노인의 사회 경제적 수준과 주관적 건강수준과의 관계 (The relationship between socio-economic factors and self-rated health among older people)

  • The purpose of this paper is to examine the relationship between socio-economic factors and self-rated health among older people living in the community. In addition, the study tries to determine whether risk differentials by these socio-economic factors can be explained by other demographic factors, chronic diseases, and functional status. We surveyed to investigate the self-rated health of 397 study samples which had been selected by stratified randomized sampling, $2.7\%$ by each Dong (district) of S-city in Gyeonggi-do. Our study found that the socioeconomic factors such as income, occupation, and insurance were significantly associated with self-rated health. The level of social economic status was positively associated with the level of self-rated health. Two-staged multivariate analysis demonstrated that this relationship was still significant even after adjustment for demographic factors, chronic diseases, and functional status. In conclusion, there are wide socio-economic disparities in self-rated health of older people in this community. It is important that government should know not only health status but also the health-associated factors in order to prepare for the aged society and improve the health status of the elderly. Further researches should uncover causality and mechanism by which SES affects changes in functional health among the elderly.

Impact of Job Satisfaction on Greek Nurses' Health-Related Quality of Life

  • Background: Employee job satisfaction and its relationship with health and quality of life has been an issue of major concern over the past decades. Nurses experience difficult working conditions that affect their job satisfaction, health, and quality of life. Methods: A cross-sectional study was undertaken in three general hospitals and their respective health centers. Stratified random sampling by level of education was used, and 508 nurses and nursing assistants were included. A self-administered anonymous questionnaire, which included the Measure of Job Satisfaction, the 36-item Short Form Health Survey, as well as demographic details, education, and work conditions data, was used. Results: Greek nurses were found to be dissatisfied with their job according to the total score of the job satisfaction scale, although personal satisfaction and satisfaction with support had had higher scores. Their general health was reported as average, because of physical and mental health problems, low vitality, low energy, and increased physical pain. Multivariate linear regression analysis revealed that males and those wishing to stay in the job had higher physical and mental health. Increased job satisfaction was related to increased physical and mental health. Conclusion: Although Greek nurses are not satisfied with their work, those with high levels of job satisfaction had better health-related quality of life. The findings suggest that improvement of the work environment would contribute to a healthier and more satisfied nursing workforce.

The Risk Factors for Musculoskeletal Symptoms During Work From Home Due to the Covid-19 Pandemic

  • Background: Online teaching and learning extend the duration of using gadgets such as mobile phones and tablets. A prolonged usage of these gadgets in a static position can lead to musculoskeletal disorders (MSD). Therefore, this study aims to identify the risk factors related to musculoskeletal symptoms while using gadgets during work from home due to the COVID-19 pandemic. Method: A cross-sectional survey with online-based questionnaires was collected from the University of Indonesia, consisting of lecturers, students, and managerial staff. The minimum number of respondents was 1,080 and was defined by stratified random sampling. Furthermore, the dependent variable was musculoskeletal symptoms, while the independent were age, gender, job position, duration, activity when using gadgets, and how to hold them. Result: Most of the respondents had mobile phones but only 16% had tablets. Furthermore, about 56.7% have used a mobile phone for more than 10 years, while about 89.7% have used a tablet for less than 10 years. A multivariate analysis found factors that were significantly associated with MSD symptoms while using a mobile phone, such as age, gender, web browsing activity, work, or college activities. These activities include doing assignments and holding the phone with two hands with two thumbs actively operating. The factors that were significantly associated with MSD symptoms when using tablets were gender, academic position, social media activity, and placing the tablet on a table with two actively working index fingers. Conclusion: Therefore, from the results of this study it is necessary to have WFH and e-learning policies to reduce MSD symptoms and enhance productivity at work.

자연공원의 분위기가 이용자의 만족도에 미치는 영향 - 국립공원과 도립공원을 대상으로 - (The Atmospheric Factors Affecting User's Satisfaction in Natural Parks)

