첨단산업에서의 안전 사고 분석 패턴 추출 모델 연구

  • 안전사고 원인에 대하여 Near-Miss Report 분석을 근거로 하여, 재해원인에 대하여 접근해 보고, 이 Data를 참고로 첨단산업에서의 재해 유형을 분석해보고, 유형에 따른 기여인자를 기존 Human-ware, Hard-ware, System-ware를 구성요소로 해서 첨단산업에서의 각각의 수행인자의 비율 분석을 통한 판단한 결과는 Human-ware와 Hard-ware 비율은 4:1로 나타났고, 첨단사업장에서의 5년간의 사고건수를 근거로 원인에 대한 수행인자의 ANOVA로 분석하여 4개인자에 대한 분산 분석을 도출하였고 이에따른 Loss와 Time과 Accident 관계와 Effect of Intervention관계와 Reason's Accident Causation Model과 Perrow's Normal Accident Theory Model를 연관시켜서 첨단산업에서의 사고이론 Model를 추출해서 첨단산업에서의 안전사고에 대한 유형을 분석해서 사고를 사전에 제거키 위한 새로운 모델을 제시하고자 한다.

냉각재 상실사고 후 격납건물내의 이상유동 연구 (A Study on the Two Phase Flow in the Floor of Containment Building after a Loss of Coolant Accident)

  • The Regulatory Guide 1.82 recommends an analysis of hydraulic performance for sump of ECCS (Emergency Core Cooing System) when LOCA(Loss of Coolant Accident) occurs in a nuclear power plant. The present study deals with 3-dimensional, unsteady, turbulent and two-phase flow simulation to examine the behavior of mixture of reactor coolant and debris near the floor of containment building in conjunction with appropriate assumptions. The dispersed solid model has been adjusted to the interfacial momentum transfer between reactor coolant and debris. According to the results, the counterclockwiserecirculation zone had been formed in the region between sump and connection aisle about 376 second after LOCA occurs. The debris thickness accumulated on a sump screen periodically increases or decreases up to 2000 second, afterwards its peak decreases.

  • In the last five years, 45 people died and 104 were wounded because of carbon monoxide poisoning accident. CO poisoning accident is higher than any other gas accident in the rate of deaths/incidents. Most of these CO poisoning accidents were caused by defective exhaust tube in the old gas boiler and multi-use facility. In this study, the spread of CO gas released from leakage hole of exhaust tube was analyzed by computational flow modeling and concentration measuring test. CO gas leaked form exhaust tube in a building was highest concentrated near the ceiling and formed the circular currents along the walls. Through these experiments and simulation, the reasonable installation location of CO alarm was made certain and suggested.

MOUSE를 활용한 미사리 조정경기장의 정량적 침수해석 (Quantifying Inundation Analysis in Misari motorboat racing stadium using MOUSE)

  • Recently, heavy rainfalls due to the climate change in Korea have caused inundation problems in urban sewer networks. In july 2006, a flooding accident at Misari motorboat racing stadium near the Han river occurred due to the effect of record-breaking outflow discharge from Paldang-dam. The purpose of this study was to simulate and analyze the flooding accident at Misari stadium by MOUSE model. The results of simulation analysis indicated that the total flood volume was $1,313,450m^3$. The effect of back water was 85.9% of the total volume which was caused by the manhole accident, and the effect of accumulated runoff was 14.1% of total volume which was caused by non-return valve shutdown. The simulation results of this MOUSE modeling that was linked to the boundary condition of the dynamic flows in the river by DWOPER model showed the potential of successful inundation analysis for sewer networks.

A Study on the Collision Accident between Ferry Golden Jindo and Ferry Princess

  • Ferry Golden Jindo collided with Ferry Princess near the No.7 light buoy of Incheon Port No.1 Passage in restricted visibility due to dense fog. The result was that Ferry Golden Jindo got a hole at the starboard midship section shell plating and Ferry Princess sustained damages at the starboard bow and 25 persons injured The aim of this paper is to investigate this collision accident, to clarify its causes, and to prevent such accident from occurring again In short, this collision resulted from Princess' high speed in restricted visibility, Golden Jindo's carelessness of watchkeeping, lack of proper safety training of crew, lack of instruction of supervisor, carelessness af PTMS Center and indifference of Korea Shipping Association, etc.

