노인의 객관적 구강건강상태와 주관적 구강건강수준간의 관련성 (Association between oral health status and oral health impact profile(OHIP-14) among the community elderlies)

  • Objectives : This study was attempted in order to grasp oral health level according to socio-demographic characteristics in elders in some communities, and to evaluate oral health status and its association. Methods : The subjects in this study were performed with 235 people, who were over 65 years and resided in Daejeon Province, from June 20 to July 10, 2011. An individual interview was held, and they got a dental checkup. As for data analysis, chi-square test, t-test, one-way ANOVA, pearson correlation were utilized. Results : The older age in the whole research subjects and the lower educational level led to the less remaining teeth and the larger missing teeth index. The decayed missing filled teeth index and the decayed missing filled teeth rate were higher in more women and older age and in the lower educational level. Tooth mortality rate was higher in the older age, the lower educational level, and the group of living together with spouse. The maxillary-mandibular fixed-bridge status in the mouth was indicated to be the highest in the full-denture mounting ratio as for elders in over 80 years old. Oral Health Impact Profile(OHIP-14) average score was $56.05{\pm}11.64$ in the whole research subjects The decayed missing filled teeth index and the decayed missing filled teeth rate showed significantly positive correlation with the decayed missing filled teeth rate, tooth mortality rate and showed significantly negative correlation with OHIP-14. Tooth mortality rate showed significantly negative correlation with OHIP-14 Oral Health Impact Profile(OHIP-14) showed significantly positive correlation with its factors. Conclusions : Accordingly, the policy effort is considered to be necessary that implements in elders in order to spend active senescence, and that elders' health and oral-health behavior can be implemented continuously and preventively through classification according to elders' physical function.

  • The autogenous tooth transplantation is an alternative treatment replacing a missing tooth when a suitable donor tooth is available. It is also a successful treatment option to save significant amount of time and cost comparing implants or conventional prosthetics. These cases, which required single tooth extraction due to deep caries and severe periodontal disease, could have good results by transplanting non-functional but sound donor tooth to the extraction site.

당뇨병 환자의 치주건강 상태에 대한 임상적 연구 (A CLINICAL STUDY ON THE PERIODONTAL STATUS OF THE PATIENT WITH DIABETES MELLITUS)

  • The purpose of the this study was to research the interrelationship of periodontal status and patient's age between diabetic group and non-diabetic group and compare that of periodontal status according to the blood sugar level and duration of diabetics. The participants of this study were 52 diabetic patients and 51 non-diabetic patients. The diabetic group and non-diabetic group were further divided into four subgroups according to the age(under 35 years old, 36 - 45 years old, 46 - 55 years old, over 56 years old) and the diabetic group was further divided into three subgroups according to the duration of diabetics (under 5 years, 6 - 10 years, over 11 years) and blood sugar level(under 140mg/100ml, 140-200mg/100ml, over 200mg /100ml), respectively. Author evaluated such periodontal status as plaque index and calculus index, gingival lndex, loss of attachment, papillary bleeding index, tooth mobility and missing teeth, and then data was statistically analyzed by SPSS/PC+ using ANOVA, respectively. The results were as follows: 1. The mean value of plaque index, gingival index, loss of attachment, papillary bleeding index, tooth mobility and missing teeth of diabetic patients were significantly higher than that of non-diabetic patients, respectively(p <0.01, p<0.01, p<0.01, p<0.01, p<0.001, p<0.001), but there was no significant difference between diabetic group and non-diabetic group in calculus index(P>0.05). 2. According to the increase of age, loss of attachment and missing teeth were increased in both group, and correlation of each subgroup was statistically significant, respectively(p<0.05, p<0.001). 3. According to the elevation of blood sugar level of diabetics, plaque index, gingival index, loss of attachment, papillary bleeding index, tooth mobility and missing teeth were increased in diabetic group, and correlation of each subgroup was statistically significant, respectively (p<0.05, p<0.05, p<0.05, p<0.05, p<0.001, p<0.001). 4. According to the duration of diabetics, plaque index, gingival index, loss of attachment, papillary bleeding index, tooth mobility and missing teeth were increased in diabetic group, and correlation of each subgroup was statistically significant, respectively(p<0.05, p<0.05, p<0.05, p<0.05, p<0.001, p<0.001).

