MBT-01108 물질에 의한 일반 소독제 및 항생물질 내성균주의 생육억제 (Growth Inhibitions of Strains Exhibiting Resistances against General Disinfectants and Antibiotics by MBT-01108 Material.)

  • 시중에서 사용되고 있는 일반 소독제를 대상으로 하여 병원 미생물의 내성 빈도를 검토한 결과는 약 30%였다. 그 중에서 푸드세프 (산화제 중 할로겐 계 소독약)와 동인테고 (양성 계면 활성제) 및 Iodo 175 (양성 계면 활성제)는 병원 미생물에 대해 낮은 감수성을 나타냈었을 뿐 아니라, 시험에 사용된 다른 소독제에 비하여 높은 내성 빈도를 나타내었다. 소독제 내성 균주를 사용해서 항생제 내성을 MIC test와 paper disc 방법으로 확인한 결과 30%의 내성 빈도를 나타내었다. 다른 항생제에 비해 aminoglycoside 계의 항생제인 gentamycin, kanamycin, streptomycin에서 높은 내성 빈도를 나타냄을 확인할 수 있었다. 본 연구실에서 제주도 손바닥 선인장으로부터 추출${\cdot}$분획한 MBT-01108 물질을 소독제와 항생제에 내성을 획득한 내성균에 상용했을 경우 내성이 생기지 않을 뿐 아니라, 내성균이 자라지 못하였다. 또한 현재 문제시되고 있는 다제 내성균인 MRSA, R-Pseudomonas aeruginosa, VRE, E. coli O157에 사용했을 경우에도 마찬가지의 결과를 얻을 수 있었다. 이상에서 살펴 본 바와 같이 손바닥 선인장에서 추출 ${\cdot}$ 분획한 MBT-01108 물질은 기존의 항생제와는 다른 기작으로 작용을 할 것으로 생각되고, 신규 항균 활성 물질의 약제 개발에 그 유용성이 매우 클 것으로 기대되며 여러 가지 항균성 생활 제품의 개발에도 유용할 것으로 기대된다.

축산폐기물에서 분리된 항생제 내성균 Escherichia coli....Brevibacterium sp.의 내성 특성 및 감마선 살균 효능 (Antibiotic Resistance of Escherichia Coli and Brevibacterium sp. Isolated from Livestock Waste and Disinfection Efficiency of Gamma-Ray Irradiation)

  • 축산 폐기물에서 항생제에 대해 내성을 가지는 균주를 분리하여, 다양한 항생제에 대한 내성특성을 확인하고, 감마선의 조사에 따른 내성균의 항생제 내성 특성변화와 살균효능에 관한 연구를 수행하였다. 그 결과, 돈분과 돈분퇴비에서 분리된 균주 중 Esherichia coli와 Brevibacterium sp.이 다제내성균으로 확인되었다. E. coli는 13종의 항생제 중 9종의 항생제에 대해 내성을 나타내었으며, Brevibacterium sp.는 7종의 항생제에 대한 내성을 나타내었다. 이 두 가지 대표 다제내성균에 대해 감마선을 이용한 살균실험을 시행한 결과, Esherichia coli는 항생제에 대한 내성이 돌연변이적으로 발생하기 쉬운 균주이나 감마선을 이용해 효율적으로 제어할 수 있는 것으로 나타났고, 그에 반해 Brevibacterium sp.는 저선량에서 감마선에 대한 내성을 나타내어 상대적으로 감마선을 이용한 제어가 용이하지 않았다. Brevibacterium sp.는 2.0 kGy의 조사량에 대해 Esherichia coli에 비해 약 100배 정도 낮은 살균 효율을 보임으로써, 감마선에 대한 내성이 강한 것으로 나타났다.

Species Transferability of Klebsiella pneumoniae Carbapenemase-2 Isolated from a High-Risk Clone of Escherichia coli ST410

