입원한 암환자의 항암화학요법 일주기 내의 오심 구토 변화 (Changes on Index of Nausea, Vomiting, and Retching in Hospitalized Cancer Patients Undergoing Chemotherapy)

  • Purpose: The purpose of this study was to determine the changes on Index of Nausea, Vomiting, & Retching (INVR) during a cycle of chemotherapy, Methods: Forty-three patients hospitalized for chemotherapy at C University Hospital during a period of 5 days from March to May, 2003 were examined, Scores of INVR were measured once a day, Anxiety, anorexia, fatigue, and sleep satisfaction were measured before chemotherapy, Data was analyzed by repeated measures of ANOVA, Results: The score of INVR increased over time during the days of hospitalization and showed a peak on the third day, The score was significantly higher on the third and consecutive cycles than on the first and second cycle, The score was significantly higher in patients in their forties and fifties rather than in their sixties, The score was higher in women than in men, and also increased as the sleep satisfaction decreased, Conclusion: These results suggested that specific interventions for relief of nausea & vomiting were needed in middle age, women, the third chemotherapy cycle, and the third day after chemotherapy.

임심오조증 임부의 증상완화를 위한 내관지압의 효과 (The Effect of P6 Acupressure for Symptom Control in Pregnant Women Having Hyperemesis Gravidarum)

  • Purpose: The purpose of this study was to determine the effects of Nei-Guan(P6) acupressure on nausea and vomiting in pregnant women having hyperemesis gravidarum. Method: The research design was a randomized control-group pretest-posttest repeated measure design with counter balancing. For the experimental treatment, P6 acupressure was carried out for ten minutes. Data was collected from April 1. 2003 to April 30. 2004. The participants were 66 patients admitted with hyperemesis gravidarum. They were divided into three groups, a P6 acupressure group, placebo point group and a control group. Result: Hyperemesis gravidarum patients who received P6 acupressure during admission experienced much less nausea and vomiting than the placebo acupressure and control group(F=8.259, p=.001). Conclusion: Nei-Guan(P6) acupressure is considered an effective intervention for reducing nausea and vomiting in pregnant women having hyperemesis gravidarum. Further more Nei-Guan(P6) acupressure maybe used as an independent nursing intervention method for pregnant women with severe nausea and vomiting.

Acupuncture effect on Chemotherapy-induced Vomiting and Nausea: A case series

  • Objectives: We carried out an observational study of the effects of Nei Guan (P6) and Gong Sun (SP4) acupuncture for the treatment of CINV, preparing for further randomized controlled trial study. This is a case series to explore the changes in the incidence of CINV by acupuncture. Methods: Patients reported a Rhodex index indicating the severity of nausea and vomiting and loss of appetite, before acupuncture, after acupuncture and 1-week follow-up examination. Twelve patients with CINV participated in this study. We included patients receiving chemotherapeutic agents that might induce moderate or severe nausea and vomiting. We conducted 5 treatment sessions with P6 and SP4 acupuncture over the course of 5 days. Results: The median Rhodex score were decreased in 5 patients over time, while we observed loss of appetite of 6 patients were improved over time. During the study, the mean of Rhodex of total patients revealed a decrease of Day 1, Day 7, Day 14; 0.74, 0.68, 0.38, respectively, while the mean of loss of appetite showed a decrease of Day 1, Day 7, Day 14; 5.25, 4.08, 4.00, respectively. Conclusions: The study gives a preliminary data that manual acupuncture of P6 and SP4 may reduce the severity of CINV and loss of appetite, and justifies further study.

