Immunoglobulin G4-Related Disease Masquerading Anaplastic Thyroid Carcinoma

  • Immunoglobulin G4-related disease (IgG4RD) is a chronic inflammatory condition characterized by tissue infiltration with lymphocytes and IgG4-secreting plasma cells, as well as varying degrees of fibrosis. We report a case of a 70-year-old man with a rapid-growing cervical mass for several months. Computed tomography and positron emission tomography showed a huge, ill-defined mass involving left thyroid lobe and encasing the common carotid artery, which was clinically and radiologically suspicious for anaplastic thyroid carcinoma. Ultrasonography-guided core needle biopsy was performed and histopathology examination revealed to be consistent with IgG4RD, and the IgG4/IgG ratio was 0.6. After oral corticosteroid was administered, the mass was dramatically resolved. Because IgG4RD often presents as a single localized and infiltrated mass lesion, it can be confused and misdiagnosed as a malignancy. Thus, clinicians should consider IgG4RD as a differential diagnosis in a rapid-growing neck mass to prevent unnecessary and excessive treatments.

In Vitro Biological Characterization of DCUN1D5 in DNA Damage Response

  • Background: Novel prognostic biomarkers or therapeutic molecular targets for laryngeal squamous cell carcinoma (LSCC) are an urgent priority. We here sought to identify multiple novel LSCC-associated genes. Methods: Using high-density microarray expression profiling, we identified multiple genes that were significantly altered between human LSCCs and paired normal tissues. Potential oncogenic functions of one such gene, DCUN1D5, were further characterized in vitro. Results: Our results demonstrated that DCUN1D5 was highly expressed in LSCCs. Overexpression of DCUN1D5 in vitro resulted in 2.7-fold increased cellular migration, 67.5% increased invasive capacity, and 2.6-fold increased proliferation. Endogenous DCUN1D5 expression was decreased in a time-dependent manner after genotoxic stress, and silencing of DCUN1D5 by siRNA decreased the number of cells in the S phase by 10.2% and increased apoptosis by 11.7%. Conclusion: Our data suggest that DCUN1D5 in vitro might have vital roles in DNA damage response, but further studies are warranted to assess its significance in vivo.

성대폴립 환자에서 수술 후 음성치료의 유효성 (The Usefulness of Postoperative Direct Voice Therapy in Vocal Polyps)

  • Background and Objectives Previous studies have suggested the usefulness and importance of postoperative voice therapy, for which there are indirect and direct methods. The aim of this study was to evaluate the efficacy of treatment according to different voice therapy methods. Subjects and Method Patients with vocal polyp were divided into three groups. Group 1 received direct voice therapy after phonomicrosurgery and Group 2 indirect voice therapy after phonomicrosurgery. Group 3 did not receive any voice therapy. Results Perceptual, acoustic, aerodynamic voice outcome parameters differed significantly between pre and post-operative treatments. In almost all of the voice analysis, Group 1, who underwent direct voice therapy, improved more significantly compared with Group 2 and 3. Conclusion Postoperative voice therapy following phonomicrosurgery may be an effective adjuvant treatment in patients with vocal polyps. In particular, direct voice therapy can be effective for improving postoperative voice outcome.

구인두 종괴로 오인된 내경동맥 사행 1예 (Tortuous Internal Carotid Artery Presenting as an Oropharyngeal Mass : A Case Report)

  • Tortuosity of the internal carotid artery is not a rare condition. A 68-year-old woman presented with a 3 years' history of a pulsatile tinnitus and throat abnormal sensation. On physical examination, a soft pulsatile mass on the left posterior oropharyngeal wall was noted. By neck CT and neck MRI the tortuous internal carotd artery was visualized. Otolaryngologists should recognize this anomaly and the patient should be made aware of their condition, because it may cause a fatal or near-fatal hemorrhage during surgical procedures in the pharynx, especially tonsillectomy and adenoidectomy and drainage of peritonsillar abscess. We report a case of a left internal carotid artery tortuosity as an oropharyngeal mass.

이소성 갑상선에 발생한 역형성 암종 1예 (A Case of Anaplastic Carcinoma Arising Ectopic Thyroid)

  • Ectopic thyroid glands generally occur in the midline as a result of abnormal median migration, and their presence lateral to the midline is extremly rare. We report a 83-year-old male with anaplastic carcinoma admix papillary carcinoma in the extrathyroid area. We suspected left lateral neck metastasis on preoperative fine needle aspiration result and computed tomography. the patient underwent total thyroidectomy and left selective neck dissection(level II, III, IV, V). The patient was diagnosed as having an ectopic thyroid gland on the lateral neck with anaplastic carcinoma admix papillry carcinoma. The patient is alive without incident of tumor recurrence at 5month after surgery and radiotherapy.

