Home Injury Pattern in One Small City: Structure and Action (지방중소도시의 가정내 안전사고: 안전구조와 안전행동의 상대적 영향)

  • The study of home injury and injury prevention has been a neglected area of research and practice. This study has the following purposes: 1) to describe the pattern of home injuries; 2) to analyze the differences in injuries by social characteristics; and 3) to compare the impact of safety structure and action. The data were collected by person-to-person interviews with 450 residents living in Wonju area. The statistical methods used for the analysis were $\chi$2-test and multiple logistic regression. The results are as follows: 1) Whether or not a certain injury has occurred varies by household type. The elderly household reported more injuries than other types of household. 2) Using logistic regression method, we found that safety-related behavioral habit was statistically significant in predicting injury occurrence. However, residence type was not a statistically significant variable. This indicates that cognitive change strategy is more important than structural change strategy.

Assessment on Location Characteristics of Urban Park as Public Service Using Geographic Information Analysis System: Focused on Cheongju City (지리정보분석시스템을 활용한 공공서비스로서의 도시공원 입지특성 평가 - 충북 청주시를 대상으로 -)

  • The Purpose of this research was to propose positioning strategies of urban park (UP) based on the assessment of location characteristics at cheongju city. To do that, this research found out urban park service area (UPSA) using GIS network analysis and built socio-economic attribute database, UP map, and other public service thematic maps such as public transportation, education, child-care, and convenience services. And this research analyzed spatial and attribute data using Pearson's correlation analysis, multiple linear regression, and binary logistic regression methods. As a result of this analysis, 1) the nearer neighborhood park and children's park, the higher land price and assumption income level (AIL). 2) children's parks were closed to living convenience facilities such as bank, hospital, and convenience store. 3) land price, AIL, population, and other public services level (PSL) in UPSA were higher than that of non-UPSA. 4) The higher land price, AIL, population, and other PSL, the higher urban park service level. The results of this research may contribute to resolve the regional UP unbalance and to improve UP service level as public service.

Triggers and Outcomes of Falls in Hematology Patients: Analysis of Electronic Health Records (혈액내과 입원 환자의 낙상 위험 요인과 환자 결과: 전자의무기록 분석)

  • Purpose: The goal was to use electronic health records to identify factors and outcomes associated with falls among patients admitted to hematology units. Methods: This retrospective case-control study included data from a tertiary university hospital. Analysis was done of records from 117 patients with a history of falls and 201 patients with no history of falls who were admitted to the hematology unit from January 1, 2013 to December 31, 2014. Risk factors were analyzed using hierarchical logistic regression; patient outcomes were analyzed using multiple logistic regression, Cox proportional hazards regression, and multiple linear regression. Results: Clinical factors such as self-care nursing (OR=4.47, CI=1.64~12.11), leukopenia (OR=6.03; CI=2.51~14.50), and hypoalbuminemia (OR=2.79, CI=1.31~5.96); treatment factors such as use of narcotics (OR=2.06, CI=1.01~4.19), antipsychotics (OR=3.05, CI=1.20~7.75), and steroids (OR=4.51, CI=1.92~10.58); and patient factors such as low education (OR=3.16, CI=1.44~6.94) were significant risk factors. Falls were also associated with increased length of hospital stay to 21.58 days (p<.001), and healthcare costs of 17,052,784 Won (p<.001). Conclusion: These findings can be a resource for fall prevention education and to help develop fall risk assessment tools for adults admitted to hematology units.

Factors Associated with Physical Activity and Sedentary Behavior among Elementary School Students (일부 초등학교 5, 6학년 학생의 신체활동과 좌식생활 관련 요인)

  • Objectives: The purpose of this study was to identify personal and social environmental factors associated with physical activity and sedentary behavior among elementary school students. Methods: Cross-sectional self-reported data were collected from a conveniently clustering sample population of 1538 grade 5 to 6 students attending 19 elementary schools in Seoul metropolitan city and Gyeonggi province. Data were statistically analyzed using Chi-square test and multiple logistic regression analysis. Results: In multiple logistic regression analyses, significant factors that were associated with schoolchildren's physical activity were gender, father's job, social support for physical activity, friend support, participation in school physical education class. Father's education level, mother's job, family functioning and urban residents were significantly associated with TV viewing and gender, age, BMI(obesity), mother's job, family functioning and urban residents were significantly associated with playing computer games among elementary schoolchildren. These results showed that physical activity among elementary school students was most associated with social environmental factors and sedentary behavior among school students was most associated with personal and family environment factors. Conclusion: Health care providers should develop interventions to improve these family and social environmental factors to increase physical activity levels and to decrease sedentary behavior among elementary schoolchildren.