Research and Development Methodology for Practical Use of Accident Tolerant Fuel in Light Water Reactors

  • Research and development (R&D) methodology for the practical use of accident tolerant fuel (ATF) in commercial light water reactors is discussed in the present review. The identification and quantification of the R&D-metrics and the attribute of candidate ATF-concepts, recognition of the gap between the present R&D status and the targeted practical use, prioritization of the R&D, and technology screening schemes are important for achieving a common understanding on technology screening process among stakeholders in the near term and in developing an efficient R&D track toward practical use. Technology readiness levels and attribute guides are considered to be proper indices for these evaluations. In the midterm, the selected ATF-concepts will be developed toward the technology readiness level-5, at which stage the performance of the prototype fuel rods and the practicality of industrial scale fuel manufacturing will be verified and validated. Regarding the screened-out concepts, which are recognized to have attractive potentials, the fundamental R&D should be continued in the midterm to find ways of addressing showstoppers.

  • 전통시장, 지하상가 등에서는 많은 유동인구가 있고 가스 전기 설비가 동일구내에 설치되어 두 에너지원의 연관성에 의한 사고 가능성이 증대되고 있으며, 이러한 사고를 예방하고 신속 대응하기 위한 방안이 필요하다. 이 연구에서는 위험인자와 관련된 영향인자 센싱을 통해 사고예측을 하는 것을 목표로 한다. 전기적요인의 누설전류, 아크 등과 폭발사고 위험과 관련된 가스누출에 대해 IOT 센서를 통한 사고예방을 할 수 있는 안전관리 플랫폼 개발의 연구이며 이 논문에서는 가스 사고와 관련된 화재위험인자를 고려한 사고예측에 대한 연구내용이다. 이러한 사고위험을 예방하기 위해서 연소기 주위에서의 온도변화 특성 분석을 수행하였고, 이를 고려하여 화재를 예방할 수 있는 사고전조 알고리듬과 관련된 모듈을 개발하였다.

사고와 아차사고 경험에 따른 원청과 협력업체 근로자 간 안전문화 인식 비교 (Comparison of Safety Culture Awareness between Client and Subcontractors' Employees according to the Experience of Accidents and Near Misses)

  • This study analyzes the impact that accidents and near misses have on clients' and contractors' awareness of safety culture. Due to the unique characteristic of employment structure in Korea, the occurrence of accidents differs by company size, which has relevant implications for the establishment of safety culture. Attention has been drawn to the importance of the management of accidents and near misses, with safety awareness acting as a core factor. A positive effect on the prevention of accidents could be achieved by noting the difference in safety awareness between clients and contractors and suggesting an associated suitable safety management system. In support of this study, a survey was distributed to workers in the automobile manufacturing industry (May-August 2020), and data from a total of 574 workers was collected and analyzed, including 399 clients' worksers and 175 contractors' workers. The questionnaire addressed participants' experiences of accidents and near misses as well as 50 items from the Nordic Occupational Safety Climate Questionnaire. Analysis of the responses was conducted using the methods of frequency analysis, Fisher's exact test, t-test, correlation analysis, and regression analysis. The results demonstrated that clients had more experiences with accidents and near misses compared to contractors. Additional differences between clients and contractors were noted in terms of the safety culture factors of learning, communication, and trust. A correlation was observed between the experience of accidents and safety justice management: for clients and contractors who experienced accidents, safety justice management was 9.4 times higher. Furthermore, clients' and contractors' awareness of employees' commitment to safety was determined to be 28.5 times higher in those who had experienced near misses This study concludes that, in order to improve accident prevention through the management of accidents and near misses, clients must focus on overseeing safety justice management and aspects of safety culture factors, while contractors must focus efforts on managing employees' commitment to safety. In further applications, this study could provide baseline data for health and safety activities in terms of the safety culture of clients and contractors. Further study on the establishment of safety culture as related to employment structure is proposed for future research.

96년 거제에서 패류 중독 사고를 유발한 진주담치의 마비성 독소 (Paralytic Shellfish Toxins in the Mussel, Mytilus edulis, Caused the Shellfish Poisoning Accident at Geoje, Korea, in 1996)

  • Food poisoning accident occurred on May, 1996 at Oepo, Geoje County, Kyeongnam Province, Korea, and two persons were died within a few hours after ingestion of the soup prepared with wild mussel, Mytilus edulis, harvested on the sea rock. Paralytic shellfish poisons (PSP) were elucidated as the responsible toxins for the food poisons accident because the wild mussels caught after three days at the near place from the accident contained high toxicity of PSP ranged $650\~1000MU/g$ of edible meat by mouse bioassay. Gonyautoxin-1+4 $(42.7\%)$ and C1+C2 $(40.0\%)$ were detected as the major toxins in the mussels by fluorometric HPLC method. Although, the poison extracted out with drip during freezing and thawing, and the toxicity gradually decreased by boiling for 20 minutes, over 30 MU/g of toxins remained in the soup and meat, which indicated that they could be able to make food poisoning.
