Determinants for further wishes for cosmetic and reconstructive interventions in 1652 patients with surgical treated carcinomas of the oral cavity

  • Background: The impairment of the appearance is a major problem for patients with carcinomas of the oral cavity. These patients want to recover their preoperative facial appearance. Some do not realize that this is not always possible and hence develop a desire for further cosmetic and reconstructive surgery (CRS) which often causes psychological problems. Method: The desire of patients for CRS (N = 410; 26%) has been acquired in this $D{\ddot{O}}SAK$ rehab study including multiple reasons such as medical, functional, aesthetic and psychosocial aspects. They relate to the parameters of diagnosis, treatment and postoperative rehabilitation. Patients without the wish for CRS (N = 1155; 74%) served as control group. For the surgeons, knowledge of the patient's views is relevant in the wish for CRS. Nevertheless, it has hardly been investigated for patients postoperatively to complete resection of oral cancer. In this retrospective cross-sectional study, questionnaires with 147 variables were completed during control appointments. Thirty-eight departments of Oral and Maxillofacial Surgery took part, and 1652 German patients at least 6 months after complete cancer resection answered the questions. Additionally, a physician's questionnaire (N = 1489) was available. Statistical analysis was performed with SPSS vers. 22. Results: The patient's assessment of their appearance and scarring are the most important criteria resulting in wishes for CRS. Furthermore, functional limitations such as eating/swallowing, pain of the facial muscles, numb regions in the operating field, dealing with the social environment, return to work, tumour size and location, removal and reconstruction are closely related. Conclusion: The wish for CRS depends on diverse functional psychosocial and psychological parameters. Hence, it has to be issued during conversation to improve rehabilitation. A decision on the medical treatment can be of greater satisfaction if the surgeon knows the patients' needs and is able to compare them with the medical capabilities. The informed consent between doctor and patient in regard to these findings is necessary.

Motion Vector Coding Using Adaptive Motion Resolution (적응적인 움직임 벡터 해상도를 이용한 움직임 벡터 부호화 방법)

  • In most conventional video codecs, such as MPEG-2 and MPEG-4, inter coding is performed with the fixed motion vector resolution. When KTA software was developed, resolution for MVs can be selected in each slice. Although KTA codec uses a variety of resolutions for ME, the selected resolution is applied over the entire pixels in the slice and the statistical property of the local area is not considered. In this paper, we propose an adaptive decision scheme for motion vector resolution which depends on region, where MV search area is divided to multiple regions according to the distance from PMV. In each region, the assigned resolution is used to estimate MV. Each region supports different resolution for ME from other regions. The efficiency of the proposed scheme is affected from threshold values to divide the search area and the entropy coding method to encode the estimated MV. Simulation results with HM3.0 which is the reference software of HEVC show that the proposed scheme provides bit rate gains of 0.9%, 0.6%, and 2.9% in Random Access, Low Delay with B picture, and Low Delay with P picture structures, respectively.

  • Small and middle sized Franchise system channels experience conflicts because of constant interaction between franchiser and franchisee. However, it is rarely that attempt has been made to identify causes of conflicts in distribution of franchise industry. The purpose of this study was to explore cause of conflicts from the franchisee. For the study, data were collected from the owner or the shop-manager with a contractor in franchise systems. By means of in-depth interviews with multiple informants, their statement was analyzed qualitatively. From analysis of the resulting data, the causes of conflicts were associated with two broad sets, attitudinal and structural differences between franchisers and franchisees. First, attitudinal sources of conflict were identified as perception of subordinated relationships between channel members, delayed announcement on order changes, difficulties in communication by doing irresponsible behavior, different expectations between channel members in market territory and lack of consistent-supporting. Second, structural sources of conflict involved unilateral decision on interior, payment method, unreasonable delivery issue, treat franchisee with discrimination by the sales, in controlling against free management. There are limitations on generalization due to the results based on interview, but this study will be a useful exploratory step before designing a large scale survey.

