중환자 전문간호행위에 대한 건강보험 상대가치 및 환산지수 개발 (Development of a Resource-based Relative Value Scale and Its Conversion Factor for Advanced Nursing Practices in the National Health Insurance)

  • Purpose: The purpose of this study was to develop a resource-based relative value scale (RBRVS) and its conversion factor for advanced nursing practices carried out by critical care nurse practitioners (CCNP) in intensive care units. Methods: The methodology was developed by calculating CCNP's RBRVS for 32 advanced nursing services based on CCNP's workload and time spent in the context of national health insurance. A cost analysis was performed to estimate the conversion factor of CCNP's RBRVS. The share of CCNP's contribution to fee-for-service in intensive care units was also analyzed. Results: Calculation of the RBRVS of 32 advanced nursing practices showed a range of points from 100.0 to 1,181.4 and an average of 296.1 points. The relevant conversion factor for advanced nursing practices in CCNP were estimated at 37.3-48.4 won. The contribution rate of CCNP's advanced nursing practices in the relative value scale of the national health insurance was estimated at 0.1-31.3%. Conclusion: Measuring the economic value of advanced nursing services will be a basis for esta-blishing a reimbursement system for CCNP's practices and thus encourage a social demand for advanced nurse practitioners.

간호요구도에 따른 중환자간호 분류도구 개발 (Development of Classification System for Critical Care Nursing Based on Nursing Needs)

  • Purpose: This study was done to develop a valid and reliable Classification System for Critical Care Nursing (CSCCN) to be used in Intensive Care Units (ICUs). Methods: Head nurses and staff nurses of 17 ICUs in 6 hospitals classified 307 patients to verify interrater reliability. To verify construct validity, the staff nurses classified 404 patients according to CSCCN comparing difference in medical department and type of stay in ICU. For conversion index, 78 patients from 4 ICUs of 'S' hospital were classified and nursing time was measured by 107 nurses and 18 nurse aids using stopwatches. Results: The developed CSCCN has 11 categories, 76 nursing activities and 101 criteria. The reliability was verified as having high agreement (r=.946). The construct validity was verified comparing differences in medical department and type of stay in ICU. According to scores, four groups in the CSCCN classification were identified. According to the conversion index, one score on the CSCCN means 7.2 minutes of nursing time. Conclusion: CSCCN can be used to measure diverse and complex nursing demands including psycho-social aspects of ICU patients and convert nursing demands to numbers.

암 병원 입원 환자들의 '좋은 간호(Good Nursing)' 경험 (Good Nursing Experience of Patients with Cancer in a Korean Cancer Hospital)

  • Purpose : This study aimed to explore and describe good nursing experiences of patients with cancer. Methods : Data were collected using individual in-depth interviews with 15 males and 8 females who were hospitalized in a Korean cancer hospital. The transcribed script was analyzed using Colaizzi's phenomenological method. Results : As a result, three theme clusters and nine themes were identified. The three theme clusters were as follows: "being kind and expert in every step along the cancer care trajectory," "taking an omnipotent mediator role supporting patients' well-being," and "providing professional care touching patients' mind and body." Three themes were assigned to each cluster to illustrate the given phenomena. Conclusion : Cancer patients wanted considerate understanding and sincere care from nurses while they experienced physical and emotional suffering owing to the cancer, treatment trajectory, and hospital system. To improve the quality of nursing care for cancer patients, patient-centered care combined with nurses' expertise in oncology care must be provided based on the insights from our study's findings.

