Patient-Specific Computational Fluid Dynamics in Ruptured Posterior Communicating Aneurysms Using Measured Non-Newtonian Viscosity : A Preliminary Study

  • Objective : The objective of this study was to analyze patient-specific blood flow in ruptured aneurysms using obtained non-Newtonian viscosity and to observe associated hemodynamic features and morphological effects. Methods : Five patients with acute subarachnoid hemorrhage caused by ruptured posterior communicating artery aneurysms were included in the study. Patients' blood samples were measured immediately after enrollment. Computational fluid dynamics (CFD) was conducted to evaluate viscosity distributions and wall shear stress (WSS) distributions using a patient-specific geometric model and shear-thinning viscosity properties. Results : Substantial viscosity change was found at the dome of the aneurysms studied when applying non-Newtonian blood viscosity measured at peak-systole and end-diastole. The maximal WSS of the non-Newtonian model on an aneurysm at peak-systole was approximately 16% lower compared to Newtonian fluid, and most of the hemodynamic features of Newtonian flow at the aneurysms were higher, except for minimal WSS value. However, the differences between the Newtonian and non-Newtonian flow were not statistically significant. Rupture point of an aneurysm showed low WSS regardless of Newtonian or non-Newtonian CFD analyses. Conclusion : By using measured non-Newtonian viscosity and geometry on patient-specific CFD analysis, morphologic differences in hemodynamic features, such as changes in whole blood viscosity and WSS, were observed. Therefore, measured non-Newtonian viscosity might be possibly useful to obtain patient-specific hemodynamic and morphologic result.

  • This study aims to investigate language society cultural aspects in a community, examining and comparing linguistic errors of English signboards (ES) at home and abroad. The data comprised the ES in 5 cities, Korea and in Paris, France. The findings showed that the errors of the ES in Korea reached to 39.2%, whereas those in Paris contained 24.7%. In Korea, ES and errors were the most in Myeong-dong area. In Paris, the most ES were in the area of Eiffel Tower, but the most linguistic errors appeared at the 3, 4 districts of Marais. Those errors belonged to most in the business of drink, food, and clothes in turn in Korea, while this was the case in the field of clothes, food, and culture in Paris. The frequent errors were classified into semantic, morphologic, and syntactic aspects in turn. The regulations on signboards of foreign languages existed but were not abided by in the both countries. Thus, more practical and reasonable devices and policies are required to establish a more harmonious atmosphere of the ES.

  • Purpose : To evaluate the MRI findings of breast cancer with BRCA mutation. Materials and Methods: We collected information of the breast cancer patients who underwent the test for BRCA gene mutation as well as preoperative breast MRI from January 2007 to December 2010. A total of 185 patients were enrolled; 33 of these patients had BRCA mutations and 152 patients did not. Among them, a total of 231 breast cancers were detected. Images of the 47 breast cancers with BRCA mutation and of the 184 breast cancers without mutations were evaluated to compare the morphologic and enhancement features on MRI. Results: With MR imaging, there were no significant difference in morphologic characteristic between two groups. However, enhancement pattern in the group with BRCA mutation were more likely to have persistent enhancement (p < 0.233), and LN metastasis was more common in breast cancers without BRCA mutation. Breast cancers with BRCA 2 mutation tend to show more persistent enhancement pattern than BRCA 1 mutation. Conclusion: In breast cancer patients with BRCA mutation, MRI didn't show significant difference in morphologic characteristics, however breast cancers with BRCA gene mutation carriers tend to have benign morphologic features on MRI, such as Type 1 kinetic curve enhancement.

  • The author ovserved a case of dentinogenesis imperfecta in a 8-year-old and a 9-year-old brother with complaints of abnormal morphologic changes of the teeth. 1. Clinically, yellowish brown colored teeth, fractured crown and numerous retained root tips of deciduous teeth with severe attrition were observed in the both deciduous and permanent dentitions. 2 Radiographically, small-sized teeth, partial or complete obliteration of the pulp chambers and pulp canals in the anterior teeth, partial obliteration of the pulp chambers and thin enamel in the permanent tooth germs were observed. 3. The familial history was their father has been lose his teeth early.

