As the number of Coronavirus Disease-19 (COVID-19) patients increases in a global pandemic situation, the usefulness of mobile computed tomography (CT) is gaining attention. Currently, mobile CT follows the basic safety and essential performance evaluation criteria of whole-body or limited-view X-ray CT in order to obtain device approval and evaluation in the Republic of Korea. Unlike stationary CT, mobile CT is not operated in shielded areas but rather areas such as intensive care units, operating rooms, or isolation rooms. Therefore, it requires a different basic safety and essential performance evaluation standard than stationary CT. In this study, four derived basic safety evaluation criteria related to electrical, mechanical, and radiation safety were included (dose indication test, protection against stray radiation, safety measures against excessive X-rays, half-value layer measurement); and seven essential performance evaluation criteria were included (tube voltage accuracy, mAs accuracy, radiation dose reproducibility, CT number of water, noise, uniformity, and spatial resolution); total eleven basic safety and essential performance evaluation criteria were selected. This study aims to establish appropriate basic safety and essential performance evaluation criteria for simultaneously obtaining images with diagnostic value and reducing the exposure of nearby patients, medical staff, and radiologic technologists during the use of mobile CT.
When using a mobile X-ray unit, primary radiation creates medical images and secondary radiation scatters in many directions, which reduces image quality and causes exposure to patients, care givers and medical personnel. The purpose of this study was to develop a radiation shielding system for effectively shielding secondary radiation and evaluate its effectiveness. Using a mobile X-ray unit, spatial dose according to presence of human equivalent phantom and spatial dose using the developed shielding device were measured, and the phantom at 80 cm equidistance from center of X-ray was compared with spatial dose according to use of a shield. Measurements were taken at intervals of 10 cm every $30^{\circ}$ from the head direction($-90^{\circ}$) to the body direction($+90^{\circ}$). In the spatial dose measurement with and without the phantom, when the human equivalent Phantom was used, the spatial dose was increased by 40% in all directions from 40 cm to 100 cm from the central X-ray, and about 88% of the space dose was reduced when using the developed shields with the phantom. The equidistance dose at 80 cm from the central X-ray was increased by 39% from $5.1{\pm}0.26{\mu}Gy$ to $7.1{\pm}0.15{\mu}Gy$ when the human equivalent phantom was used, and when phantom was used and shielding was used, the spatial dose was reduced by about 90% from $7.1{\pm}0.15{\mu}Gy$ to $0.7{\pm}0.07{\mu}Gy$. The spatial dose of natural radiation was measured to be about $0.2{\pm}0.04{\mu}Gy$ when using the developed shielding with Phantom at a distance of 1 m or more. It is expected that by using the developed shielding system, it will be possible to effectively reduce secondary radiation dose received in all directions and to ensure safe imaging.
본 연구에서는 PPG 신호에 포함되어 있는 동잡음을 제거하기위해 사용되는 가속도 센서를 대체하기 위한 새로운 PPG 센서와 그 신호처리 기법을 제안하며, 제안된 방법은 청색광으로부터 추정된 동잡음을 기반으로 하여 광용적 맥파의 동잡음을 제거하는 것이다. 제안된 방법의 성능을 평가하기위해 청색광 LED와 적색광 LED, 포토센서를 이용하여 센서 프로브 설계하고 제작하였고, 제작된 센서 프로브와 적응필터를 이용하여 동잡음 제거 처리를 하였다. 실험의 결과에서는 제안된 센서와 신호처리 방법이 동잡음이 발생하더라도 신호 복원이 잘되었으며, 신호 대잡음비도 기존의 이동평균필터 보다 4.5배 이상 향상되었다. 이 실험의 결과로부터 제안된 방법이 저가의 헬스케어 단말기 설계에 적용될 수 있을 것이다.
