The Effect of Gaze Angle on Muscle Activity and Kinematic Variables during Treadmill Walking

  • Objective: The purpose of this study was to determine how gaze angle affects muscle activity and kinematic variables during treadmill walking and to offer scientific information for effective and safe treadmill training environment. Method: Ten male subjects who have no musculoskeletal disorder were recruited. Eight pairs of surface electrodes were attached to the right side of the body to monitor the upper trapezius (UT), rectus abdominis (RA), erector spinae (ES), rectus femoris (RF), bicep femoris (BF), tibialis anterior (TA), medialis gastrocnemius (MG), and lateral gastrocnemius (LG). Two digital camcorders were used to obtain 3-D kinematics of the lower extremity. Each subject walked on a treadmill with a TV monitor at three different heights (eye level; EL, 20% above eye level; AE, 20% below eye level; BE) at speed of 5.0 km/h. For each trial being analyzed, five critical instants and four phases were identified from the video recording. For each dependent variable, one-way ANOVA with repeated measures was used to determine whether there were significant differences among three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed using the contrast procedure. Results: This study found that average and peak IEMG values for EL were generally smaller than the corresponding values for AE and BE but the differences were not statically significant. There were also no significant changes in kinematic variables among three different gaze angles. Conclusion: Based on the results of this study, gaze angle does not affect muscle activity and kinematic variables during treadmill walking. However, it is interesting to note that walking with BE may increase the muscle activity of the trapezius and the lower extremity. Moreover, it may hinder proper dorsiflexion during landing phase. Thus, it seems to reasonable to suggest that inappropriate gaze angle should be avoided in treadmill walking. It is obvious that increased walking speed may cause a significant changes in biomechanical parameters used in this study. It is recommended that future studies be conducted which are similar to the present investigation but using different walking speed.

ICD-10 분류로 살펴본 저단계 레이저 치료 임상 논문 고찰 (The Clinical Indication of Low-Level Laser Therapy Using ICD-10)

  • Objectives The purpose of this study was to improve the knowledge of the low-level laser therapy (LLLT) field and to review research reports on LLLT to understand the current situation with respect to the clinical indication and current research trends.Methods A survey was carried out on the subject of low-level laser therapy to September 2012, using the PubMed search engine. Selected literature was checked by two reviewers and was classified according to the International Classification of Diseases 10th (ICD-10) over 10 years.Results We selected 469 studies in total, of which 142 were case reports, 118 were case-controlled trials, and 209 were randomized controlled trials of LLLT. According to the ICD-10 classification of diseases, the K code and M code being the most common, 399 studies have been published in the last 10 years. This shows that the study and clinical indications of low-level laser therapy have rapidly increased over the past 10 years.Conclusions Low-level laser therapy has been used most frequently with respect to dentistry and pain and musculoskeletal disorders. Recently, interest in and research into LLLT has increased for various diseases. With the establishment of standard conditions for low-level laser therapy, supported by aggressive clinical utilization and systematic clinical research, LLLT will be a very useful treatment and a useful alternative method in many medical fields.

대칭형 들기 작업에서 사회심리적 요인이 근육 동원 형태에 미치는 영향 분석 (Effects of Box Color and Precision Demand on the Muscles' Recruitment Pattern in Repetitive Lifting Tasks)

  • In this study, the effects of psychosocial stress (box color and precision demand) on muscle activity were evaluated in laboratory setting. Eight subjects performed sagittally symmetric lifting tasks. Box color (yellow, black), precision demand (yes, no), and box weight (5%MVC, 10%MVC, 15 %MVC) were varied and surface EMG signals from seven muscles(medial deltoid right, biceps brachii right, lateral triceps right, latissimus dorsi right, erector spinae right, external oblique right, internal oblique right) were recorded. EMG signals were band-pass filtered($10{\sim}400\;Hz$), rectified, RMS smoothed and normalized (NEMG). Analysis of variance tests were conducted on the total NEMG (TNEMG: the sum of the seven muscles' NEMGs) and on the individual muscle's NEMGs. Box color had no effect on the TNEMG and on the seven muscles activities(p>0.05). When precision demand was required at the end point of lifts, the mean NEMG showed higher values than no precision demand conditions: TNEMG (14% increase) and medial deltoid(40% increase), biceps brachii(10% increase), lateral triceps(26% increase), latissimus dorsi(25% increase) muscles. Those increases showed more conspicuous as the box weight increased in the muscles of medial deltoid, lateral triceps, and latissimus dorsi.

