취약계층 당뇨병 여성노인의 대사증후군지표와 심혈관 위험정도가 우울증상 여부에 미치는 요인 (Influencing Factors of the Metabolic Index and Cardiovascular Risk Factors on Depressive and Non-depressive Groups in the Vulnerable Diabetic Elderly Women)

  • Purpose: The purpose of this study was to identify influencing factors of metabolic index and cardiovascular risk factors, on depressive and non-depressive groups, in vulnerable diabetic elderly women. Methods: Participants were 137 vulnerable diabetic elderly women, using health centers in D city. Data were collected through interviews September though December 2017. The metabolic index was measured using National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III), and cardiovascular risk factors were measured using Framingham Risk Score (FRS). Depressive and non-depressive groups were divided by the score of Geriartric Depress Scale Short Korea Version (GDSSF). Collected data were analyzed using a x2 test, independent t-test, and binary logistic regression, with the SPSS/WIN 25.0 program. Results: Vulnerable diabetic elderly women, did not exercise in the depressive groups, and had higher triglyceride (TG), total cholesterol (TC) and larger waists, than in the non-depressive group. Results show that lack of exercise (OR= 6.30), is the highest risk factor, influencing the depressive symptom in vulnerable diabetic elderly women. Conclusion: These results suggest that to reduce depressive symptom levels among vulnerable diabetic elderly women, nursing interventions are needed to increase exercise and decrease TG, TC, and waist size, particularly in improving exercise of vulnerable diabetic elderly women.

  • 목 적: 본 연구는 국내 소아청소년의 대사증후군의 기준치 설정의 참고자료가 되고자 BMI, 혈압, 허리둘레, 중성지방, 중성지방, HDL-콜레스테롤, 공복혈당의 연령별, 성별 백분위수 분포를 분석하였다. 방 법: 1998년, 2001년 및 2005년 국민건강영양조사에서 조사된 10-19세 연령 총 4,164명(남아 2,139명, 여아 2,025명)을 대상으로 하였다. 신장, 체중, 허리둘레를 측정하였고 BMI, 혈압, 지질 농도와 혈당의 분포를 분석하였다. 결 과: BMI 는 여아 17세 90 백분위수에서, 남아 17세의 85 백분위수에서 성인 비만 위험군으로 정의하는 25 이상으로 측정되고 있었다. 허리둘레는 17세 이상 90 백분위수 남아 여아 모두에서 아시아 성인 복부비만의 기준치 이상이 측정되었다. 수축기 혈압은 남아 15세 이상 90 백분위수 이상, 13세 이상 95 백분위수 이상에서 성인 대사증후군의 진단 기준인 수축기 혈압 130 mmHg 이상이 측정되었고 이완기 혈압은 남아 17세 이상의 90 백분위수 이상, 15세 이상 95 백분위수 이상에서 80 mmHg 이상이었다. 혈장 중성지방은 NCEP-ATP III의 기준인 110 mg/dL 이상은 남아 여아 모두 75 백분위수에 해당하였고 IDF의 기준인 150 mg/dL 이상은 남아 여아 모두 90 백분위수에 해당하였다. HDL-콜레스테롤의 대사증후군 진단 기준인 40 mg/dL 이하는 남아에서 25 백분위수 이하가 여아에서 10 백분위수 이하가 해당되었다. 공복 혈당은 NCEP-ATP III 기준인 110 mg/dL 이상은 남아 여아에서 95 백분위수에 해당하였고 IDF 기준인 100 mg/dL 는 남아 여아 모두 90 백분위수에 해당하였다. 결 론: 한국 청소년 후기 남아에서 혈압, 허리둘레, 중성지방의 90 백분위수와 여아에서 허리둘레, 중성지방의 90 백분위 수치가 한국 성인 대사증후군의 기준치에 근접하여 매우 높았다.

한국 성인의 대사증후군 및 그 구성요소가 성별에 따른 건강관련 삶의 질 손상에 미치는 영향: 2016-2018 국민건강영양조사 (The Influence of Metabolic Syndrome and its Components on the Impaired Health-related Quality of Life by Gender in Korean Adults: The 2016-2018 Korea National Health and Nutrition Examination Survey)

  • 본 연구의 목적은 국민건강영양조사 자료를 활용하여 한국 성인의 대사증후군 및 그 구성요소가 성별에 따른 건강관련 삶의 질 손상에 미치는 영향을 알아보기 위한 것이다. 참가자는 19세 이상 성인 16,657명이었으며, 대사증후군은 NCEP-ATP III에 따라 정의되었고, HRQOL는 EQ-5D를 사용하여 평가되었다. 연구 대상자의 대사증후군 유병율은 34.3%였고 남성이 여성보다 높았다. 건강관련 삶의 질 평균 점수는 남성이 96.7점, 여성이 94.5점으로 남성이 높았다. 인구학적, 건강행위, 건강관련특성을 보정변수로 통제 한 후 다중 로지스틱 회귀분석을 실시한 결과, 대사 증후군이 있는 여성은 대사증후군이 없는 여성에 비해 건강관련 삶의 질 손상의 교차비가 1.27(95% CI: 1.09-1.46)이었다. 대사증후군의 구성요소 중 복부비만(OR=1.72, 95% CI: 1.50-1.97)과 고혈압(OR=1.26, 95% CI: 1.06-1.50)은 건강관련 삶의 질 손상과 연관이 있었다. 반면, 남성에서는 유의하지 않았다. 결론적으로 여성의 건강관련 삶의 질 향상을 위해서는 여성의 특성을 고려한 대사증후군 위험요인을 감소시키는 접근전략이 필요함을 시사한다.