최근, 병원정보시스템의 환경은 다양한 스마트 기술을 접목하고 있는 추세이다. 따라서, 스마트 폰, 테블렛 PC와 같은 다양한 스마트 디바이스가 의료 정보 시스템에 활용된다. 또한, 이러한 환경은 이기종 센서, 디바이스, 시스템 및 네트워크에서 실행되는 다양한 응용 프로그램으로 구성된다. 이들 병원 정보 시스템 환경에서, 기존의 접근 제어 방식에 의한 보안 서비스를 적용하는 것은 문제가 된다. 기존 보안 방식의 대부분은 접근제어 리스트 구조를 사용한다. 이는 클라이언트 이름, 서비스 객체 메소드 이름으로 접근 제어 매트릭스에 의해 정의된 접근만을 허용한다. 가장 큰 문제점으로는 정적인 접근 방법은 변화되는 상황에 신속하게 적응하지 못한다. 따라서, 우리는 보다 유연하고, 매우 상이한 보안 요구와 다양한 환경에 적용 할 수 있는 새로운 보안 메커니즘을 필요로 한다. 또한, 환자중심의 의료 서비스 형태로 변화되고 있어, 이를 해결하기 위한 연구가 요구된다. 본 논문에서는 스마트 모바일 환경에서 의료정보 동적접근 시스템을 제안하고자 한다. 우리는 기존 병원정보 시스템의 환경을 기반으로 동적접근 제어 방법으로 의료정보 시스템에 접근하는 방법에 중점을 두었다. 물리적인 환경은 모바일 x-ray 영상 디바이스와 전용 모바일 스마트 디바이스, PACS, EMR 서버와 인증 서버로 구성하였다. 소프트웨어 환경은 모바일 X-ray영상기기는 Windows7 OS를 기반으로 동기화 및 모니터링 서비스를 위해 .Net Framework를 기반으로 개발하였다. 그리고 전용 스마트 디바이스는 Android OS를 기반으로 JSP와 Java SDK를 통한 동적접근 응용 서비스를 구현하였다. 병원의 의료영상정보 서버와 모바일 X-ray영상기기, 전용 스마트 디바이스간의 의료정보는 의료영상정보 표준인 DICOM을 기준으로 한다. 또한 EMR 정보는 H7을 기반으로 한다. 동적접근 제어 서비스를 제공하기 위해, 우리는 산소포화도, 심박수, 혈압과 체온과 같은 생체 정보의 값에 대한 조건에 의해 환자의 상황을 분류하고, 의료진의 의료정보 접속 인증 방법으로 동적인 접근 방법을 설계했다. 이는 일반 상태와 응급상태로 2부분으로 구분하여 이벤트 추적 다이어그램으로 보였다. 그리고, 인증 정보는 ID/PWD와 위치, 역할, 작업시간 그리고 응급 환자를 위한 응급 코드를 포함하였다. 동적접근 제어 방법의 일반적인 상황은 인증 정보의 값에 의해 의료정보에 접근 할 수 있다. 그러나 응급상황의 경우는 인증 정보 없이 응급 코드에 의해 의료정보에 접근하도록 하였다. 또한, 우리는 의료정보 표준에 따라 환자, 의료진 및 의료 영상 정보로 구성되는 의료정보 통합 데이터베이스 스키마를 구축했다. 끝으로, 우리는 제안 시스템의 수행 결과를 일반과 응급상황과 같은 환자의 상태에 따라 스마트 디바이스 기반으로 동적접근 응용 서비스의 유용성을 보였다. 특히, 제안 된 시스템은 동적 액세스 제어 방법에 의해 응급상황에서 스마트 디바이스기반의 효과적인 의료 정보 서비스를 제공한다. 이 결과, 제안한 시스템이 u-병원 정보 시스템과 서비스에 유용할 것으로 기대한다.
In ophthalmology, a pupillometer, a device to measure the diameter of the pupil of the eye, can provide information on the function of the autonomic nervous system. The current pupillometers on the market are either too large to be a handheld instrument, or relatively expensive. In this study, a pupillometer based on a smartphone was designed. Both white and infrared LEDs and a 3M pixel camera of a smartphone were applied for the visual stimuli to an eye and for the acquisition of the eye images, respectively. Contrary to the existing method of pupil measurement that usually observe the variation of pupil diameter, the proposed algorithm in this study was applied to calculate the constriction ratio of the pupillary area in response to pupillary light reflex. The results showed that the constriction ratio of the pupillary area were all in the normal range (above 4.0) from the sixteen healthy participants. It is believed that the approach to pupil measurement used in this study is suitable for a mobile interface, and this system can be applied to clinical research, home-use healthcare, and distributed to some areas which suffer from problems like a lack of medical support.
최근 모바일 스마트 기기의 급격한 보급과 확산으로 인해 모바일 스마트 기기 간의 멀티미디어 콘텐츠 공유 기술이 확산되고 있다. 이를 위해 계층적으로 구성된 이종 무선 네트워크 상에서 멀티미디어 스트리밍 중계 시 중계 기기와 다수의 멤버 기기들 간의 과도한 네트워크 자원 경쟁으로 인해 재생 화면 정지 문제가 발생한다. 본 논문에서는 이러한 문제를 해결하기 위해 중계 기기와 멤버 기기의 스트림 데이터 버퍼링을 제어함으로써 과도한 네트워크 점유를 방지하고 버퍼 고갈로 인한 재생 화면 정지 문제를 개선하는 스트리밍 중계 기법을 제안한다. 또한 제안한 중계 기법을 적용한 스트리밍 중계 엔진을 설계하고 구현한다. 제안한 중계 기법의 성능 평가를 위해 스트리밍 중계 시의 화면 정지 횟수와 정지 시간을 측정하여 기존의 중계 엔진과 비교한다. 성능 측정 결과, 본 논문에서 제안하는 미디어 버퍼 제어를 이용한 중계 엔진이 기존의 중계 엔진보다 더 적은 화면 정지 횟수와 정지 시간을 보였으며 이를 통해 멀티미디어 스트리밍 품질을 향상시킬 수 있다.