  • The purpose of this paper is to examine atmospherical factors affecting user's satisfaction in natural park to answer the research question: what are the effects of atmosphere on user's satisfaction in natural parks(NP). After reviewing the literature, mechanism of NP, and use elements in NP, We constructed the conceptual framework and have formulated the hypothesis of this research. We had obtained data through a questionnaire, which surveyed 508 visitors at 6 of the 73 NP in Korea in 2001, based on stratified sampling method. We have analyzed the data using descriptive statistical methods, the mean difference test, Pearson's correlation analysis, and the multiple linear regression method. We found that 1) the five atmospheric variables, j.e., number of users(NOU), crowding, damage to park resources(DPR), and maintenance of park resources and facilities(MPRF), encounter level(EL) affecting user's satisfaction, have tuned out to be statistically significant at a five percent level. The direction of the relationship between user's satisfaction and MPRF, NOU, EL is the same as that of the dependent variable and the opposite of crowding, and BPR, 2) in bivariate analysis, the positive relationships between user's satisfaction and park resources, MPRF are fairly high and statistically significant. The higher the value of DPR, and crowing, the lower the degree of user's satisfaction, 3) in multivariate analysis, such variables as NOU, crowding, DPR, EL, and MPRF affecting user's satisfaction have been statistically significant at five percent level, and 4) the relative contribution of MPRF, park resources, park facilities, NOU, crowding, DPR, and size of activity space on user's satisfaction have been determined to have respectively 6.00, 4.78, 2.53, 1.83, 1.64, 1.59 and 2.03 times more important than that of EL. Among the atmospheric variables, MPRF is the most important at 1.26 times higher than that of park resources. The research results suggest that the development of devices for the increase in user's satisfaction and user management program based on the knowledge we have found, be recommended in the planning and development process of natural park. The approach adopted by this research is valid and useful for evaluation criteria of NP. It is recommended that more empirical studies by activity types, activity spaces, and seasons on atmospheric elements affecting user's satisfaction be performed in the future.

전국 병원영양사의 직무만족도 현황 및 관련요인 (Job Satisfaction Level and Related Factors among Korean Hospital Dietitians)

  • The objective of this study was to examine job satisfaction status and its related factors among dietitians working in hospitals. A survey questionnaire was sent to dietitians working in tertiary, general, and convalescent hospitals in Korea. The study sample hospitals included all tertiary hospitals (n=43), and the sample of general hospitals (n=114) and convalescent hospitals (n=240) was randomly selected using a stratified sampling method. A total of 665 dietitians completed the survey questionnaire. The questionnaire included 27 job satisfaction questions on task, stability vision, working conditions, and relationship areas, and the questions on task improvement need. The job satisfaction scores in all four areas ranged from 2.53/5.00 to 3.89/5.00, implying generally low job satisfaction level in hospital dietitians. The job satisfaction levels in the convalescent hospital group were significantly lower compared to those in the tertiary and general hospital groups. Whereas job satisfaction levels of tertiary and convalescent hospital groups did not differ by number of beds, those of the general hospital group increased significantly with the number of beds for all four areas. Multivariate regression analysis results showed that hospital type was a significant predictor of job satisfaction level for all four areas. Job satisfaction of relationship area was also significantly affected by the number of beds, whereas that of working conditions was influenced by annual income and working experience. The study findings provide useful information in planning clinical dietitians' optimal supply outlook for the improvement of clinical nutrition services.

우리나라 소아.청소년 장애인의 치아우식증 실태와 관련요인 (DENTAL CARIES STATUS AND RELATED FACTORS AMONG DISABLED CHILDREN AND ADOLESCENT IN KOREA)

  • 2004년 보건복지부에서 시행한 전국 장애인 구강보건실태조사 자료 중 만 6-17세 사이의 소아 청소년 장애인 677명을 대상으로 하여 장애유형을 신체장애, 정신지체 및 감각장애로 분류한 후 이들의 치아우식증 실태를 파악하여 다음과 같은 결론을 얻었다. 소아 청소년 장애인들의 전체 평균 우식경험영구치지수와 우식경험영구치면지수는 각각 3.06개 및 5.24면이었고, 각 연령대별 우식경험영구치지수와 우식경험영구치면지수는 6-8세는 0.90개 및 1.45면, 9-11세는 2.32개 및 3.76면, 12-14세는 3.63개 및 6.33면 그리고 15-17세는 4.93개 및 8.35면이었다. 또한 소아.청소년 장애인들의 우식경험영구치지수에 유의한 영향을 미친 변수로는 연령, 잇솔질 횟수 및 잇솔질 의존도 그리고 저작 및 기능장애이었다. 연령이 증가할수록(b=1.34, p<0.001), 잇솔질 횟수가 적을수록(b=-0.27, p=0.034), 잇솔질을 자력으로 하는 경우에(b=-1.43, p<0.001), 그리고 구강 내에 저작 및 기능장애가 있는 경우에(b=1.24, p=0.011) 우식 경험영구치지수가 유의하게 높았다. 또한 우식경험영구치면지수에 유의한 영향을 미친 변수는 연령, 잇솔질 횟수 및 잇솔질 의존도로, 연령이 증가할수록(b=2.49, p<0.001), 잇솔질 횟수가 적을수록(b=-0.66, p=0.024) 그리고 잇솔질을 자력으로 하는 경우(b=-1.75, p=0.042)에 우식경험영구치면지수가 유의하게 높게 나타났다. 따라서 우리나라 소아 청소년 장애인들의 치아우식증의 진행과 중증도를 감소시키기 위해 각 연령대별로 보다 효과적인 잇솔질 교습을 포함한 구강건강증진 프로그램을 개발하여 적용하는 것이 필요한 것으로 사료되었다.

