총의치교합면의 사용재료에 따른 마모도 측정에 관한 실험적 연구 (An Experimental Study on the Abrasion Tests as a Result of using Materials of Full Denture Occlusal Surface)

  • I got a conclusion as a result of that in case of constructing a full denture using acrylic resin teeth, porcelain teeth, and forming posterior occlusal surface of the artificial teeth by Type III Gold and Nickel crome alloy thus construdting a full denture and therefore comparing the above case with abrasion of Natural teeth. The result were as follows : 1. The abrasion of Natural teeth and the abrasion of full denture constructed by Type III gold has a close resemblance. 2. The one that has the hightest degree of abrasion is full denture that's using, with acrylic resin teeth, maxillary and mandibular And the lowest abrasion is natural teeth-natural teeth. 3. In case of single denture, that's opposing to natural teeth, the one that has the hightest degree of abrasion is a full denture that's using Acrylic resin teeth and the lowest abrasion is a full denture that's forming occlusal surface by Type III gold. 4. The single denture, which is opposing to type III gold teeth, was abraded above everything by full denture which is constructed by porcelain teeth. 5. In the same teeth, the abrasion of mandibular teeth was greater than that of mandibular teeth was greater than that of maxillary teeth. 6. The abrasion degree of Acrylic resin teeth has no direct mutual connection with material hardness which is opposing to.

방문구강보건사업 대상 노인의 현존 자연치아 수에 영향을 미치는 요인 (Factors influencing the number of remaining natural teeth in elderly people visiting dental care services)

  • Objectives : The purpose of this study was to investigate the number of remaining natural teeth in elderly people visiting dental care services and the factors affecting dental visit. This study will contribute the development of oral health promotion programs for the elderly people. Methods : Subjects were 172 elderly people recruited from 217 senior citizens visiting public oral health care services in 16 districts in Busan. They completed self-Corresponding Author reported questionnaires. Results : Elderly people had less remaining natural teeth (p<0.001). The remaining natural teeth ($15.07{\pm}8.75$) of the health insurance beneficiary were majority than those of the medicaid ($8.78{\pm}8.45$)(p<0.001). The respondents with better oral health condition had more remaining natural teeth (r=0.317, p<0.001), and those who were more worried about oral health had less remaining natural teeth (r=-0.599, p<0.001). Aging accelerates loss of natural teeth (p<0.001) of 3.203. Approximately 2.188 remaining teeth will be preserved by oral health care improvement (p=0.009). Conclusions : Frequent dental clinic visit will prevent natural teeth loss in the elderly people. Toothbrushing is the most efficient method of oral health care in the elderly people. Awareness towards oral health care is the motivation to preserve natural teeth in the elderly people.

  • Osseointegrated dental implants are now an accepted part of the prosthetic treatment of fully and partially edentulous patients. Clinical studies have shown that osseointegrated dental implants may successfully be used as abutments for prosthetic rehabilitations, and that the supporting tissue can be kept in a stable and healthy clinical state for prolonged periods of time. The purpose of this study was to look for differences in the distribution in the bacterial morphotypes around natural teeth and osseointegrated dental implants. The subgingival plaque around natural teeth and implants was analysed by means of differential phase-contrast microscopy. In addition, clinical indices used to analyse natural teeth condition were used to compare the condition of natural teeth and implants. The results were as follows : 1. In implants of partially edentulous patients, the plaque index, sulcus bleeding index(P<0.05), pocket depth(P<0.01) were greater than natural teeth. 2. Gingival crevicular fluid volume was not significantly different between natural teeth and implants. 3. In implants of fully edentulous patients, the plaque index was greater than natural teeth(P<0.01) and implants of partially edentulous patients(P<0.05), but sulcus bleeding index was not significantly different. 4. In partially edentulous patient, coccoid cell proportion was greater in implants than natural teeth (P<0.05). 5. In implants of fully edentulous patients, coccid cell proportion was even greater than implants of partially edentulous patients(P<0.05) and natural teeth(P<0.01). 6. Among three groups, the proportions of non-motile rods, motile rods, spirochetes and others were not significantly different. 7. The proportion of motile bacteria was not significantly different among three group.

  • Author have studied with electron microscope on the effect of biscuit solution dissolved in natural saliva on the enamel surface of the primary teeth. Four of the teeth were natural primary teeth and the other four teeth were treated in acidulated phosphate fluoride during four minute. The obtained results were as follows; 1. The pH value of the solution was decreased according to the time passed, and gradually became 3.75 at the end of 48 hours. 2. Natural teeth appeared normal enamel surface appearance during first 12 hours, and at 24 hours, it appeared some degree of decalcified appearance near the enamel rods and surfaces. 3. The decalcified appearances of the enamel surfaces of the fluoride treated teeth were not significant compared with the fluoride untreated teeth. 4. Streptococci of the enamel surface were increased according to the time passed.

