A Case of Forearm Muscle Herniation after Radial Forearm Sensory Tendocutaneous Free Flap (요골 전완부 감각신경 유리건피판술 후 생긴 근육탈출증의 증례보고)

  • Purpose: Although muscle hernia has been well described in the lower-extremity, muscle hernias in the upper extremity are extremely rare. As with lower extremity muscle hernias, the forearm muscle hernia may result from forced exertion of strenuous activity or following blunt trauma. The objective of this paper is to report an extraordinary case of forearm muscle hernia after radial forearm sensory tendocutaneous free flap with references. Methods: A 58-year-old male patient received wide excision and radical neck dissection and lower lip reconstruction with radial forearm sensory tendocutaneous free flap for squamous cell cancer on the lower lip. 16 weeks after the operation, he complained of protruding mass on the forearm and the size was increasing. In postoperative 18 weeks, MRI showed herniation of flexor digitorum superficialis. For unaesthetic cause and preventing progress, the authors performed direct fascial closure and Mesh graft. Results: In 12 months after the surgery there was no recurrence and the patient remained symptom-free. Conclusion: Pain on extremity exertion and unaesthetic buldge of forearm due to forearm muscle hernia were the primary indications for surgery which consist of direct closure, fasciotomy, fascia lata onlay graft, fascia lata inlay graft, etc. The authors experienced uncommon forearm muscle hernia after radial forearm free flap and satisfying result of treatment.

Surgical Corrections of Perineal Hernia by Muscle Transposition in Dogs (근육전위을 이용한 회음허니아의 교정)

  • Two dogs (case 1:2-year-old intact male German Shepherd-mixed dog and case 2: 4-year-old intact female Jindo miked) with perineal hernia were referred to the Chonbuk Animal Medical Center, Chonbuk National University. In the both cases, there were unilateral perineal swelling involving the right side of the anus. In addition, in case 2, swelling was also present ventral to the anus. Rectal palpation of pelvic diaphragm revealed weakness of the muscles. Both cases were surgically treated using muscle transposition technique for hemiorrhaphy. In case 1, transposition of the superficial gluteal and internal obturator muscles and in case 2, transposition oi semitendinosus muscle was performed for hemiorrhaphy and reconstruction of the pelvic diaphragm. Both dogs became recovered after the surgical correction and no complications were observed during 10-month (casel) and 9-month(case2). Muscle transposition can be a useful technique for the treatment of perineal hernia in the dog.

A Case of Traumatic Ventral Hernia Repair with a Porcine Dermal Collagen Graft (Permacol) (외상성 복벽 탈장 환자에서 biologic mesh를 이용한 치료)

  • Resection of the bowel is necessary for the repair of a ventral hernia after recovery from trauma in some cases. In such instances, polyester or polypropylene meshcannot be used due to the possibility of infection; we had to use biological mesh instead. We report a case in which a traumatic hernia was repaired with Permacol (Covidien, Norwalk, CT, USA). A 42-year-old male patient had been injured by a factory machine seven months prior to admission. At that time, he had abdominal wall injury and small bowel perforation. His abdominal wall had been a defect after operation. A CT scan of the abdomen showed that the left abdominal wall, which is lateral to left rectus abdominis muscle had only one muscle layer, an external oblique muscle, and that a previous abdominal incision had a defect along the entire incision. During the exploration, 10 cm of small bowel was removed due to firm adhesion to the previous surgical scar. Permacol mesh was applied and fixed with transfascial fixations and tacks by using the intraperitoneal onlay mesh technique. There were no complications after the surgery and the patient was discharged without any problems.

