Neuroprotective Effects of Multi-vitamin Therapy in Transgenic Mouse Model of Amyotrophic Lateral Sclerosis (근위축성측삭경화증의 유전자 이식 마우스 모델에서 비타민 복합요법의 신경보호효과)

  • Background: There is no currently effective treatment for amyotrophic lateral sclerosis (ALS), although this disorder is a progressive neurodegenerative disease resulting in death within several years. Because recent evidence suggests that homocysteine (HC) is highly related to neurodegenerative disorders with aging, we tried to elucidate the effects of multi-vitamin therapy on G93A SOD1 transgenic mice. Methods: We treated this murine model of ALS with multi-vitamin (folic acid 1.97 mg/day, pyridoxine 0.98 mg/day, cyanocobalamin 0.1 mg/day) from 45 days of age, per oral. We performed the rotarod test from postnatal $10^{th}$ week, weekly. Results: We found that multi-vitamin reinforcement significantly prolonged average lifespan and delayed disease onset with improvement of motor performance. However, it did not significantly slow disease progression and statistical differences of weight loss were not observed between in transgenic mice and controls. Conclusions: These results suggest that multi-vitamin can be a potent therapeutic strategy for familial forms of ALS.

Examination of Dose Change at the Junction at the Time of Treatment Using Multi-Isocenter Volumetric Modulated Arc Therapy (용적조절호형방사선치료(VMAT)의 다중치료중심(Multi- Isocenter)을 이용한 치료 시, 접합부(Junction)의 선량 변화에 대한 고찰)

  • This study examined dose change depending on the reposition error of the junction at the time of treatment with multi-isocenter volumetric modulated arc therapy. This study selected a random treatment region in the Arccheck Phantom and established the treatment plan for multi-isocenter volumetric modulated arc therapy. Then, after setting the error of the junction at 0 ~ 4 mm in the X (left), Y (upper), and Z (inner and outer) directions, the area was irradiated using a linear accelerator; the point doses and gamma indexes obtained through the Phantom were subsequently analyzed. It was found that when errors of 2 and 4 mm took place in the X and Y directions, the gamma pass rates (point doses) were 99.3% (2.085) and 98% (2.079 Gy) in the former direction and 98.5% (2.088) and 95.5% (2.093 Gy) in the latter direction, respectively. In addition, when errors of 1, 2, and 4 mm occurred in the inner and outer parts of the Z direction, the gamma pass rates (point doses) were found to be 94.8% (2.131), 82.6% (2.164), and 72.8% (2.22 Gy) in the former part and 93.4% (2.069), 90.6% (2.047), and 79.7% (1.962 Gy) in the latter part, respectively. In the X and Y directions, errors up to 4 mm were tolerable; however, in the Z direction, error values exceeding 1 mm were beyond the tolerance level. This suggests that for high and low dose areas, errors in the direction same as the progress direction in the treatment region have a more sensitive dose distribution. If the guidelines for set-up errors are established at the institutional level through continuous research in the future, it will be possible to provide good quality treatment using junctions.

Development of Dual-mode Signal Processing Module for Multi-slit Prompt-gamma Camera (다중 슬릿 즉발감마선 카메라를 위한 이중모드 신호처리 모듈 개발)

  • In proton therapy, in vivo proton beam range verification is very important to deliver conformal dose to the target volume and minimize unnecessary dose to normal tissue. For this purpose, a multi-slit prompt-gamma camera module made of 24 scintillation detectors and 24-channel signal processing system is under development. In the present study, we have developed and tested a dual-mode signal processing system, which can operate in the energy calibration mode and the fast data acquisition mode, to process the signals from the 24 scintillation detectors. As a result of performance test, using the energy calibration mode, we were able to perform energy calibration for the 24 scintillation detectors at the same time and determine the discrimination levels for the detector channels. Further, using the fast data acquisition mode, we were able to measure a prompt-gamma distribution induced by a 45 MeV proton beam. The measured prompt gamma distribution was found similar to the proton dose distribution at the distal fall-off region, and the estimated beam range was $17.13{\pm}0.76mm$, which is close to the proton beam range of 16.15 mm measured by an EBT film.