  • Abutment dies which resemble the actual size and shape of crown abutment is essential in most of the research area of fixed prosthodontics like marginal accuracy, crown seating, behavior of luting agent and so on. Seeing the large portion of research is done with round shaped dies in different size and cone angles, the necessity of research on the crown abutment is self-evident. 500 molar abutments were collected randomly through the commercial dental laboratoy, regrdless of their position in the dental arch, sex, and age. The measurements of 22 points of a die were done, and the results were as fogbows : 1. The height of the molar dies was $3.9{\pm}1.2mm$ 2. The bucco-lingual width was $8.9{\pm}1.2mm$ at the base, and $7.4{\pm}1.2mm$ at the occlusal. 3. The desio-sistal width was $8.2{\pm}1.2mm$ at the base, and $7.0{\pm}1.3mm$ at the occlusal.

  • Metastatic melanoma is often difficult to diagnose morphologically. However, the prognostic and therapeutic decision in cases of metastatic melanoma depends upon the morphologic documentation of the metastatic disease, which may rapidly and accurately be done by cytologic methods such as aspiration cytology of clinically suspicious lesion. Cytological features of 12 metastatic melanoma are described. Evaluation for melanin pigment, background composition, cytoplasmic vacuoles, intranuclear inclusions, bi- or multinucleated cells, eccentric nuclei, nucleoli, and degree of cellular cohesion were made. the presence of melanin pigment, prominent nucleoli, intranuclear cytoplasmic inclusions, lack of cell cohesion, and eccentric nuclei are the important and useful cytologic features in the diagnosis of metastatic melanoma. Bi or multinucleated cells, cytoplasmic vacuoles, and bloody background are also frequently found in smears from metastatic melanoma.

Adenocarcinoma of the parotid gland with calcification (석회화를 수반한 이하선내 선양암종)

  • A 78-year-old woman was referred to Chonbuk National University Dental Hospital complaining of facial palsy and palpable mass on the right parotid gland area. Clinical examination showed non-specific findings of the intraoral region, but showed asymmetrical facial appearance. Panoramic view showed a large amorphous calcified mass on the posterior to the mandibular ramus and thin cortical plate of the posterior ramus. Sialogram showed constriction of the main duct and no further filling of striated, intercalated ducts and parenchymal areas. CT scans demonstrated an irregular, infiltrating mass with slight enhancement in the right parotid gland. The mass showed necrotic areas and calcifications. Bone scan showed marked accumulation of /sup 99m/Tc-MDP on the right posterior maxilla. Microscopic findings demonstrated the minimal morphologic alterations and rare mitotic figures within tumor cells, and diagnosed as adenocarcinoma (NOS, Grade II). This report could be aid in the diagnosis of calcified lesions of the salivary gland.

Morphological characteristics of the mandibular asymmetry in adult patients (하악골비대칭 환자의 형태학적 특징에 관한 연구)

  • Morphological characteristics of the asymmetric mandible in 135 adult male patients were investigated. Panoramic X-ray images were recorded. Age, ramus height, antegonial notch height, condylar height, coronoid process height, ramus body height and condylar neck angle were calculated and measured. Statistical analysis of unpaired two-tailed t-test and correlation coefficients was performed to find the morphologic differences between short ramus height side and the other. Condylar height and ramus body height in short ramus height side were smaller than those of the other side. Antegonial notch height, coronoid process height, condylar neck angle showed no differences. Condylar neck height was correlated with condylar neck angle.

  • Introduction of electron microscope in biological and medical sciences change the concept of functional and morphologic unit of biological phenomena from the cell to subcellular unit, and it formulated the basis for molecular biology and pathology. Until recently, electron microscopy has mainly been applied to basic research works. However, practical clinical application of electron microscopy is being actively tried. The major clinical fields in which electron microscopy is helpful or even essential include viral diseases, metabolic diseases, glomerular diseases and in the identification of certain types of neoplasms. A brief introduction of characteristics of each conditions are made to encourage more active application of electron microscopy in clinical medicine.

  • Ganglioneuroma is a well-differentiated, benign tumor of the sympathetic nervous system. These tumors belong to a family of neoplasm that exhibit a wide range of differentiation, with neuroblastoma at one end and ganglioneuroma at the other. Because it share morphologic features with other both benign and malignant neural tumors, accurate preoperative diagnosis is often difficult. Nonetheless, it is critical for proper management. Fine needle aspiration cytology (FNAC) in the diagnosis of the ganglioneuroma has been a little documented. We describe a case of mediastinal ganglioneuroma in a 33-month-old girl. The diagnosis was suggested on FNAC and was confirmed by histopathologic examination later.