Current Research Trends in Randomized Controlled Trials Investigating the Combined Effect of Korean Medicine and Western Medicine Treatment

  • This review examined recently published (July 2014 to June 2017), randomized controlled trials (RCTs) which investigated the safety and effectiveness of combined Korean medicine/complementary alternative medicine (CAM) and Western medicine, to indicate the direction for integrative medical practice. The Korean Medicine Convergence Research Information Center evidence-based medicine database (KMCRIC EBM DB) was used to retrieve relevant RCTs indexed in the last 3 years. Study design, country, sample size, disease/condition with the Korean Standard Classification of Diseases code, interventions, direction of outcomes, and adverse events were extracted and summarized. A total of 93 RCTs were included in this review. Acupuncture/moxibustion was the most commonly used intervention (n = 47; 51%), and 19% (n = 18) of the studies treated musculoskeletal disorders, followed by circulatory disorders (n = 16; 17%), and mental and behavioral disorders (n = 9; 10%). Integrative treatment was reported as more effective than monotherapy in approximately 83% of these studies. Adverse events were poorly reported in most studies. This review suggests that integrative treatments are feasible, effective, and safe for various diseases/conditions, based on the evidence from recently published RCTs. Future studies on integrative healthcare are warranted.

Comparative study of the Korean Regulations, Standards and Guidelines for the Human Vibration with Other Countries

  • Objective: The aim of this study is to review the literatures on the regulation, standard and guideline for the human vibration in Korea and other countries. Background: This review can be used to prevent various diseases caused by the human vibration as a basis for the development of the policy. Results: In Korea, the general employers' duties related to human vibration are set forth the Health Measures(Article 24) in the Occupational Safety and Health Act. And then an employer shall take measures to protect the health of the workers concerned by improving other working conditions relating to working hours for the vibration prevention measures referred to in Article 24 of the Act. The European Union adopted a Directive in 2002 on minimum requirements for the health and safety of workers exposed to vibration. New Regulations on Vibration at Work will be introduced in Great Britain on 2005 to implement the Directive. In the U.S., both ANSI and ACGIH adopted the ISO standard for measurement and suggested exposure action and limit values. In Japan, the Ministry of Labor decided that the vibration syndrome among operators of rock drills and riveters etc. could be included in an occupational disease(1947). In addition, ISO standard was based on proposals and draft documents of many countries such as U.K, Japan and European, etc. Conclusion: In Korea, Occupational Safety and Health Act prevent vibration to health, but do not include exposure limits. It is therefore important to consider the new duties regarding to vibration risks added to the general duties.

건강한 성인에서 인위적 다리길이 차이가 보행 중 3차원 엉덩관절 모멘트에 미치는 효과 (Effect of Artificial Leg Length Discrepancy on 3D Hip Joint Moments during Gait in Healthy Individuals)

  • Purpose: This study investigated the three-dimensional moment values of the hip joint for subjects with artificial leg length alterations and subjects with unaltered leg lengths. Methods: Forty-two healthy adults (8 men, 34 women) participated in this study. The selected subjects were able to walk normally, had less than a 1 cm leg length discrepancy, and were instructed to wear shoes that fit their feet. The study participants performed 8 dynamic gait trails to measure the hip joint moment using a three-dimensional motion analysis system. Kinetic and dynamic three-dimensional gait analysis data were collected from infrared cameras, and a force plate was used to standardize the weight of each subject. Results: There were significant correlations between the differences in the leg length discrepancy during right extension, right flexion, right internal rotation, and left extension in hip joint moments (p<0.05). There were significant correlations between the differences in shoe conditions during left extension, right flexion, right extension, and right internal rotation in the hip moments (p<0.05). Conclusion: This study suggests that a leg length discrepancy can affect hip joint moment, which may further exacerbate musculoskeletal disorders, such as osteoarthritis in lower extremity joints. Therefore, further studies should be conducted to verify the impact of clinical interventions on differences in hip joint moment values to correct leg length discrepancies and prevent osteoarthritis in lower extremity joints.