Predictors associated with Occurrence of Needlestick Injuries in Clinical Practicum among Nursing Students (간호대학생의 임상실습 중 주사침 자상 발생 예측요인)

  • Purpose: The purpose of this study was to identify prevalence and risk factors of needlestick injuries (NSI) among nursing students. Methods: Data were collected using self-administered questionnaires which included general and practical characteristics, safe environment risk factors, knowledge, attitude, and preventive behavior associated with NSI. Data were conducted from December 14-20, 2014 and analyzed using $x^2$ test and multiple logistic regression. Results: The incidence rate of NSI was 20.2%. Multiple logistic regression analysis identified four predictors for NSI which were statistically significan; carelessness (OR=1.49, 95% CI=1.04~2.11), status of hospital, university affiliated (OR=1.93, 95% CI=1.35~2.75), knowledge about handing the needle after NSI (OR=1.42, 95% CI=1.00~2.03), and barriers to NSI preventive behavior (OR=1.48, 95% CI=1.04~2.12). Conclusion: Management of carelessness and barriers to NSI preventive behavior are essential elements for reducing the risk of NSI in nursing students. It is necessary to develop a specific NSI precaution program for nursing students.

Effects of job satisfaction and patients satisfaction on medical profit at public hospitals (공공병원 직원의 직무만족도 및 환자 만족도가 의료수익에 미치는 영향)

  • Purpose: This study was conducted to examine the factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Method: A cross-sectional survey was conducted through staff and patients in 39 hospitals (34 local province hospitals, and 5 red-cross hospitals) from September to October in 2011. A questionnaire was designed to collect information on job satisfaction, inpatients and outpatients satisfaction. The data of medical profit per bed was collected in 2011 annual financial reports from each 39 hospitals. Finally, data from 5,521 staff, 1,730 in-patients and 1,730 out-patients were analyzed. Multiple logistic regression analysis was performed to determine factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Results: The medical profit was positively correlated with job satisfaction of staff. As the results of multiple logistic regression, the significant variables of medical profit per bed, were communication satisfaction of staff(OR=0.48, 95% CI=0.25-0.91), hospital environment satisfaction of in-patients(OR=1.53, 95% CI=1.05-2.22). Conclusion: The satisfaction of patient and staff were significantly effect to medical profit in public hospitals. The Government and administrators have make an effort to improve satisfaction of staff and patients in public hospitals.

Breast Cancer Screening and Repeat Screening (유방암 조기검진수검 및 반복수검 실태와 관련요인)

  • Purpose: This study was to investigate the characteristics and related factors of breast cancer screening and repeat screening, and to propose nursing interventions to increase the rate of breast cancer screening and repeat screening. Method: Study subjects was 236 women residing in the community, teachers and nurses who were older than 45 were recruited. Data was collected with self administered questionnaires from July 1st to August 31st, 2003 and analysed using SPSS/WIN 10.0 with $X^2$test, t-test, and stepwise multiple logistic regression at a significant level of =.05. Result: The breast cancer screening rate was 57.2%, and repeat screening rate was 15.3%. With the multiple logistic regression analysis, factors associated with mammography screening were age and perceived barriers of action, and factors related to the repeat mammography screening were education level and other cancer screening experience. Conclusion: Based on the results, we recommend the development of anintervention program to decrease the perceived barrier of action, to regard mammography as an essential test in regular check-up, and to give active advertisement and education to the public to improve the rates of breast cancer screening and repeat screening.

Homebound status and Related Factors According to Age in Female Elders in the Community (지역사회 재가 여성노인의 칩거와 영향 요인)

  • Purpose: The purpose of this study was to investigate homebound status and significant related factors for community-dwelling female elders according to age. Methods: The participants were female elders over 65 years of age registered in public health centers. Data were collected by interviewing the elders, who voluntarily completed the entire survey. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 18.0. Results: There were statistically significant differences in homeboundness and related factors between the young-old (65 to 74 years of age) and the old-old (75 years of age or older). The level of homeboundness of the old-old was higher than that of the young-old. Multiple logistic regression showed, timed "up and go", depression, and fear of falling as significantly associated with homebound status of the young-old, while hand grip strength (right), timed "up and go", static balance ability, severity of urinary incontinence, and fear of falling as significant for the old-old. Conclusion: The findings of this study indicate that homebound status and related factors for elders are different according to age, and therefore, interventions to prevent and help homebound elders get over being homebound should be developed according to age.

Factors Associated with Depression among Workers by Socio-economic Factors, Health Behaviors, and Characteristics of Work Environment (근로자의 우울 관련 요인: 사회경제적 특성, 건강행태, 근무환경을 중심으로)

  • Objectives: The purpose of this study is to analyze factors associated with depression among workers in South Korea by socio-economic factors, health behaviors, and working conditions. Methods: Study subjects include 4,251 adults (19 and older) who are currently working using the data from 2009 National Health and Nutrition Survey. Data were analyzed using frequency test, $X^2$ test, and multiple logistic regression. Results: Multiple logistic regression analysis showed that gender (OR=1.91, p<.001), age (OR=1.53, p=.030), marital status (31-39: OR=1.88, p=.029) were significantly related to depressing among socio-economic factors. Next, among health behaviors factors, perceived health status (Poor: OR=1.76, p=.049; Very poor: OR=3.46, p=.002) was statistically significant. Among characteristics of work environment, trust and respect (OR=1.34, p=.049) and working with hidden emotion (OR=1.88, p<.001) were significantly related to depression. Conclusions: In order to manage and reduce depression among workers, it is important to develop appropriate mental health promotion programs both in work places and in regulations.
