  • Articular cartilage is a unique tissue with no vascular, nerve, or lymphatic supply. This uniqueness may be one of the reasons why chondral injuries will hardly heal and may progress to osteoarthritis over time. Currently, there are several surgical options for the treatment of articular cartilage lesions. Although there is some discrepancy as to which procedures work best in certain patients. The spectrum of treatment alternatives for articular cartilage defects can range from simple lavage and debridement, drilling, micro-fracturing, and abrasion to osteochondral grafting and autologous chondrocyte implantation. In 1984, for the first time, results of autologous chondrocyte implantation in a rabbit model were presented, showing hyaline cartilage repair. Clinical study using autologous cultured chondrocyte implantation in chondral defects of the human knee has been reported in 23 patients in 1994. In 14 out of 16 patients treated for chondral injuries on the femoral condyles, the results were good to excellent. It is important for the surgeon to understand the autologous chondrocyte implantation technique and to be aware on the postoperative management. Attention to surgical technique and selection of appropriate patient for the autologous chondrocyte implantation will provide with the best results.

Field Experiments on the Cutoff Grouting Around Waterway Tunnel (도수터널의 차수 그라우팅 현장시험)

  • In order to clarify an effect of the cutoff grouting, a series of field experiments were performed during construction of the waterway tunnel from the River Gilancheon(Andong) to the Youngcheon dam. The experiments were conducted in three different ways based on the grouting time in the construction sequence, i.e., the pre-grouting, after-grouting and consolidation grouting tests. And those were also planned to compare the efficiency of grouting in relation to the material types of grout, base rock types and other geologic factors such as discontinuities, depth and direction of grouting holes, and number of grouting stages. Among the materials of grout employed in the experiments, such as a common Portland cement, a micro-cement, a micro-cement with sodium silicate, and a urethane, the urethane was the most effective as the cutoff grouting. And for the same grout material, the pre-grouting was more effective to cutoff the water inflow comparing to the after-grouting and the consolidation grouting. For the rock types, the grouting efficiency in the sedimentary rocks as a base rock was less than the other rocks such as granite and volcanic rocks, which is believed due to the smaller separation of joints and the abundance of infilling materials in the joints developed in the sedimentary rocks. There was no direct relationship between the total RMR value of the rock mass and the grouting efficiency, however, the joint separation which is one of the RMR criteria is believed to have positive relation to the grouting efficiency. And the direction of the grouting holes might not so much affect on the grouting efficiency while increasing the number of grouting stage showed the better results.

Effect of h-BN Content on Microstructure and Mechanical Properties of Si3N4 (질화규소의 미세조직과 기계적 성질에 미치는 h-BN 첨가의 영향)

  • $Si_{3}N_{4}$-BN based machinable ceramics were fabricated by hot-pressing at $1800^{\circ}C$ for 2 h under a pressure of 25 MPa. The microstructure, mechanical properties, and machinability were investigated. With increasing h-BN content from 5 vol% to 30 vol%, three point flexural strength decreased from 1000 MPa of monolithic S $i_3$ $N_4$ to 720~400 MPa. The fracture toughness, $K_{IC}$ , was decreased from 7.6 MPaㆍ$m^{1/2}$ of monolithic S $i_3$ $N_4$ to 6.5~4.1 MPaㆍ$m^{1/2}$. The grain size and aspect ratio of $\beta$-S $i_3$ $N_4$ slightly decreased with increasing h-BN content. S $i_3$ $N_4$ monolith could not be machined due to brittle fracture, but S $i_3$ $N_4$-BN based machinable ceramics could be machined without fracture, showing excellent machinability. With increasing h-BN content, the thurst force during cutting and micro-drilling process was decreased.

Effect of platelet-rich plasma on bone regeneration in ovariectomized osteoporotic rats (골다공증 유발 쥐에서 혈소판 농축 혈장이 골 재생에 미치는 영향)

  • Purpose: The aim of this experimental study is to observe the effect of platelet-rich plasma (PRP) on early bone regeneration of rats both in normal condition and in osteoporosis induced by ovariectomy. Material and methods: Total 40 Sprague-Dawley female rats were divided into 4 groups. A 8-mm-diameter calvarial critical-sized defect (CSD) was made by drilling with trephine at the center of calvaria in cranium of every rat. Every CSD was augmented with an osteoconductive synthetic alloplastic substitute ($MBCP^{TM}$) and PRP as follows. Group A; 10 non-ovariectomized rats grafted with only $MBCP^{TM}$. Group B; 10 non-ovariectomized rats grafted with $MBCP^{TM}$ and PRP. Group C; 10 ovariectomized rats grafted with only $MBCP^{TM}$. Group D; 10 ovariectomized rats grafted with $MBCP^{TM}$ and PRP. At 4 weeks after graft, every rat was sacrificed. And histomorphometric analysis was performed by measuring calcified area of new bone formation within the CSD. Results: Comparing four groups, results were obtained as follows. 1. In non-ovariectomized groups, PRP showed a positive effect somewhat but not significant (P > .05). 2. In ovariectomized groups, PRP showed a positive effect significantly (P < .05). 3. In PRP untreated groups, ovariectomy diminished bone regeneration significantly (P < .05). 4. In PRP treated groups, ovariectomy diminished bone regeneration somewhat but not significant (P > .05). Conclusion: Within the limitation of this study, it can be concluded that PRP in combination with an osteoconductive synthetic alloplastic substitute has an effect on bone regeneration more significantly in ovariectomized osteoporotic rats than in normal rats.

Report on Extended Leak-Off Test Conducted During Drilling Large Diameter Borehole (국내 대구경 시추공 굴진 중 Extended Leak-Off Test 수행 사례 보고)

  • We report results of Extended Leak-Off Test (XLOT) conducted in a large diameter borehole, which is drilled for installation of deep borehole geophysical monitoring system to monitor micro-earthquakes and fault behavior of major fault zones in the southeastern Korean Peninsula. The borehole was planned to secure a final diameter of 200 mm (or more) at a depth of ~1 km, with 12" diameter wellbore to intermediate depths, and 7-7/8" (~200 mm) to the bottom hole depth. We drilled first the 12" borehole to approximately 504 m deep and installed American Petroleum Institute standard 8-5/8" casing, then annulus between the casing and bedrock was fully cemented. XLOT was carried out for several purposes such as confirming casing and cementing integrity, measuring rock stress states. To that end, we drilled additional 4 m long open hole interval to directly inject water and pressurize into the rock mass using the upper API casings. During the XLOT, flow rates and interval pressures were recorded in real time. Based on the logs we tried to analyze hydraulic conductivity of the test interval.
