주기성 사지운동장애와 사망률 (Periodic Limb Movement Disorder and Mortality)

  • Periodic limb movement disorder (PLMD) is a sleep-related movement disorder characterized by involuntary, rhythmic limb movements during sleep. While PLMD itself is not considered life-threatening, its association with certain underlying health conditions raises concerns about mortality risks. PLMD has been found to be associated with cardiovascular diseases such as hypertension and cardiovascular disease. The fragmented sleep caused by the repetitive limb movements and associated arousals may contribute to sympathetic activation, chronic sleep disruption, sleep deprivation, and subsequent cardiovascular problems, which can increase mortality risks. The comorbidities and health factors commonly associated with PLMD, such as obesity, diabetes, and chronic kidney disease, may also contribute to increased mortality risks. PLMD is often observed alongside other neurological disorders, including restless legs syndrome (RLS) and Parkinson's disease. The presence of PLMD in these conditions may exacerbate the underlying health issues and potentially contribute to higher mortality rates. Further research is needed to elucidate the specific mechanisms linking PLMD to mortality risks and to develop targeted interventions that address these risks.

조음장애아동과 비장애아동의 말운동통제 보상능력 비교 (Compensation Ability in Speech Motor Control in Children with and without Articulation Disorders)

  • This study attempted to reveal the physiologic etiology or related factors associated with speech processing by comparing the compensation ability in speech motor control in children with and without articulation disorders. Subjects were 35 children with articulation disorder and 35 children without articulation disorder whose age ranged from 5 to 6 years. They were asked to rapidly repeat /$p^ha$/, /$t^ha$/, /$k^ha$/, /$p^hat^hak^ha$/ diadochokinetic movement while mandible was free and mandible was stabilized with bite block. The results showed that children with articulation disorder revealed significantly greater difference in elapsed time for diadochokinetic movement between mandible free and stabilized state compared to the without articulation disorder group. But the correlation between the percentage of consonants correct and the compensation ability in speech motor control in the articulation disorder group was irrelevant. These results point out to the fact that children with articulation disorder have poor compensation ability in speech motor control compared to the children without articulation disorder. On the other hand, the poor ability does not have any relation with the severity of articulation disorder. These results suggest either general or individual characteristics of children with articulation disorder.

Stimulant Induced Movement Disorders in Attention Deficit Hyperactivity Disorder

  • Stimulants, such as amphetamine and methylphenidate, are one of the most effective treatment modalities for attention deficit hyperactivity disorder (ADHD) and may cause various movement disorders. This review discusses various movement disorders related to stimulant use in the treatment of ADHD. We reviewed the current knowledge on various movement disorders that may be related to the therapeutic use of stimulants in patients with ADHD. Recent findings suggest that the use of stimulants and the onset/aggravation of tics are more likely to be coincidental. In rare cases, stimulants may cause stereotypies, chorea, and dyskinesia, in addition to tics. Some epidemiological studies have suggested that stimulants used for the treatment of ADHD may cause Parkinson's disease (PD) after adulthood. However, there is still a lack of evidence that the use of stimulants in patients with ADHD may cause PD, and related studies are only in the early stages. As stimulants are one of the most commonly used medications in children and adolescents, close observations and studies are necessary to assess the effects of stimulants on various movement disorders, including tic disorders and Parkinson's disease.

감정표현 표정의 영상분석에 의한 인지동작치료가 정서·행동장애아 감성재활에 미치는 영향 (The Effect of Cognitive Movement Therapy on Emotional Rehabilitation for Children with Affective and Behavioral Disorder Using Emotional Expression and Facial Image Analysis)

  • 본 연구는 인간발달 과정을 신경과학, 심리학, 운동학습, 근육 생리학 등의 행동과학 자료와 생체역학, 인체운동 분석, 운동조절 기반의 인지 동작치료 프로그램을 정서 행동장애 아동들에게 실시하여 표정에 따른 얼굴 움직임이 감정과 정서변화에 따라 어떤 특성을 갖는지를 표정 움직임의 변화로 정량화 하고자 하였다. 본 연구에서는 영상측정 및 키네마틱 분석의 정서 행동장애 아동의 표정변화를 중재 프로그램 피드백 자료로 활용하였고, 표정 변화를 통해 인지동작 치료프로그램 효과를 알 수 있었다. 또한 정서 및 행동치료의 영상분석과 키네마틱 분석의 정량적 데이터를 통하여 인간발달에 대한 융 복합적 측정 및 분석법을 적용하여 발달장애의 조기발견과 치료과정에 따른 데이터를 축적하는 것도 기대할 수 있었다. 따라서 본 연구의 결과는 아동뿐만이 아니라 자기표현이 부족한 장애와 노인, 환자에게도 확대 적용할 수 있을 것이다.

