PNF를 이용한 경부 운동이 뇌졸중 환자의 연하기능과 경부 통증에 미치는 영향 (Effect of Neck Exercise with PNF for Dysphagia and Pain of Neck in Patients with Stroke)

  • Purpose: This study aimed to investigate the effect of neck exercises using proprioceptive neuromuscular facilitation (PNF) on dysphagia and neck pain in stroke patients. Methods: This study included 20 patients with stroke who performed the following neck exercises using PNF: flexion, lateral flexion, and rotation. The PNF technique was repeated under the guidance of verbal commands from a physical therapist. The exercise program consisted of 5 times, 30-minute sessions each week for four weeks. The Mann Assessment of Swallowing Ability (MASA) and the visual analogue scale (VAS) were used to evaluate the patients before and after the training. Swallowing ability was evaluated using MASA, and the patients' pain was evaluated using VAS. All data were analyzed using SPSS 21.0. Results: Significant differences were observed for the MASA and VAS in the subjects: MASA was significantly improved (p<0.00) and VAS was significantlyincreased (p<0.00). Conclusion: The neck is a very important part of the body in stroke patients. Neck exercises using PNF are effective for improving dysphagia and decreasing neck pain in patients with stroke. Thus, neck exercises using PNF are very effective for stroke patients, and this approach would be effective in clinical practice.

슬링을 이용한 목 안정화 운동 시 아래턱 위치가 고유수용성감각과 관절가동범위에 미치는 영향 (The Effect of Mandible Position on Proprioception and Range of Motion during Neck Stabilization Exercise using a Sling)

  • Purpose: The aim of this study was to investigate the effect of mandible position on proprioception and range of motion (ROM) during neck stabilization exercise using a sling in healthy adults. Methods: The subjects were randomly assigned to either a sling exercise and mandible open group (n=10) or a sling exercise and mandible closed group (n=12). The sling exercise-mandible open group and sling exercise-mandible closed group took part in an exercise program for 30min, three times per week for 4 weeks. After each training session, head repositioning accuracy (HRA) and the ROM of the cervical spine were measured. Wilcoxon's test was conducted to verify changes within each group, and the Mann-Whitney U test was performed to examine between-group differences. Results: The HRA of the cervical spine was significantly increased during left rotation and extension in the sling exercise-mandible open group. In addition, there were significant differences in both rotations and extension in the two groups. The ROM of the cervical spine increased significantly during both rotations in the sling exercise-mandible closed group. In addition, there was a significant difference in right rotation and extension in both groups. Conclusion: Cervical stabilization exercise using a sling, with the mandible closed increased proprioception and the ROM of the cervical spine.

앞쪽머리자세가 있는 만성 목통증 환자에게 압력 바이오피드백 장비를 이용한 목안정화운동 적용이 목통증과 앞쪽머리자세, 음향학적 특성 변화에 미치는 효과 (Effects of Cervical Stabilization Exercise Using Pressure Biofeedback on Neck Pain, Forward Head Posture and Acoustic Characteristics of Chronic Neck Pain Patients with Forward Head Posture)

  • PURPOSE: This study was conducted to measure the effects of cervical stabilization exercises on neck pain, forward head posture, and the acoustic characteristics frequency and amplitude modulation of patients with chronic neck pain caused by forward head posture using pressure biofeedback. METHODS: 20 patients with chronic neck pain and voice disorders presenting at the S Exercise Center in Daegu, Korea, were included in the study. A cervical stabilization exercise program of 50 minutes per session was performed three times a week for eight weeks. Pressure biofeedback was utilized to determine the impact of the exercises on neck pain, forward head posture, and the acoustic characteristics of the patients. The measurements were taken prior to and after the intervention to determine any changes. RESULTS: A significant improvement in neck pain, craniovertebral angle and the acoustic characteristics frequency and amplitude modulation of the patients was demonstrated after the intervention (p<.05). CONCLUSION: Cervical stabilization exercises were demonstrated to have a significantly positive effect on neck pain, forward head posture, and vocalization stability in patients with chronic neck pain in the current study based on measurements taken using a pressure biofeedback system. This indicates that an improvement in forward head posture positively impacts postural stability and vocalization. Future studies investigating a greater range of interventions designed to improve neck pain and acoustical effects in patients with chronic neck pain and forward head posture patients are warranted.