자연공원 휴양자원의 이용적합성 평가 (An Evaluation on Use Suitability of Recreation Resource in Natural Parks)

  • 본 연구는 자연공원의 각 휴양자원이 이용적합성에 미치는 영향을 구명하여 합리적인 자연공원 휴양자원의 관리에 필요한 지식을 제공함을 목적으로 한다. 본 연구에서는 11점 등간척도의 점수부과체계에 따라 자연공원 6개소에서 총 385명의 이용자를 대상으로 한 설문조사를 통해 평가하였으며, 다중회귀분석 방법으로 검정하여 다음의 결과를 얻었다. 1) 지형, 사회적 자원. 문화적 자원, 경관, 향기$.$$.$소리가 5%의 유의수준에서 이용적합성에 통계적 유의성이 있는 것으로 판명되었으며. 이들 자원의 수준이 우수할수록 자연공원 휴양자원의 전체적인 이용적 합성이 높아지는 것으로 판명되었다. 2) 휴양자원 중에서 이용적합성의 값의 변화에 영향력이 가장 큰 것은 지형이었다. 3) 자연공원의 휴양자원들 중에서 이용적합성에 대한 상대적 기여도의 크기는 문화적 자원을 1.00으로 할 때. 이에 비해 향기$.$$.$소리는 1.05배. 사회적 자원은 1.56배. 경관은 2.16배. 지형은 2.70배 더 중요한 것으로 평가되었다. 본 연구의 결과는 휴양자원의 평가와 관리의 우선순위를 결정하기 위한 하나의 기준으로 활용될 수 있을 것이다.

고령친화환경 요인들이 삶의 만족 및 노화불안에 미치는 영향에 대한 연령집단별 비교 연구 (The Effects of Age-friendly Environment Factors on the Life Satisfaction and Aging Anxiety: An Analysis by Age-Cohort Groups)

  • 본 연구는 크게 두 가지의 연구목적을 갖는다. 첫째, 생애과정관점에 따라 중고령자들의 연령집단별 삶의 만족과 노화불안의 수준을 살펴봄으로써 대도시에서 노년기를 맞이한 사람들의 삶의 현황을 파악한다. 둘째, 노화과정에서 심리적 안녕감을 나타내는 삶의 만족 제고와 노화불안 감소에 기여하는 영향요인, 즉 개인의 객관적인 삶의 조건을 의미하는 인구사회학적 변수들의 영향력과 고령친화 환경의 영향력을 상호 검토함으로써 고령친화환경 조성을 통한 시사점을 제안한다. 이를 위해 본 연구는 서울에 거주하는 베이비부머를 포함하는 노년기 세대를 대상으로 비례층화표집 방법을 활용하여 조사한 4,000명의 자료를 분석하였다. 주요 분석결과를 중심으로 논의를 정리하면 다음과 같다. 첫째, 연령집단의 연령대가 증가함에 따라 삶의 만족 수준은 낮아지는 반면, 노화불안의 수준은 높아지는 것으로 나타났다. 둘째, 연령집단의 연령대가 높아질수록 삶의 만족과 노화불안에 대한 개인의 객관적 삶의 조건들의 설명 분산은 높아지는 경향성을 나타냈으며, 연령집단별 영향변수는 상이하게 나타남에 따라 노년기 연구에 있어서 생애과정 관점의 유용성을 보여주었다. 셋째, 인구사회학적 변수들을 통제한 상태에서 연령집단별 고령친화환경 요인들이 삶의 만족에 대한 설명분산이 높게 나타난 반면, 노화불안에 대한 설명분산은 상대적으로 낮게 나타났으며, 후기노인의 경우 장년층에 비해 설명분산이 높게 나타났다. 한편 연령집단별 삶의 만족과 노화불안에 영향을 미치는 고령친화환경 요인들은 연령집단별 공통적인 요인들이 존재하는 반면, 연령집단별 상이한 요인들도 존재하는 것으로 나타났다. 이와 같은 결과들에 따라 노년기를 보내는 개인의 삶의 만족을 제고하고 노화불안을 감소시키기 위한 정책설계 방향에 대해 논의하였다.

