Tonsillectomy as prevention and treatment of sleep-disordered breathing: a report of 23 cases

  • Background: The paradigm of tonsillectomy has shifted from a treatment of recurrent throat infection to one of multi-discipline management modalities of sleep-disordered breathing (SDB). While tonsillectomy as a treatment for throat problems has been performed almost exclusively by otorhinolaryngologists, tonsillectomy as a part of the armamentarium for the multifactorial, multidisciplinary therapy of sleep-disordered breathing needs a new introduction to those involved in treating SDB patients. This study has its purpose in sharing a series of tonsillectomies performed at the Seoul National University Dental Hospital for the treatment and prevention of SDB in adult patients. Methods: Total of 78 patients underwent tonsillectomy at the Seoul National University Dental Hospital from 1996 to 2015, and 23 of them who were operated by a single surgeon (Prof. Jin-Young Choi) were included in the study. Through retrospective chart review, the purpose of tonsillectomy, concomitant procedures, grade of tonsillar hypertrophy, surgical outcome, and complications were evaluated. Results: Twenty-one patients diagnosed with SDB received multiple surgical procedures (uvulopalatal flap, uvulopalatopharyngoplasty, genioglossus advancement genioplasty, tongue base reduction, etc.) along with tonsillectomy. Two patients received mandibular setback orthognathic surgery with concomitant tonsillectomy in anticipation of postoperative airway compromise. All patients showed improvement in symptoms such as snoring and apneic events during sleep. Conclusions: When only throat infections were considered, tonsillectomy was a procedure rather unfamiliar to oral and maxillofacial surgeons. With a shift of primary indication from recurrent throat infections to SDB and emerging technological and procedural breakthroughs, simpler and safer tonsillectomy has become a major tool in the multidisciplinary treatment modality for SDB.

Investigation of Long-term Survivors with Stage IV Gastric Cancer (제4기 위암 환자 중 장기 생존 예의 검토)

  • Purpose: The prognosis of stage IV gastric cancer is very grave. However, some of these patients survive long periods after surgery. This study was undertaken to investigate various clinico-pathological profiles related to the prognosis for these long-term survivors. Materials and Methods: One hundred fifty-five patients with stage IV gastric cancer who underwent a gastric resection from 1992 to 1997 at Hanyang University Hospital were evaluated. Thirty-three patients who survived more than 5 years after surgery were designated as long-term survivors (LTS); on the other hand, one hundred twenty-two patients who died within 5 years after surgery were named as short-term survivors (STS). Results: The rate of the patients with T4, preoperative serum level of CA19-9 greater than 37 U/g protein, and peritoneal dissemination was lower for the LTS than in for the STS (P=0.002, P=0.045, and P=0.0000, respectively). Tumors were smaller (7.3 cm vs. 8.9 cm, P=0.030) and metastatic lymph node were fewer (19.7 vs. 28.8, P=0.019) for the LTS than for the STS. Curative surgery ($\76\%\;vs.\;\46\%$, P=0.002) and a subtotal gastrectomy ($\76\%\;vs.\;46\%$, P=0.026) were performed more frequently for the LTS than for the STS. From a univariate survival analysis, depth of invasion, distant metastasis, extent of gastric resection, postoperative chemotherapy, and curability were statistically significant factors. From a multivariate survival analysis, curability, depth of invasion, and extent of gastric resection were independent prognostic factors. Conclusions: If feasible, we have to exert our efforts to achieve curative surgery although the tumor is considered to be a stage IV gastric cancer. Thereafter, multi-modality treatments including chemotherapy can be considered to improve the prognosis.

  • The purpose of this study is to inquire into the theoretical background of music therapy interventions for the improvement of mathematical concepts among young children with special needs. The researcher provides a basis of theoretical background about musical activities as an effective tool for young children to understand and promote their mathematical concepts, and the necessity of practical application in the field of mathematics education is suggested. Music, as a multi-sensory modality, has an ability to hold and maintain one's attention, and can be used as a memory aid and a powerful and effective motivator and reinforcer for young children. Therefore, musical activities can be used to facilitate mathematical concepts in the field of education for young children. Possible musical activities for promoting mathematical development are suggested, and the necessity for developing various musical activities is discussed.

