한방음악치료(韓方音樂治療)의 기전(機轉)에 따른 치료법(治療法) 연구(硏究) - 신허자음음악요법(腎虛滋陰音樂療法)을 중심(中心)으로 - (The Study of Deficiency of the Kidney-Eum Nourishing Therapy by Methodology of Oriental Medicine Music Therapy)

  • In this study, I investigated physiology and pathology of the kidney and then researched deficiency of the kidney-nourishing the Eum music therapy to apply for clinical use, because symptoms such as chronic fatigue, morbus asthenia, anxiety, dizziness, tinnitus, and amnesia are prevalent in our modern societies. These symptoms are due to deficiency of the kidney. The kidney stores vital essence and has a function to keep activities as motive power. If the kidney is abnormal in storing the essence and holding Gi, various symptoms like dizziness, tinnitus, amnesia are caused by deficiency of the kidney. In deficiency of the kidney-nourishing the Eum music therapy, Water-Gi music which can store the essence is mainly used. In case of patients with deficiency of the kidney, dizziness, tinnitus, and amnesia, we can make use of NO.3, BWV 1068, Air of J.S.Bach and Jinyangjo of Geomungo. If there are flaring-up and fidgetiness due to deficiency of vital essence, Earth-Gi music that can help the vigor of spleen and stomach should be used prior to Water-Gi music. In the concrete, Hahyeondodeuri of Yeongsanhoesang played on Geomungo can be used. The tinnitus is caused by wind-heat and fire in the gallbladder. It is good to nourish the Eum through Water-Gi music after dispelling pathogens through Jajinmori among Gayageum sanjo.

A Case of Hemifacial Spasm Caused by an Artery Passing Through the Facial Nerve

  • Hemifacial spasm (HFS) is a clinical syndrome characterized by unilateral facial nerve dysfunction. The usual cause involves vascular compression of the seventh cranial nerve, but compression by an artery passing through the facial nerve is very unusual. A 20-year-old man presented with left facial spasm that had persisted for 4 years. Compression of the left facial nerve root exit zone by the anterior inferior cerebellar artery (AICA) was revealed on magnetic resonance angiography. During microvascular decompression surgery, penetration of the distal portion of the facial nerve root exit zone by the AICA was observed. At the penetrating site, the artery was found to have compressed the facial nerve and to be immobilized. The penetrated seventh cranial nerve was longitudinally split about 2 mm. The compressing artery was moved away from the penetrating site and the decompression was secured by inserting Teflon at the operative site. Although the facial spasm disappeared in the immediate postoperative period, the patient continued to show moderate facial weakness. At postoperative 12 months, the facial weakness had improved to a mild degree. Prior to performing microvascular decompression of HFS, surgeons should be aware of a possibility for rare complex anatomy, such as compression by an artery passing through the facial nerve, which cannot be observed by modern imaging techniques.

전문직 정체성 형성을 위한 의학교육 현장의 과제 (The Tasks of Medical Education to Support the Formation of Medical Professional Identity)

  • Building professional identity is the most basic purpose of medical education. Students who enter medical schools do not have an identity rooted in the medical profession, and universities should therefore take steps to help students form their identity as doctors, attitudes, beliefs, and values through the curriculum. However, while medical knowledge and clinical skills are fully reflected in basic medical education, issues persist regarding education on values, attitudes, and beliefs that are important for professional identity. Regarding the process of professional identity formation, it is important to keep in mind that rapid changes in modern society lead to corresponding changes in socio-cultural expectations and demands related to professional identity, resulting in discrepancies between the reality of medical education and the actual field of medicine. Medical schools need to prepare students for these discrepancies, and in-depth discussions should address what is important and what should be solved first at medical education sites. However, it is difficult to generalize the tasks of professional identity formation in the field of medical education because each medical school may have unique circumstances. This article discusses the tasks that medical education should solve for professional identity formation education in terms of five aspects: establishing learning outcomes, training educational experts, introducing transformative learning, utilizing self-directed learning, and developing evaluation methods.

