유아 발 전체의 뼈에 대한 합성 전후축방향 엑스선촬영법 연구 (A Composite DorsoPlantar Axial X-ray Projection Study on the Bones of the Whole Infant's Foot)

  • 본 연구는 유아 발의 합성 전후축방향 엑스선 촬영에서 발전체가 다리와 겹침 없이 균일한 농도와 선예한 영상을 평가하기 위하여 Receiver Operating Characteristic 와 Signal to Noise Ratio 분석 결과 Cronbach Alpha 값이 0.813로 유의하게 높았으며 신호대잡음비(SNR)의 연구에서는 발허리뼈에서 발꿈치뼈를 향하여 엑스선관을 10° 기울여 검사 시 5.948 높게 나타났으며, 발꿈치뼈에서 발허리뼈를 향하여 엑스선관을 20° 기울여 검사 시 6.957으로 가장 높았다. 또한, 주관적평가에서는 발허리뼈에서 발꿈치뼈로 10° 기울려 검사시 18점으로 높은 점수를 얻었으며, 발꿈치뼈에서 발허리쪽뼈을 향하여 엑스선관을 20° 기울여 검사 시 18 점을 얻었다. 본 연구의 결과 유아 발 합성 전후축방향 검사 시 엑스선관을 발허리뼈 뒤쪽으로 10° 기울였을 경우와 발꿈치뼈에서 전방으로 20° 기울였을 때 균일한 농도와 선예한 영상을 얻을 수 있다.

변형 Weil 절골술의 중족지 단축 효과를 이용한 Freiberg병의 치료 (Treatment of Freiberg's Disease Using the Shortening Effect of the Modified Weil Osteotomy)

  • Purpose: This study sought to evaluate the clinical effectiveness of the shortening effect of the modified Weil osteotomy for the treatment of Freiberg's disease. Materials and Methods: We reviewed 21 cases treated with the modified Weil osteotomy for Freiberg's disease from November 2005 to June 2019. The average follow-up period was 32.5 months and the mean age of the patients was 38.3 years. The clinical results were analyzed using the American Orthopaedic Foot and Ankle Society (AOFAS) lesser metatarsophalangeal-interphalangeal scale, the visual analogue scale (VAS), and the range of motion (ROM) of the metatarsophalangeal joint. In the radiologic evaluation, the length of preoperative and postoperative metatarsal shortening was compared. Results: The average AOFAS lesser metatarsophalangeal-interphalangeal scale showed an improvement from 60.5 preoperatively to 90.9 at the latest follow-up. VAS showed a decrease from 5.4 preoperatively to 0.9 at the latest follow-up. ROM of the affected metatarsophalangeal joint increased from 40.2 degrees preoperatively to 58.6 degrees at the latest follow-up. The mean length of metatarsal shortening was 6.7 mm. There was no transfer metatarsalgia, osteonecrosis, and definite joint space narrowing. Conclusion: Modified Weil osteotomy with second layer cutting is an effective treatment option to restore the joint surface and painless joint motion for patients with Freiberg's disease.

Pathological fracture induced by Halicephalobus gingivalis (Nematoda: Rhabditida) in a horse limb

  • Halicephalobus gingivalis is a free-living nematode that occasionally causes infections in horses. We report a rare case of limb fracture of horse caused by infection with H. gingivalis. An 8-year-old mare was referred to the Veterinary Hospital of the Federal University of Lavras with claudication grade 5 of the right hind limb, that had been started 3 months ago. The patient had aseptic arthritis in the tarsal joint and edema that extended to the quartile. The radiographic examination showed punctate osteolysis with exacerbation of bone trabeculation along the calcaneus, talus, proximal epiphysis of the third metatarsal and distal epiphysis of the tibia. Treatment for arthritis was initiated, and the animal showed a slight improvement in limb function. However, 21 days after hospitalization, due to a comminuted fracture of the tibia, it was euthanized. At necropsy, yellowish masses were found from the metatarsal to the tibia, and around the tarsal bones and joint. Similar masses were also found in the left kidney. Numerous nematodes compatible with H. gingivalis were identified. This is the first description of a pathological fracture caused by H. gingivalis infection in an equine limb.