중장년기 주요 구강건강 지표 (Major oral health indicators in mature and middle age)

  • Although the Korea National Health and Nutrition Examination Survey (KNHANES) calculates oral health functional restriction rate and chewing discomfort rate every year, these two indicators are not all indicators of oral health. Therefore, indicators are needed to subdivide by age group and cover dental caries, periodontal disease, remaining teeth, and oral care use. The purpose of this study is to identify the key indicators of oral health in mature and middle age. The average number of existing natural teeth, the rates of 20 or more natural teeth, complaints of chewing discomfort, oral examination, periodontal disease and dental caries, were analyzed using KNHANES VI (2013-2015) as a complex sample. In the age group between 40 and 64, there were 25.2 natural teeth remaining, 91.4% natural teeth retention rate, 23% chewing discomfort rate, 34.7% oral examination rate, 38.7% periodontal disease prevalence, 6.46 the number of caries experience teeth, 21.4% interdental brush usage rate.

자연치와 도재관에 대한 색조선택의 동일성 (Shade Matching Identification of in Vivo Natural Teeth and Porcelain-Fused-to-Metal Crowns)

  • The purpose of this study was to evaluate shade selection using conventional visual assessment in vivo natural teeth and porcelain-fused-to-metal (PFM) crown. Maxillary central incisors, lateral incisors and canines of one hundred twenty four college women were used as vivo natural teeth. Fifty one PFM crown for maxillary central incisor fabricated by dental laboratory were used as experimental materials. Using Vitapan Classical Shade Guides, shade selection of natural teeth was measured by each college woman and shade selection of PFM crown was measured by three ceramists with more than ten years career. Both natural teeth and PFM crown shade selection were measured through Shade Eye-Ex. From the shade selection comparing, following results were obtained. The results were as follows: 1. The shade matching identification of natural teeth between the shade selection using Vitapan Classical Shade Guides and the shade selection using Shade Eye-Ex was 27.4% in maxillary central incisor, 13.7% in lateral incisor and 18.5% in canine. 2. Among the shade selection of PFM crown by three ceramists, the shade evaluation of three ceramists were same only in ten cases. In twenty case, those of two ceramists were same. 3. The shade matching identification of PFM crown between the shade selection using Vitapan Classical Shade Guides and the shade selection using Shade Eye-Ex was 38.6% in average. These results suggest that the shade selection using conventional visual assessment should be dealt with care in clinic and need a credible method for shade matching color.

대합되는 재료에 따른 합성수지 인공치의 마모저항성에 관한 비교 연구 (Comparison Study of Wear Resistance Among Several Denture Teeth Opposing Various Restorative Materials)

  • Partial or complete prosthesis is needed when teeth are lost due to various kinds of reason. Artificial teeth recover occlusion instead of natural teeth. Artificial teeth are required of esthetics, fragile resistance and abrasive resistance. Artificial tooth is made of acrylic resin or porcelain. Nowadays, acrylic resin artificial teeth are mainly used. Acrylic resin teeth are occluded with natural teeth, gold alloy, Ni-Cr alloy or porcelain etc. Acrylic resin teeth have similar translucency, gloss of natural teeth. And it has good chemical bond with denture base material, but it has low wear resistance. The aim of this study is to compare wear resistance among several denture teeth(Endura, SR-orthosit-PE, Planustar) and between artificial resin denture teeth and opposing 3 restorative materials(gold, Ni-Cr alloy, porcelain). Wear tests were conducted with a rotating wear testing apparatus(pin-on-disk type wear tester) under conditions of rpm 180, 75 minutes and constant loading of 50N. The upper part was the cusp of maxillary first molar and the lower part was a disk type restorative materials. To make similar oral environment, water was supplied continually. The acrylic resin teeth wear was determined by weighing the cusp each 5 minutes during 75 minutes test. Vicker's hardness tester was used to evaluate the surface hardness of test specimens. The SEM was used to evaluate the wear surfaces. The results were as follows: 1. Wear rates of acrylic resin teeth opposing to the restorative materials were high in order of Porcelain, Gold, Ni-Cr alloy (p<.05). 2. Wear resistance rate opposing to the Porcelain disk, was shown in order of Endura, SR-orthosit-PE, Planustar. The wear rate of opposing to porcelain disk was above two times more than that of other groups (p<.05). 3. Wear resistance rates opposing to the Gold, Ni-Cr alloy disk, was shown in order of Endura, SR-orthosit-PE, Planustar (p<.05). 4. A degree of the surface hardness is directly proportional to the degree of wear resistance. There are statistically significant differences between each groups (p<.05).