중환자 통증사정 도구 개발 (Development of a Pain Assessment Tool for Critically Ill Patients)

  • Purpose: The purpose of this study was to develop a reliable and valid pain assessment tool suitable for critically ill patients with communication problem in Korean intensive care unit. Methods: This pain assessment tool, Critical Care Non-verbal Pain Scale (CNPS), was developed based on review of national and international researches. Data were collected from ten intensive care units at five major general hospitals in Seoul. Reliability and validity were tested during performance of position change and endotracheal suctioning. Pain was measured before, during, and 20 minutes after the two procedures. Results: Interrater reliability of the CNPS was analyzed by ICC (Intraclass correlation coefficients). ICC values were significant from .833 to .883. Significant correlation between the FPS (Face Pain Scale) scores and the CNPS scores verified concurrent validity of the CNPS. For position change, CNPS scores increased significantly between before and during (t=-23.399, p<.001) and decreased significantly between during and 20 minutes after (t=22.760, p<.001). For endotracheal suctioning, CNPS scores increased significantly between before and during (t=-29.064, p<.001) and significantly decreased between during and 20 minutes after (t=28.194, p<.001), verifying construct validity of the CNPS. Conclusion: Results indicate that the CNPS can be used to assess pain of critically ill patients who have communication problem.

Experience of Treating Critically Ill COVID-19 Patients in Daegu, South Korea

  • At the beginning of February 2020, Daegu faced a serious situation due to the rapid outbreak of coronavirus disease 2019 (COVID-19), which is caused by severe acute respiratory syndrome coronavirus-2. The healthcare system in Daegu Metropolitan City was ill-prepared for this sudden disaster situation. The COVID-19 pandemic is still ongoing in South Korea. Daegu has limited medical resources compared to the Seoul Metropolitan Area, and it is hypothesized that a review of the outbreak therein could provide information that will be valuable for dealing with future outbreaks. The likelihood of death due to COVID-19 depends on the capacity of the area for the treatment of critically ill patients. This report reviews the overall treatment process followed for critically ill patients in Daegu.

  • 본 연구는 중증도가 높은 신경계중환자를 대상으로 중환자 중증도 분류도구와 Glasgow coma scale 적용의 유용성을 검정하고자 하는데 있다. 본 연구에서 대상자의 일반적 특성 및 임상 관련 특성에 따른 사망률 확인, 중환자 중증도 분류도구(CPSCS)의 일반적 특성, 임상관련 특성에 따른 중증도 차이, GCS의 일반적 특성과 임상관련 특성에 따른 중증도 차이를 파악하고, 임상적 유용성을 검정하고자 한다.

The Effects of Kangaroo Care on Maternal Self-esteem and Premature Infants' Physiological Stability

  • Purpose: This paper identified the effects of kangaroo care (KC) on maternal self-esteem and the growth and physiological stability of premature infants hospitalized in the neonatal intensive care unit. Methods: The experiment was conducted with an experimental group of 17 infants, and a control group of 17 infants selected by convenience sampling. KC was given to the infants one time a day for 30 minutes after feeding for two weeks. Their weight was measured every morning, and their physiological stability was measured in terms of their body temperature, respiratory rate, heart rate, oxygen saturation, and stability of the cardio-respiratory system in premature infants (SCRIP) score, as physiological responses at every 10 minutes during the intervention. Maternal self-esteem was assessed by a maternal self-report inventory. Results: The experimental group showed significant increase in the self-esteem of mothers. Also, KC was effective on physiological stabilization of preterm infants. Conclusion: This study suggests that KC can be provided in clinical settings more widely in Korea as one of the nursing interventions aimed at promoting maternal self-esteem as well as infants' physiological stabilization.

어떤 다발성 외상환자가 중환자실에 오래 있게 되는가?; Injury severity score와 손상부위 수의 비교 (Why do Multiple-trauma Patients Stay Longer in the Intensive Care Unit?; - A Comparison of Injury Severity Score and The Number of Injured Regions -)

