Analysis of Carbon Emissions According to Combustion of Tree Branch, Bark and Living Leaf in Pinus Densiflora (소나무 부위별 연소에 따른 탄소배출량 분석)

  • 본 연구에서는 산불발생 시 온실가스의 배출량을 추정하기 위한 기초 연구로서 산림연료의 연소에 따른 탄소배출량을 분석하였다. 산림연료는 소나무를 대상으로 생엽, 가지, 수피 등 부위별로 연소실험을 수행하였으며, 콘칼로리미터를 이용하여 일산화탄소와 이산화탄소의 배출량을 분석하였다. 중량 50g의 연료 기준, 일산화탄소의 배출량은 1.8~4.0g 정도였으며, 이산화탄소의 배출량은 49.3~84.7g 정도를 나타냈다. 부위별로 큰 차이를 나타냈는데 수피는 생엽과 가지보다 상대적으로 많은 일산화탄소와 이산화탄소를 배출하는 것으로 나타났다.

An Emergency Management Architecture Using Personalized Emergency Policy for Smart Healthcare (스마트 헬스케어를 위한 사용자 맞춤형 응급 정책을 활용한 응급 관리 구조)

  • In smart healthcare service, the accurate and prompt emergency detection and notification are very critical to patients' lives. Since these detection and notification of emergency situation are usually performed by the medical staffs, it is difficult to simultaneously support many patients in real-time. This article presents a methodology for emergency bio-data transmission for smart healthcare using personalized emergency policy. It consists of three steps: In step 1, the bio-data is collected by wireless body area network. In step 2, the decision on emergency is made using personalized emergency policy. In step 3, the emergency message including the health condition information is converted between IEEE 11073 PHD message and HL7 CDA. By doing this, the emergency status of the individual bio-data collected from wireless body area network is detected automatically using personalized emergency policy. When the emergency is detected, the quick emergency rescue service can be provided to the patient by delivering to the emergency notification and the emergency bio-data. We have verified the service and functions of the proposed system architecture by realizing it.

  • Thailand had set the National Water Management Strategy which covered main six areas in the next 12 years, i.e., by priority: (1) water for household, (2) water for agricultural and industrial production, (3) water for flood and drought management, (4) water for quality issue, (5) water from forest conservation and soil erosion protection, (6) water resources management. However due to the climate change impact, there is a question for all strategies is whether to complete this mission under future climate change. If the impact affects our target, we have to clarify how to mitigate or to adapt with it. Vulnerability assessment was conducted under the framework of ADB's (with the parameters of exposure, sensitivity and adaptive capacity) and the assessments were classified into groups due to their different characteristic and the framework of the National Water Management Strategy, i.e., water supply (rural and urban), water for development (agriculture and others), water disasters (floods (flash, overflow), drought, water quality). The assessments identified the parameters concerned and weight factors used for each groups via expert group discussions and by using GIS mapping technology, the vulnerability maps were produced. The maps were verified with present water situation data (floods, drought, water quality). From the analysis result of this water resources management strategy, we found that 30% of all projects face the big impacts, 40% with low impact, and 30% for no impact. It is clear that water-related agencies have to carefully take care approximately 70% of future projects to meet water resources management strategy. It is recommended that additional issues should be addressed to mitigate the impact from climate risk on water resource management of the country, i.e., water resources management under new risk based on development scenarios, relationship with area-based problems, priority definition by viewpoints of risk, vulnerability (impact and occurrence probability in past and future), water management system in emergency case and water reserve system, use of information, knowledge and technology in management, network cooperation and exchange of experiences, knowledge, technique for sustainable development with mitigation and adaptation, education and communication systems in risk, new impact, and emergency-reserve system. These issues will be described and discussed.