A study of fracture loads and fracture characteristics of teeth

  • PURPOSE. The purpose of this in vitro study was to investigate the fracture loads and modes of failure for the full range of natural teeth under simulated occlusal loading. MATERIALS AND METHODS. One hundred and forty natural teeth were taken from mandibles and maxillas of patients. There were 14 groups of teeth with 10 teeth in each group (5 males and 5 females). Each specimen was embedded in resin and mounted on a positioning jig, with the long axis of the tooth at an inclined angle of 30 degrees. A universal testing machine was used to measure the compression load at which fracture of the tooth specimen occurred; loads were applied on the incisal edge and/or functional cusp. RESULTS. The mean fracture load for the mandibular first premolar was the highest (2002 N) of all the types of teeth, while the mean fracture load for the maxillary first premolar was the lowest (525 N). Mean fracture loads for the mandibular and maxillary incisors, and the first and second maxillary premolars, had significantly lower values compared to the other types of teeth. The mean fracture load for the teeth from males was significantly greater than that for the teeth from females. There was an inverse relationship between age and mean fracture load, in which older teeth had lower fracture loads compared to younger teeth. CONCLUSION. The mean fracture loads for natural teeth were significantly different, with dependence on tooth position and the sex and age of the individual.

  • The purpose of this study was to evaluate the mechanical effects when one implant fixture was connected to the natural teeth with reduced alveolar bone height. This study also examined the effects of increasing the number of abutment teeth and the effects of the intramobile connector and the titanium connector as they were inserted between the implant superstructure and the fixture. The distribution and concentration load was applied to the fixed partial denture(FPD) supported by implant and the natural teeth with reduced alveolar bone height. The stress and displacement of each element was observed and compared by the two-dimensional finite element method. The following results were obtained : 1. The greater the loss of alveolar bone in natural teeth area, the greater the displacement of FPD and the stress concentration in alveolar bone around implant, especially at the stress concentration in the mesial alveolar bone crest around implant fixture. 2. The displacement of FPD was increased more and that of implants fixture was decreased more when intramobile connector was used than titanium connector was used. Also the stress concentration in alveolar bone around implant fixture was greater when intramobile connector than titanium connector. One implication of this finding was that the difference in stiffness of implant and the natural teeth with reduced alveolar bone height could be partially compensated in case of the POM intramobile connector. 3. The amount and direction of displacement and the stress distribution of the 4-unit FPD was better than those of the 3-unit FPD. It implied that the difference of stiffness of implant and natural teeth with reduced alveolar bone height could be partially compensated in case of the 4 unit FPD.

한국인 무치악환자의 인공치아선택방법에 관한 연구 (A STUDY ON THE SELECTION OF ARTIFICIAL TEETH FOR THE EDENTULOUS PATIENTS IN KOREANS)

  • An esthetically pleasing result in denture service is dependent upon several consideration. Esthetic considerations for the edentulous patient include tooth size, shape, arrangement, positioning, gingival contour, age, sex, personality differences, and ethnic type. Especially, the form of anterior artificial teeth is an important factor on the esthetics. The selection of artificaial teeth requires understanding and knowledge on physical and biologic factors and has to meet the indivisul esthetics and functional needs of each patient. However, the selection of artificial teeth is based on the large degree of subjective judgement of the dentist, Therefore, this is one of the most unscientific processes. Many attempts have been made to find a guideline for the selection of artificial teeth. Temperamental theory by White and Hall utillized with the physical characteristics such as body size, body form, color of eyes and hair, and disposition. SPA theory by Frush and Fisher utilized with the basis of sex, personalities, and age of the indivisual. There has not been provided for a guideline and study on the selection of artificial teeth for Koreans yet. This study was aimed to evaluate the William's typal matching theory in Koreans. 1. The facial forms of korean adult were ovoid(46.3%), square(36.7%) and tapered(17.0%) form. 2. The anatomic forms of natural maxillary incisors were ovoid(45.3%), tapered(31.3%), square(23.3%) form. 3. The forms of face and natural maxillary incisors were ovoid(21.9%), squared(11%), tapered(6.3%). 4. The natural maxillary incisors were similar in form to the facial form only in 39%. There was no correlation between the form of the face and the form of natural maxillary central inisors in Koreans. 5. Artificaial teeth which was selected according to the typal matching theory did not represent the form of the natural teeth in 61 per cents of the fatal, but it felt that they harmonized with the form of the patient's face and produced good results.
