and anterior portal, we make a 2 cm lateral skin incision approximately 3 cm distal to 1/3 of lateral margin of the acromion and place short gray cannula to prevent adjacent soft tissue injury when insert guide pin. Under C-arm fluoroscopic and arthroscopic guidance, 3 to 4 guide pins are inserted toward the necrotic area. Then we perform drilling using 7.0 mm reamer. Conclusion: We believe this technique is a less invasive approach and avoids the complications comparing to deltopectoral approach. Arthroscopic assisted core decompression is an effective procedure in management of early stage of the humeral head osteonecoris.

Usefulness of Full-thickness Skin Graft from Anterolateral Chest wall in the Reconstruction of Facial Defects (안면부 재건에서 전외측 흉벽을 공여부로 하는 전층 피부이식술의 유용성)

  • Purpose: Full thickness skin grafts are useful in the reconstruction of facial skin defects when primary closure is not feasible. Although the supraclavicular area has been considered as the choice of donor site for large facial skin defect, many patients are reluctant to get a neck scar and some patients do not have enough skin to cover the defect owing to the same insult occurred to the neck such as burn accident. We present several cases of reconstruction of facial skin defects by freehand full-thickness skin graft from anterolateral chest wall resulting aesthetically acceptable outcome with lesser donor site morbidity. Methods: Retrospective review was performed from March, 2007 to September, 2009. 15 patients were treated by this method. Mean age was 31.5 years. The ethiology was congenital melanocytic nevus in 7 cases, capillary malformation in 5 cases and burn scar contracture in 3 cases. Mean area of lesion was measured to 67.3 cm2 preoperatively. The lesion was removed beneath the subcutaneous fatty tissue layer. The graft was not trimmed to be thin except defatting procedure. For the larger size of defect, two pieces of grafts were harvested from both anterolateral chest wall in separation and combined by suture. Results: The mean follow up period was 9.7 months. All the grafts survived without any problem except small necrotic areas in 4 cases, which healed spontaneously under conventional dressings in 6 weeks postoperatively. Color match was relatively excellent. There were 2 cases of hyperpigmentation immediately, but all of them disappeared in a few months. Conclusion: In cases of large facial skin defects, the anterolateral chest wall may be a good alternative choice of full-thickness skin graft.

CASE REPORTS OF FASCIAL SPACE ABSCESS CAUSED BY ODONTOGENIC INFECTION (치성 감염에 의한 근막간극 농양의 치험례)

  • Infections involved with the oral and maxillofacial area are associated with various anatomical structures. If the proper treatment is not done in an immediate period, the infections will be quite fatal. The causes of the infections are numerous, but the most common cause of odontogenic infections in children is a dental caries. It is known to lead to some kinds of diseases such as periapical abscess, cellulitis, osteomyelitis, Ludwig's angina, toxic shock syndrome and so on. The common pathogenic sequence of fascial abscess is a necrotic pulpal inflammation in the form of dentoalvelor abscess which spreads over and gradually penetrates into the fascial membranes through the cortical bones and finally contracts the potential fascial spaces. If the infections of oral maxillofacial area were penetrated into the surrounding soft tissues, then they would diffuse into the directions of the least tissue resistance along with the connective tissues and the fascial spaces. These infections can be properly cured by tooth extraction, endodontic therapy, surgical treatment including Incision & drainage and antibiotics. The purpose of the cases is to report the satisfactory treatment results in the patients derived from the canine fascial space abscesss or buccal fascial space ones of the odontogenic origin.

Diffusion-Weighted MR Imaging of Various Intracranial Diseases : Clinical Utility (다양한 두개강내 질환의 확산강조 자기공명영상 : 임상적 유용성)

  • Purpose : To evaluate the clinical utility of diffusion-weighted imaging by analyzing the signal intersity of lesions in patients with various intracranial diseases. Materials and Methods : difusion-weighted MR imaging was prospectively perormed in randomly selected 70 patients with various intracranial idseases. They consisted of 20 patients with acute infarct, 21 patients with chronic infarct of small vessel disease, 14 patients with primary intracranial tumor, three patients with brain metastasis, five patient with brain abscess, five patients with brain abscess, five patients with cerebral hemorrhage, one patient with neurocysticercosis, and one patient with epidermoid cyst. the diffusion-weighted images were obtained immediately after routine T2-weighted imaging on a 1.5T MR unit using single shot spin echo EPI technique with 6500 ms TR, 107ms TE, $128{\times}128$ matrix, 1 number of excitation, $24{\times}24$ field of view, 5-7 mm slice thickness, 2-3 mm inter-slice gap. The diffusion-gradients (b value of ($1000s{\;}/{\;}textrm{mm}^2$)) were applied along three directions(x, y, z). On visual inspection of diffusion-weighted images, the signal intersity of lesions was arbitrarily graded as one of 5 grades. In quantitative assessment, we measured the signal intensity of all the lesions and the contralateral corresponding normal area using round region of interest(ROI), and then calculated the signal intensity ratio of the lesion to the normal brain parenchyma. Results : On visual inspection, markedly hyperintense signals were seen in all cases of acute infarct, brain abscess, epidermoid cyst, and neurocysticercosis in degenerating stage. In all cases of cerebral hematoma, the very high signal internsity was intermingled with low signal intensity. focal very high signal intersity was also seen in a solid portion of the tumor in a patient. the mean signal intensity ratios of all those lesions to the normal brain parenchyma were above 2.5. Gliosis, solid component of brain tumor, brain metastasis, and vasogenic dedma appeared isointense to the normal brain parenchyma in 71%, 64%, 100%, and 67%, respectively ; the mean signal intensity ratios of those lesions to the normal brain parenchyma ranged 1.15 to 1.28 and there was no significant difference among these(p>0.1). Cystic cerebromalacia and necrotic or cystic portions in tumor were markedly or slightly hypointense, and the mean signal intensity ratios were 0.45 and 0.42, respectively. Conclusion : Very high signal intensity of acute infarct, brain abscess, epidermoid cyst, and cystic neurocysticercosis in degenerating stage on diffusion-weighted images may be helpful in differentiating from other diseases that are hypointense or isointense to the normal brain parenchyma. It may be especially useful differentiation of brain abscess from brain tumor with necrotic or cystic portion.