Healthcare and Emergency Response Service Platform Based on Android Smartphone

  • As the elderly population is becoming an aging society, the elderly are experiencing many problems. Social security costs for the elderly are increasing and the un-linked social phenomenon is emerging. Thus, the social infrastructure and welfare system established in the past economic growth period are in danger of not functioning properly. People socially isolated or with chronic diseases among the elderly are exposed to various accidents. Thus, an active healthcare management service is imperative. Additionally, in the event of a dangerous situation, the system must have ways to notify guardians (family or medical personnel) regarding appropriate action. Thus, in this paper, we propose the smartphone-based healthcare and emergency response service platform. The proposed service platform aggregates movement of relevant data in real-time using a smartphone. Based on aggregated data, it will always recognize the user's movements and current state using the human motion recognition mechanism. Thus, the proposed service platform provides real-time status monitoring, activity reports, a health calendar, location-based hospital information, emergency situation detection, and cloud messaging server-based efficient notification to several subscribers such as family, guardians, and medical personnel. Through this service, users or guardians can augment the level of care for the elderly through the reports. Also, if an emergency situation is detected, the system immediately informs guardians so as to minimize the risk through immediate response.

Validation study of integrated intubation tube with stylet(IITS) in tracheal intubation (기관내삽관에서 속심일체형 삽관튜브 용이성에 관한 연구)

  • Purpose: Difficult organs or locations or inadequate tube intubations can cause complications. There are some cases in which the tube location changes or the tube is removed due to processing inside the organ while installing the stylet or rapid stylet removal. Thus, this study aimed to evaluate and develop an integrated intubation tube with stylet (IITS) for easier intubation of organs in emergency cases and reduce complications caused by the stylet. Methods: This study used a "Laerdal Airway Management Trainer". For stylet intubation, procedure No. 14 of the national practical test protocol was followed, but the removal step was omitted. In this study, each emergency case was intubated with an IITS, in which the stylet was not inserted or removed separately even though it has the function of an organ intubation stylet. Results: The existing classic ET intubation method had a success rate of 100% and had an average intubation time of 21.75 seconds, The developed IITS method was also successful in all cases and had an average intubation time of 15.78 seconds. Conclusion: Application of an IITS is expected to reduce intubation time and decrease inappropriate depth and intubation failure due to stylet removal, therefore improving the efficiency of airway maintenance.

MAINTENANCE COST ANALYSIS FOR LARGE HOSPITAL BUILDINGS

  • Large hospitals such as medical centers provide not only medical services, but also carry the responsibilities for emergency refuges, medical researches and education. The function of large hospitals is as important as other infrastructure systems such as highways, bridges, and utilities. When disasters occur, the hospitals have to provide emergency medical services for victims and support the patient's needs of health. In order to keep a smooth operation of hospitals, the maintenance management of hospital buildings should be carefully investigated. However, there are few researches focused on maintenance management issues of hospital buildings. This paper investigated the National Taiwan University Hospital (NTUH) and established a maintenance cost database. The NTUH is the best-known and most high-renowned medical center in Taiwan in which more than 4,000 employees serving approximately 2,000 in-patients and 7,000 out-patients daily. The data were collected from the NTUH which consisted of 16,228 maintenance records in the past ten years. This paper analyzed these data to obtain various characteristics of maintenance records, and revealed the key items of maintenance cost for large hospital buildings, which can provide the facility manager of hospital buildings to execute a proper maintenance policy for hospital buildings.

Study of water drowning death patients on clinical care and prevention (수중익사 사망 환자의 임상 치료 및 예방에 대한 연구)

  • The purpose of this study was to investigate drowning death patients on clinical care and prevention. Drowning accidents that occurred in the Republic of Korea were identified by using the Emergency Activity Daily Report Data from the National Emergency Management Agency (NEMA). The age of drowning accidents was 20-29 years (25.4%). Drowning cases for the months of August was 17.6%. The main circumstance involved in the drowning cases was swimming (45.0%). Half of swimming incidents related the drowning accidents of people attempting to save a life of a family member or friend from the water. The most common location was at a lake or reservoir (40.2%). Drowning is a main cause of accidents in the Republic of Korea. Preventive strategies that may be beneficial for persons contain life jackets or life rings supply, upgrading swimming ability, placing lifeguards based on public education activities.

