Developmental anomalies arising from the branchial apparatus include cysts, external sinuses, internal sinuses, and complete fistulas. Second branchial cleft cysts are by far the most common among these anomalies. It may occur at any age, being most common in the third decade, and more frequent in the male than in the female. It usually presents a smooth, round, nontender fluctuant mass located between the level of the tragus and the clavicle along the anteromedial border of the sternocleidomastoid muscle. It is lined by respiratory or squamous epithelium unless inflammation is present. The considerable amount of lymphoid tissue may be found beneath the epithelium. The treatment of choice of branchial cleft cyst is surgical excision. If the lesion is acutely infected, however, it is essential to relieve the infection prior to the surgery. This report deals with two cases of second branchial cleft cyst. In case 1, the cyst had rapidly increased in size over pregnant period. In case 2, the patient presented the swelling in the left neck, and had the history of incision and drainage because of misdiagnosis as submandibular space abscess. The infection was treated by antibiotic therapy in the first place, and then complete surgical excision was made. There was no evidence of any recurrence or complications for these $3{\sim}4$ years.
Objective: This study aims to identify important physical design variables in designing a necklace type wearable device, and to present design guidelines to maximize comfort that a user feels upon wearing the device. Background: Interests in fitness culture and personal health are on the rise recently. In such a situation, demand for necklace type wearable devices is projected to increase a lot, as the devices enable users to use their hands freely and to enjoy various contents through connection with mobile devices. However, the necklace type wearable device's comfort was assessed to have the lowest comfort in a running situation, where human body moves up and down and left and right more than other devices wearable on other human body parts. Therefore, the usability of a necklace type wearable device was low. In this regard, studies on identification of the variables affecting user comfort upon wearing a necklace type wearable device and on physical design direction maximizing comfort and usability are needed. Method: A pretest and a main test were carried out to draw the direction of necklace type wearable device design. In the pretest, wearing evaluation on the diverse types of devices released in the market was conducted to draw physical design variables of the devices affecting comfort. Furthermore, variables significantly affecting the comfort of a device were selected through an analysis of variance (ANOVA). In the main test, anthropometry was performed, and information on anthropometric items corresponding to the design variables selected in the pretest was acquired. Based on the pretest results and the anthropometric information in the main test, the present study produced design guidelines maximizing the comfort of a necklace type wearable device with regard to major design variables upon dynamic tasks. Results: According to the pretest results, the variables having effects on comfort were the angle of side points, width, and height. Due to interactions between variables, those need to be simultaneously considered upon designing a device. Upon dynamic tasks, the angle of side points and width of a device was designed to be smaller than mean angle of the trapezius muscle and neck width, and thus attachment to human body was high. As height was designed to be larger than mean neck front and rear point width, comfort was higher due to feeling of stability. Conclusion: Because user sensitivity to comfort was high at human body's inflection points, a device needs to be designed for users not to feel high pressure on specific body parts with the device fitting human body shape well. A design considering user's situation is also required in further studies.
Purpose: Hyoid bone is a U-shaped bone in the anterior of the neck. Hyoid bone fractures are exceedingly rare and represent only 0.002% of all fractures because of its protective position relative to the mandible and its suspension by elastic musculature. We report a patient who presented hyoid bone fracture associated with hypoglossal nerve palsy. We also discuss the possible complication and treatment. Methods: A 69-year-old man was transferred from another institution because of persistent purulent discharge from the left chin. He had a history of trauma in which a knuckle crane grabbed his face and neck in the construction site. A CT scan at the time of the accident demonstrated a comminuted fracture of the right side of the mandible and hyoid bone fracture at the junction between body and right greater cornua. The displaced fracture of hyoid bone and fullness in the pre-epiglottic space were noted, probably indicating some edema. The patient was transferred into ICU after treatment of emergency tracheostomy because the patient showed respiratory distress rapidly. When the patient was hospitalized in our emergency room, he complained of dysphagia and pain when swallowing. On examination of oral cavity, the presence of muscle wasting with fasciculation of the tongue was noted and the tongue deviates to the left side on protruding from the mouth. Pharyngolarygoscopy was performed to make sure that there was no evidence of progressive swelling and pharyngeal laceration. Results: The patient underwent surgical removal of dead and infected tissue from the wound and reconstruction of mandibular bony defect by iliac bone grafting. Hyoid bone fracture was managed conservatively with oral analgesics, soft diet and restricted movement. Hypoglossal nerve palsy was resolved within 7 weeks after trauma without complications. Conclusion: Closed hyoid bone fracture is usually uncomplicated and thus it can be treated conservatively. Surgical intervention for hyoid bone fracture is recommended for patient with airway compromise, pharyngeal perforation and painful symptoms which show no response to conservative care. Furthermore, since respiratory distress syndrome may develop quickly, close observation is required. Besides, hypoglossal nerve palsy is a rarely recognized complication of hyoid bone fracture.