The Effect of Nonpharmacologic Interventions on Behavioral and Psychological Symptoms of Dementia : A Meta-Analysis (치매환자의 행동심리증상에 비약물적 중재가 미치는 효과 - 메타분석)

  • To investigate a variety of nonpharmacologic interventions have confirmed what the symptoms and effects, mainly interventions by acting on behavioral and psychological symptoms of dementia was conducted a meta-analysis. Data were collected from online search engines using search words from domestic and foreign article database. The researcher independently and among the pre-post experimental studies published from January 2000 to June 2016, recalled applying for the elderly with dementia presents the effect of treatment group and the control group RCT in this study were included in the study. The results of this meta-analysis showed that, effect size of the nonpharmacologic interventions of total -0.33, occupational therapy - 0.26, multi-sensory stimulation intervention -0.65 was significant beneficial effects to elderly with dementia(p<.05). According to interventions as a major affected the symptoms associated with behavioral problems, mainly aggression, memory-related problem behavior in the home-based program. Also, reminiscence therapy and occupational therapy is generally apathy, multi-sensory stimulation and music therapy was confirmed that there was a major change in behavior anxiety or agitation. The results of this study confirmed that various nonpharmacologic interventions were effective on behavioral psychological symptoms of dementia patients and confirmed the main symptoms of intervention.

Surfactant Replacement Therapy for RDS: a Collaborative Study of 72 Multi-center Trials in Korea (2010) and a Review of Korean Experiences over 20 Years (2010년 전국 72개 병원에서 신생아 호흡곤란증후군의 폐 표면활성제 치료 결과: 지난 20년간의 비교)

  • In Korea, pulmonary surfactant (PS) replacement therapy in respiratory distress syndrome (RDS) was started in 1991 since when Surfacten$^{(R)}$ was imported from Japan. At the present time, Surfacten$^{(R)}$, Newfactan$^{(R)}$, Curosurf$^{(R)}$, and Infasurf$^{(R)}$ are available in Korea. The governmental health insurance covers the expense for multiple dose treatment since 2002 and the early prophylactic treatment (BW: <1,250 g or GP: <30 wks) since 2011. We undertook a multi-institutional collective study to evaluate the outcomes of PS over past 20 years in Korea (Period-I; 1990/91, P-II; 1996, P-III; 2002, and P-IV; 2007, P-V; 2010). There were 60 RDS neonates with PS treatment in P-I (16 hospitals), 1,179 in P-II (64), 1,595 in P-III (62), 1,921 in P-IV (57), and 3,160 in P-V (72). Decreased mortality rate, defined as the percentage of neonates who died within 28 days of birth, was seen between periods, P-V vs P-I, II, III, and IV (mortality rate: 10.1% vs. 40.0%, 30.0%, 18.7%, and 14.3%). We conclude that PS therapy contributed to improve remarkable outcome in RDS neonates over the last 20 years in Korea. However, more efforts should be made to optimize PS therapy for better outcome. Multiple PS doses for relapse and poor response, early prophylactic use, and better supportive care for pre-term infants are mandatory.