학동기 소아에서 보완대체요법의 이용실태 (A study on the utilization of complementary and alternative medicine for elementary children)

  • 목 적:만성질환자가 아닌 일반 학동기 아동을 대상으로 보완대체요법 이용실태를 파악하며 소아청소년과 의사로서 대처 방향을 모색하고자 연구를 실시하였다. 방 법:3개 초등학교 1학년부터 6학년 1,688명의 학부모를 대상으로 설문조사를 시행하였다. 그중 설문지가 회수된 794명(47%)을 분석하였다. 결 과:일반적인 학동기 아동에서 보완대체요법의 이용률은 278명(35%)이었고, 보완대체요법의 종류는 한약제 62.6%, 보조식품 31.3%, 비타민 30.2%, 선식 11.2%, 침술 11.2%, 체 냄, 수지침 순이었다. 보완대체요법의 이용률은 어머니의 학력이 대학 이상 일 때, 종교가 있을 때, 가정 월 총수입이 100만 원 이상일 때 의미 있게 높았으며 이용하게 된 동기는 질병예방목적이 가장 많았고, 주로 주변 사람이나 매체를 통해 이용하였다. 보완대체요법에 대한 만족하지 못하는 경우는 16%였고, 재사용 의향이 없는 경우는 20.5%였고, 환아가 건강할 때 주로 사용하는 경향을 보였다. 환아가 영양결핍, 아토피 피부염, 관절통증, 알레르기비염, 비만, 천식 때 주로 이용하는 경향을 보였다. 결 론:보완대체 의학의 사용이 아이의 건강상태와는 밀접하지 않으며 사용 경로 또한 주변 사람의 권유나 방송 매체 등을 통해 이루어지고, 병원치료와 병행 시 좋을 것이라는 막연한 기대감으로 사용되고 있는 경우가 많아 보완대체요법에 대한 부작용, 효과, 안전성에 대한 연구가 필요하며 만성질환 및 알레르기 질환의 경우 소아청소년과 의사로서 현대의학의 적극적인 치료 및 교육이 필요할 것으로 사료된다.

일제강점기의 한의학 교재 중 하나인 『한방의학강습서(漢方醫學講習書)』 - 서문과 발문(跋文)의 번역과 인용서적의 분석을 중심으로 (A Lecture Book on Traditional Korean Medicine in the Period of Japanese Occupation, 『Eihak Gangseupseo(醫學講習書)』 - focused on its preface, epilogue and reference books)

  • In July 2008, I obtained a copy of "Hanbang Eihak Gangseupseo" through Uibangseowon. It was recorded that the book was compiled by Seong Ju-bong and reviewed by Ji Seok-young. According to previous studies, this book was the lecture book that was used in teaching Traditional Korean Medicine in Daejeon, Chungcheongnam-do. This book gave insight to the system and curriculum of the school for Traditional Korean Medicine in Daejeon, Chungcheongnam-do. It also exhibited the academic characteristics of Traditional Korean Medicine in the Period of Japanese Occupation and the medical viewpoint of Seong Ju-bong. The summary is as follows: First, an independent School for Traditional Korean Medicine was run in Daejeon, Chungcheongnam-do, with reasonable curriculums and systematic textbooks. Second, the medical viewpoint and treatment methods of Huang Yuan-Yu of Qing Dynasty was actively introduced. Then it was reorganized for the society and stimulated the progress of Traditional Korean Medicine. Third, while absorbing Chinese Medicine, it still inherited our heritage of Traditional Korean Medicine Especially, Seong Ju-bong's original opinions and clinical experiences are shown in surgery, gynecology and pediatrics. Fourth, in a break from the past, when Chinese culture could not be introduced due to diplomatic problems between Joseon and Qing, efforts were made to overcome limitations of lagging behind by adopting and educating Warm Disease study. Fifth, while working side by side with Ji Seok-young who introduced the modern Western Medicine through vaccination, it still searched for a traditional Korean medical treatment for chickenpox. I hope that the report of my findings through reading "Eihak Gangseupseo" could make up for the fact that the medical history during the colonial period is scarce.

뇌(腦)와 심(心)의 한의학적 상관성에 대한 연구 (Study on the Relationship of Brain and Heart Based on Oriental Medicine)