Purpose: The purpose of this study was to investigate whether Qihyul-therapy was effective against subjects of bad Qihyul circulation who had narrowed muscles of the buttocks, a distorted body shape, or pain. Methods: Qihyul-therapy used. Results: The muscles of the buttocks remained almost untouched. The elasticity of the muscles restored. The skin tone brightened, and the anal muscle strengthened after Qihyul-therapy. The subject's body was shifted to the right by -7°. The right arm was about 6 cm longer than the left arm. But after correction, the right arm was about 5 cm shorter. The distorted body was almost balanced. After neck and shoulder correction, the distance between the collarbone and neck was 7~10 cm wider. After the pelvic correction, the pain in the buttocks was relieved, and the position of the hip joint returned correctly. The right leg that was turned 45° to the left was corrected, and the length became the same. Subjects had no knees attached, their right knees were up 5 cm above the left, and their ankles bent at an angle of 30 ° relative to the left, but after correction, the knees of the two legs stably attached. The height of the knee became the normal angle. Conclusion: If the Qihyul-therapy, the spine correction Qihyul-exercise and the pelvic correction Qihyul-exercise performed steadily, the deformed body shape restored adequately, and it is useful to prevent and cure related diseases by alleviating the pains.
After transplantation of groin free flap was sucessed by the Daniel and Taylor in 1973, the reconstruction of plastic surgery was extensive and universal due to rapidly developement of anatomic study of the donor site and technique of microvascular surgery. The free tissue transfers is possible to be early activity and rehabilitation by one stage operation. It currently available allow transfer of specific tissue quality as bone, muscle, nerve to achieve a functional and cosmetic result as well as the most favorable secondary defect. But free flaps require critical, skillful technique and lengthy operating time. Also it has disadvantage of donor site morbity at the large tissue transfer. Authors were transferred with 107 cases in 103 patients from May 1987 to June 1996, and then we analysed free tissue transfer to acquire more increased sucess rate, satisfactory functional and cosmetic results. The sexual distribution was male prominent in 79 cases(76.7%), female in 24(23.3%) and age was variable distribution from 3 to 76 years old. The cause of defects was most prevalent in trauma of traffic and industrial accident in 51 cases(49%). The common recipient site were lower extremities in 47 cases(43.9%), upper extremities in 28 cases(26.5%), head and neck in 25 cases(23.4%), and trunk in 7 cases(6.5%). The type of transfer were free skin flaps in 46 cases(43%), free muscle or musculocutaneous flaps in 31 cases(29%), free vasculized or osteocutaneous flaps in 10 cases(9.3%), and specilized free flaps in 20 cases(18.7%). The anastomosis of artery was end to end anastomosis in 94 cases(87.9%), end to side anastomosis in 13 cases(12.1%) and all vein was end to end anastomosis. The number of anastomosed vessels were one artery one vein in 62 cases(57.9%), one artery two vein in 45 cases(42.1%) and vein graft was performed only one case. The postoperative mornitoring were used with temperature, color of flap, capillary refilling time, ultrasonogram, bone scan, doppler, and endoscopy. The reexploration was performed in 9 cases(8.4%), and then flap was loss in 3 cases(2.8%). Accordingly overall success rate was 97.2%. The postoperative complication was early vascular occlusion, hematoma, partial necrosis and late bulkiness, scarring, color dismatch etc. Therefore, free tissue transfer is the preferred method of treatment, even through conventional local and distant flaps are available.
