이중미세나사산 임플란트의 임상적 및 방사선학적 1년 평가 (Clinical and radiographic evaluation of implants with dual-microthread: 1-year study)

  • Purpose: The stability of periodontal condition and marginal bone level were important to achieve long-term success of dental implant treatment. The aim of this study was to evaluate periodontal conditions and marginal bone loss around 67 GSII(OSSTEM, Seoul, Korea) dental implants with dual-microthread at the neck portion, 1 year after prosthetic loading. Materials and methods: Sixty-seven GS II dental implants in 27 patients(mean age; $47.4{\pm}14.0$ years) who received implant treatments at Pusan National University Hospital, were included in this study. Thirteen US II(OSSTEM, Seoul, Korea) implants with smooth neck design were selected for the control group. Periodontal and radiographic evaluations were carried out at baseline, 6 months and 12 months after prosthetic loading. Results: In the GS II group, plaque index(PI), gingival index(GI) and probing depth(PD) increased as time passed. In the US II group, GI and PD increased. Although marginal bone level was lower in the US II group in all evaluation periods, the changes between the periods were not statistically significant(p>0.05). In each period, periodontal parameters were not statistically significant between groups. Conclusion: One year after prosthetic loading, GS II and US II dental implants showed similar periodontal conditions and marginal bone response, and were within the criteria of success.

  • For improving the image quality in case of neck Lateral projections, the Al compensation filter were used and conclusion that, it could be got the high image quality able to observe the shadow of cervical spine, hyoid bone, trachea, soft tissue simultaneously.

경기 지역 폐경 전.후 여성들의 골밀도와 신체 계측, 모성요인 및 운동과의 상관성 연구 (A Study on Bone Mineral Density, Anthropometric Measurements, Maternal Factors, and Exercise and Their Correlation in Premenopausal and Postmenopausal Women in Gyeonggi-do Korea)

  • This study was performed to assess the relationships among bone mineral density, anthropometric measurements, maternal factors and exercise in premenopausal and postmenopausal women. Anthropometric measurements were taken by a trained practitioner and the maternal factors of the 78 subjects in Gyeonggi-do were acquired by an interview questionnare. The BMD of the lumbar spine(L2~L4), femoral neck, Ward's triangles and trochanters were measured by dual energy X-ray absorptiometry. The mean age of the premenopausal women was 45.23 years and that of the postmenopausal women whose ase of menopause was 49.37 years was 61.27 years. The age, waist and waist to hip ratio of postmenopausal women were significantly higher than those of premenopausal women. BMD of the lumbar spine(L2~L4) in postmenopausal women was significantly higher than that of premenopausal women. Number of children and age at last delivery were significantly higher in postmenopausal women than those in premenopausal women. The BMDs of the lumbar spine and femoral neck of premenopausal women was positively correlated with weight, body mass index, waist and WHR and the BMDs of these two sites in postmenopausal women were positively correlated with height, and weight. In postmenopausal women, BMD of the lumbar spine was negatively correlated with duration time after menopause and BMD of the femoral neck was positively correlated with age at last delivery. These results suggest that it is necessary to maintain adequate body weight. Health management and education about performing more exercise are recommended for postmenopausal women to prevent osteoporosis.

Osteoradionecrosis of Jaw in Head and Neck Cancer Patient Treated with Free Iliac Bone and Umbilical Fat Pad Graft

  • Osteoradionecrosis is one of the most serious complications of patients receiving radiation therapy. It is characterized by hypovascularity, hypocellularity, and hypoxia-inducing necrosis of bone and soft tissue following delayed healing. In this case, a 72-year-old man was referred to the Department of Oral and Maxillofacial Surgery complaining of trismus following extraction three months before first visit. He had a history of right tonsillectomy, radical neck dissection and radiotherapy performed due to right tonsillar cancer seven years prior. After the diagnosis of osteoradionecrosis on right mandibular body and angle, conservative antibiotic therapy was used first, but an orocutaneous fistula gradually formed, and extensive bony destruction and sequestrum were observed. Sequestrectomy, free particulated iliac bone and umbilical fat pad graft were performed via a submandibular approach under general anesthesia. Preoperative regular exams and delicate wound care led to secondary healing of the wound without vascularized free flap reconstruction.

한국 여성 노인의 대퇴경부 및 요추 골밀도 관련 요인 (Factors Associated with the Lumbar Spine and Femoral Neck Bone Mineral Density in Korean Elderly Women)

  • 본 연구는 국민건강영양조사 제 4기 2 3차년도, 제 5기 1차 년도에 참여한 60세 이상 여성노인을 대상으로 하였다. 연구의 목적으로는 신체계측, 생활습관, 식이섭취, 골절 과거력, 골다공증 가족력, 병력, 월경력 및 산과적 요인 등의 제 요인들과 대퇴경부 골밀도와 요추골밀도의 관련성을 알아보고자 시도하였다. 각 변수의 단위별 BMD의 변화를 표현하기 위해 비율차이 계산식 ${\beta}{\times}$(단위/BMD의 평균)를 이용한 다중선형회귀분석을 실시하였다. 모든 연속 변수의 단위는 1 표준편차를 사용하였다. 연구결과, 대퇴경부 및 요추 골밀도에 조사된 각 독립변수 중 여성노인의 골밀도에 가장 큰 영향을 미치는 것은 대퇴경부 부위에서는 제지방량(B: 0.257), 요추 부위에서는 체지방(B: 0.237)으로 확인되었다. 따라서 여성노인에서는 제지방량을 늘리고, 적절한 체중을 유지하는 것이 골다공증을 예방하는 효과적인 방안이 될 것이다.