  • In this study, using brain phantom for multi-modality imaging, we acquired CT, MR and PET images and performed registration of these anatomical images and nuclear medicine functional images. The algorithms and program applied for registration were Chamfer Matching and Mutual Information Maximization algorithm which have been using frequently in clinic and verified accuracy respectively. In result, both algorithms were useful methods for CT-MR, CT-PET and MR-PET. But Mutual Information Maximization was more effective algorithm for low resolution image as nuclear medicine functional image.

  • 본 연구는 2011년 8월 12일부터 8월 17일까지 일개 광역시에 대학병원에서 근무하는 안과인턴, 레지던트, 전문의, 방사선사, 안경사, 간호사를 대상으로 안과 PACS의 지능형 영상진단 및 치료지원 시스템 구축을 위한 사용자 요구도 조사를 실시하였다. 구성 대상은 의사 7명(38.9%), 의료기술직 4명(22.2%) 간호사 3명(16.7%), 기타 4명(22.2%)로 나타났다. 안과 PACS 도입 시 가장 중요한 부분으로 8(44.4%)명이 편리성이라고 하였고, 안과 PACS Viewer 중요분야는 9(50.0%)명이 편리한 기능이라고 하였다. 안과 PACS Viewer 필요분야에서 가장 필요한 분야는 14(77.8%)명이 망막분야라고 하였다. 이는 망막 검사의 대부분이 영상을 이용한 진단이 활용되어 있어 나타나 결과로 볼 수 있었다. 안과 PACS Viewer의 환경 관련 문항에서 검사 환자 리스트에서 가장 중요한 것은 생년월일, 부위 ,장비명이었으며, Annotation 및 사진 편집기능에서는 길이측정, 선 및 화살표 삽입, 글자입력으로 진료 및 치료에 직접적인 영향을 미치는 부분을 가장 중요한 환경 구성으로 응답하였다. 안과 PACS의 지능형 영상진단 및 치료에 대한 지원 시스템 구성은 기존 PACS에서 이루어져 있는 Viewer 형태의 UI 구성보다는 더 세밀하고 정확한 진단을 요하는 길이, marking, 장비의 특성, Color Palette 변화, Multi Modality Image Display를 요구하고 있다. 앞으로 방사선 분야의 기존 Viewer 형태의 PACS UI 구성보다는 각 분야에 맞는 PACS UI를 구축함으로써 더 정확하고 지능화된 영상 진단 및 치료 지원을 할 수 있어야 하겠다.

  • With the abundance of digital contents, the necessity of precise indexing technology is consistently required. To meet these requirements, the intelligent software entity needs to be the subject of information retrieval and the interoperability among intelligent entities including human must be supported. In this paper, we analyze the unifying framework for multi-modality indexing that Snoek and Worring proposed. Our work investigates the method of improving the authenticity of indexing information in contents-based automated indexing techniques. It supports the creation and control of abstracted high-level indexing information through ontological concepts of Semantic Web skills. Moreover, it attempts to present the fundamental model that allows interoperability between human and machine and between machine and machine. The memory-residence model of processing ontology is inappropriate in order to take-in an enormous amount of indexing information. The use of ontology repository and inference engine is required for consistent retrieval and reasoning of logically expressed knowledge. Our work presents an experiment for storing and retrieving the designed knowledge by using the Minerva ontology repository, which demonstrates satisfied techniques and efficient requirements. At last, the efficient indexing possibility with related research is also considered.