  • The Ministry of Public Safety and Security was established in order to handle all sorts of disasters because of Sewol ferry tragedy and the fundamental reform on the emergency management system will be performed. The establishment of the Ministry of Public Safety and Security is considered as the landmark for the emergency management when it is compared to those in the United States and Japan in terms of the size and the authority. However, there are many tasks to be done for the settlement of the Ministry of Public Safety and Security. More specifically, a real and substantial plan should be prepared because the organization was established without blueprints or detailed implementation plan. This study suggests that all-hazard approach which is the one of the disaster management principles should be applied when the substantial plan is prepared. All possible hazard should be analyzed, assessed, and prioritized. In addition, Based on the results, the effective policies should be established with the consideration of Korean context.

Successful minimally invasive management using transcatheter arterial embolization in a hemodynamically stable elderly patient with mesenteric vascular injury in a hybrid emergency room system in Korea: a case report

  • Mesenteric injury occurs rarely in cases associated with blunt abdominal trauma. Despite its low incidence, mesenteric injury can lead to fatal outcomes such as hypovolemic shock due to hemoperitoneum or sepsis due to intestinal ischemia, or perforation-related peritonitis. For mesenteric injuries, especially those involving massive bleeding, intestinal ischemia, and perforation, the standard treatment is surgery. However, in the case of operative management, it should be borne in mind that there is a possibility of complications and mortality during and after surgery. The usefulness of transcatheter arterial embolization (TAE) is well known in solid organs but is controversial for mesenteric injury. We present a 75-year-old man with mesenteric injury due to blunt abdominal trauma. Initial abdominal computed tomography showed no hemoperitoneum, but a mesenteric contusion and pseudoaneurysm with a diameter of 17 mm were observed near the origin of the superior mesenteric artery. Since there were no findings requiring emergency surgery such as free air or intestinal ischemia, it was decided to perform nonoperative management with TAE using microcoils in hybrid emergency room system. TAE was performed successfully, and there were no complications such as bleeding, bowel ischemia, or delayed bowel perforation. He was discharged on the 23rd day after admission with percutaneous catheter drainage for drainage of mesenteric hematoma. The authors believe that treatment with TAE for highly selected elderly patients with mesenteric injuries has the positive aspect of minimally invasive management, considering the burden of general anesthesia and the various avoidable intraoperative and postoperative complications.

Comparison of tracheal intubation using the Macintosh laryngoscope versus the intubating laryngeal mask airway in novice users - A manikin study - (초보자에서 마네킨에게 직접 후두경과 삽관용 후두마스크기도기를 이용한 기관내삽관의 비교)

  • Purpose : This research was designed to provide basic data for advanced pre-hospital airway management by comparing the time to ventilation and success rate for tracheal intubation performed with Macintosh laryngoscope and intubating Laryngeal Mask Airway(ILMA) in a manikin. Methods : All participants were novice users among EMT-Paramedic students and were divided into two groups: (1) the group for Macintosh laryngoscope guided tracheal intubation(MLG-TI) (2) the group for ILMA guided tracheal intubation(ILMA-TI). After an introductory lecture and demonstration, each group made an attempt ten tracheal intubation to compare the ventilation time and success rate for tracheal intubation. Results : 1) There was significant difference in the time to ventilation through MLG-TI, the time to first and second ventilation through ILMA-TI of the 10 attempts. 2) The time to first ventilation through ILMA-TI was significantly shorter than that of ventilation through MLG-TI. 3) There was no significant difference between the time to ventilation through MLG-TI and the time to second ventilation through ILMA-TI. 4) The success rates of ILMA-TI were significantly higher than those of MLG-TI. Conclusion : ILMA-TI can be an alternative method for MLG-TI in advanced pre-hospital airway management.