측두하악장애 환자에서 악관절 세정술과 교합안정장치를 동반한 치료의 효과 (AN EFFECT OF COMBINATION WITH ARTHROCENTESIS AND STABILIZATION SPLINT TREATMENT OF TEMPORPMANDIBULAR JOINT DISORDER PATIENT)

  • The purpose of this study was to evaluate the clinical outcomes of patients with temporomandibular joint disorder before and after performance of arthrocentesis and stabilization splint therapy. The subjects of this study were 33 patients with limited mouth opening or pain or joint effusion who visited the Department of Oral and Maxillofacial Surgery, St Mary's Hospital, Catholic University of Korea. The arthrocentesis was performed and the stabilization splint was worn immediately after the arthrocentesis. Comparing the clinical outcomes of treatment at the times of first medical examination, 3 months and 6 months later on treatment. The results are as follows; 1. Statistically significant increase in the amount of maximum mouth opening occurred 3 months and 6 months later on treatment. 2. Statistically significant decrease in the average value of pain on test measured by VAS during maximum mouth opening, protrusive movement, right lateral movement, left lateral movement occured 3 months and 6 months later on treatment. The results suggested that arthrocentesis and stabilization splint therapy provide a improvement on patients with temporomandibular joint disorder when the exact diagnosis were provided.

측두하악 장애환자의 하악 운동 Simulation에 관한 연구 (A STUDY ON SIMULATION OF THE MANDIBULAR MOVEMENT OF THE PATIENTS WITH TEMPOROMANDIBULAR JOINT DISORDER)

  • To understand the biomechanics of the mandibular movement of the patients with T.M.J. disorder, it is necessary to analyze the mandibular movement by the least moving points of instantaneous center of rotation on computer screen. In this study, two subjects without T.M.J. disorder and with normal occlusion and few patients with T.M.J. disorder were selected for obtaining the processing data. The habitual opening and dosing movements were recorded by image recognition system of video and computer and then the mandibular movement and the least moving point were analyzed using computer program for image anaysis of specialized points. The results obtained were as follows: 1. The least moving points of control group during opening and dosing were dose to the mandibular foramen, but in the test group there were differences between two positions of the least moving points during opening and closing. 2. The variations of the least moving point were in the range of $0.02\sim0.05cm$ of X-coordinate and $0.07\sim0.10cm$ of Y-coordinate for control group, whereas in the range of $0.05\sim0.30cm$ of X-coordinate and $0.08\sim1.65cm$ of Y-coordinate for test group. 3. The index of variation during opening and dosing movements was shown in $0.8\sim1.0$ for the control group and in $0.56\sim2.6$ for the test group. 4. After the treatment of the test group No. 4 by occlusal splint the least moving point was dose to the mandibular foramen and the index of variation was changed from 25 to 05. 5. The tracing shape of the least moving point in the test group represented the irregularities compared to the control group. 6. The mandibular opening and dosing movements were simulated on the computer screen following the movement of the least moving point of instantaneous center of rotation.

포비유증후군을 호소하는 남성 한방 치험 1례 (A case of a patient with Foville syndrome treated with Traditional Korean Medicine)

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine (TKM) on a patient with horizontal eye movement disorder due to Foville syndrome. ■ Methods A patient with horizontal eye movement disorder as Foville syndrome was treated with herbal medication, acupuncture, electro-acupuncture and transcutaneous electrical nerve stimulation. Then we evaluated the improvement with mobility impairment, length of movement and House-Brackmann scale before and after treatment ■ Results Improvements of mobility impairment, distance at left gaze and House-Brackmann scale were observed after the TKM treatment. ■ Conclusion This case report shows the effect of TKM treatment on horizontal eye movement disorder due to Foville syndrome.