Arthscopic Treatment of Osteochondritis Dissecans in Femoral Condyle (관절경을 이용한 대퇴골과 박리성 골연골염에 대한 치료)

  • Purpose : We analyzed clinical and radiological results of the treatment of osteochondritis dissecans in the femoral condyle under arthroscopic guidance. Materials and Methods : The study group consists 19 cases in 17 patients. Average follow up period was 34 months and average age was 16 years. The cases were classified by 4 different groups, using the fellowing system: Group 1-stable lesion and no specific treatment after arthroscopic examination; Group 2-early separation and multiple drilling; Croup 3-unstable lesion and Herbert screw fixation; Croup 4-loose body removal and/or crater curettage. The results were analyzed by the criteria of Hughston which including clinical and radiologic outcomes. Results : There were 14 cases$(74\%)$ of good and excellent results in 19 knees in which, $75\%$(3/4) in Group 1, $75\%$(3/4) in Group 2, $86\%$(7/8) in Group 3 and $33\%$(1/3) in Group 4. The result of Herbert screw fixation group was better than that of other groups with statistically significant differences. Conclusion : In the treatment of osteochondritis dissecans of skekletally immature patients, arthroscopic finding was reliable guidance in decision of treatment method and active fixation was recommended in patients with large, unstable lesion.

The Effects of Positive Psychological Capital and Professor Support on Career Preparation Behavior in University Students (대학생들의 긍정심리자본과 교수지지가 진로준비행동에 미치는 영향)

  • The purpose of this study was to confirm the relationship between positive psychological capital, professor support, and the career preparation behavior of college students, and to identify the factors influencing the career preparation behavior. The convenience sampling method was used on 180 students from August 30 to October 30, 2020, Data was collected using a structured questionnaire and analyzed by frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheff test, Pearson's Correlation Coefficients, and multiple regression. The research results showed that there were significant differences in positive psychological capital and professor support according to gender, majors, and economic levels. There were significant differences in career preparation behavior according to gender (t=3.52, p<.001), major (F=10.85, p<.001), and career decision (t=4.98, p<.001). There were significant differences in career preparation behavior according to gender, majors, and career decisions. Positive psychological capital (r=.45, p<.001) and professor support (r=.40, p<.001) showed a positive correlation with career preparation behavior. Factors influencing career preparation behavior were majors, career decisions, positive psychological capital, and the explanatory power of these factors was 32.5%. Therefore, effective career and counseling guidance should be planned to increase positive psychological capital according to the major. It is necessary to provide opportunities for exploration of various career options to enable students to select suitable careers or jobs and to set goals, and provide an environment with positive motivation.

  • Business environment is always full of challenges. Despite various strategic efforts, there are so many failure cases of misfit. With the weaker resource base and institutional foundation, startup firms find it more difficult to find the right spot in the stiff competition. In the middle of evolutionary process, the startup firms need proper strategies meeting the differential challenges along the multiple stages of growth. Following the idea of product life cycle, this study applies the four stages of growth-startup, initial growth, accelerated growth, matured, and decliing. The next step for the startup manager is meeting each stage of growth with proper strategic efforts, including strategy, structure, decision-making pattern, and control method. When the knowledge factor is introduced, there is a potential for higher performance. Based on the 'Detailed Survey on Startup Ventures in 2017,' this study explores the impact of the government subsidy program on the firm competitiveness and performance-along the four stages of growth. In each stage, the strategy factors showed differential impact.

Korean physicians' attitudes toward the prenatal screening for fetal aneuploidy and implementation of non-invasive prenatal testing with cell-free fetal DNA

  • Purpose: Physicians' attitudes may have a strong influence on women's decision regarding prenatal screening options. The aim of this study is to assess the physicians' attitudes toward prenatal screening for fetal aneuploidy including non-invasive prenatal testing (NIPT) in South Korea. Materials and Methods: Questionnaires were distributed and collected at several obstetrics-gynecological conferences and meetings. The questionnaire included 31 multiple choice and 5 fill-in-the-blank questions. Seven questions requested physicians' demographic information, 17 questions requested information about the NIPT with cell-free fetal DNA, and 12 questions requested information about general prenatal screening practices. Results: Of the 203 obstetricians that completed the survey. In contrast with professional guidelines recommending the universal offering of aneuploidy screening, only 53.7% answered that prenatal aneuploidy testing (screening and/or invasive diagnostic testing) should be offered to all pregnant women. Physicians tended to have positive attitudes toward the clinical application of NIPT as both primary and secondary screening methods for patients at high-risk for fetal trisomy. However, for patients at average-risk for fetal trisomy, physicians tended to have positive attitudes only as a secondary screening method. Physicians with more knowledge about NIPT were found to tend to inform their patients that the detection rate of NIPT is higher. Conclusion: This is the first study to investigate expert opinion on prenatal screening in South Korea. Education of physicians is essential to ensure responsible patient counseling, informed consent, and appropriate management after NIPT.