  • Purpose: The current study seeks to identify the effect of neck muscle strengthening exercise using proprioceptive neuromuscular facilitation (PNF) on the swallowing ability of patients diagnosed with dysphagia due to stroke. Methods: As a single case study, the current research conducted neck muscle strengthening exercise using PNF on the patient with dysphagia for 40 minutes, four times per week for a six-week period. At the same time, typical rehabilitation therapy for dysphagia was provided. This type of therapy included food-swallowing and the relation of the muscles surrounding the neck. The functional dysphagia scale and the penetration-aspiration scale were used to assess swallowing ability. Results: After the therapy, the functional dysphagia scale and the penetration-aspiration scale decreased by 18 points and 3 points, respectively, which proves the effectiveness of this type of therapy for dysphagia. Conclusion: The results of the current study indicate that neck muscle strengthening exercise using PNF reduces penetration-aspiration in patients with dysphagia, and that PNF can be clinically utilized to improve the swallowing ability of dysphagic patients.

방사선 사진상 경추 후만을 보인 만성 경항통 환자의 만곡 이상 치험 2례 (Two Cases Report of Chronic Neck Pain Patients with Kyphotic Cervical Curvature Measured by Radiography)

  • We researched two patients complaining about chronic neck pain with kyphotic cervical curvature. We assumed that the patients' symptom was caused by weakened deep neck flexor and activated superficial muscles around neck. So, acupuncture therapy, SCENAR therapy and self exercise with wooden neck pillow were used to treat the patients. We measured their pain by numerical rating scale (NRS) and neck disability index (NDI) before and after treatment. And cervical curvature was evaluated by Cobb method (C1-C7) and Ishihara Index. As a result, NRS and NDI significantly reduced and cervical curvature was also improved. Therefore, we conclude that acupuncture therapy with SCENAR therapy and self exercise using wooden neck pillow is an effective treatment to reduce chronic neck pain with kyphotic cervical curvature. But there is a limit on this study due to insufficient number of cases and absence of control group. Further studies will be needed.

VDT 증후군을 가진 사무직 근로자를 위한 심부경부굴곡 운동의 효과 연구 (The Effects of a Deep Cervical Flexion Exercise for Sedentary Workers with VDT Syndrome)

  • The aims of this study were to compare and analyze two types of deep cervical flexion exercises, craniocervical flexion and cervical flexion, and to ascertain their relative effects on neck and shoulder pain and functional improvement. The participants of the study were individuals who work in sedentary jobs in an office environment. 54 appropriate subjects were chosen and randomly divided into two groups: one group underwent a craniocervical flexion exercise program and the other a cervical flexion exercise program. The six week exercise program consisted of home exercises performed by each subject five times a week and education once a week. Neck-shoulder pain, muscle strengthening, cervical alignment were measured prior to commencement of the exercise programs and again after six weeks. At completion of the six week, both exercise programs decreased neck pain (p<.05). Forward head postures were reduced, and the craniocervical flexion exercise program was more effective in reducing forward head posture (p<.05). The maximum muscle strength and 50% of maximum muscle strength maintaining time of the deep cervical flexor muscles were significantly increased. There were no significant changes of the cervical lordotic curve. The results of this study showed deep cervical flexion exercise was effective in the treatment of neck and shoulder pain, however craniocervical flexion exercise was more effective than cervical flexion exercise.

컴퓨터 단층촬영을 통한 깊은목굽힘운동 수행 시 일자목 환자와 정상인에게 나타나는 목근육의 단면적 비교 (Comparison of Cross-sectional Areas using Computerized Tomography of the Cervical Muscles between Straight Neck Patients and Normal Participants during Cranio-cervical Flexion Exercise)

  • PURPOSE: This study compared the cross-sectional areas (CSA) of the cervical muscles of straight neck patients and normal participants during a craniocervical flexion exercise (CCFE) using computerized tomography (CT) to investigate the effects of CCFE on the cervical curve. METHODS: Eighteen subjects were recruited for this study. Nine subjects were allocated to the straight neck group (subjects with pain and a cervical lordosis angle of less than 20°); the remainder formed the control group (subjects with a cervical lordosis angle greater than 20°). The CSA of the subjects' neck flexors (longus colli, longus capitis, and sternocleidomastoid) were measured by CT during rest and CCFE in the supine position, and the range of motion (ROM) of neck flexion was measured using a C-ROM instrument in a sitting. RESULTS: The straight neck group had a significantly smaller CSA of the longus colli, longus capitis, and sternocleidomastoid than the control group (p < .05). Both the straight neck and control groups showed statistically significant increases in the CSA of the neck flexors during CCFE compared to that at rest (p < .05). In addition, the straight neck group showed a significantly smaller ROM of neck flexion than the control group (p < .05). CONCLUSION: The results of this study provide more concrete evidence for therapists by demonstrating that CCFE improves the neck function by strengthening the neck flexors and increasing the neck stability for straight neck patients. Therefore, it is necessary to perform CCFE and neck extension exercises to rehabilitate straight neck patients.