In recent years, some researchers reported that myofascia was innervated by the autonomic nervous system. However, there is no neurophysiological explanation and evidence for the effects of myofascial release(MFR). Thus, the aim of this study was to determine whether the excitability of the autonomic nervous system is modulated by MFR. In this study, thirty healthy subjects in their 20s were randomly assigned to a myofascial release group(MG) and a placebo control group(PCG); each group had 15 subjects. The MG conducted 5 minutes of cranial base release in supine position, and the PCG performed sham cranial base release. Muscle flexibility was measured with the neck range of motion and the changes of the autonomic nervous system excitability was measured by heart rate, blood pressure, and concentration of plasma epinephrine and norepinephrine. The results were as follows: 1. The percentage changes in the cervical range of motion for extension and side flexion were significantly increased in the MG, signifying that more muscle relaxation. 2. There was no significant percentage changes in heart rate, blood pressure, and concentration plasma epinephrine between MG and PCG. 3. The percentage change in concentration plasma norepinephrine was significantly different between MG and PCG. The result of this study suggests that there is no evidence that MFR can modulate the autonomic nervous system excitability.

We proposed that, especially, J$\bar{i}$n ji$\acute{e}$(根結) has got a close relationship with a operation of the defense qi(衛氣). The ji$\acute{e}$(結) of three yang(三陽) located in ears and eyes, that are a starting point of the defense qi(衛氣) operation in the daytime and a one of five sense organs(五官). Gin, Liu, Zou and Ru (根, 溜, 注, 入) of the three yang(三陽) distinguished between the three yang(三陽) in the extremities. and in the symptoms of a disease of the three yang(三陽) on the bolt - leaf - hanges(關闔樞) theory, Greater yang(太陽) is related to the skin and flesh, Yang brightness(陽明) is related to the flesh and Lesser yang(少陽) is related to the muscle or bone. These skin, flesh, muscle and bone belonged to the five bodies(五體). The five bodies(五體) have relationship with the defense qi(衛氣)'s operation and function part. The ji$\acute{e}$(結) of three yin(三陰) located in neck, chest and abdomen. If we could catched the concepts on the ji$\acute{e}$(結) of three yin(三陰) and The Gin, Liu, Zou, Ru and ji$\acute{e}$(根, 溜, 注, 入, 結) position of three yang(三陽) altogether, we could suggested the theory of the entire area completed in the surface of body. so the defense qi(衛氣)'s protecting function of the whole body surface is achieved. In the symptoms of a disease of the three yin(三陰)'s the bolt - leaf - hanges(關闔樞) theory, greater yin(太陰) and reverting yin(厥陰)'s symptoms indicates the defense qi(衛氣)'s main action of a chest and abdomen. And lesser yin(少陰)' symptoms is about a vessle, that is not to mention on the five bodies(五體) of the three yang(三陽)'s symptoms, so here is mentioned the relationship of the defense qi(衛氣) and the five bodies(五體) strengthened. In the "J$\bar{i}$n ji$\acute{e}$ chapter(根結編) of Lin Shu(靈樞)", as the meridians of the foot (足經) was described, except the meridians of the hand(手經), it is reasonable to infer that the defense qi (衛氣) is relevant to the meridians of the foot(足經) than the meridians of the hand(手經).