Predictive Analysis of Fire Risk Factors in Gyeonggi-do Using Machine Learning (머신러닝을 이용한 경기도 화재위험요인 예측분석)

  • The seriousness of fire is rising because fire causes enormous damage to property and human life. Therefore, this study aims to predict various risk factors affecting fire by fire type. The predictive analysis of fire factors was carried out targeting Gyeonggi-do, which has the highest number of fires in the country. For the analysis, using machine learning methods SVM (Support Vector Machine), RF (Random Forest), GBRT (Gradient Boosted Regression Tree) the accuracy of each model was presented with a high fit model through MAE (Mean Absolute Error) and RMSE (Root Mean Squared Error), and based on this, predictive analysis of fire factors in Gyeonggi-do was conducted. In addition, using machine learning methods such as SVM (Support Vector Machine), RF (Random Forest), and GBRT (Gradient Boosted Regression Tree), the accuracy of each model was presented with a high-fit model through MAE and RMSE. Predictive analysis of occurrence factors was achieved. Based on this, as a result of comparative analysis of three machine learning methods, the RF method showed a MAE = 1.765 and RMSE = 1.876, as well as the MAE and RMSE verification and test data were very similar with a difference between MAE = 0.046 and RMSE = 0.04 showing the best predictive results. The results of this study are expected to be used as useful data for fire safety management allowing decision makers to identify the sequence of dangers related to the factors affecting the occurrence of fire.

Trends in the Use of Artificial Intelligence in Medical Image Analysis (의료영상 분석에서 인공지능 이용 동향)

  • In this paper, the artificial intelligence (AI) technology used in the medical image analysis field was analyzed through a literature review. Literature searches were conducted on PubMed, ResearchGate, Google and Cochrane Review using the key word. Through literature search, 114 abstracts were searched, and 98 abstracts were reviewed, excluding 16 duplicates. In the reviewed literature, AI is applied in classification, localization, disease detection, disease segmentation, and fit degree of registration images. In machine learning (ML), prior feature extraction and inputting the extracted feature values into the neural network have disappeared. Instead, it appears that the neural network is changing to a deep learning (DL) method with multiple hidden layers. The reason is thought to be that feature extraction is processed in the DL process due to the increase in the amount of memory of the computer, the improvement of the calculation speed, and the construction of big data. In order to apply the analysis of medical images using AI to medical care, the role of physicians is important. Physicians must be able to interpret and analyze the predictions of AI algorithms. Additional medical education and professional development for existing physicians is needed to understand AI. Also, it seems that a revised curriculum for learners in medical school is needed.

The Phenomenological Study on School Dropout of Specialized Vocational High School Students (특성화고등학교 학생의 학업중단에 대한 현상학적 연구)

  • The purpose of this study was to analysis school dropout of specialized vocational high school students using phenomenological research method. The interview for the research was carried out with 10 adolescents who dropped out specialized vocational high school from December 8 to 23, 2018. According to the result of the research, 31 themes were extracted from interviews with 10 research participants. And 10 theme clusters were categorized from these. And these clusters were divided into 3 domains : 'before school dropout', 'causes & process of school dropout, feeling about school dropout', 'after school dropout' Based on the finding of the study, major conclusions of this study were as follow: First, adolescents who dropped out specialized vocational high school suffered from hard school life, disappointing lead from teachers, stereotypical lesson. And they committed misdeeds, and had psychological difficulties. Some of them kept up the good relationship with their friends, teachers, parents, some of them did not. Some of them kept up the good relationship with their friends, teachers and parents, while some of them did not. Second, they chose dropout due to various different causes. The procedures of school dropout proceeded with comparative ease. The effect of dropout prevention program is very limited. The feeling they felt at the time of school dropout varied individually. Some adolescents who dropped out specialized vocational high school were satisfied while others were stressed out, regretting their decision. Third, they lived diligently working part-time jobs or preparing General Equivalency Diploma (GED) test after dropout. They experienced positive changes in their daily lives after dropout. But sometimes they experienced various difficulties and negative changes. Most of them had their goals, and they were preparing for them. Their expectation was low that their life will succeed if they returned to school. They wanted people to understand their decision about dropout. And multiple institutions are supporting adolescents who dropped out specialized vocational high school. They need practical support : Various information, activity for career experience, counseling etc.