PNF를 이용한 아래등세모근 강화 운동이 만성 목 통증 환자의 통증, 관절가동범위, 장애에 미치는 영향 (The Effect of Lower Trapezius Strengthening Exercise Using PNF on Pain, Range of Motion, and Disability in Patients with Chronic Neck Pain)

  • Purpose: The study aimed to determine the effect of proprioceptive neuromuscular facilitation (PNF) lower trapezius muscle strengthening exercise on pain, cervical range of motion (ROM), and neck disability index (NDI) in patients with chronic neck pain. Methods: Following baseline measurements, the subjects (n = 30) with chronic neck pain were randomized into two groups: the PNF group (n = 15) that received PNF strength training of the lower trapezius muscles or a control group (n = 15) that received gentle palpation of the skin. Each group participated in the intervention for 30 min, three times per week for six weeks. The visual analogue scale for pain, ROM, and NDI of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with pre-intervention, and independent t-tests were used to analyze differences in the dependent variables between the two groups. Results: After the six-week intervention, both groups experienced significantly decreased pain and NDI (p < 0.05) and significantly increased cervical flexion, extension, lateral flexion, and rotation ROM (p < 0.05). The PNF group that received PNF strength exercise of the lower trapezius muscles showed greater improvements in pain and NDI and cervical rotation of ROM than those of the control group (p < 0.05). Conclusion: These results suggest that the PNF lower trapezius strengthening exercise reduces neck pain and disability level and enhances cervical ROM in patients with chronic neck pain.

다이나믹 테이핑을 적용한 복합 운동프로그램이 긴장성 두통과 전방머리자세를 동반한 만성 목 통증 환자에게 미치는 효과 (Effects of a Complex Exercise Program using Dynamic Taping on Patients with Tension-Type Headache and Chronic Neck Pain with Forward Head Posture)

  • Purpose : This study examined the effects of pain, neck dysfunction, psychosocial level, headache impact test (HIT), postural alignment, and trapezius muscle tone of the complex exercise program using dynamic taping in patients with tension headache and chronic neck pain with forward head posture. Methods : The design of this is a randomized controlled trial (RCT). Thirty-four patients with chronic neck pain were screened using a randomized assignment program and assigned to experimental group (n=17) and control group (n=17). Both groups underwent a complex exercise program. In addition, the experimental group dynamic taping was applied to the upper trapezius muscle. All interventions were applied three times per week for four weeks. The visual analogue scale (VAS), the neck disability index (NDI), short form-12 health survey questionnaire (SF-12), Headache impact test-6 (HIT-6), Craniovertebral angle (CVA), Cranial rotation angle (CRA), upper trapezius muscle tone were compared to evaluate the effect on intervention. Results : Both groups showed significant differences before and after the intervention in VAS, NDI, SF-12, HIT-6, and CVA, CRA (p<.05). In addition, significant differences in NDI and upper trapezius muscle tone were observed between the experimental group and control group (p<.05). Conclusion : A complex exercise program using dynamic taping for patients with tension headache and chronic neck pain with forward head posture are effective method with clinical significance in improving the function and reducing upper trapezius muscle tone.

교각운동에서 다리의 위치에 따른 목폄근의 활성도에 미치는 영향 (Effects of the Trunk and Neck Extensor Muscle Activity According to Leg Positionon in Bridging Exercise)

  • PURPOSE: This study aims to determine the optimal knee joint angle and hip joint angle for minimizing the cervical muscle tension and maximizing the muscle activity of the trunk during the bridging exercise for trunk stabilization. METHODS: The bridging exercise in this study included seven forms of exercise: having a knee joint flexion angle of $120^{\circ}$, $90^{\circ}$, $60^{\circ}$, $45^{\circ}$ and hip joint abduction angle of $15^{\circ}$, $10^{\circ}$, $5^{\circ}$. The posture of the bridging exercise was as follows. To prevent the increase of hyper lumbar lordosis during the bridging exercise, the exercise was practiced after maintaining the lumbar neutral position through the pelvic posterior tilting exercise. RESULTS: The abduction angles did not result in statistically significant effects on the cervical erector, external oblique, rectus abdominis and erector spinae muscles. However, in relation to the knee joint angles, during the bridging exercise, statistically significant results were exhibited. CONCLUSION: The knee joint angle affected the muscle activity of the neck muscle. The greater the knee joint angle, the lower the load placed on the neck muscle. In contrast, the load increased as the knee joint angle decreased. In addition, the muscle activity of the neck muscle and trunk muscle increased as the knee joint angle decreased.
