One Case Report of Surgical Treatment of Left Atrial Myxoma (좌심방내 점액종 치험예)

  • It is well known that the atrial myxoma is the most common intracardiac tumor which is hardly able to be diagnosed preoperatively and is completely curable if surgical treatment is appropriate. A case of the atrial myxoma, which was misdiagnosed as mitral valvular disease preoperatively and was successfully removed with the aid of the cardiopulmonary bypass, was reported. The common symptoms,signs,diagnostic measures and treatments of the intracardiac tumor were discussed with the review of literatures.

  • We report a rare case of an extremely large right ventricular myxoma involving the ventricular side of the tricuspid valve. The tumor was excised along with the entire posterior leaflet and part of the anterior leaflet. The tricuspid valve was repaired by sliding valvuloplasty combined with ring annuloplasty.

  • Primary cardiac tumors usually are benign and are left atrial myxoma. We experienced with two cases of left atrial myxoma recently, and underwent surgical resection successfully. Diagnoses were obtained by echocardiogram and the angiocardiography was not performed. The operation consisted of excision of the tumor with a segment of atrial septum or wall through a right atriotomy, transseptal approach. Postoperative functional results were good and the literature is reviewed.

  • Myxomas in the orofacial regions are relatively uncommon tumors that occur in two different types: odontogenic myxomas and soft tissue myxomas. Odontogenic myxomas, derived from the mesenchymal cells such as papillae, dental follicles, or periodontal ligament, occur commonly in the maxilla and mandible. However, soft tissue myxomas in the oral cavity are rare and have been previously reported in only about 30 cases of unknown pathogenesis. The aim of this case report is to present a new case of oral soft tissue myxoma in a 37-year-old male patient who presented with a soft, exophytic round mass on the palate.

  • Nerve sheath myxoma is a benign tumor of the peripheral nerves that rarely occurs in the lip area. Among the few reported cases, no lesion has previously been reported on the lip in Korea. We report a case of nerve sheath myxoma occurring on the lip of a 34 year-old woman with a brief review of the literature.

  • Hyuk An, M.D.Atrial myxoma constitutes the most significant of all cardiac neoplasm. About 75% occur in the left atrium and 25% in the right. They may cause severe and progressive disease mimicking valvular heart disease. But recently attention to differential point in the clinical features along with various diagnostic techniques have allowed pre-operative diagnosis in the most cases. From April, `77 to Aug., `78, three cases of left atrial myxoma and one case of right atrial myxoma were operated in Seoul National University Hospital. Pre-operative diagnosis was established with echocardiography and cardiac angiography. In all cases, tumors were resected with cardiopulmonary bypass successfully, and discharged with good results.

Surgical Treatment of Intracardiac Myxoma (심장 점액종의 외과적 치료)

  • Since November 1979, 20 patients have undergone excision of an intracardiac myxoma, which was located in the left atrium in 18[90%], in the right atrium in 1[5%], and in the right ventricle in 1[5%], There were 17 female and 3 male patients with a mean age of 43.5 years [range 23 to 68 years]. Only one patient was asymptomatic, the others were seen mostly exertional dyspnea, palpitation, sings of systemic illness. Diagnosis was confirmed by echocardiography in all cases and angiography in two cases, preoperatively, The myxomas were successfully removed in all patients, either by shaving them from the atrial septum or by excising a portion of normal atrial septum with tumor. All heart chambers were carefully explored for presence of multi-centric myxoma or tumor debris. We conclude that excision of intracardiac myxoma is curative and long term survival is excellent and long term clinical and echocardiographic follow-up are recommended since late recurrence, although rare, has been reported.

  • Although odontogenic myxoma (OM) has various radiographic appearances, the characteristic features of OM are the multilocular radiolucent lesion, straight bony septa along the margin forming either square or triangular spaces. We present a case of OM in a 25-year old-male patient. Multilocular radiolucent lesion on the left mandible body showed tennis racket appearance. Cone beam computed tomography (CBCT) showed straight bony septa along the margin and cortical perforation. This CBCT features would have significantly contributed to allowing a diagnosis of OM. We think that this case shows characteristic radiographic features of odontogenic myxoma.

  • Myxoma of the heart can now be successfully treated but the success is obviously dependent upon the correct diagnosis and treatment prior to development of catastrophic complications such as sudden death, embolism or cardiac failure. The right ventricular myxoma is very rare, and we treated successfully a case of giant right ventricular myxoma, sessile tumor originated from ventricular apex, weighed 175 gm. The tumor base was broad, about 3 cm x 3cm, and it was hardly adhesed to the tricuspid valvular structures, but could remove with the preservation of adequate residual ventricular chamber size and the maintenance of functional tricuspid valve anatomy. The postoperative course was excellent and uneventful.

  • From August 1990 to October 1991, we performed operation on 5 patients with cardiac myxoma, which was located in the left atrium, There was 1 male and 4 female patients ranging in age 45 years to 61 years. Chief complaints included exertional dyspnea, palpitation, cough, orthopnea, general weakness. M-mode and two dimensional echocardiography provided an accurate diagnosis in all cases which was utilized as safe, reliable and noninvasive imaging modalities. In all cases, myxoma was excised successfully. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.