현존자연치아수와 만성폐쇄성폐질환과의 연관성 (Association between the Number of Existing Permanent Teeth and Chronic Obstructive Pulmonary Disease)

  • 본 연구는 2009년 국민건강영양조사 자료를 이용하여 우리나라 성인에서 다양한 혼란변수를 보정한 후 현존자연치아수와 COPD와의 연관성을 알아보고자 하였다. 40세 이상 성인 3,107명을 대상으로 구강검진과 폐기능검사를 시행하여 다음과 같은 결론을 얻었다. 첫째, 폐기능검사에 의한 COPD 유병률은 11%였다. 현존자연치아수를 0~19개군, 20~27개군, 28개군으로 나누었을 때, COPD의 분포는 현존자연치아수가 0~19개인 군에서 21.0%로 가장 높았다. 둘째, COPD는 65세 이상군, 남자, 교육수준이 낮을수록, 정상적인 BMI를 갖는 군, 고혈압군에서 높았다(p<0.001). 셋째, 인구사회학적 특성, 사회경제적 위치, 건강관련행태, 구강건강 및 행태, 전신건강상태를 보정한 다중로지스틱 회귀분석에서는 현존자연치아수 20~27개군(OR, 1.90)과 0~19개군(OR, 3.93)에서 COPD와 연관성을 보여, 현존자연치아수가 적을수록 COPD의 위험이 증가함을 확인하였다. 특히 치주질환과 COPD의 중요한 위험요인인 흡연상태에 따른 층화분석결과, 현재흡연군에서만 모든 치아를 가지고 있는 군에 비해 0~19개 치아를 가진 군에서 OR이 8.90배 높은 결과를 보여 강한 연관성을 보여주었다(OR, 8.90; 95% CI, 2.53~31.33). 본 연구 결과를 통해 현존자연치아수는 COPD와 연관성이 있음을 확인하였다.

일부 도시지역 주민들의 일상생활습관과 스트레스와의 관련성 (Health Related Lifestyle and Stress Among Inhabitants of a City in Korea)

  • The purpose of this study was to evaluate the patterns of stresses arising from various life styles and their related sociodemographic factors in urban residents. The subjects were recruited from the population of two ‘Dong’s (administrative blocks) representative of Daejeon city through stratified cluster random sampling during the period ranging from June I st to Aug. 31 st, 2003. Self-administered questionnaires, including items asking about subjects' sociodemographic characteristics, daily life styles, measurement of stresses by General Health Questionnaire (Goldberg, 1978), were delivered to 396 residents and their responses were analyzed with the following results. 1. Based on the discriminant scores of HPI, 46.7% of the subjects were found to have less than 4 points (poor life style), whereas 53.3% had points higher than 5 (good life style). Higher than 5 points were scored more frequently in females than in the male, in the age group of 40's - 60's than the 30's, in the group with spouse than without, in the group with both spouse and offsprings than without, and in the group owning a house than not. 2. Based on the degree of stress, 17.7% of the subjects were determined to be healthy, 74.5% were potentially under stress, and 7.8% were at higher risk of stress. The proportion of healthy individuals were significantly higher in the male, advanced aged group(40's and 60's), the group with higher education years( over highschool), with higher monthly income over two million Won, and with spouse, than their respective counterparts. On the other hand the proportion of the individuals potentially under stress and at higher risk of stress was significantly higher in the female, in the age group of 30's and over 60's, in the group with academic career lower than middle school, with monthly income lower than two million Won, and without spouse. 3. Based on the relation of HPI with degree of stresses, subjects with HPI scores lower than 4 had increased rate of falling into the groups under potential stress and at higher risk of stress, while on the other hand those with over 5 points were found to be healthy in light of stress. 4. Based on the specific relation of each item of HPI with degree of stress, the proportion of healthy individuals was higher in the groups who take appropriate hours of sleep(7-8hours), who take breakfasts everyday, who take physical exercises everyday, who don't smoke, who don't drink alcohol, who take snacks everyday, who are overweight and obese, whereas the proportion of the group under potential stress and at higher risk of stress was higher in their comparable counterparts. 5. The relation of mean scores of HPI with stress scores in both male and female subjects showed negative correlation that the higher HPI scores, the lower stress scores. 6. Multivariate regression analysis to reveal the factors influencing the stress of the subjects showed that for men the significant factors were age, education, presence of job or not, exercise, subjective health status, with the explanatory power of 26.3%. For women, they included educational years, presence of spouse or not, job, owning a house or not, sleeping hours, drinking habit, taking snacks, subjective health status, with the explanatory power of 31.8%. The above study results suggests that stresses of urban residents have significant correlation with daily life styles and this correlation is also remarkably distinguished by different age and sex.