  • This study aims to define the relationship between brain and heart through several literatures about oriental medicine and the conclusions are as follows. Heart in oriental medicine is called as Sinmyeongjishim(神明之心) which has a close connection with Mind, Consciousness, Emotion, and Physiological instinct of Drain in modern medicine. According to Oriental medicine, Brain stores Wonsin(元神) as Heart stores mind(神). Heart is where mind rests whereas Brain is where mind reveals. The external evidences that prove the relationship of Heart and Mind are as follows: First, with ears, eyes, mouth, and nose the subject of cognition is recognized as Sinmyeongjishim(神明之心). Second, Bulin(不仁), which means decreased movement power and sensibility of limbs, proves that Sinmyeongjishim(神明之心) is involved with movement power and sensibility of limbs. The physiological evidences that prove the relationship of Heart and Mind are as follows; First, Heart as the operation of Sinmyeongjishim(神明之心) manages language. Second, Heart is related with Tongue. Third, Heart is linked to Ears through the ear hole. Fourth, Heart is a store of Mind. Fifth, the five viscera control emotional and psychological activities. The pathological evidence of the relationship of Heart and Mind is that the symptoms of heart disease which are related to Sinmyeongjishim(神明之心) are also related to the functions of Brain. Though Brain has a close connection with Heart in oriental medicine, it is recognized that there are distinctive symptoms of disease of Brain and Hyeolyookjishim(血肉之心) respectively. The relationship of Heart and Brain has been researched in this study, even though there are not enough written materials about oriental medicine. But the fact that the majority of Heart operation is deeply connected with Brain activities cannot be denied. Therefore the research of Heart should be done as well as Brain in the clinical study of Brain.

한의학의 비과학성 논란에 대한 반박 (Refutation against the Non-Scientificity Argument on Korean Medicine)

  • This study is aimed to refute against medical opponent's claim that Korean medicine does not conform to the conditions of science. Analyses and refutations against a journal treatise and a Facebook column formally written for a logical criticism and attack were conducted. As an example of the logic of the knowledge production process in Chinese medicine, 8 principle theory was exemplified in the Han danasty Classics "Neijing" and "Shanghanlun" at first. The knowledge was continuously revised and accumulated historically and then completed through Ming、Cheng period. The differential diagnosis and treatment theory is a logical process of forming knowledge through the process of abduction, deduction and induction begun from "Shanghanlun" succeeded to nowadays and it is essentially equaled with the process of experimental inference by Claude Bernard. Examples of normal science status based on Kuhn's scientific standards include the theory of 8 principle, differntial diagnosis of viscera and bowel, 3 yang and 3 yin diseases of "Shanghanlun" and warm disease theory. Examples of science lost or get its normal status by refutation following Popper's standards were cold damage theory and warm disease theory respectively. This allowed Chinese medicine to follow the general principles of science that form scientific knowledge and to correspond with the demarcation standards and concepts of science. However, as one of the conditions for becoming a science, Chinese medicine is partially lacking in terms of the interpretation of text language or the accordance with modern knowledge. Therefore methods are required to supplement this lack through multi-faceted research such as literature-based, theoretical and clinical studies.

도진우(都鎭羽)의 『동서의학요의(東西醫學要義)』에 대한 연구 (A Study of Do Jinwoo's Dongseo uihak youi (東西醫學要義))

  • This paper analyzes the historical context, the author, and the organization of contents of Dongseo uihak youi (Essentials of Eastern and Western Medicines), which was written by Do Jinwoo. In the colonial situation of the early twentieth century, the tradition of Korean medicine faced crises and challenges in many ways. Members of the Korean medicine community were simultaneously faced with continuing the tradition of Korean medicine and becoming healthcare providers with a specific role within the healthcare system of the time. Dongseo uihak youi is the result of the collective and official efforts of the Association of Korean Medicine of the time to maintain its tradition where only Western medicine was officially allowed to be taught and tested after the promulgation of the Rules of the Medical Student (ŭisaeng). Dongseo uihak youi was the first Korean medicine book to precisely describe and compare the names of diseases in Eastern and Western medicines. Dongseo uihak youi contained not only medical theories and prescriptions but also laws and forms, in that the purpose of the book was not simply to cultivate clinical skills but also to demarcate the boundary of medical knowledge and activities required of a practitioner of Korean medicine in the modern colonial health care system of the time.

화침요법(火鍼療法)의 연구동향(硏究動向)과 D.I.T.I를 활용(活用)한 임상적(臨床的) 고찰(考察) (A Literature Review and A Clinical Examination of Burning Acupuncture Therapy of using D.I.T.I.)