  • Traumatic abdominal wall hernia after blunt abdominal trauma is rare. The prevalence of traumatic abdominal wall hernia in published series is approximately 1%. Recently, by the use of computed tomography has increased the number of occult traumatic abdominal wall hernias (TAWH). A 47-year-old woman presented to the emergency room soon after a traffic accident. She was fully conscious and complained of diffuse, dull, abdominal pain. She had a seat belt on at the time of the accident. Initial computed tomography showed that the lower left abdominal wall had a defect and that a part of the small bowel had herniated through the defect. During the operation, we made an incision at the defect site and confirmed the defect. The defect size was about $15{\times}5cm$. The muscle layers were repaired in layers with absorbable sutures. Prolen mesh was layed down and fixed on the site of the repaired muscle defect. After 6 months, hernia had not recurred, and no weakness of the repaired abdominal wall layers was identified. The patient's postoperative body functions were normal.

Ureterosciatic hernia causing obstructive uropathy successfully managed with minimally invasive procedures

  • Ureterosciatic hernia is extremely rare. In ureteral herniation, ureter prolapses occur through either the greater or lesser sciatic foramen. Atrophy of the piriformis muscle, hip joint diseases, and defects in the parietal pelvic fascia are predisposing factors for the development of ureterosciatic hernia. Most symptomatic patients have been treated surgically, with conservative treatment reserved only for asymptomatic patients. To the best of our knowledge, long-term follow-up outcomes after ureterosciatic hernia management are sparse. In this paper, we report the case of a 68-year-old woman who presented with colicky left abdominal pain. After computed tomography (CT) scan and anterograde pyelography, she was diagnosed ureterosciatic hernia with obstructive uropathy. We performed ureteral balloon dilatation and double-J ureteral stent placement. After this minimally invasive procedure, CT scan demonstrated that the left ureter had returned to its normal anatomical position without looping into the sciatic foramen. The patient remained asymptomatic with no adverse events 7 years after the minimally invasive procedures. This brief report describes ureterosciatic hernia successfully managed with minimally invasive procedures with long-term follow-up outcomes.

Complication and Morbidity of Donor Site after Free TRAM Flap (횡복직근 유리 피판술후 공여부 합병증과 이환율)

  • Transverse rectus abdominis muscle (TRAM) free flap is widely used for breast reconstruction, however donor-site morbidities such as abdominal wall weakness, hernia, bulging are troublesome. For the purpose of minimizing donor-site morbidity, there has been a surge in interests in muscle sparing free TRAM flap preserving the anatomy of rectus abdominis muscle, fascia, and motor nerve. The purpose of this study is to investigate complication and morbidity after muscle sparing free TRAM flap. Between August, 1995 and May, 2003, there were 108 cases of muscle sparing free TRAM flap of breast reconstruction. There was no abdominal hernia. There were 4 cases of dog ear, 3 cases of marginal necrosis of apron flap, 2 cases of asymmetry of umbilicus. At 1 year after operation, most patients feel comfortness in physical exercise. Muscle sparing free TRAM flap provides ample amount of well vascularized soft tissue with small inclusion of rectus abdominis muscle and fascia. Also it minimizes donor-site morbidity with rapid recovery of abdominal strength.

The Effects of Medx Exercise and Sling Exercise Program on the Lumbar Trunk Muscle Strength of Patients with Lumbar Disc Hernia Operation (8주간의 Medx운동과 Sling운동이 요추 추간판 수술환자의 체간근육의 근력에 미치는 영향)

  • This study planed to analyze durability of effect and result that the Medx and Sling exercise gets to the strength of lumbar extensors. 15 patients who had enforce the minimal invasive lumbar surgery were executed 3 times per week for 8 weeks. The purpose of this study was to identify the influence of the mixed exercises of the Medx and Sling program on lumbar trunk muscles and to present basic data for the proper exercise prescription for lumbar patients. The results were as follows: 1) Left: Patients by lumbar hernia operation of trunk muscle strength by Sling-exercise: 0, 45, 90, 135, 180 (degree) treatment periods (P<0.05). 2) Right: Patients by lumbar hernia operation of trunk muscle strength by Sling-Exercise: 0, 45, 90, 135, 180 (degree) treatment periods (P<0.05). Medx treatment, a muscle strengthening lumbar extension exercise program, was now being used at local hospitals. In addition, Sling exercise, which is designed to develop lumbar muscle by way of reducing gravity in a new way so that it can accelerate the growth of muscles and ligaments in-depth in the patients, also has begun to be introduced gradually. In this study, therefore, the new mixed program (Sling and Medx training)can used as an exercise program that can reduce pain and increase lumbar muscles, not only for disk disease patients but also for all those who have undergone surgery or who haven't undergone surgery, who have chronic pain, and it also can be utilized as basic data for the new method of exercise.