영양교육프로그램이 항암화학요법을 받는 소화기암환자의 오심구토, 식욕부진, 식이섭취 및 영양상태에 미치는 효과 (The Effects of Nutritional Education Program on Nausea and Vomiting, Anorexia, Food Intake, and Nutritional Status of GI Cancer Patients Undergoing Chemotherapy)

  • Purpose: The purpose of this study was to identify the effects of nutritional education program (NEP) on nausea and vomiting, anorexia, food intake, and nutritional status among GI cancer patients undergoing chemotherapy. Methods: A nonequivalent, non-synchronized posttest design was used. Study subjects were conveniently selected 30 patients (15 subjects for each group) who were undergoing chemotherapy. The subjects of the experimental group received NEP which consisted of 2 educations and 1 counselling while they were hospitalized, and telephone counselling with nutritional supports after discharge for four times. Results: The score of anorexia was lower and the amount of food intake was higher in the experimental group than those of the control group. However, there were no significant differences in levels of nausea and vomiting, weight, hemoglobin, albumin, and total protein level between the experimental and the control group. Conclusion: The findings demonstrate that the NEP was effective in alleviating anorexia and improving subjective food intake of GI cancer patients undergoing chemotherapy. These findings suggest that NEP can be a useful nursing intervention for preventing nutritional disorders for patients with GI cancer after chemotherapy.

국내 암환자의 오심구토에 적용한 비약물적 중재효과의 메타분석 (A Meta-Analysis of Intervention Studies on Nausea and Vomiting in Cancer Patients)

  • Purpose: This study was designed to analyze the characteristics and effect size of published intervention studies related to nausea & vomiting among cancer patients. Methods: A total of 1,083 studies were retrieved. From these studies, 20 studies met the inclusion criteria with a total of 698 participants. Two authors independently extracted data from the selected studies and assessed the methodological quality. The data were analyzed by the RevMan 5.0 program of Cochrane library. Results: The twenty studies utilized various non-pharmacological interventions: Nine studies on acupuncture (45%), Five studies utilized massage (25%), Two studies used oral cryotherapy (10%) and Four studies used other therapies. In the twenty studies the effect size of the intervention studies reported a higher effect sizes for massage (d=-1.62) and acupuncture (d=-0.89). Conclusion: This study suggests that non-drug therapy can reduce the levels of nausea and vomiting intensity. Massage and acupuncture interventions studies were more numerous and this may account for the higher effect rate.

근육이완요법이 항암화학요법으로 인한 오심구토, 불안, 불편감의 감소에 미치는 영향 (Efficacy of Progressive Muscle Relaxation Training in Reducing Nausea and Vomiting, Anxiety and Symptom Distress of Cancer Chemotherapy)

  • Twenty five cancer patients were assigned to two groups: The experimental group which received Progressive Muscle Relaxation (PMR) training and the control group without the training. Base line data for dependent variables -nausea and vomiting, state anxiety and symptom distress- were collected when the subjects of both groups received the first cycle chemotherapy. Then the experimental group was trained for PMR between the first and the second cycle chemotherapy. The same dependent variables were measured during the second cycle chemotherapy from the subjects. Results indicated that the PMR group showed no significant difference in severity of nausea and vomiting compared to the control group. Although the significance was not supported, the severity of nausea and vomiting in experimental group decreased while control group increased between the first and fifth day of the second cycle chemotherapy. The experimental group showed significantly less state anxiety during the second cycle than the control group (p<0.01). Significantly low symptom distress was also reported in the experimental group compared to the control group(p<0.01). The result suggests that PMR training may be an effective procedure for helping cancer patients cope with the adverse effects of their chemotherapy.

  • 1. Objectives This case is to report that a Soyangin patient with nausea and vomiting treated by Soyangin's Constitutional medications 2. Methods This patient was treated by several Constitutional medications and acupuncture treatment according to the result of Sasang Constitutional diagnosis with western management. We used Visual analogue scale(VAS) for the assessment of nausea and vomiting. 3. Results and Conclusions This patient's chief complants were nausea and vomiting. So he was treated by Soyangin's Constitutional medications(especially Hyeongbangpaedok-san, Dojeokganggi-tang and Hyeongbangsabaek-san) and showed a positive response about his symptom. But then we consider that studies about that case like this are more needed.