  • Purpose: Injury severity score (ISS), a widely used scoring system, is used to define the severity of trauma in multiple-trauma patients. Nevertheless, ISS cut-off value for predicting the outcome of multiple-trauma patients has not been confirmed. Thus, this study was performed to determine the more useful method for predicting the outcome for multiple-trauma patients: the ISS or the number of anatomical Abbreviated injury scale (AIS) injury regions. Methods: for 195 consecutive patients who a regional emergency medical center, we analyzed the ISS and the number of anatomical AIS injury region. The patients were divided into four groups based on the ISS and the number of anatomical AIS regions. We compared intensive-care-unit (ICU) admission days and hospitalization days and ICU stay ratio (ICU admission days/hospitalization days) between the four groups. Results: In the groups with an ISS more than 17, the results were not significantly different statistically the group with 2 anatomical AIS injury regions and more than 3 anatomical AIS injury regions. Also, in the group with an ISS of 17 or less, the results were the same as those for patients with an ISS more than 17 (p>0.05). Among the patients with 2 anatomical AIS injury regions, patients with an ISS more than 17 patients had more ICU admission days and a higher ICU stay ratio than patients with an ISS 17 or less. Also, Among the patients with 3 anatomical AIS injury regions, the results were the same as those for patients with 2 anatomical AIS injury regions. Conclusion: Patients with high ISS, regardless of the number of anatomical AIS injury regions had significantly longer ICU stays and higher ICU admission ratio. Thus, the ISS may be a better method than the number of anatomical AIS injury regions for predicting the outcomes for multiple-trauma patients.

Hospital-Acquired Pressure Injury: Clinical Characteristics and Outcomes in Critical Care

  • Electronic health records (EHRs) enable us to use and re-use electronic data for various multiple purposes, such as public reporting, quality improvement, and patient outcomes research. Current hospital-acquired pressure injury (HAPI) risk assessment instruments have not been specifically developed for intensive care unit (ICU) patients and showed false positive rates in this specific populations. Previous research studies report a number of risk factors; however, it is still not clear what factors influence ICU HAPI in this population. As part of a larger research study, we performed an exploratory analysis by using a large electronic health record data. The aims of this study were to compare characteristics of patients who developed HAPIs during their ICU stay with those who did not, and to determine whether the two groups were different in the aspects of length of ICU stay, discharge disposition, and discharge destinations. We conducted chi-square test and t-test for group comparison. Association was examined by using bivariate analyses. Pearson correlation coefficients were used to examine correlation between LOS and number of medications. Our findings suggest a number of consistent and potentially modifiable risk factors, such as sedation, feeding tubes, and the number of medications administered. The mortality of the HAPI group was significantly higher than the non-HAPI group in our data. Discharge disposition was significantly different between the groups. 67% of the HAPI group transferred to intermediate or long-term care hospitals whereas 57.7% of the non-HAPI group went home after discharge. Awareness of these risk factors can lead to clinical interventions that can be preventative in the ICU setting.

Suggestions for the Work of Dental Hygienists and a Reflection of Health Insurance Charges according to Demographic Changes

  • Given that the demand for dental care for elderly individuals is expected to increase according to demographic changes, long-term roadmaps for the current health insurance system should be actively planned. The study aimed to investigate the health insurance system in Korea, compared to that of Japan, and suggest directions for the work of dental hygienists to provide basic data on efficient improvements in the health insurance system. Based on the collected data, the website was used to collect additional related data from Korea and Japan. The most common cause of death in Korea was malignant neoplasms, and most patients were hospitalized for diseases and disorders of the musculoskeletal system and connective tissue. Dental care covered by Korean medical insurance for individuals aged 65 years or older was treatment oriented, such as dentures and implants, while in Japan, treatment-intensive items, such as visiting a dental hygienist for guidance and provision of home-care professional oral hygiene treatment, were covered. The percentages of remaining teeth in Korea and the percentage of remaining 20 teeth were 68.9% in those aged 65 to 74 years and 51.2% in those aged 75 to 84 years in Japan. A strategy for promoting oral health among elderly individuals should be established, and a multilateral intervention approach is required to prevent oral problems from leading to deterioration of whole-body health. The role of dental hygienists in providing comprehensive assessment to elderly individuals is important. In order to provide systemic oral care, it is necessary to introduce oral care systems according to the national policy.