신체활동과 좌식생활에 따른 비만남성과 여성의 건강상태의 차이 비교 (Comparison of Physical Activity and Sedentary Life on Health in Obesity Men and Women)

  • 본 연구의 목적은 대사적으로 비만한 건강인과 비건강강인의 신체활동과 좌식생활의 행태를 비교분석하는 것이었다. 본 연구의 대상자는 2014년 국민건강영양조사 중 1585명을 대상으로 하였으며, 대사적으로 건강한 비만인(metabolically healthy obesity; MHO)와 대사적으로 비건강한 비만인(metabolically unhealthy obesity; MUO)의 분류는 NCEP-ATP III(National Cholesterol Education Program-Adult Treatment Panel III)의 기준을 근거로 분류하였다. 연구결과, 남자대상자의 경우 MHO와 MUO 간의 신체활동 수준 및 규칙적인 운동 참여 수준은 차이가 없었다. 여자대상자의 경우 MHO와 MUO 간에 총 신체활동량과 근력운동 참여에서 MHO 집단이 더 많은 신체활동량과 근력운동 참여를 하는 것으로 나타났다. 결론적으로 여성의 경우, 신체활동과 운동참여 수준은 동일한 수준의 비만인에서 건강상태에 영향을 주는 하나의 중요한 요인이라 사료된다.

경찰공무원의 대사증후군 유병률과 영향 요인 (A Study on the Prevalence and Influencing Factors of Metabolic Syndrome among Police Officers)

  • Objectives: The purpose of this study was to evaluate the predictors of metabolic syndrome among police officers and determine its prevalence. Methods: Data were collected through a questionnaire survey and medical examination among 1,089 male police officers from 2010 to 2013. Information about age, marital status, smoking, drinking and exercise were based on data from the self-administered questionnaire. We collected job type, working position, and work duration from company personnel data and the indices of metabolic syndrome through the medical examination. Metabolic syndrome was assessed according to the NCEP-ATP III definition and the Asia-Pacific obesity guidelines. Results: The prevalence of metabolic syndrome among the 1,089 police officers was 33.4%. The prevalence of metabolic influencing factors were 47.5% for abdominal obesity, 30.1% for hypertension, 37.1% for hypertriglyceridemia, 22.0% for low HDL-cholesteralemia and 13.2% for hyperglycemia. Factors such as age, drinking, and family history were significantly associated with metabolic syndrome. Conclusions: Weight control and a moderate drinking program may decrease the prevalence of metabolic syndrome among male police officers.

대사증후군 대상자들의 건강코칭프로그램 평가 (The Evaluation of a Health Coaching Program on Metabolic Syndrome Patients)

  • Objectives: We assessed the feasibility of health coaching for health coaching program on metabolic syndrome. Methods: We developed a 6 month health coaching program on metabolic Syndrome. We recruited people with metabolic syndrome according to modified NCEP-ATP III. The participants were 9 men over 30 years of age who had taken a health screening at general hospital. We collected data such as demographics, BMI, body fat, blood pressure, HDL-cholesterol blood sugar and triglyceride. The program was analyzed by using Wilcoxon signed rank test. Results: Participants showed significantly decreased BMI, weight, waist circumference, body fat after 6 month program. They talked the awareness about their own behavior. They changed into better for eating habits, physical activities, and self management. Their discipline increased and eating habits became regular. They were satisfied to this program and showed strong confidence about their own change. Conclusions: Coaching did not direct certain behavioral change but guided self awareness and practice. Health coaching program showed long maintained effect to participants. We suggested health coaching as a helpful individual program to intervene risky health behavior especially for metabolic Syndrome.

Relationship among Bone Mineral Density, Body Composition, and Metabolic Syndrome Risk Factors in Females

  • Osteoporosis is a disease that increases the fracture rates and a major cause of increased mortality and morbidity in the elderly people. This study is to determine which components of body composition and metabolic syndrome risk factors are important to bone health, we analysed the relationship among bone mineral density (BMD), body composition and metabolic syndrome risk factors in females. Totally 630 females participated in a medical check-up program (mean age 47 years) were selected for this study. Body composition analysis was performed by segmental bioelectrical impedance method, muscle mass, and percent body fat were measured. We also measured metabolic syndrome risk factors including abdominal obesity, HDL-cholesterol, triglyceride, blood pressure and fasting glucose level. Metabolic syndrome was defined by NCEP-ATP III criteria. The lumbar spine and femoral neck BMD were measured using the dual energy X-ray absorptiometry. Osteopenia and osteoporosis were observed in 180 and 51 persons, respectively. Muscle mass and HDL-cholesterol decreased in osteopenia and osteoporosis groups compared to the control group, and the grade was shown progressively by the symptoms. Significant positive correlation between BMD and muscle mass was observed. Multi variable regression analyses showed that % body fat and muscle mass were independent predictors of BMD after adjustment of age, height and weight. In conclusion, the BMD showed negative correlation with the metabolic and body composition was associated with BMD.