Investigating Functional Level in Patients with Stroke using ICF Concept (ICF core-set를 이용한 뇌졸중 환자의 기능수행 분석)

  • Purpose: The purpose of this study was to investigate level of functioning in patients with stroke using Modified Bathel Index (MBI), World Health Organization Disability Assessment Schedule 2.0 (WHODAS 2.0), and ICF core-set for stroke. Methods: Sixty-four patients with stroke were recruited for this study from nine medical institutes. The ICF core-set for stroke, WHODAS 2.0, and MBI were used to collect subjects' functional levels. ICF core-set was employed here as a standard frame to observe multi-dimension of functioning, that is physiological bodily function, activity and participation (AP) in daily life, and current environmental factors (EF) in patients with stroke. WHODAS 2.0 and MBI were also used in order to have a specific functioning level for subjects. The linkage of each item in WHODAS 2.0 and MBI into the ICF core-set for stroke was examined. Pearson correlation coefficient was used for analysis of their relationships. Results: Functioning level of participants showed moderate resulting from MBI and WHODAS 2.0 ($73.48{\pm}22.27$ and $35.55{\pm}12.53$, respectively). Strong relationship was observed between ICF core-set and WHODAS 2.0, and with MBI. Each item of disability scales was obtained its linkage into ICF in the domain of AP. However, lack of correlation between MBI and ICF in the domain of EF was found due to absence of related factors. Conclusion: MBI was found to be linked mainly into ICF in the domain of AP and to have limited linkage into EF. Therefore, it should be suggested that the ICF concept frame should be used as a multi-dimensional approach to patients with stroke.

Virtual Reality Program Model for Trauma Intervention in Multi-cultural Adolescents (다문화청소년의 트라우마 중재를 위한 가상현실 프로그램 모형)

  • As the technology of virtual reality has recently developed, virtual reality treatment programs for game addiction and alcoholism are being developed. The purpose of this study is to present a model of developmental process of virtual reality based cognitive behavior therapy program for trauma intervention in multicultural adolescents. The development of virtual reality program of cognitive behavior therapy consists of three stages. In the first step, In the first stage, the trauma characteristics including the main emotions and the expression problem due to the trauma experienced by the multicultural youth should be grasped. In the second stage, the contents of the development process and scope of the scenario for the cognitive behavior therapy for trauma should be presented. In the final stage 3, the study will examine the feasibility of implementing the scenario contents in the virtual reality program. Through this process, the intervention program development model that can lead the positive change of the multicultural youth was presented.

Effects of Non-Saponin Red Ginseng Components on Multi-drug Resistance

  • Multi-drug resistance (MDR) is a major problem in cancer chemotherapy and has often ended up with termination of the therapy. The aim of this study was to identify any fractions of Korean red ginseng that would be effective in modulating for MDR. Although ginsenosides have been reviewed as possible MDR modulators, the MDR modulation activity of the other component is unknown. Therefore, a red ginseng was extracted with methanol, ether, ethylacetate, and n-butanol, followed by several fractionations by silica gel chromatography. And the activity of MDR modulating for these fractions was examined via sulforrhodamine B assay. We have found that several ether fractions, as nonsaponin components are effective on MDR modulation. We have expect that these results helpful to improvement of cancer chemotherapy.

Innovation Status of Gene Therapy for Breast Cancer

  • Background: To analyze multi-source data including publications and patents, and try to draw the whole landscape of the research and development community in the field of gene therapy for breast cancer. Materials and Methods: Publications and patents were collected from the Web of science and databases of the five major patent offices of the world, respectively. Bibliometric methodologies and technology are used to investigate publications/patents, their contents and relationships. Results: A total of 2,043 items published and 947 patents from 1994 to 2013 including "gene therapy for breast cancer" were retrieved. The top five countries in global publication share were USA, China, Germany, Japan and England. On the other hand, USA, Australia, England, South Korea and Japan were the main producers of patents. The universities and enterprises of USA had the highest amount of publication and patents. Adenovirus- and retrovirus-based gene therapies and small interfering RNA (siRNA) interference therapies were the main topics both in publications and patents. Conclusions: The above results show that global research in the field of gene therapy for breast cancer is increasing and the main participants in this field are USA and Canada in North America, China, Japan and South Korea in Asia, and England, Germany, and Italy in Europe. Also, this article demonstrates the usefulness of bibliometrics to address key evaluation questions and define future areas of research.