Measurement of the occipital alpha rhythm and temporal tau rhythm by using magnetoencephalography

  • Developing Magnetoencephalography (MEG) based on Superconducting Quantum Interference Device (SQUID) facilitates to observe the human brain functions in non-invasively and high temporal and high spatial resolution. By using this MEG, we studied alpha rhythm (8-13 Hz) that is one of the most predominant spontaneous rhythm in human brain. The 8-13 Hz rhythm is observed in several sensory region in the brain. In visual related region of occipital, we call to alpha rhythm, and auditory related region of temporal call to tau rhythm, sensorimotor related region of parietal call to mu rhythm. These rhythms are decreased in task related region and increased in task irrelevant regions. This means that these rhythms play a pivotal role of inhibition in task irrelevant region. It may be helpful to attention to the task. In several literature about the alpha-band inhibition in multi-sensory modality experiment, they observed this effect in the occipital and somatosensory region. In this study, we hypothesized that we can also observe the alpha-band inhibition in the auditory cortex, mediated by the tau rhythm. Before that, we first investigated the existence of the alpha and tau rhythm in occipital and temporal region, respectively. To see these rhythms, we applied the visual and auditory stimulation, in turns, suppressed in task relevant regions, respectively.

Multi-Modality Treatment for Intracranial Arteriovenous Malformation Associated with Arterial Aneurysm

  • Objective: Intracranial arteriovenous malformation (AVM) associated with aneurysm has been infrequently encountered and the treatment for this malady is challenging. We report here on our clinical experience with AVMs associated with arterial aneurysms that were managed by multimodality treatments, including clipping of the aneurysm, microsurgery, Gamma-knife radiosurgery (GKS) and Guglielmi detachable coil (GDC) embolization. Methods: We reviewed the treatment plans, radiological findings and clinical courses of 21 patients who were treated with GKS for AVM associated with aneurysm. Results: Twenty-seven aneurysms in 21 patients with AVMs were enrolled in this study. Hemorrhage was the most frequent presenting symptom (17 patients: 80.9%). Bleeding was caused by an AVM nidus in 11 cases, aneurysm rupture in 5 and an undetermined origin in 1. Five patients were treated for associated aneurysm with clipping followed by GKS for the AVM and 11 patients were treated with GDC embolization combined with GKS for an AVM. Although 11 associated aneurysms remained untreated after GKS, none of them ruptured and 4 aneurysms regressed during the follow up period. Two aneurysms increased in size despite the disappearance of the AVM nidus after GKS and then these aneurysms were treated with GDC embolization. Conclusion: If combined treatment using microsurgery, GKS and endovascular treatment can be adequately used for these patients, a better prognosis can be obtained. In particular, GKS and GDC embolization are considered to have significant roles to minimize neurologic injury.

  • User, contents exchanged between user and computer system, and media are axes among the vanous factors related to multimedia interface. The points of contact resulted by three mam factors bring many design problems. User factor is most important among and its cognitive constraints become mam design principle for multi-modality features of multimedia. Because media could be understood differently depending on the level of information delivery, it is necessary that various characteristics and representation mode of media are clarified. This thesis develops design evaluation process that tells multimedia interface as a entity integrated with contents, media, and representation elements and assesses its sub elements whether they are correspond with design principal.

Size distributions of atmospheric particulate matter and associated trace metals in the multi-industrial city of Ulsan, Korea

  • Particulate matter (PM) was collected using micro-orifice uniform deposit impactors from a residential (RES) site and an industrial (IND) site in Ulsan, South Korea, in September-October 2014. The PM samples were measured based on their size distributions (11 stages), ranging from $0.06{\mu}m$ to over $18.0{\mu}m$. Nine trace metals (As, Se, Cr, V, Cd, Pb, Ba, Sb, and Zn) associated with PM were analyzed. The PM samples exhibited weak bimodal distributions irrespective of sampling sites and events, and the mean concentrations of total PM (TPM) measured at the IND site ($56.7{\mu}g/m^3$) was higher than that measured at the RES site ($38.2{\mu}g/m^3$). The IND site also showed higher levels of nine trace metals, reflecting the influence of industrial activities and traffic emissions. At both sites, four trace metals (Ba, Zn, V, and Cr) contributed to over 80% of the total concentrations in TPM. The modality of individual trace metals was not strong except for Zn; however, the nine trace metals in $PM_{2.5}$ and $PM_{10}$ accounted for approximately 50% and 90% of the total concentrations in TPM, respectively. This result indicates that the size distributions of PM and trace metals are important to understand how respirable PM affects public health.