  • Metatarsalgia is one of the most common causes of patients complaining of pain in their feet. This pain is the plantar forefoot, including the second to fourth metatarsal heads and arises from either mechanical or iatrogenic causes. On the other hand, it is frequently accompanied by a deformity of the toes as well as of the first and fifth rays. The pain has a variety of causes, and sometimes the cause is difficult to distinguish. The variability of possible causative factors necessitates an individualized approach to treatment. To determine these causes, this paper presents an overview of the gait mechanics, plantar pressure, and the classification according to the etiology.

  • Calcaneal or calcaneovalgus deformity can occur after surgical treatment of equinus or eguinovarus deformity in cerebral palsy patient. It is a serious complication and the results of many conservative and operative treatments are reported unsatisfactory. We experienced one case of both calcaneovalgus deformity following operative treatment of diplegic equinovarus deformity in a spastic cerebral palsy patient and report about the result of the treatment.

  • There are many reports on the various methods of treatment for hallux valgus. 1st metatarsal proximal osteotomy with distal soft tissue procedure or 1st metatarsophalangeal joint arthrodesis has been conventionally used for treatment of severe hallux valgus. We performed a new double chevron osteotomy in 3 cases and obtained satisfying results. We present this procedure as a new method of treatment for severe hallux valgus.

  • Slight protrusion of the dorsum of midfoot without degenerative arthritis is common and usually can be treated by avoiding pressure of the shoe. Primary or posttraumatic degenerative arthritis accompanied by severe functional disability is treated by realignment and arthrodesis of the involved joints. We report a case of osteophytic changes in the form of lipping at the base of the first metatarsal and medial cuneiform which was treated by resection of the spurs. The cause of these spurs may be the congenital coalition of the first metatarsocuneiform joint.

  • A female Holstein calf with five legs and two tails was examined macroscopically and radiographically. The external feature included two normal forelimbs, two normal hindlimbs, a normal tail and each of underdeveloped extra hindlimb and tail, which was attached to the pelvic region. The extra hindlimb consisted of an underdeveloped femur, crural meromelia, duplicated calcaneous, partially duplicated metatarsal bone, three rows of digits with hoofs. This extra hindlimb was connected to an extra os coxa. The cervical and thoracic vertebrae were fused partially. The lumbar, sacral and coccygeal vertebrae were duplicated. This calf is a dipygus associated with pygopagus parasiticus.

복잡계를 응용한 인체 골절치료 모델링과 해석에 관한 연구 (Simulation of Bone Fracture Healing by the Complex System Rule)

  • The bone fracture healing is simulated by using one of the complex system rules, named cellular automata method. It is assumed that each cell has property of Bone, Cartilage or Fibrous connective tissue. Nine local rules are adopted to change the property of each cell against the mechanical stimulus, which consists of the strain energy density, and the existence of bone in the surroundings. Two dimensional sheep metatarsal model is considered and the bone fracture healing is simulated. The simulation results agree well with those obtained by using fuzzy logic model and experimental data. The cellular automata method found to be one of the simulation methods to express the bone fracture healing. The cellular automata method is expected to be effective in representing biological phenomenon.

장애물 높이에 따른 성인과 노인의 족저압 분포 비교 (The Comparison of Plantar Foot Pressure Distribution in Adult and Elderly according Obstacle Heights)

  • Purpose: The purpose of this study was to compare plantar foot pressure distribution in adults and elderly according to obstacle height. Methods: Nine healthy adults and nine older adults were recruited and the subjects provided written informed consent consent prior to participation. Both groups walked and crossed obstacles with heights of 0%, 10%, 20%, and 30% of their height. Foot pressure was measured by peak pressure using the Pedar System (Novel Gmbh, Germany) during obstacle walking with barefeet in shoes. Three trails were calculated on eight areas and then averaged for data analysis. Results: A significant difference in great toe, little toes, and lateral metatarsal area was observed between adults and elderly groups, but other areas did not show significant differences. Foot pressure was increased in groups according to obstacle height. Conclusion: These findings showed that change in foot pressure distribution is more lateral in elderly in order to maintain postural control during obstacle crossing.