면류 섭취 여부에 따른 영양소 섭취와 건강지표의 관련성 - 2010 국민건강영양조사 자료를 바탕으로 - (Comparison of Nutrient Intake and Health Indices by the Intake of Noodles - Using the Korea National Health and Nutrition Examination Survey (KNHANES, 2010) -)

  • The purpose of this study was to evaluate the nutrient intake, diversity and prevalence of diseases according to noodle consumption using data from the Korean National Health and Nutrition Examination Survey (KNHANES) of 2010. Korean adults (>20) were classified into two groups, noodle group (NG) and non-noodle group (NNG). NG was found to have higher intakes of protein, calcium, iron, sodium, potassium, and niacin. The DVS and DDS according to noodle consumption were found to be 21.76 and 3.51 for NG, and 18.56 and 3.40 for NNG, this result was thought to show that the diet of NG was more satisfactory than that of NNG in terms of dietary diversity. In the bone mass of entire subjects, NG showed higher figures in the femoral neck and lumbar spine than NNG. For bone mineral density, NG exhibited higher figures in the femoral neck, and for T-scores, the values of NG was significantly higher in the femoral neck than those of NNG. However, the prevalence of osteoporosis of NG and NNG was 11.6% and 9.5%. The prevalence of osteopenia of NG and NNG was 43.0% and 51.7%. Thus, these results showed that overall bone health of NG was better than that of NNG. Dietary balance and quality were also more satisfactory in NG. These results showed that noodle consumption may have an impact on the prevalence of chronic diseases.

운동이 중년이후 여성의 골밀도에 미치는 효과에 대한 메타 분석 (A Meta Analysis on the Effects of Exercise on Bone Mineral Density among Middle-aged and Older Women)

  • Purpose: This study was to examine the effectiveness of exercise for bone mineral density (BMD). Methods: Four investigators reviewed English articles from Pub Med and CINAHL, selecting randomized controlled trials on exercise programs for middle-aged and older women. Out of 25 studies identified, 14 that satisfied with the inclusion criteria were included in the meta-analysis. The quality of the studies was assessed using recognized methods and the effect size was calculated as a Hedges'g using Comprehensive Meta-analysis Version 2.0. Primary outcomes were changes in BMD at femoral neck, trochanter, and lumbar spine. Subgroup analysis included changes in BMD according to exercise style. Results: Weight bearing exercise was effective(Q=20.1, p>.05, ES=0.32), and resistance exercise was effective in case of comparing to pre and post intervention (Q=4.15, p=.98, ES=0.14). At the femoral neck, 9 study groups were homogeneous and the experimental groups demonstrated a positive effect on BMD (Q=19.5, p>.05, ES=0.33). In contrast, marked heterogeneity (Q=33.3, p<0.01) was apparent in 7 study groups evaluating trochanter. Conclusion: These findings suggest that weight bearing is effective for BMD of the femoral neck, and is relevant to the non-pharmacological treatment of bone loss for middle-aged and older women.

Retrospective study of osteoradionecrosis in the jaws of patients with head and neck cancer

  • Objectives: Osteoradionecrosis (ORN) is one of the most severe complications resulting from radiotherapy (RT) in patients with head and neck cancer (HNC). It is characterized by persistent exposed and devitalized bone without proper healing for greater than 6 months after a high dose of radiation in the area. To describe the profile and dental management of ORN in HNC patients undergoing RT in an oncological clinical research center. Materials and Methods: A retrospective descriptive study was performed to analyze dental records from HNC patients with ORN treated at an oncological clinical research center from 2013 to 2017. A total of 158 dental records for HNC patients were selected from a total of 583 records. Afterwards, this number was distributed to three examiners for manual assessments. Each examiner was responsible for selecting dental records that contained an ORN description, resulting in 20 dental records. Results: Mean patient age was 60.3 years with males being the most affected sex (80.0%). The most affected area was the posterior region of the mandible (60.0%) followed by the anterior region of the mandible (20.0%) and the posterior region of the maxilla (10.0%). The factors most associated with ORN were dental conditions (70.0%) followed by isolated systemic factors (10.0%) and tumor resection (5.0%). There was total exposed bone closure in 50.0% of cases. The predominant treatment was curettage associated with chlorhexidine 0.12% irrigation (36.0%). Conclusion: Poor dental conditions were related to ORN occurrence. ORN management through less invasive therapies was effective for the closure of exposed bone areas and avoidance of infection.

갑상설관 낭종에서 발생한 유두상암 (Two Cases of Papillary Thyroid Carcinoma Arising in Thyroglossal Duct Cyst)

  • Thyroglossal duct cyst is a congenital anomaly generally appearing as an asymptomatic midline neck mass. Generally, the duct cyst is benign, but 1 percent of cases may be malignant. These present the same histologic types as thyroid carcinomas. The most common pathologic type is papillary carcinoma. Regional lymph node metastasis of papillary carcinomas in thyroglossal cyst occurs in 7.7 percent of cases. Prognosis of papillary carcinoma is excellent. The treatment has been quite variable, but the most common initial treatment is Sistrunk's operation. Adequate excision of cyst and its tract including the mid-portion of the hyoid bone is the treatment of choice. We experienced two cases of papillary carcinoma arising in thyroglossal duct cyst between 1986 and 2002.