광주지역 길고양이 구강질환의 임상적 특성 (Clinical characteristics of oral diseases on stray cats in Gwangju)

  • Demand on specialized feline dentistry has increased over the years, partly due to growing feline population in Korea. Despite the notable increase in the population, not much researches has been conducted on oral disease for stray cats. Thus this research was conducted on 81 adult stray cats from Gwangju in Korea (42 males and 39 females) with average weight of 3.4 kg and also focused on occurrence of periodontal disease (plaque and calculus), feline odontoclastic resorptive lesion (FORL), stomatitis and tooth anomaly. Formation of dental plaque and calculus were abundant on maxilla than mandible, especially distributed much on 4th premolar teeth and 1st molar teeth. After thorough examination of oral cavity, 33 stray cats (40.7%) were found to have oral disease. Eighteen stray cats (22.2%) had tooth fracture which was evident on cuspid and missing teeth were present on 2nd premolar tooth of maxilla and 2nd incisor of mandible amongst 20 stray cats (24.7%). FORL was present on cuspids and 2nd premolar tooth of maxilla among 10 stray cats (12.3%). Stomatitis was involved with FORL, missing tooth, and tooth fracture. Therefore, it was determined that stomatitis, FORL, and periodontal disease were closely associated.

  • Class I malocclusion is essentially a dental dysplasia. Rotations, individual tooth malpositions, missing teeth, tooth size discrepancies, etc., fall under this classification. There are two types of class I malocclusions. One is identified by and insufficient denture base to accommodate the teeth; the other has more denture base than tooth material, creating spaces in the arch. The tooth material-to denture base discrepancies may be slight, calling for only a little increase in arch length for alignment and the correction of minor rotations. Discrepancies may also be great, in which case it becomes necessary to reduce tooth material by extraction, so as to make the tooth material more in proportion to the size of the denture base. The author had attempted orthodontic treatment of a class I malocclusion case of 13-year old boy in which high canines and impacted mandibular second premolars were involved. The author obtained good results.

Evaluation of prognosis and cause of failure in 182 cases of autogenous tooth transplantation

  • Autotransplantation is a viable option for the treatment of a missing tooth or for replacement of avulsed and traumatized tooth when there is a donor tooth available. The maintenance of healthy periodontal ligament cells in the donor tooth is one of the most important factors for successful tooth transplantation. The purpose of this study was to evaluate the prognosis and the cause of failure in 182 auto-transplanted teeth. A total of 182 cases which received the auto-transplantation in Department of Conservative dentistry, Yonsei Dental Hospital were included in this study.(omitted)

하악 4전치 상실시 치료 계획 (Treatment plan for missing mandibular 4 incisors)

  • 상실한 하악 4전치에서의 치료 계획은 어느 다른 부위보다 수술의 성공률이나 보철치료시 유리한 환경을 지니고 있다고 흔히 생각되는 부위이다. 하지만 하악 전치들의 M-D 사이즈나 crowding으로 인한 공간적 제한, 임시치아 문제, 치조골 흡수로 인해 생기는 심미적인 문제 등 의외로 까다로운 경우가 종종 발생하는 부위기도 한다. 본문에서는 하악 4전치 상실 시의 치료 계확에 대해서 케이스를 통해 알아 보겠다. 하악 4전치 상실시 가능한 옵션들: 하나 혹은 4전치중 일부 상실의 경우나 잔존 치조골의 흡수가 거의 없어 임플란트 위치 관계가 심미적으로 중요하다고 판단되는 경우라면 직경이 작은 임플란트들을 원래 발치와 중앙에 식립(Narrow type 이나 One body mini implant type) 최종 보철물 형태가 Pink porcelain을 포함하는 이미 광범위한 치조골의 흡수가 일어난 경우 즉 임플란트 식립 위치가 덜 중요한 상황이라면 regular size의 임플란트들을 측절치와 중절치의 사이 Interseptal bone위치에 식립 4전치중 하나 혹은 일부가 상실됐고 남아 있는 전치들의 예후가 불량하다고 의심되나 환자가 당장 나머지 전치들의 발치에는 동의하지 않을 때 남아 있는 전치들이 발치 될 때까지의 Tentative restoration로서 Resin Bonded Bridge(Resin Retained Bridge/Resin Bonded Fixed Partial Denture)의 적용.