  • Sequence type 410 (ST410) of Escherichia coli is an extraintestinal pathogen associated with multi drug resistance. In this study, we aimed to investigate the horizontal propagation pathway of a high-risk clone of E. coli ST410 that produces Klebsiella pneumoniae carbapenemase (KPC). blaKPC-encoding E. coli and K. pneumoniae isolates were evaluated, and complete sequencing and comparative analysis of blaKPC-encoding plasmids from E. coli and K. pneumoniae, antimicrobial susceptibility tests, polymerase chain reaction, multilocus sequence typing, and conjugal transfer of plasmids were performed. Whole-genome sequencing was performed for plasmids mediating KPC-2 production in E. coli and K. pneumoniae clinical isolates. Strains E. coli CPEc171209 and K. pneumoniae CPKp171210 were identified as ST410 and ST307, respectively. CPEc171209 harbored five plasmids belonging to serotype O8:H21, which is in the antimicrobial-resistant clade C4/H24. The CPKp171210 isolate harbored three plasmids. Both strains harbored various additional antimicrobial resistance genes. The IncX3 plasmid pECBHS_9_5 harbored blaKPC-2 within a truncated Tn4401a transposon, which also contains blaSHV-182 with duplicated conjugative elements. This plasmid displayed 100% identity with the IncX3 plasmid pKPBHS_10_3 from the K. pneumoniae CPKp171210 ST307 strain. The genes responsible for the conjugal transfer of the IncX3 plasmid included tra/trb clusters and pil genes coding the type IV pilus. ST410 can be transmitted between patients, posing an elevated risk in clinical settings. The emergence of a KPC-producing E. coli strain (ST410) is concerning because the blaKPC-2-bearing plasmids may carry treatment resistance across species barriers. Transgenic translocation occurs among carbapenem-resistant bacteria, which may spread rapidly via horizontal migration.

2007년 서울 지역에서 판매되고 있는 반찬류의 세균 오염과 항생제 내성실태 (Assessment of Bacterial Contamination and Antibiotic Resistance Patterns in Ready-To-Eat Side Dishes in Seoul Area, 2007)

  • The purpose of this research was to investigate the microbiological quality and antimicrobial susceptibilities of the ready-to-eat side dishes, which are were collected from the grocery stores, big markets, and department stores in Seoul throughout November, 2007. Of total 124 samples, presence of staphylococci and enteric bacilli was observed in 38 samples (31%) and 53 samples (43%), respectively. And 30 samples (24%) were shown to be of unsatisfactory quality for total bacterial count (>$10^5$). Antimicrobial susceptibilities of the staphylococci isolated from the side dishes were tested for six different antimicrobial agents, which are in widespread clinical use in Korea, as well as four new antimicrobials, daptomycin, linezolid, quinupristin/dalfopristin and tigecycline. As a result, the staphylococcal isolates were found to be resistant to oxacillin (${MIC}_{90}$, >128 ${\mu}g$/ml), teicoplanin (${MIC}_{90}$, >128 ${\mu}g$/ml), mupirocin (${MIC}_{90}$, >128 ${\mu}g$/ml), linezolid (${MIC}_{90}$, 128 ${\mu}g$/ ml) and quinupristin/dalfopristin (${MIC}_{90}$, 32 ${\mu}g$/ml). Especially, some of the staphylococcal isolates exhibited high level and multi-drug resistance. Moreover, these bacteria were also resistant to new antimicrobials, except tigecycline.

Colistin resistance and plasmid-mediated mcr genes in Escherichia coli and Salmonella isolated from pigs, pig carcass and pork in Thailand, Lao PDR and Cambodia border provinces

  • Background: Colistin and carbapenem-resistant bacteria have emerged and become a serious public health concern, but their epidemiological data is still limited. Objectives: This study examined colistin and carbapenem resistance in Escherichia coli and Salmonella from pigs, pig carcasses, and pork in Thailand, Lao PDR, and Cambodia border provinces. Methods: The phenotypic and genotypic resistance to colistin and meropenem was determined in E. coli and Salmonella obtained from pigs, pig carcasses, and pork (n = 1,619). A conjugative experiment was performed in all isolates carrying the mcr gene (s) (n = 68). The plasmid replicon type was determined in the isolates carrying a conjugative plasmid with mcr by PCR-based replicon typing (n = 7). The genetic relatedness of mcr-positive Salmonella (n = 11) was investigated by multi-locus sequence typing. Results: Colistin resistance was more common in E. coli (8%) than Salmonella (1%). The highest resistance rate was found in E. coli (17.8%) and Salmonella (1.7%) from Cambodia. Colistin-resistance genes, mcr-1, mcr-3, and mcr-5, were identified, of which mcr-1 and mcr-3 were predominant in E. coli (5.8%) and Salmonella (1.7%), respectively. The mcr-5 gene was observed in E. coli from pork in Cambodia. Two colistin-susceptible pig isolates from Thailand carried both mcr-1 and mcr-3. Seven E. coli and Salmonella isolates contained mcr-1 or mcr-3 associated with the IncF and IncI plasmids. The mcr-positive Salmonella from Thailand and Cambodia were categorized into two clusters with 94%-97% similarity. None of these clusters was meropenem resistant. Conclusions: Colistin-resistant E. coli and Salmonella were distributed in pigs, pig carcasses, and pork in the border areas. Undivided-One Health collaboration is needed to address the issue.