Congenital Intercostal Lung Herniation Combined with an Unusual Morgagni's Hernia

  • A 70-year-old male visited urgent care due to coughing for 1 month and left chest pain. He had no history of trauma. The initial chest computed tomography (CT) showed the 7th left intercostal lung herniation. A follow-up CT showed an intercostal lung herniation combined with a bowl herniation, which had developed due to a Morgagni's hernia. An emergency operation was performed due to the incarceration of the bowl and lung. The primary repair of the diaphragm was performed and the direct approximation of the 7th intercostal space was determined. We concluded that the defect of the diaphragm and the intercostal muscle was a congenital lesion, and the recurrent coughing was the aggravating factor of herniation.

Repair of Bilateral Perineal Hernia with Semitendinosus Muscle Transposition Along with Colopexy and Cystopexy in a Bitch (암컷 개의 양측 회음 허니아에서 결장 고정술과 방광 고정술 후 반힘줄 근육 전위술의 이용)

  • An 11 year-old intact female Yorkshire Terrier weighing 3.5 kg was presented with the complain of having a perineal mass, fecal incontinence and tenesmus. Physical examination revealed reducible bilateral ventral perineal mass, enlarged mammary gland and vaginal discharge. Bilateral ventral perineal hernia (BVPH) and pyometra were diagnosed based on the clinical and radiographic findings. Ovariohysterectomy was performed after general anaesthesia to treat pyometra. The BVPH was repaired by herniorrhaphy with semitendinosus muscle transposition along with colopexy and cystopexy. The postoperative radiographs and CT revealed a good correction of the BVPH. The patient showed good activity after 10 days and no complications were observed during a one year follow up period. BVPH can be successfully surgically managed by herniorrhaphy with semitendinosus muscle transposition along with cystopexy and colopexy in the dog.

Usefulness of Muscle Plication and Synthetic Mesh in Breast Reconstruction Using TRAM Pedicled Flap (배곧은근피판을 이용한 유방재건 시 복벽의 주름형성과 합성그물을 이용한 복부성형술의 유용성)

  • Purpose: The transverse rectus abdominis myocutaneous(TRAM) flap has become a mainstay of breast reconstruction. The chief disadvantage of the TRAM flap is its potential to create a weakness in the abdominal wall. Nowadays true hernia is less frequent, but bulging that appears at the muscle donor site, or at the contralateral side, or at the epigastric area is still remained as a problem. To prevent this complications, we have used synthetic mesh as well as abdominal muscle plication. Now we report the result of our methods. Methods: We started to use synthetic mesh and muscle plication as supplementary reinforcement for entire abdominal wall, after TRAM flap harvesting, in an attempt to stabilize it and achieve a superior aesthetic result since 2002. We observed complications of TRAM flap donor site, and compared our results (from January, 2002 to January, 2006) with other operator's result (before 2001) at the same hospital in aspect of incidence of abdominal complications. Results: 42 consecutive patients have been performed routine reinforcement with the extended mesh technique and muscle plication from January, 2002 to January, 2006. Mean patient follow up was 25.2 months. No hernia or mesh related infection were encountered and only one patient had a mild abdominal bulging. Nevertheless the our good results, there were no significant statistical differences were observed between two groups. Conclusion: We recommend the using of synthetic mesh and muscle plication for donor site reconstruction after TRAM flap breast reconstruction to improve strength as well as aesthetic quality of the abdominal wall.