Effects of hallux valgus angle on one-legged stance and gait parameters in young adults: a preliminary study

  • Objective: Hallux valgus (HV) is a common musculoskeletal deformity that is accompanied with pain and continues to decrease one's quality of life and ability to perform daily life activities by affecting gait and static stability. Therefore, this study aimed to investigate the effect of the angle of HV (HVA) and to compare the one-legged stance and gait parameters in young adults with less HV and severe HV. Design: Cross-sectional study. Methods: Forty young adults were divided into two groups, where HVA ≥15° (n=20) was defined as HV, and HVA <15° (n=20) was defined as normal. For balance ability, the center of pressure (COP) path, velocity, length of axis of the COP path, deviation of the x-axis and y-axis, and percentage of foot pressure were measured, and gait, the foot rotation angle, step length, percentage of each phase of the gait cycle, time change from the heel to forefoot, and maximum pressure of the forefoot and midfoot were measured. Results: Significant differences were found in sway length and time change from heel to forefoot during walking between the normal and HV groups (p<0.05). Most parameters were not associated with the HVA, but parameters such as length of axis and time to change from heel to forefoot were significantly associated with the HVA (p<0.05). Conclusions: These results suggest that most one-legged stance and gait parameters were not significantly affected by the HVA in young adults; therefore, future studies are needed in order to address other dynamic parameters and other methods of gait analysis for detecting clinically meaningful conditions.

경추에서 발생한 호산구 육아종의 수술적 치료 - 3예 보고 - (Surgical Treatment of Eosinophilic Granuloma in Cervical Spine - 3 Cases Report -)

  • 척추에서의 호산구 육아종은 흔하지 않으며, 특히 경추 및 후방 신경궁에서의 발생은 드물다. 또한 호산구 육아종은 자기 공명 영상을 포함한 방사선학적 검사상 추체의 붕괴, 척추경의 소실 및 척추 주변 연부 조직의 종물 형성 등의 소견을 보이므로 다른 악성 골종양이나 전이성 종양 또는 결핵성 척추염 등과 감별을 요한다. 저자들은 각각 제 4 경추, 제 7 경추의 후방 신경궁, 그리고 제 4 경추체에서 발생한 호산구 육아종에 대하여 수술적 치료 후 완치를 보인 3례를 경험하였기에 문헌 고찰과 함께 보고하고자 한다.

하박 장착용 스마트 개인무장의 발사충격에 의한 인체거동 해석 (Simulation of Shot Impact by a Wearable Smart Individual Weapon Mounted on a Forearm)

  • One of the future weapon systems is the individual smart weapon which has a structure mounted on the forearm of soldiers. The structure may cause injuries or affect the accuracy of fire due to its impact on joints when shooting. This paper proposes human-impact interaction modeling and a verification methodology in order to estimate the impact of fire applied to the forearm. For this purpose, a human musculoskeletal model was constructed and the joints' behavior in various shooting positions was simulated. In order to verify the simulation results, an impact testing device substituting the smart weapon was made and the experiment was performed on a real human body. This paper compares the simulation results performed under various impact conditions and the experimental values in terms of accuracy and introduces methods to complement them. The results of the study are expected to be a basis for a reliable human-impact interaction modeling, and smart individual weapon development.

탄소나노튜브 인솔 착용에 따른 드롭 착지 동작의 생체역학적 분석 (Biomechanical Analysis of Wearing Carbon Nanotube-Based Insole during Drop Landing)

  • The purpose of this study was to determine the biomechanical effect of wearing carbon nanotube-based insole on cushioning and muscle tuning during drop landing. Twenty male university students(age: $21.2{\pm}1.5yrs$, height: $175.4{\pm}4.7cm$, weight: $70.2{\pm}5.8kg$) who have no musculoskeletal disorder were recruited as the subjects. Average axial strain, average shear strain, inversion angle, linear velocity, angular velocity, vertical GRF and loading rate were determined for each trial. For each dependent variable, a one-way analysis of variance(ANOVA) with repeated measures was performed to test if significant difference existed among different three conditions(p<.05). The results showed that Average axial strain of line 4 was significantly less in CNT compared with EVA and PU during IP phase. The average shear strain was less in CNT compared with EVA and PU during other phases. The inversion angle was increased in CNT compared with EVA and PU during all phase. In linear velocity, angular velocity, vertical GRF and loading rate, there were no significant difference between the three groups. This result seems that fine particle of carbon nanotube couldn't make geometric form which can absolve impact force by increasing density through eliminating voids of forms. Thus, searching for methods that keep voids of forms may play a pivotal role in developing of insole. This has led to suggestions of the need for further biomechanical analysis to these factors.