Antibacterial Mode of Action of Cinnamomum verum Bark Essential Oil, Alone and in Combination with Piperacillin, Against a Multi-Drug-Resistant Escherichia coli Strain

  • This study aims to investigate the mechanism of action of the cinnamon bark essential oil (CB), when used singly and also in combination with piperacillin, for its antimicrobial and synergistic activity against beta-lactamase TEM-1 plasmid-conferred Escherichia coli J53 R1. Viable count of bacteria for this combination of essential oil and antibiotic showed a complete killing profile at 20 h and further confirmed its synergistic effect by reducing the bacteria cell numbers. Analysis on the stability of treated cultures for cell membrane permeability by CB when tested against sodium dodecyl sulfate revealed that the bacterial cell membrane was disrupted by the essential oil. Scanning electron microscopy observation and bacterial surface charge measurement also revealed that CB causes irreversible membrane damage and reduces the bacterial surface charge. In addition, bioluminescence expression of Escherichia coli [pSB1075] and E. coli [pSB401] by CB showed reduction, indicating the possibility of the presence of quorum sensing (QS) inhibitors. Gas-chromatography and mass spectrometry of the essential oil of Cinnamomum verum showed that trans-cinnamaldehyde (72.81%), benzyl alcohol (12.5%), and eugenol (6.57%) were the major components in the essential oil. From this study, CB has the potential to reverse E. coli J53 R1 resistance to piperacillin through two pathways; modification in the permeability of the outer membrane or bacterial QS inhibition.

  • Food safety is a term broadly applied to food quality that may adversely affect human health. These include zoonotic diseases and acute and chronic effects of ingesting natural and human-made xenobiotics. There are two major areas of concern over the presence of residues of antibiotics in animal-derived foodstuffs with regard to human health. The first is allergic reactions. Some antibiotics, such as penicillins can evoke allergic reactions even though small amounts of them are ingested or exposed by parenteral routes. The second is development of antibiotic resistance in gut bacteria of human. Recently multi-resistant pneumococcal, glycopeptide-resistant enterococci and gram negative bacteria with extended-spectrum $\beta$-lactamases have spread all over the world, and are now a serious therapeutic problem in human. Although it is evident that drugs are required in the efficient production of meat, milk and eggs, their indiscriminate use should never be substituted for hygienic management of farm. Drug should be used only when they are required. In addition to veterinary drugs, environmental contaminants that were contaminated in feed, water and air can make residues in animal products. Mycotoxins, heavy metals, pesticides, herbicides and other chemicals derived from industries can be harmful both to animal and human health. Most of organic contaminants, such as dioxin, PCBs and DDT, and metals are persistent in environment and biological organisms and can be accumulated in fat and hard tissues. Some of them are suspected to have endocrine disrupting, carcinogenic, teratogenic, immunodepressive and nervous effects. The governmental agencies concerned make efforts to prevent residue problems; approval of drugs including withdrawal times of each preparation of drugs, establishment of tolerances, guidelines regarding drug use and sanitation enforcement of livestock products. National residue program is conducted to audit the status of the chemical residues in foods. Recently HACCP has been introduced to promote food safety from farm to table by reducing hazardous biological, chemical and physical factors. Animal Production Food Safety Program, Quality Assurance Programs, Food Animal Residue Avoidance Databank are para- or non-governmental activities ensuring food safety. This topic will cover classification and usage or sources of chemical residues, their adverse effects, and chemical residue status of some countries. Issues are expanded to residue detection methodologies, toxicological and pharmacokinetic backgrounds of MRL and withdrawal time establishments, and the importance of non-governmental activities with regard to reducing chemical residues in food.

35종 해조류 추출물의 병원성 세균 및 Candida sp. 진균에 대한 항균 활성 평가 (Evaluation of the Antimicrobial Activities of 35 Seaweed Extracts against Pathogenic Bacteria and Candida sp.)