랑거씨 액와 아치 1예 (One Case of Langer's Axillary Arch(Axillopectoral Muscle))

  • Langer's axillary arch(axillopectoral muscle) is a rare disease. It is a variant muscular structure of the axilla, which was described as Langer's muscle by Ramsay and Langer. It could evoke chronic ambiguous neck pain. Therefore, it should be considered, when the patient failed to find the origin of neck pain which do not react any treatment. If it were diagnosed, it can be easily managed. However, it is not well known to otorhinololaryngologitis. Here we reports the 53 years old female who developed pain on supraclavicular area as a result of an Langer's axillary arch.

  • The Horner's syndrome includes anisocoria as a result of miosis of the involved pupil with ptosis of the upper and lower lids, which results in slight narrowing of the palpebral fissure. Ipsilateral facial hyperemia and anhidrosis over the face and neck are less common features. The findings with Horner's syndrome are a result of the loss of sympathetic innervation to the ipsilateral eye and face. Recently we experienced 3 cases of Horner's syndrome that developed postoperatively and report briefly with literature.

비강 내 소세포형 악성 흑색종의 면역치료 1예 (A Case of Immunotherapy in Small Cell Type Malignant Melanoma of Nasal Cavity)

  • There are many treatment options for the malignant melanoma. Wide excisional surgery is one of the most acceptable treatments for locoregional treatment. Depending on the pathologic classification, however, some other treatment option can be included such as chemotherapy, radiotherapy and immunotherapy as adjuvant treatment. Small cell type malignant melanoma is a rare variant of malignant melanoma. It is known that melanomas manifesting this morphology are invariably in vertical growth phase and have an aggressive course. The authors encountered small cell type malignant melanoma and would like to share the experience of successful treatment with surgery plus immunotherapy as one of adjuvant treatment options.

관통성 경부 손상에 관한 임상적 고찰 (Clinical Analysis of Penetrating Neck Injury: Review of 42 Operated Cases)

  • Background: Penetrating neck injuries are potentially dangerous and require emergency management because of the presence of vital structures in the neck. The risk of airway, vascular, neurological, and pharyngoesophageal injuries leads to many difficult diagnostic decisions. The purpose of this retrospective study is to evaluate our experience with management of penetrating neck injuries, and to assess treatment outcome. Material and Method: Forty-two consecutive patients were identified (26 patients from Korea university Ansan hospital, 16 patients from Guro hospital) as having penetrating neck injuries from 2003 to 2009. With review of medical records, variables were collected and evaluated including the location of injury, mechanism of injury, number of significant injuries, diagnostic modalities, duration of hospital stay and outcome. Results: The location of injury was zone I (lower neck) in 13 cases (31%), zone II (midportion of the neck) in 22 (52%), and zone III (upper neck) in 7 (17%). Injuries were caused by stab wounds in 23 patients, penetrating foreign bodies in 12. Among 35 patients who had deep injuries that violated the platysma, significant injuries, including major vascular (20), trachea (5) Pharyngoesophageal injuries (5) were identified in 24 patients. The mean hospital stay was 9.5 days. Conclusion: The penetrating trauma in the neck may show various degrees of severity. However, Cervical penetrating injury should not be underestimated in spite of the minimal width of the lesion.

두경부암종 수술 후 결손부위 재건에 사용된 유리피판술 51예의 고찰 (Clinical Analysis of 51 Cases of Free Flap Reconstruction after Ablative Surgery of Head and Neck Cancer)

  • Background and Objectives:Microvascular free flap reconstruction has been revolutionized in last two decades, and became a standard option in the reconstruction of head and neck defects. We intended to review our experiences of 51 microvascular free flap for head and neck defects during 5-year period and to analyze the types of flaps according to primary sites, success and complication rates. Subjects and Methods:From Oct. 2001 through Dec. 2005, fifty one free flap reconstructions were performed in forty nine patients at ENT department of Soonchunhyang university bucheon hospital. Primary sites, pathology, T-stage, operative time, time interval of oral feeding, and various reconstructive factors such as recipient and donor vessels, free flap related complications, failure rates and salvage rates were retrospectively analyzed. The relation between complication rates and preoperative risk factors were statistically analyzed. Results:Methods of reconstruction were radial forearm free flap(RFFF)(n=28, 54.9%), anterolateral thigh free flaps(n=9, ALTFF)(17.6%), rectus abdominis free flap(n=7, RAFF)(13.7%), jejunal free flap(n=5, JFF)(9.8%), and miscellanous(n=2, 4.0%) in order. In free flap related complications, failure of free flap occurred in seven cases(13.7%) and pharyngocutaneous fistula occurred in five cases(9.8%) among fifty one free flaps. The overall success rate of free flaps was 86.3%. Salvage of free flaps was possible only one among eight cases(12.5%). In positive preoperative risk factor groups, failure of free flap was higher than in negative risk factor group. However, it was not statistically significant. Conclusion:We confirmed that free flap reconstructions are highly versatile and reliable options for use in the reconstruction of various soft tissue defects of the head and neck. Free flaps have gained great popularity given its versatility, ability for a two-team approach, and minimal donor site morbidity. However, complications related to microvascular surgery may be overcome by increased surgical experience and by intensive flap monitoring in early postoperative period.