위성통신은 이동통신처럼 많은 사람이 사용하지는 않으나 군사용, 재난용, 원격 교육용 및 의료용, 도서 지역, 인프라 취약 지역에서의 인터넷 제공 등 공공성, 통신서비스를 위하여 꼭 필요한 기술이다. 하지만 선박이나 항공기에서는 기지국이 필요한 휴대용 이동통신들은 사용 불가능하거나 사용에 제한을 받는다. 본 논문에서는 본 논문에서는 단말 장치의 제어기로 라즈베리 파이를 사용하여 위성 모뎀과 PPP 프로토콜을 통한 네트워크 통신을 하였으며 모뎀의 문자 메시지 기능을 이용하여 양방향 데이터 링크를 구현하여 Thraya 정지궤도위성 네트워크망에 접속하였다. 그리고 단말장치의 제어기에 I/O 장치들을 이에 연결하여 통신 시설이 갖춰지지 않은 곳에서 제어 대상 시스템에 원격으로 접속하여 I/O들을 제어하고 각종 센서를 통하여 계측한 데이터를 전송할 수 있는 선박용 IoT 단말 장치 시스템을 설계하고 구현하였다.

Objective: Using biofeedback in instructing pelvic floor muscle (PFM) activation is a great method to provide information on muscle contraction. This study aimed to determine the immediate effectiveness of a mobile game-based instruction with an extracorporeal biofeedback device (EBD) to improve PFM contraction in healthy subjects. Design: Cross-sectional study. Methods: Sixteen healthy subjects (4 men and 12 nulliparous women; age, 31±5 years) were enrolled. The subjects were randomly categorized into two groups, those who were to receive instructions on how to contract the PFM by using the EBD (n=7) and those who were to undergo a transabdominal ultrasonography (TAUS; n=9) with biofeedback. PFM function was measured as the displacement (mm) of PFM elevation by using the TAUS before and after the instructions in each group. Results: The EBD and TAUS groups showed a significant increase in the displacement of PFM elevation after the instructions from 5.93±4.03 mm to 7.62±3.77 mm and from 5.27±3.39 mm to 7.47±2.79 mm, respectively (p<0.05). No significant differences were found between the two groups. Conclusions: The results of this study indicated that instructions for PFM contraction using the EBD and TAUS showed an immediate effect; however, no significant difference in effectiveness was observed between the two instruction methods. Therefore, not only can the TAUS method be used but the EBD method can also be used as a PFM instruction method for noninvasive procedures. However, further studies are needed to demonstrate the effectiveness of training and exercise on larger sample sizes that includes patient populations with PFM dysfunction.
Human can learn how to shape their brain electrical activity in a desired direction through continuous feedback of the electroencephalogram (EEG), and this technique is known as Neurofeedback (or EEG biofeedback), which has been used since the late 1960s in clinical applications. In this study, a portable wireless EEG (named wEEG) has been designed and implemented, which consists of a mobile station (a wireless two-channel EEG acquisition device) and a base station (a bridge between mobile station and computer). Moreover, a SensoriMotor Rhythm (SMR) training system was also implemented with the wEEG for enhancing attention with virtual environment. Experiment results based on 16 volunteers' (8 females and 8 males, average age is $27{\pm}4$) were reported in this paper. The results show that the SMR ratio of 87.5% subjects increased about 0.7% in training status than that in the stable status. With the proposed system, many training protocol scan be designed easily and can be done at home in our daily life conveniently. Additionally, the proposed system will be useful for disabled and aged people.
According to the frequent use of mobile devices, risk of repetitive strain injury for the finger joints was increasing recently. The purpose of this study was to investigate the difference of muscular endurance limit of the thumb between adults and children. A total of thirty subjects (15 adults: $41.0{\pm}7.9$ years and 15 children: $9.5{\pm}2.7$ years) was recruited for this experiment. Maximum voluntary contraction (MVC) of the thumb was measured for 100 trials for each subject by using a push-pull gauge. From the maximum peak value obtained, three main factors (MVC, endurance limit, and fatigue index) were defined to quantify fatigue characteristics of the thumb flexor muscle. The experimental results showed that the mean of MVC, endurance limit, and fatigue index of the children were approximately 50% compared to the adults (p < 0.001). From the results of Pearson correlation test, the endurance limit was highly correlated with weight, age, hand length rather than height. Based on this study, children are more vulnerable to repetitive task using the thumb such as text/SNS messaging, gaming, and scrolling.