  • 해양생물 유래의 기능성 물질 개발 연구의 일환으로, 35종 해조류로부터 메탄올 추출물을 조제하여 항생제 내성균주를 포함한 다양한 세균 및 진균에 대한 항균력을 평가하였다. 그 결과, 패, 넓패, 알쏭이 모자반 및 야마다 모자반에서 광범위한 세균에 대해 우수한 항세균 활성을 확인하였으며, 상기 4종과 함께 곰피, 대황, 감태 및 톳의 8종에서 Candida albicans 생육억제력을 확인하였다. 이중 패, 넓패, 야마다모자반 추출물은 500 ${\mu}g/mL$ 농도에서 55-93%의 인간적혈구 용혈활성을 나타내었다. 상기 8종 해조류의 메탄올 추출물을 n-hexane, ethylacetate, butanol을 이용하여 순차적 용매 분획물과 물 잔류물을 조제하여 다양한 Candida sp.에 대한 항균력을 평가한 결과, 패 및 넓패로부터 C. albicans 및 항생제 내성 Candida sp.에 대한 신규의 항균제 개발 가능성을 확인하였다. 본 연구 결과는 해조류 특히 패, 넓패, 알송이모자반, 야마다모자반이 다양한 세균 및 항생제 내성진균의 제어를 위한 생물자원으로 개발 가능함을 제시하고 있다.

Synthesis of New VO(II), Co(II), Ni(II) and Cu(II) Complexes with Isatin-3-Chloro-4-Floroaniline and 2-Pyridinecarboxylidene-4-Aminoantipyrine and their Antimicrobial Studies

  • The complexes of tailor made ligands with life essential metal ions may be an emerging area to answer the problems of multi drug resistance. The coordination complexes of VO(II), Co(II), Ni(II) and Cu(II) with the Schiff bases derived from isatin with 3-chloro-4-floroaniline and 2-pyridinecarboxaldehyde with 4-aminoantipyrine have been synthesized by conventional as well as microwave methods. These compounds have been characterized by elemental analysis, molar conductance, electronic spectra, FT-IR, FAB mass and magnetic susceptibility measurements. FAB mass data show degradation of complexes. Both the ligands behave as bidentate and tridentate coordinating through O and N donor. The complexes exhibit coordination number 4, 5 or 6. The Schiff base and metal complexes show a good activity against the bacteria; $Staphylococcus$ $aureus$, $Escherichia$ $coli$ and $Streptococcus$ $fecalis$ and fungi $Aspergillus$ $niger$, $Trichoderma$ $polysporum$, $Candida$ $albicans$ and $Aspergillus$ $flavus$. The antimicrobial results also indicate that the metal complexes are better antimicrobial agents as compared to the Schiff bases. The minimum inhibitory concentrations of the metal complexes were found in the range 10-40 ${\mu}g/mL$.

다약제 내성 Acinetobacter baumannii 에 의한 폐렴에서 Colistin 분무치료의 효과 (Effects of Aerosol Colistin Treatment of Pneumonia Caused by Multi-drug Resistant Acinetobacter baumannii)

  • 연구배경: Acinetobacter baumannii는 항생제에 대한 내성균이 자주 발생하며 MDR Acinetobacter baumannii 감염의 치료 시 항생제 선택에 어려움이 있다. Colistin은 그람 음성균에 효과적이며 MDR Acinetobacter균에 감수성을 보이나 전신투여 시 신독성, 신경독성 등의 부작용으로 인해 잘 쓰이지 않는 약제이다. 전신투여에 의한 부작용을 회피하는 방법으로 분무치료가 있다. 그러나 MDR Acinetobacter baumannii 페렴환자에서 colistin 분무치료의 효과와 부작용에 대한 국내연구는 부족한 실정이어서 저자들은 다음과 같은 연구를 시행하였다. 방법: 2006년 12월부터 2007년 8월까지 중환자실의 폐렴환자 중에서 호흡기검체 배양검사를 통해 imipenem에 내성을 포함한 MDR Acinetobacter baumannii균에 의한 폐렴이 진단된 환자를 대상으로 하였다. 이들 중 항생제 정맥주사와 더불어 하루 3번의 colistin 분무치료를 시행했던 환자들의 의무기록을 후향적으로 분석하여 임상 소견, 신기능, 인공호흡기의 최대기도압의 변화를 분석 하였다 결과: 총 31명의 환자에서 colistin 분무치료를 시행하였다. Colistin 사용기간은 $14{\pm}7$일이었으며, 하루 흡입용량은 225~300 mg이었다. Colistin 분무치료 후 25명/31명(80.6%)에서 Acinetobacter균이 제거되었으나 이 중 11명(44%)에서는 2차 감염에 의한 새로운 균이 배양되었다. Colistin 분무치료 전, 후로 백혈구 수는 감소하였다. Colistin 분무치료 후 신기능의 장애는 발생하지 않았으며 의미 있는 기관지경련의 소견도 관찰되지 않았다. 결론: 적절한 항생제를 선택하기 어려운 MDR Acinetobacter baumanii에 의한 폐렴을 치료하는데 colistin 분무치료는 비교적 높은 균 제거율과 낮은 부작용을 보이므로 고려해 볼 수 있는 치료방법으로 사료된다.