  • We have studied the clinical application of burning acupuncture by examining 18recent-published papers in the journal 'ZHONGGUO ZHENJIU(中國鍼灸)' for the bibliographical study and clinical study and have given medical treatment of burning acupuncture on 21 Patients who suffered 'the body chill symptoms(冷症)' at the oriental hospital of Won-Kwang University, Kwang-Ju. As a result, we have come to some conclusions as below. 1. The diseases for which the modern borning acupuncture is efficacious are widely extended to various kinds of field such as surgery, internal medicine, gynecology, ophthalmology, otolaryngology. etc. as well as numbness, meridian muscle disease(經筋病) mentioned in 'Neijing(內徑)' . 2. The effects of burning acupuncture therapy are wen zhuang yang qi(溫壯陽氣), sheng ji lian chuang(生肌斂瘡), san han chu shi(散寒除濕), qu fens zhi yang(祛風止痒), qu yu chu fu pai nong(祛瘀除腐排膿), san jie xiao zhong(散結消腫), zhi tong huan ji chu ma mu(止痛緩急除麻木), qing re xie hue jie du(淸熱瀉火解毒). 3. The recently reported diseases for which burning acupuncture is good are internal and external humeral epicondylitis, atheroma, menorrhalgia, thecal cyst, tragomaschalia, pruritus, traumatic onychophemia, gout of feet, prostatomegaly, aacne, supprative infection of body surface, snapping finger, backache, numbness, pyocyst etc. 4. The subjective symptoms of 'the body chill symptoms(冷症)' were, most of all, feeling cold accompanied by pain(16 case), while just 4 cases were feeling cold only. 5. In the related diseases of the body chill symptoms(冷症), feeling pain like arthralgia was the most case, and then dysmenorrhea, menorralgia, depression, anemia in order. 6. In the D.I.T.I before and after burning acupuncture treatment, 6 patients had shown 'excellent' effects and 8 patients had turned out 'good', while 7 patients had become'fair'. 7. In the degree of patient-satisfaction, 5 patients announced 'excellence' and 6 patients expressed 'good', 4 expressed 'fair', while the other 6 showed no change. 8. In the correlation of D.I.T.I and patient-satisfaction, the better the result of clinical treatment was, the more satisfied the patient was, however, in the case 'fair', we saw the degree of patient-satisfaction was relatively low, so we could admit the judging significance of D.I.T.I.

한의사·한약사 임무 및 공공제도 중심의 의약법규 제·개정 고찰 (A study on the Legislations and Amendments of the Medical and Pharmaceutical Laws and Regulations - Focusing on the Duties of Korean (Oriental) Medicine Doctors and Korean (Oriental) Pharmacists as well as the Public Health System -)

  • Purpose : The current Medical Law and the Pharmaceutical Affairs Act, which are incapable of utilizing the research results and the advanced academic, clinical, and pharmaceutical system of the present-day Korean (Oriental) medicine, have limitations and create a paradox by provoking social conflict among the professionals in the field. The aim of this study was to find out the legal and systematic problems that contributed to a complicated conflict amongst Korean (Oriental) medicine doctors, doctors, pharmacists, and Korean (Oriental) pharmacists regarding the classification of their functions. Methods : We reviewed the history and characteristics of the legislation regarding the duties of Korean (Oriental) medicine doctors and Korean (Oriental) pharmacists as well as the relevant and important public health policies since the enactment of the National Medical Services Law in 1951. We focused on the laws and regulations that are made in the process of the separating functions of physicians and pharmacists and the dispute between the Korean (Oriental) medicine doctors and the Korean (Oriental) pharmacists in the 1990s and 2000s. Results : The legislations and amendments of the medical and pharmaceutical laws and regulations that reflect the modern academic, clinical, and pharmaceutical system of the Korean (Oriental) medicine and the research results could be summarized as follows: 1) A partial amendment of the Medical Law in 1987, which added the provision of "Oriental health guidance" as one of the duties of Korean (Oriental) medicine doctors, assured a place for Korean (Oriental) medicine doctors in the field of public health. 2) A partial revision of Pharmaceutical Affairs Act in 1994 established a new system for Korean (Oriental) pharmacists, bringing about the creation of dualistic pharmaceutical system that complements the dualistic medical system. 3) The Promotion of the Research and Development of Wonder Drugs by Using Natural Substances Act was legislated in 2000 in order to stimulate research and development of Korean (Oriental) medicine and its industrialization. 4) Oriental Medicine Promotion Act in 2003 was enacted to lay foundation to specify and promote technology and industry that are related to Korean (Oriental) medicine. Discussions and conclusions : Although the dualistic medical and pharmaceutical system is set up by the Medical Law and Pharmaceutical Affairs Act, it is shown that the relevant regulations have been developed from a perspective of the western medicine.