Long-term survival of retained deciduous mandibular second molars and maxillary canine incorporated into final occlusion

  • Orthodontic treatment of a complex case that involves retained deciduous mandibular second molars with missing permanent successors is challenging. Usually, congenitally missing teeth are manifested with other dental anomalies that further complicate orthodontic treatment, such as retained deciduous teeth, impactions, transpositions and peg-shaped lateral incisors. Even though the long term prognosis of the retained deciduous tooth is not fully predictable, if the teeth are in good condition, the patient and clinician may incline towards a decision to preserve the deciduous teeth as long as possible. This case report demonstrates that deciduous teeth, in this case the mandibular second molars and maxillary canine, can be incorporated into final occlusion with clinically stable long-term results.

유합치와 쌍생치에 관한 방사선학적 연구 (A RADIOGRAPHIC STUDY OF FUSED AND GERMINATED TOOTH)

  • The incidence and several characteristic features of fused and geminated teeth were studied radiographically, with full mouth periapical radiogram and pantomogram, in 4201 patients of mixed dentition and 5358 patients of permanent dentition. The obtained results were as follows: 1. The prevalence was revealed to 2.86%, 0.32%, 0.33%, and 0.06% in deciduous fused tooth, permanent fused tooth, deciduous geminated tooth and permanent geminated tooth respectively, and these anomalies were occured in female more than male. 2. Fused teeth were observed predominantly in lower anterior teeth area, especially in lateral incisor and canine region, and many cases of deciduous geminated tooth were observed in upper central incisor region. 3. Congenital missing rates of succedaneous tooth in deciduous fused teeth were 57.1 %, 85.7%, 71.0%, 69.0% in upper right and left central-lateral incisor regions, lower right and left lateral incisor-canine regions, respectively. 4. Prevalence of dental caries was 42.3%, 18.8% and 5.6% in deciduous fused, deciduous geminated and permanent fused tooth, respectively. 5. In classifying of fused and geminated teeth into 9 types, by following appearance such as number of crown, root, pulp chamber and pulp canal of those teeth, it was more favorable that Type Ⅰ(2 crown, 2 root, 2 pulp chamber, 2 pulp canal) in deciduous fused tooth and Type Ⅸ (I crown, 1 root, 1 pulp chamber, 1 pulp canal) in permanent fused tooth, deciduous and permanent geminated tooth.

Occlusal rehabilitation of posterior fixed prostheses: A clinical report

  • Masticatory function is more important than esthetics in posterior fixed restoration. The usual technique-mounting the casts on a semi-adjustable articulator, etc. -cannot make all patients satisfied with their restorations. For example, functionally generated path technique can be an easier and more satisfactory method in the restoration of group function. These clinical reports describe various approaches for occlusal restoration of relatively simple posterior fixed protheses according to patients occlusal patterns. The 3-unit bridge restoration is one of the most popular treatment options in prosthodontics. Because dentists have much experiences of it, they restore a missing span of one tooth mechanically, that is, without special consideration. While esthetics is important in making an anterior 3-unit fixed prostheses, mastication is more focused on in posterior 3-unit bridge restoration. Many dentists are concerned about various aspects in esthetics, such as morphology of the tooth, value, chroma, hue, translucency, surface texture, etc. But they do not usually consider various methods to restore occlusion. They treat one-tooth-missing area in a similar way in spite of patients having variety of occlusal patterns. Three cases are presented here in 3 or 4-unit bridge restoration. They show some methods to restore patients' occlusal patterns.