  • This study reviewed about Health Management of Women's with Menstrual Disorder and the differences in opinions between the Eastern- Western Nursing Sciences. The purpose of this study was to find out a possible integration of the Eastern-Western Nursing Sciences for Health Management of Women's with Menstrual Disorder and to suggest a distinct frame of Korean Nursing Intervention Method. In the last decade, the women's movement has succeeded in changing in inappropriate and poor treatment. The health consumer movement has also achieved many gains in improving the quality of health care. Yet the medical care offered women during their life stages is still characterized by a lack of effectiveness, safety, and consideration. Today, Although there are differences between Oriental and Western Medicine in approaches to clients, the Eastern-Western Nursing Sciences in approach to clients can be hoped that contribution will be made to the development of nursing intervention which is suitable to Korean context.

양극성 장애 환자에서 비정형 항정신병약물 치료 후에 나타나는 지연성 운동장애와 지연성 근긴장이상의 유병률 및 위험요인 (Tardive Dyskinesia and Tardive Dystonia with Second-Generation Antipsychotics in Bipolar Disorder Patients Unexposed to First-Generation Antipsychotics)

  • Objectives Second-generation antipsychotics (SGAs) are frequently used in the treatment of bipolar disorder. However, there is still no consensus on their risk of tardive movement syndromes especially for first-generation antipsychotics (FGAs)-naïve patients. This study aimed to investigate the prevalence and associated factors of SGAs-related tardive dyskinesia and tardive dystonia in patients with bipolar disorder, in a naturalistic out-patient clinical setting. Methods The authors assessed 78 non-elderly patients with bipolar (n = 71) or schizoaffective disorder (n = 7) who received SGAs with a combined use of mood stabilizers for more than three months without previous exposure to FGAs. Multiple direct assessments were performed and hospital records longer than one recent year describing any observed tardive movement symptoms were also reviewed. Results The prevalence rates of tardive dyskinesia and tardive dystonia were 7.7% and 6.4%, respectively. These patients were being treated with ziprasidone, risperidone, olanzapine, quetiapine, or paliperidone at the time of the onset of the movement symptoms. Tardive dyskinesia was mostly observed in the orolingual area, and tardive dystonia was most frequently detected in oromandibular area. A past history of acute dystonia was significantly associated with presence of both tardive movement syndromes. Conclusions Our findings suggest that SGAs-related tardive movement syndromes occur in a substantial portion of bipolar disorder patients. Acute dystonia, a reported risk factor of tardive movement syndromes in the era of FGAs is confirmed as a risk factor of both tardive dyskinesia and tardive dystonia that were induced-by SGAs.

편마비를 주소증으로 입원한 뇌경색 환자의 한방 치료 시기에 따른 치료 효과 분석 : 후향적 의무기록 분석 (Analysis of Treatment Effect According to the Period of Herbal Treatment in Cerebral Infarction Patients Admitted with Hemiplegia: Retrospective Medical Chart Review)

  • Objective: This study was aimed at evaluating the effect and clarifying the treatment period of Korean medicine treatment for cerebral infarction. Method: This study was carried out on patients with hemiplegia who were hospitalized in the Department of Korean Internal Medicine of Jecheon Korean Medicine Hospital of Semyung University from June 2014 to May 2019. A retrospective study was performed on 253 patients who were diagnosed with cerebral infarction by brain CT or nuclear magnetic resonance imaging. Results and Conclusion: 1. Korean medicine treatment has a significant effect on improving the movement disorder and daily life independence of cerebral infarction. 2. In the group that started Korean medical treatment within one month after the onset of cerebral infarction, with the exception of MMSE-K, the indicators related to the movement disorder and daily life independence showed significant effect. 3. MMSE-K showed no statistically significant change in any of the patient groups. 4. Within three months after the onset date, the longer the period of treatment with Korean medicine, the better the symptom improvement of upper extremity movement disorder. 5. The longer the hospital stays, the better the symptom improvement of the lower extremity movement disorder. 6. The combination of Korean and Western medicine did not affect liver or kidney function.
