심실중격결손과 폐동맥유출로 협착을 동반한 교정형 대혈관전위증의 해부학적 교정수술 (Modified Anatomic Repair of Corrected Transposition of the Great Arteries with Ventricular Septal Defect and Pulmonary Outflow Obstruction)

  • A modified procedure for the operative management of the corrected transposition of the great arteries with ventricular septal defect and pulmonary stenosis or atresia was performed in 4 patients. (age ; 18 months-9 years). The operation consists of a venous switch procedure (Senning), intraventricular baffling directing the blood flow from the morphologic left ventricle into the aorta via ventricular septal defect through the right ventriculotomy, and insertion of valved conduit between the morphologic right ventricle and the pulmonary artery. All the four patients had large non-restrictive ventriular septal defects and no evidence of significant mitral valve regurgitation. Both ventricles were well-developed. Three cases had pulmonary atresia, and the remainder had severe subpulmonic stenosis. Postperatively, all patients showed no findings of systemic or pulmonary venous obstruction withnormal sinus rhythm. One patient died of sepsis due to infection by Methicillin-resistant S. aureus on the thirteenth postoperative day. Follow-up is done on the survivors, and all of them are dong well with regular sinus rhythm, with their functional class I or II at 2 to 8 months after the operation.

Comparative In vitro Antibacterial Activity of DA-1131, A New Carbapenem Antibiotic(II)

  • 배지의 영향에 대하여 검토하기 위하여 SDA, MHA, NA 및 BHIA를 사용하였으며, 시험균주 중 Pseudomonas aeruginosa에 대하여만 NA에서의 MIC 값이 낮게 나타났을 뿐 나머지 균주에 대하여는 배지에 따른 항균력 변화가 관찰되지 않았으며, 배지의 pH를 5.4-8.4까지 변화시켰을 때 pH 변화에 따른 DA-1131의 유의적인 항균력 변화와 접종균량에 따른 항균력 변화가 관찰되지 않았다. Methicillin-resistant Staphylococcus aureus(MRSA)에 대하여는 DA-1131과 vancomycin 간의 상승 및 상가작용이 관찰되었고, P. aeruginosa, Klebsiella pneumoniae, Enterobacter cloacae 및 Serratia marcescens 등의 균종에 대하여는 ciprofloxacin의 상가작용이 확인되었다. Sisomicin은 P. aeruginosa에 대한 상승 및 상가작용이 인정되었으나 ceftazidime은 DA-1131에 의한 길항작용이 관찰되었다.

알지네이트 인상체에 대한 상업성 소독제의 효과 (The effect of disinfectants for Alginate impression)

  • To evaluate the effect of disinfectants for Alginate impression, several commercially available disinfectants were tested to Alginate model contaminated by Methicillin resistant Staphylococcus aureus. Alginate specimens were artificially contaminated by Staphylococcus aureus and soaking in four commercially available disinfectants for 1, 5, 10 minutes and colonies were counted for each case. Significant reduction of bacterial colony counts was noted after soaking in 0.5% and 0.05% sodium hypochlorite and after soaking in 2% glutaraldehyde, but not after soaking in Hypochlorus add. Commercially available sodium hypochlorite and glutaraldehyde were found to be effective disinfectants for Alginate impression. But hypochlorus add was not adequate disinfectant.

  • The resistances to multiple antibiotics of strains of Gram-positive Staphylococci. methicillin-resistant Staphylo coccus aureus (MRSA), are now significant clinical problem. One of the major efforts of our laboratory has been the search and design and synthesis of novel lead compound for the purpose of obtaining highly potent anti-MRSA drug.   • An anacardic acid analog (1) was isolated from the fungus Paecilomyces variotii which was derived from the giant jellyfish Nemopilema nomurai. Compound 1 was isolated from a natural source for the first time and was evaluated for antibacterial activity against human and marine pathogens, including MDR (multidrugresistant) strains. Compound 1 exhibited mild antibacterial activity against Escherichia coli DC 2, Streptococcus iniae, and methicillin-resistant Staphylococcus aureus 3089 (MRSA).

  • Bioactivity guided fractionation of the cultured filtrates of Aspergillus versicolor, which was derived from a marine sponge Petrosia sp., yielded three polyketides: decumbenones A (1),B (2), and versiol (3). These compounds were identified on the basis of 1D and 2D NMR spectroscopic and MS analysis. The absolute configuration was defined by the modified Mosher's method. The isolated compounds were tested for cytotoxicity against a panel of five human solid tumor cell lines and antibacterial activity against twenty clinically isolated methicillin-resistant strains. This is the first report on the isolation of these compounds from a marine source.

  • Water-soluble chitosan and water-insoluble chitosan with molecular weight of 2,000,000, 500,000, 80,000, and 40,000 and more than 90% degree of deacetylation were controlled to evaluate the antibacterial activity of chitosan against a pathogenic bacteria, methicillin resistant Staphylococcus aureus(MRSA), which is being issued in the world. The Shake Flask Method and Modified Shake Flask Method were used to find out the antibacterial activities of 5types of chitosan/acetic acid solution, and the other antibacterial activities test with the cotton filter treated with chitosan /acetic acid solution. Those test methods showed the great differences ,but the results of the antibacterial activites showed the same difference.

A New Method for Antimicrobial Susceptibility Testing of Vitro-cultured Bacteria by Means of Resonance Light Scattering Technique

  • A new method for antimicrobial susceptibility testing of vitro-cultured bacteria on an ordinary fluorescence spectrometer was developed. The viable bacteria reduced 3-(4,5 dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) to produce insoluble particles that displayed intense resonance scattering light. The assay showed a linear relationship between the number of viable bacteria and the intensity of resonance scattering light. Dead bacteria were unable to reduce MTT. Methicillin-resistant Staphylococcus aureus exposed to flavonoids from Marchantia convoluta showed a flavonoids concentration-dependent inhibition of the ability to reduce MTT. In the assay, less than 12 h was required to attain susceptibility results and fewer bacteria were utilized than in traditional methods. The RLS technique could, in combination with the MTT assay, be a rapid and sensitive measuring method to determine the in vitro activity of new antimicrobials.

The Synergistic Antibacterial Activity of 1-Acetyl-$\beta$-Carboline and $\beta$-Lactams Against Methicillin-Resistant Staphylococcus aureus (MRSA)

  • 1-Acetyl-$\beta$-carboline was isolated as an anti-MRSA agent from the fermentation broth of a marine actinomycete isolated from marine sediment. The producing strain was identified to be Streptomyces sp. by phylogenetic analysis of the 16S rRNA gene sequence. The anti-MRSA agent was isolated by bioactivity-guided fractionation of the culture extract by solvent partitioning, ODS open flash chromatography, and purification with a reversed-phase HPLC. Its structure was elucidated by extensive 2D NMR and mass spectral analyses. Combination of 1-acetyl-$\beta$-carboline with ampicillin exhibited synergistic antibacterial activity against MRSA.