Association between Obesity factors and Homocysteine Levels with the Metabolic Syndrome

  • Metabolic syndrome and homocysteine are associated with increased independent risk factors of cardiovascular disease. We investigated the difference between the degree of obesity, metabolic syndrome risk factors, plasma homocysteine and anthropometric obesity factors. Totally 398 participated in a medical check-up program were selected for this study. Subjects were categorized into three groups according to the number of metabolic syndrome components present as defined by the NCEP-ATP III criteria; Absent (0 criteria, n=124), Pre-MetS (1-2 criteria, n=220) and MetS (${\geq}3$ criteria, n=54). Body mass index (BMI) is a measure used to distinguish between normal weight, overweight and obesity. MetS presented higher homocysteine than Absent (p<.05) and obesity higher than normal weight (p<.01). When Absent+Pre-MetS was used to classify obese or not, obesty presented higher homocysteine than non-obese (p<.05). Further homocysteine levels positively correlated with weight, BMI, waist circumference, hip circumference and waist-hip ratio (WHR). Especially WHR is not only MetS (r=0.378, p<.001) but also Absent+Pre-MetS (r=0.305, p=0.029) significantly positively correlated with homocysteine. The results of our study indicate that homocysteine is related closely to obesity. Although obesity has not been diagnosed with metabolic syndrome, obesity related with increased homocysteine.

노인의 대사증후군 유병에 따른 영양소 섭취 특성 - 국민건강영양조사 제5기 1차년도(2010) 자료를 이용하여 - (Characteristics of Nutrient Intake According to Metabolic Syndrome in Korean Elderly - Using Data from the Korea National Health and Nutrition Examination Survey 2010 -)

  • This study was performed to investigate the characteristics of nutrient intake according to metabolic syndrome in Korean elderly. Data on 1,200 elderly (men 545, women 655) over 65 years was obtained from the Korea National Health and Nutrition Examination Survey 2010. The subjects were divided into metabolic syndrome (MetS) and normal groups based on NCEP-ATP III guidelines and by applying the Asia-Pacific definition with waist circumference. The prevalence of MetS was 48.9% in all subjects, 35.6% in men and 58.9% in women, respectively. All nutrient intake was significantly lower in MetS than normal subjects, except iron, vitamin A, carotene, and retinol. In the carbohydrate: protein: fat (C:P:F) ratio, MetS showed a higher rate for carbohydrate, but a lower rate for protein and fat than normal. MetS was lower in calcium, vitamin B1, and vitamin C by the nutrient adequacy ratio than normal. Mean adequacy ratio of MetS (0.69) was lower than that of the normal group (0.72). These results demonstrated that undernutrition, not excess intake of specific nutrients - energy, fat, or cholesterol was associated with the risk for MetS in Korean elderly with MetS. Therefore, it is recommended to have a healthy dietary pattern with diverse nutrients for the prevention of MetS.

Association of Metabolic Syndrome with Exercise Capacity and Heart Rate Recovery after Treadmill Exercise Test

  • Heart rate recovery (HRR) immediately after a treadmill exercise test is a function of vagal reactivation. A delayed heart rate recovery is associated with an increased risk for overall cardiovascular mortality. The purpose of this study is to find out if metabolic syndrome is associated with autonomic nerve function and exercise capacity in healthy adults. We measured the treadmill exercise capacity (METs) and heart rate recovery in 119 subjects through a medical checkup at $J$ General Hospital. The metabolic syndrome was defined according to the National Cholesterol Education Program Adult Treatment Panel III (NCEP-ATP III) criteria. The value for the HRR was defined as the difference between the heart rate obtained during the peak exercise and the heart rate obtained at first minute during the recovery period. The subjects with the metabolic syndrome had significantly lower exercise duration ($9.6{\pm}1.5$ vs $8.7{\pm}1.4$), METs ($11.6{\pm}1.7$ vs $10.4{\pm}2.5$), and HRR ($37.5{\pm}14.3$ vs $27.1{\pm}8.9$). The waist circumference in subjects with the metabolic syndrome was more strongly correlated with HRR ($r$=-.517, $P$ <.001) than in normal subjects. Furthermore, delayed HRR was associated with high resting heart rate and increased waist circumference ($P$=.032, $P$ <.001, respectively). In conclusion, delayed HRR during the first minute after a treadmill exercise test was associated with the metabolic syndrome risk factors. Delayed HRR was also associated with high resting heart rate and increased waist circumference.