  • 1. Objectives This case is to report that a Taeumin patient with nausea and vomiting treated by Taeumin's Contitutional medications. 2. Methods This patient was treated by several Constitutional medications and acupuncture treatment according to the result of Sasang Constitutional diagnosis with western management. We used Visual analogue scale(VAS) for the assessment of nausea and vomiting. 3. Results and Conclusions This patient's chief complaints were nausea and vomiting. So She was treated by Taeumin's Constitutional medications(especially Galgeunhaegui-tang, Cheongsimyeonja-tang) and showed a positive response about her symptom. Bur then we consider that studies about that case like this are mote needed.

임신오조(姙娠惡阻) 관련 무작위대조군연구(RCT)에 관한 고찰 - 경혈을 이용한 치료를 중심으로 - (Review on RCT Research Papers Related Nausea and Vomiting in Pregnancy - Focusing on Acupoint Treatment -)

  • Objectives: To observe randomized controlled trials(RCTs) related to investigating the effects of acupoint treatments on nausea and vomiting in pregnancy to establish the basis of korean medical treatments, and suggest the problems of future clinical research. Methods: After searching for national and international papers published after 2000 and restricting the parameters to 'clinical trial', 'controlled clinical trial', 'randomized controlled trial', then comparing the subjects, methods, research tools, and results to understand the effects and re-comparing the conflicting results to assume the cause. Results: Of the 9 research papers that applied acupressure, four of them used control and experimental groups, and three(75%) research papers indicated significant reduction in vomiting and nausea, and of the seven research papers that utilized the experimental and placebo groups, five(71.4%) reported significant reducing effects. Of the four research papers the used acupuncture treatments, four research papers using control and placebo groups, two showed improvements in symptoms but did not have significant results compared to sham acupuncture groups, and two showed significant reduction of symptoms compared to the control groups. The one research paper that used electrical stimulation showed significant results in the reduction of vomiting and nausea compared to the control group. Conclusions: Acupressure and electrical stimulation showed efficacy reducing the symptoms. Acupuncture treatments did not indicate efficacy some experiments. Therefore it is thought that future acupuncture treatments should use methods that resemble actual clinical medicine to establish efficacy to produce significant results.

Ondansetron과 Droperidol의 혼합 투여가 술 후 오심과 구토 예방에 미치는 효과 (The Effect of Combination of Ondansetron and Droperidol on Prevention of Postoperative Nausea and Vomiting)

  • Background: Ondansetron is both a central and peripheral serotonin (5HT) receptor antagonist and droperidol is a dopaminergic blocking drug which acts centrally at the chemoreceptor trigger zone. We assessed the efficacy and adverse effects of ondansetron, droperidol or both, in the prevention of postoperative emesis during postoperative intravenous patient-controlled analgesia (PCA) using butorphanol and ketorolac medication. Methods: We studied 60 women, aged 25-60 yrs, who underwent total abdominal hysterectomy (TAH), under general anesthesia using $N_2O-O_2$-enflurane. A bolus dose of 1 mg of butorphanol and 4 mg of ondansetron were given to patients and thereafter, PCA was started using 10 mg of butorphanol and 240 mg of ketorolac mixed into the 5% D/W solution (total volume; 100 ml, 1 ml of bolus dose, and 10 min of lockout interval). We also added ondansetron 4 mg (Group O, n = 20), ondansetron 4 mg and droperidol 2.5 mg (Group OD, n = 20), or droperidol 2.5 mg (Group D, n = 20) to the PCA drug. The severity of pain, nausea, vomiting, sedation and other side effects were assessed at 0, 1, 2, 6, 12, 24, 36 and 48 hr after awakening. Results: There was no difference in the incidence of nausea and vomiting between the three group [Group O: 4 (20%) and 3 (15%), respectively; Group OD: 1 (5%) and 1 (5%), respectively; Group D: 3 (15%) and 3 (15%), respectively]. Group O showed a lower sedation score than the other groups (P < 0.05). The pain score and other side effects did not show any difference between the groups. Conclusions: The combination of ondansetron and droperidol showed no clinical benefit compared with ondansetron or droperidol alone for prevention of postoperative nausea and vomiting during postoperative PCA using butorphanol and ketorolac.