This study was carried out to compare the bone mineral density and risk factors of osteoporosis between normal and rheumatoid arthritis in postmenopausal women. Sixty-eight postmenopausal patients with rheumatoid arthritis(RA) were compared with 124 postmenopausal normal women. Data were collected from october, 1998 to April, 1999 at Dong-a university hospital in Pusan. From all subjects, individual characteristics such as age, body weight, height, age of menarche, duration of menopause, gravity, parity, and breast feeding period and factors of life style such as milk consumption, exercise, alcohol intake, cigarette smoking, coffee consumption were identified as influencing factors of osteoporosis by questionnaire. From RA patients, health assessment score, Ritchie articular index, erythrocyte sedimentation rate, C-reactive protein and steroid dosage were measured by rheumatologist on measuring bone mineral density. Bone mineral density was measured at the Lumbar spine, femoral neck, femur Ward's triangle, and femur trochanter using dual x-ray absorptiometry. The data was analyzed by using a frequency, t-test, Chi-square, ANCOVA with SPSS PC program. The results could be summarized as follows : 1) There was a significant difference in age and breast feeding period between RA patients and normal women. 2) RA patients took less calcium in the past and practiced less regular exercise in past and present than normal women. 3) There was no difference in lumbar bone mineral density between RA patients and normal women. 4) There was a significant difference in femur Ward's triangle and femur trochanter between RA patients and normal women after adjustment for age and breast feeding period. 5) The prevalence of osteoporosis of all subjects was the highest in femur Ward's triangle. In summary, our findings suggest that the bone mineral densities of femur Ward's triangle and trochanter in postmenopausal women with RA is significantly lower than normal women. Also the exercise participation rate of postmenopausal women with RA is lower than normal women. For the further study, we recommend to develop exercise program that improve the bone mineral density in femur Ward's triangle and trochanter and to test the effect of that exercise program.
The purpose of this study was to investigate muscle activity of cervical erector spinae & upper trapezius while using a smartphone according to anterior pelvic tilt & posterior pelvic tilt in sitting position. & we also want to obtain basic data necessary for development of IoT devices. Fifteen healthy men & women aged 20-30 were enolled, After anterior pelvic tilt & posterior pelvic tilt in sitting position were adjusted, they used a smartphone for 5 minutes & EMG signal was measured simultaneously. We used median 3 minutes of measured EMG signal. The results of this study were as follows: In the sitting position, the use of smartphone in anterior pelvic tilt had significantly lower %RVC values in both cervical erector spinae (p <.001), left upper trapezius (p <.001) & right upper trapezius (p <.002) in comparison with posterior pelvic tilt. This means that anterior pelvic tilt make vertebrae maintain normal curvature & prevent forward head posture occurred while using a smartphone. It reduces loads around neck & shoulders, so that %RVC value is significantly lowered. In the future, new posture corrected IoT devices with an aspect of pelvic tilt should be developed.

The purpose of this study was to examine contraction of abdominal muscles on surface electromyographic (EMG) activity of superficial cervical flexors, rib cage elevation and angle of craniocervical flexion during deep cervical flexion exercise in supine position. Fifteen healthy subjects were participated for this study. All subjects performed deer cervical flexion exercise with two methods. The positions of two methods were no volitional contraction of abdominal muscles in hook-lying position with 45 degree hip flexion (method 1) and 90 degrees hip and knee flexion with feet off floor for inducing abdominal muscle contraction (method 2). Surface EMG activities were recorded from five muscles (sternocleidmastoid, anterior scaleneus, recuts abdominis, external oblique, internal oblique). And distance of rib cage elevation and angle of craniocervical flexion were measured using a three dimensional motion analysis system. The EMG activity of each muscle was normalized to the value of reference voluntary contraction (%RVC). The EMG activities, distance of rib cage elevation. and angle of craniocervical were compared using a paired t-test between two methods. The results showed that the EMG activities of sternocleidmastoid and anterior scaleneus during deep cervical flexion exercise in method 2 were significantly decreased compared to method 1 (p<.05). Distance of rib cage elevation and angle of craniocervical flexion were significantly decreased in method 2 (p<.05). The findings of this study indicated that deep cervical flexion exercise with contraction of abdominal muscles could be an effective method to prevent substitute motion for rib cage elevation and contraction of superficial neck flexor muscles.
